Paraspinal abscess associated with epidural in labour.

A 24-year-old presented in labour requesting an epidural. She had been diagnosed as having "pelvic arthropathy" at 37 weeks. After an uneventful epidural and instrumental delivery, she discharged herself home. She re-presented 19 days later with left hip pain and abnormal neurological signs in the lower limbs. On MRI, there was a large paraspinal abscess with an epidural granulation mass compressing her spinal cord. She had an urgent surgical decompression. In hindsight, it is likely that the paraspinal mass was present at the time of epidural insertion. The discussion highlights that complica

A two-dose epidural morphine regimen in cesarean section patients: pharmacokinetic profile.

The maternal pharmacokinetics, metabolism of, and possible neonatal transmission of epidural morphine in cesarean section patients were investigated. Maternal plasma, breast milk, and maternal and neonatal urine concentrations of unconjugated and conjugated (UM and CM) morphine were measured in patients given two 5-mg doses of epidural morphine for post-cesarean section analgesia. The first dose was administered after delivery and the second dose 24 h later. Maternal venous blood samples (n = 10) were collected at times 0, 0.25, 0.5, 1, 2, 3, 4, 6, 12, and 24 h after each dose, and maternal ur

Ventilatory effects of epidural clonidine during the first 3 hours after caesarean section.

Many authors have shown the analgesic efficacy of 150-800 micrograms of epidural clonidine in the postoperative period. Its use as an analgesic after caesarean section has recently been studied with higher dosages (400-800 micrograms). Our study aimed at assessing the analgesic and ventilatory effects of two smaller doses of epidural clonidine (150 and 300 micrograms), which were compared to the effects of 10 mg of parenteral morphine (M) during the first 3 h after caesarean section. The duration of the analgesic effect was longest with 150 micrograms of epidural clonidine. Arterial blood pres

Mastectomy pain blocks: A comparison of preoperative versus intraoperative pectoralis nerve blocks: Mastectomy pectoralis nerve blocks comparison.

BACKGROUND: Pectoralis nerve blocks (PNB) are increasingly used for post-mastectomy analgesia. We evaluated whether block timing (preoperative vs intraoperative) affected postoperative opioid use. METHODS: A retrospective review of mastectomy patients receiving preoperative (PPNB) or intraoperative (IPNB) PNB was performed. Multivariable analysis assessed post-anesthesia care unit (PACU) opioid use and morphine milligram equivalents (MME). Subgroup analyses of unilateral (UM) and bilateral mastectomy (BM) were conducted. RESULTS: Among 122 patients, 69 received PPNB and 53 IPNB. No block-rel

Neuraxial anesthesia in pregnant women with surgically corrected scoliosis: a case series.

Women with a history of surgical correction for adolescent idiopathic scoliosis are often denied neuraxial anesthesia during labor and delivery due to concerns of technical difficulties, anesthetic failures, inadvertent dural punctures, and hardware infections. We report four successful neuraxial procedures by four different anesthesiologists in three patients with previous posterior spinal instrumentation and fusion who delivered at our institution. The procedures include spinal anesthesia for cerclage, epidural analgesia for labor followed by successful conversion to epidural anesthesia for

Extended pulsed radiofrequency as a part of multimodal pain management in a refractory case of Bernhardt-Roth syndrome.

Bernhardt-Roth syndrome (BRS) is a neurological condition characterised by pain, burning or numbness in anterolateral thigh due to entrapment of the lateral femoral cutaneous nerve (LFCN). The possible aetiologies can be mechanical, iatrogenic, neuropathic or idiopathic. After consent for possible publication, we are discussing a case of pain management in a 38-year-old patient with BRS secondary to diabetes. The coherent history, uncontrolled glycaemic status and reduced nerve conduction velocity for LFCN helped reach the diagnosis. Initial treatment with pharmacotherapy, steroid LFCN block a

Pre- versus postoperative intraarticular application of local anesthetics and opioids versus femoral nerve block in anterior cruciate ligament repair.

BACKGROUND: Often anterior cruciate ligament (ACL) reconstruction is performed as outpatient surgery. This requires a patient friendly postoperative pain management. Three common procedures were compared in this trial. METHODS: In a prospective, randomized study the effect of postoperative pain management using preoperative intraarticular anesthesia (0.1 mg Fentanyl + 8 ml Bupivacain 0.5%) was compared with postoperative intraarticular anesthesia (0.1 mg Fentanyl + 8 ml Bupivacain 0.5%) and the femoralis 3-in-1 nerve block (20 ml Prilocain 1% + 20 ml Bupivacain 0.5%) in 157 patients who under

Effect of relaxation practices on labour pain and choice of epidural: An exploratory mixed-method study.

BACKGROUND: Theoretical literature suggests utilising relaxation practices during labour can mitigate perceived pain. However, empirical evidence regarding their effectiveness in reducing epidural use remains inconclusive. Investigating women's experiences with relaxation techniques during labour, alongside their use of epidural, may provide further insights into these conflicting results. OBJECTIVE: To investigate women's experiences regarding the impact of relaxation practices on labour pain and the utilisation of epidurals, following attendance at an antenatal relaxation class (ARC). Interventions for fear of childbirth including tocophobia.

BACKGROUND: Many women experience fear of childbirth (FOC). While fears about childbirth may be normal during pregnancy, some women experience high to severe FOC. At the extreme end of the fear spectrum is tocophobia, which is considered a specific condition that may cause distress, affect well-being during pregnancy and impede the transition to parenthood. Various interventions have been trialled, which support women to reduce and manage high to severe FOC, including tocophobia. OBJECTIVES: To investigate the effectiveness of non-pharmacological interventions for reducing fear of childbirth

Intracranial hematoma as the cause of headache after subarachnoid anesthesia for cesarean section--a case report.

BACKGROUND: Intracranial subdural hematoma is an exceptionally rare but life-threating complication of epidural and spinal anesthesia. The diagnosis is rather difficult because the initial symptoms mimic post-dural puncture headache. CASE REPORT: A 33-year-old primipara was admitted to the hospital at 38 weeks gestation for a cesarean section due to premature rupture of membranes and meconium stained amniotic fluid. During the procedure a single puncture between L2 and L3 vertebrae was made with the use of a 26-gauge, pencil-point needle. The amount of 2.8 ml of analgesic solution was adminis

Maternal temperature monitoring during labor: concordance and variability among monitoring sites.

OBJECTIVE: Elevated maternal temperature in labor is associated with adverse immediate and long-term neonatal outcomes. Conventional methods of temperature measurement may not reflect the intrauterine temperature, which constitutes the fetal environment. The purpose of this study was to ascertain the most reliable noninvasive method of temperature monitoring in labor that would best reflect changes in intrauterine temperature. METHODS: Women in labor receiving epidural analgesia had temperature readings taken every 10 seconds from the uterine cavity, ear canal, and skin surface of the leg and

Effect of erector spinae plane block on postoperative nausea and vomiting in lumbar disc herniation surgery.

BACKGROUND: Postoperative nausea and vomiting (PONV) is the most frequent surgical complication after pain. We assessed the effect of the erector spinae plane (ESP) block on PONV incidence, nausea severity, pain levels, rescue antiemetic and analgesic requirements, intraoperative remifentanil use, and the correlation between pain and nausea severity in patients undergoing elective single-level lumbar disc herniation (LDH) surgery. METHODS: A prospective, randomized, double-blind study included 92 patients with American Society of Anesthesiologists I-II status who were scheduled for lumbar dis

Racial Disparities in Unmet Pain Treatment Preference, Pain Treatment Satisfaction and Subsequent Opioid Misuse: A Secondary Analysis of a National Multisite RCT.

BACKGROUND: Racial disparities in opioid prescriptions for pain are well documented. Evidence shows undertreated acute pain increases the risk of developing chronic pain, which puts patients at risk of long-term opioid use and misuse. We sought to determine the association between satisfaction with analgesia, unmet opioid preference, and opioid misuse risk by race in a diverse, longitudinal cohort. METHODS: We conducted a secondary analysis of participants with complete data in an RCT of 1301 patients who presented to the emergency department (E.D.) for acute kidney or back pain. Our primary

Pregnancy and childbirth after adolescent idiopathic scoliosis surgery: A study of 80 pregnancies.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) is a common spinal disease affecting 2% of adolescents, and women in 90% of the cases. When a surgical treatment is opted for, many questions are frequently asked by families and patients about the course of pregnancy and childbirth after the spinal fusion. This subject remains little studied in the literature, especially with modern instrumentation techniques. HYPOTHESIS: The goal was to describe pregnancy and childbirth after AIS surgery in terms of access to epidural analgesia, need for cesarean section (c-section), and low back pain during

Opioid sparing effect of intravenous dexmedetomidine in orthopaedic surgery: a retrospective analysis.

BACKGROUND: Dexmedetomidine is a highly selective alpha-2 receptor agonist without any effect on the GABA receptor. It provides an excellent sedative and analgesic profile with few side effects. We report our experience with dexmedetomidine use during orthopaedic surgery under locoregional anaesthesia to ensure adequate sedation and optimal postoperative pain control. METHODS: In this retrospective analysis, we included 128 patients who underwent orthopaedic surgery between January 2019 and December 2021. All patients received the same local anaesthetic dose of 20 ml of ropivacaine 0.375% + m
