Coccidioides immitis presenting as a mycelial pathogen with empyema and hydropneumothorax.

A previously healthy Caucasian male developed hydropneumothorax and a pleural peel filled with pleomorphic, septate hyphae. The only organism grown from cultures of the lung and pleural fluid was Coccidioides immitis, confirmed by exoantigen testing. Spherule-endospore forms were produced, however, following injection of the arthroconidial tissue isolate into BALB/c mice. The patient had a positive immunodiffusion complement-fixation test and developed a positive coccidioidin skin test during therapy. He recovered following thoracotomy and wedge resection of the ruptured coccidioidal cavity, a

Surgical treatment for a case of postoperative Pseudallescheria boydii endophthalmitis.

Pseudallescheria boydii (P. boydii) is an uncommon ocular pathogen which previously has been identified in only 10 of 905 fungal isolates identified by the Sid Richardson Microbiology Laboratory at the Cullen Eye Institute of Baylor College of Medicine. Furthermore, only one case of postoperative P. boydii endophthalmitis and four cases of endogenous P. boydii endophthalmitis have been reported. Three of the four patients with endogenous endophthalmitis died within 4 weeks of diagnosis. We describe a second case of postoperative endophthalmitis due to this fungus. The infection was successfull

[Disseminated histoplasmosis after a tropical visit in a patient with known sarcoidosis].

A 40-year-old man with grade II sarcoidosis went on a two-week visit to Ecuador. Afterwards he was treated with low-dose corticosteroids because of slight deterioration on the X-ray film. Unilateral hilar enlargement followed by bouts of high fever and a greatly increased erythrocyte sedimentation rate, suggested an acute exacerbation of the sarcoidosis and was, therefore, treated with an increase in steroid dosage plus azathioprine. Bronchoalveolar lavage fluid and tissue samples from lung, bone-marrow and spleen were suggestive of an infection with Histoplasma capsulatum. There was now bilat

Clinical and experimental mycotic corneal ulcer caused by Aspergillus fumigatus and the effect of oral ketoconazole in the treatment.

Aspergillus fumigatus was isolated from a case of keratomycosis. The patient, a 12-year-old boy presented with large corneal ulcer with hypopyon. The direct microscopic examination of scrapings revealed hyaline, septate mycelium. In vitro some antimycotics (amphotericin B,5-fluorocytosine, oxiconazole, amorolfine and ketoconazole) were tested against A. fumigatus by agar dilution method. Ketoconazole with minimum inhibitory concentration of 30 micrograms/ml after 11 days of incubation was most effective against A. fumigatus. Experimental corneal ulcer was produced by injecting intralamellary s

[Evaluation of antimicrobial schemes in episodes of fever and granulocytopenia in children with cancer].

The efficacy of two antibiotic regimens used as initial empiric therapy was evaluated in 299 episodes of fever and granulocytopenia in children with cancer. Of these, 148 were treated with the combination cefoxitin-amikacin-carbenicillin and the remaining 151 with ceftriaxone-amikacin. All of the patients were evaluated at the beginning and 72 hours after starting treatment with laboratory tests and based on their clinical condition. Each was classified according to the type of infection. A few modifications were made to the initial treatment plan and in accordance with the clinical condition

Orofacial aspergillosis in acute leukemia.

The clinicopathologic characteristics of orofacial aspergillosis in thirteen hospitalized patients who developed the infection while receiving chemotherapy for acute leukemia are described. Clinically, the primary sites of infection, in decreasing order of frequency, were the paranasal sinuses, nasal cavity, mouth, and facial skin; the corresponding order for the secondary sites was orbit, nasal cavity, facial skin, and mouth. Pathologically, the fungal lesions in the nasal, oral and sinusoidal cavities were black, ulcerated, and escharotic due as a direct result of tissue destruction by the o

Penicilliosis marneffei in Thailand: report of five human cases.

The first two cases of Penicillium marneffei infection in humans were reported in 1959 and 1973. There had been no additional clinical reports of penicilliosis marneffei, until the five new cases of human infection described in this paper, the first from Thailand. The patients, three of whom died, came from various parts of the country. Their common clinical manifestations were fever and generalized lymphadenopathy, with multiple soft tissue, bone, joint and pulmonary involvement. Pericarditis with effusion was also seen. The diagnosis was established by isolating and identifying a dimorphic P

[Isolation and characteristics of polymyxin-resistant mutants of Shigella flexneri].

Some characteristics of S. flexneri 2a mutants resistant to various concentrations of polymyxin M were studied. The data indicate that mutations resulting in low (50 microgram/ml) and high (300 microgram/ml) levels of the antibiotic resistance were determined by different genes. Polymyxin resistance led to changed permeability of the outer membrane with respect to detergents and some antibiotics, such as aminoglycosides, penicillins, chloramphenicol and amphotericin B but did not change sensitivity of the strains to some bacteriophages, except phage PI. Mutants resistant to 50 microgram/ml of

Spore siege: diagnostic and therapeutic challenges in fungal sternal osteomyelitis-a case series.

BACKGROUND: Deep sternal wound infections (DSWI) are rare but potentially fatal complications of cardiac surgery. While bacterial pathogens predominate, fungal involvement is a rare phenomenon coupled with high morbidity and mortality. AIM: This study aimed to describe the clinical spectrum, management strategies, and outcomes of culture-proven fungal deep sternal wound infections following cardiac surgery at a tertiary care center, with particular emphasis on pathogen distribution, diagnostic challenges, therapeutic approaches, and patient outcomes, in order to enhance awareness of this rare

Pharmacokinetics and pharmacodynamics of antifungals in children and their clinical implications.

Invasive fungal infections are a significant cause of morbidity and mortality in children. Successful management of these systemic infections requires identification of the causative pathogen, appropriate antifungal selection, and optimisation of its pharmacokinetic and pharmacodynamic properties to maximise its antifungal activity and minimise toxicity and the emergence of resistance. This review highlights salient scientific advancements in paediatric antifungal pharmacotherapies and focuses on pharmacokinetic and pharmacodynamic studies that underpin current clinical decision making. Systemic histoplasmosis: a 15-year retrospective institutional review of 111 patients.

To our knowledge, an institutional review of systemic histoplasmosis has not been conducted in the United States since the major outbreaks in Indianapolis in 1978-4982. We conducted a retrospective review of all patients with systemic histoplasmosis diagnosed at Mayo Clinic over a 15-year period. The case definitions employed were based on an international consensus statement by the European Organization for Research and Treatment of Cancer/Invasive Fungal Infections Cooperative Group (EORTC/IFICG) and the National Institute of Allergy and Infectious Diseases Mycoses Study Group (MSG). Drug-induced nephrotoxicity. Aetiology, clinical features and management.

There is a growing number of hospitalised patients who develop a drug-induced renal problem because increasing numbers of potent drugs have been added to the therapeutic arsenal in recent years. The 3 clinical syndromes that can be recognised in drug-induced nephropathy are acute renal failure, chronic interstitial nephritis and the nephrotic syndrome. The first can be caused by prerenal problems, acute interstitial nephritis, acute tubular necrosis and intratubular obstruction. The most important drugs that cause prerenal failure are NSAIDs, captopril and cyclosporin. NSAIDs inhibit the synth

Managing fungal and viral infection in the immunocompromised host.

The increasing number of opportunistic fungal infections in neutropenic cancer patients has become of major concern. Postmortem examinations have shown that more than half of the deaths attributed to infection were due to disseminated fungal infections. Candida species are the major pathogens. Unfortunately, the early diagnosis of serious fungal invasion is exceedingly difficult in these patients, since the manifestations of infection are ill-defined and the organisms can only rarely be isolated from blood cultures. Therefore, empirical antifungal therapy in febrile patients early in the cours

Cellular and material-specific responses of Candida parapsilosis biofilms to biocides.

BACKGROUND: Candida parapsilosis biofilms pose significant challenges in clinical settings due to their resilience to biocides and the presence of persister cells. This study evaluated the efficacy of amphotericin B (AmB), chlorhexidine (CLX), and ortho-phthalaldehyde (OPA) against biofilms formed by fluconazole-resistant C. parapsilosis isolates and examined the role of persister cells and catheter materials in biofilm resilience. METHODS: Clinical isolates of C. parapsilosis from hospital surfaces were exposed to CLX (0.5%) and OPA (0.55%). Minimum inhibitory concentrations (MICs) were dete

Understanding Heterogeneity and Tolerance of Dual Candida albicans-Staphylococcus aureus Biofilms to Cold Atmospheric Plasma and Antimicrobial Combinations.

Interkingdom polymicrobial biofilms of Staphylococcus aureus and Candida albicans increase pathogenicity and complicate treatment strategies, contributing to greater morbidity and mortality. These infections are typically investigated using the C. albicans strain SC5314, despite its uncertain clinical relevance. Here, we evaluate how different C. albicans (high, intermediate, low biofilm formers) and S. aureus strains influence biofilm tolerance to both conventional antimicrobials and cold atmospheric plasma (CAP). CAP is an emerging technology that has been shown, including in our prior work,

Nitroxoline and its combination with antifungals: An alternative for the treatment of fungal biofilm.

Fungal infections represent a significant public health challenge due to their high mortality rates, which can be exacerbated by the ability of fungi, especially Candida species, to form biofilms on medical devices. These opportunistic pathogens are causing candidemia in debilitated patients, contributing to increased hospitalization costs and prolonged recovery times. The existing antifungal therapy has some limitations, including high costs, significant toxicity and the emergence of fungal resistance. In this context, drug repositioning and combined therapy may be an alternative. The 8-hydro

Prevalence and diversity of antifungal resistance in Fusarium isolates across clinical and agricultural settings in the United States.

Disseminated fusariosis and chronic superficial fusariosis require effective strategies to control fungal growth and prevent systemic spread. This study presents an updated, comprehensive analysis of antifungal susceptibility in 174 Fusarium isolates collected from clinical and agricultural sources across 26 US states. Using a standardized panel of 14 antifungal agents, we compared resistance profiles across isolates to identify key epidemiological and resistance trends. Among clinical isolates, over 60% exhibited minimum inhibitory concentrations ≥ 32 µg/mL to all four clinical azoles, 49% to

Primary cutaneous mixed infection with Cryptococcus uniguttulatus and Mycobacterium tuberculosis.

Cutaneous mixed infections involving multiple pathogens present significant diagnostic and therapeutic challenges. Here, we report the first documented case of concurrent cutaneous infection caused by Cryptococcus uniguttulatus and Mycobacterium tuberculosis in a 43-year-old male. Initial histopathological examination revealed fungal organisms, and subsequent culture identified C. uniguttulatus as the causative pathogen. Despite treatment with oral itraconazole and amphotericin B, complete resolution was not achieved. Further molecular diagnostic testing using real-time PCR and next-generation

Spinal subdural abscess following chronic meningitis - A rare manifestation of Mucormycosis.

A 57-year old man with uncontrolled diabetes presented with features suggestive of chronic meningitis. Cerebrospinal fluid (CSF) analysis revealed a polymorphonuclear pleocytosis with low glucose and high protein levels in the CSF. Bacterial and fungal cultures and tests for M. tuberculosis were negative. MRI spine showed leptomeningeal enhancement. On ruling out other causes, fungal meningitis was considered. The patient developed paraparesis in the hospital. MRI showed peripherally enhancing subdural lesion with dorsal cord involvement at the level of D4 and D5 vertebrae. On laminectomy and
