Immediate effects of arousal from sleep on cardiac autonomic outflow in the absence of breathing in dogs.

To determine the immediate effects of arousal from non-rapid-eye-movement (non-REM) sleep on cardiac sympathetic and parasympathetic activities, six dogs were studied breathing through an endotracheal tube inserted into a chronic tracheostomy. Mean heart rates (HRs) during non-REM sleep were compared with 1) awake periods immediately after spontaneous arousals (ARs) and 2) later periods of stable relaxed wakefulness (RW). During spontaneous breathing, HR increased after AR (mean = 31.0%; P < 0.001) and in RW (mean = 7.6%; P < 0.001). To avoid the confounding influence of changes in breathing p

Pharmacological profile of moclobemide, a short-acting and reversible inhibitor of monoamine oxidase type A.

The novel antidepressant moclobemide is a reversible inhibitor of monoamine oxidase (MAO), preferentially of type A. Moclomide was active in three animal models considered predictive for antidepressant activity: 1) it prevented dose-dependently akinesia and blepharospasm induced in mice and rats by Ro 4-1284, a short-acting amine releasing agent. Prevention of akinesia by moclobemide also depended upon the dose of Ro 4-1284. For comparison also, effects of cimoxatone, harmaline, tranylcypromine and clorgyline are presented: 2) in cats, it selectively and dose-dependently suppressed rapid eye m

The effects of nasal anesthesia on breathing during sleep.

Inability to breathe through the nose is an increasingly recognized cause of disordered breathing during sleep. To test the hypothesis that this respiratory dysrhythmia could result from loss of neuronal input to respiration from receptors located in the nose, we anesthetized the nasal passages of 10 normal men during sleep. Each subject spent 4 consecutive nights in the sleep laboratory while sleep stages, breathing patterns, respiratory effort, and arterial oxygen saturation were monitored. Night 1 was for acclimatization with Nights 3 and 4 being randomized to nasal spraying with either 4%

The effects of armodafinil on objective sleepiness and performance in a shift work disorder sample unselected for objective sleepiness.

BACKGROUND: Armodafinil is a medication used to treat excessive sleepiness in individuals with shift work disorder (SWD). In the present study, we investigate whether armodafinil can normalize nocturnal sleepiness in a group of typical SWD patients. METHODS: Participants were 12 night workers (aged 33.8 ± 8.57 years, 7 female subjects) with excessive sleepiness (≥10 on the Epworth Sleepiness Scale; mean, 14.8 ± 3.16), meeting the International Classification of Sleep Disorders, Second Edition criteria for SWD, with no other sleep or medical disorders verified by polysomnogram. The multiple sl

Morning headaches in patients with sleep disorders: a systematic polysomnographic study.

OBJECTIVES: Patients with sleep disorders suffer more often from headache after awakening than healthy subjects. However, it still is a matter of controversy whether this applies only to patients with sleep apnea syndrome (SAS) or also to patients with other diagnoses of sleep disorders. METHODS: We asked all patients in our sleep laboratory about the frequency of past headaches and also ascertained the occurrence of morning headaches after awakening in the sleep laboratory. Polysomnographic recordings from nights before morning headache were compared with nights without following headache. F

Understanding the relationship between sleep, psychological and musculoskeletal health from a neuroimmune perspective.

BACKGROUND: Sleep, psychological wellbeing, and musculoskeletal health are more closely interconnected than previously recognised. Shared neuroimmune pathways between these domains mean that changes in one can substantially impact the others-either beneficially or detrimentally. For clinicians, a thorough understanding of these relationships is imperative to inform more holistic and effective clinical strategies for managing and potentially preventing musculoskeletal conditions. METHODS: Narrative review of neuroimmune pathways that interconnect sleep, psychological wellbeing, and musculoskel

Chronic Sleep Disturbance Enhances Inflammation and Collagen Production in Neural- and Myofascial Tissues in Uninjured Rats.

Chronic sleep disturbance is postulated to enhance pain and inflammatory responses, although systemic inflammation has been primarily investigated to date. We sought to examine the effects of chronic intermittent sleep disturbance on pain-related behaviors and peripheral forelimb neural- and myofascial tissues in the context of aging. Uninjured young adult and mature female rats (3 mo. and 9-10 mo. of age) were exposed to either normal or disturbed sleep for 9 h/day on 4 d/week (using in-cage environmental stimulation), for 6 weeks. Compared to rats with normal sleep, sleep disturbed rats of b

Mental health in endurance running: Beyond the prevalence of mental health symptoms toward understanding underlying mechanisms.

Endurance runners appear to experience a higher prevalence of several mental health symptoms, including anxiety, exercise addiction, sleep disorders, and suicidal ideation. Existing evidence suggests that this pattern is multifactorial and cannot be explained by a single mechanism. In this commentary, we highlight the complex interplay among self-selection processes, sport-specific demands, and broader biopsychosocial factors that may contribute to these outcomes. We also emphasize that demographic characteristics may interact with training-related and psychological factors to shape mental hea

Mindful endoscopy: holistically supporting patients through awake examinations of the path of swallowing.

BACKGROUND: Awake procedures such as aerodigestive endoscopies can be distressing. As a result, definitive diagnostic assessments often require sedation or general anaesthesia, limiting ability to identify early-stage cancers. OBJECTIVE: To create a process and a team to deliver mindful endoscopy, in order to support patients through awake procedures, thereby extending the scope of definitive assessments in outpatient settings. METHODS: The mindful endoscopy team, consisting of an endoscopist and endoscopy support practitioners (ESPs) was created. Nurse-led mindfulness comprised patient educ

Prevalence of sleep-related symptoms in patients receiving methadone maintenance treatment: a systematic review and meta-analysis.

BACKGROUND: Sleep disturbances are sometimes reported in clinical settings among individuals with opioid use disorder (OUD) receiving methadone maintenance treatment (MMT). Although numerous studies have explored this issue, a systematic assessment of the prevalence of these disturbances is lacking. METHODS: We searched for trials reporting the prevalence of sleep disturbances in patients with OUD receiving MMT. Studies assessing sleep-related symptoms using a standardized scale were included in a prevalence meta-analysis. All other studies were synthesized narratively and in tabular form. R

The role of physical activity in children and adolescents with chronic pain: the moderating effects of psychological symptoms and sleep difficulties.

Chronic pain in children and adolescents affects physical, psychological, social, and academic function. Although physical activity may play a role, its direct effects and its interactions with sleep difficulties and psychological symptoms remain unclear. This cross-sectional study examined whether physical activity is associated with chronic pain, and whether these associations vary by sleep difficulties and psychological symptoms. We analyzed data from 212,105 individuals (49% girls, age: 11-15 years) from the 2018 Health Behavior in School-aged Children study. Chronic pain, psychological sy

Sleepless in America: A social sensing study of pandemic-era sleeplessness using nighttime social media data.

Sleeplessness is a widespread public health concern that was further intensified during the COVID-19 pandemic, yet large-scale, real-time monitoring of sleep disturbance remains limited. This study introduces a scalable social sensing framework that leverages temporally filtered nighttime social media activity (10:00 p.m. - 6:00 a.m. local time) to infer population-level patterns of sleep disturbance. To demonstrate the framework, we conducted a comprehensive analysis of geotagged tweets posted by users in the United States from March 1, 2020 to June 30, 2021, using pre-pandemic tweets as a ba

Dream tyranny: Hyperonirism diagnostic criteria.

BACKGROUND: Hyperonirism, or "epic dreaming," is an emerging clinical phenomenon characterized by excessive, vivid, and continuous dream activity associated with non-restorative sleep and daytime fatigue. Despite historical descriptions, it remains poorly defined and unrecognized by current nosological systems. METHOD: We describe four patients assessed in expert sleep and psychiatry centers, presenting with persistent complaints of excessive dream activity not attributable to nightmares, REM sleep behavior disorder, or medication-induced parasomnias. Comprehensive clinical, psychiatric, and

Leveraging animal models to understand non-motor symptoms of Parkinson's disease.

Parkinson's disease is diagnosed based on motor symptoms, but non-motor symptoms of the disease, such as cognitive impairment, autonomic dysfunction, hyposmia, sleep disorders, and psychiatric disorders heavily impact patient and caregiver quality of life. It has proven challenging to faithfully reproduce and quantify these non-motor phenotypes. Indeed, many non-motor signs in animals that may phenotypically resemble features in patients may be caused by different mechanisms or may not be consistent within the same or similar models. In this review, we survey the existing literature on the ass

Impact of childhood trauma on depressive symptoms in bipolar adolescents: the mediating effect of sleep disturbance.

BACKGROUND: Childhood trauma exacerbates bipolar disorder in adolescents, particularly intensifying depressive symptoms. While the potential mechanisms are unclear, sleep disturbances might mediate the link between childhood trauma and depression. This study investigated if sleep problems mediate the relationship between childhood trauma and depression severity in adolescents with bipolar disorder. METHODS: 426 adolescents with bipolar I or II disorder were recruited from Renmin Hospital of Wuhan University in 2024. They completed questionnaires on the Childhood Trauma Questionnaire, the Pitt

Awake Venovenous Extracorporeal Membrane Oxygenation in the Intensive Care Unit: Challenges and Emerging Concepts.

Extracorporeal membrane oxygenation (ECMO) is an advanced treatment for severe respiratory failure. Implantation of ECMO before invasive ventilation or extubation during ECMO has been reported and is becoming increasingly popular. Avoidance of sedation and invasive ventilation during ECMO (commonly referred to as "awake ECMO") may have potential advantages, including a lower rate of delirium, shorter mechanical ventilation time, and the possibility of undergoing early rehabilitation and/or physiotherapy. However, awake ECMO is also associated with several risks, such as self-inflicted lung inj

Practical issues in the management of sleep, anxiety, and mood disorders in primary headaches.

There are many conditions associated with primary headaches, including mood (depression), anxiety, and sleep disorders, which are highly prevalent in the general population and in tertiary headache centers. We call this set of symptoms migraine, anxiety, mood, and sleep (MAMS). The presence of one or more of these symptoms can alter the clinical course and represent a practical challenge. In the present study, we aimed to describe a practical approach to treat these symptoms frequently associated with headaches. Diagnostic aspects and secondary causes were addressed as well as the shared mecha

Sleep abnormalities in bipolar disorders across mood phases: A systematic review and meta-analysis.

Sleep abnormalities are core features of bipolar disorders (BD), but they have not been thoroughly examined across mood phases. This meta-analysis investigated sleep disturbance prevalence and sleep characteristics differences in BD across mood phases. A systematic search through September 2024 identified 44 studies (7614 BD cases, 3164 controls), including 11 prevalence and 34 case-control studies. Poor sleep quality prevalence was 52% during euthymia, and insomnia prevalence was 63% during the depressive phase. Individuals with euthymic BD reported worse sleep quality and objectively measure

An early surge of norepinephrine along brainstem pathways drives sensory-evoked awakening.

The locus coeruleus-norepinephrine (LC-NE) system regulates arousal and awakening; however, it remains unclear whether the LC does this in a global or circuit-specific manner. We hypothesized that sensory-evoked awakenings are predominantly regulated by specific LC-NE efferent pathways. Anatomical, physiological, and functional modularities of LC-NE pathways involving the mouse basal forebrain (BF) and pontine reticular nucleus (PRN) were tested. We found partial anatomical segregation between the LC → PRN and LC → BF circuits. Extracellular NE dynamics in BF and PRN exhibited distinct sound-e
