Nonsteroidal Anti-inflammatory Drugs (NSAIDs) and Collision Sports: The Need for Further Education in Rugby.

Rugby is a collision sport and the fastest growing sport in the United States. Nonsteroidal anti-inflammatory drugs (NSAIDs), which can be acquired without prescription, are used by millions of individuals daily. Athletes in collision sports such as rugby overuse them, and the management of these analgesic anti-inflammatory drugs is gaining importance to healthcare providers in sports because of their potential organ toxicities. Athletes, amateur and elite, may misconstrue the perceived advantages of NSAIDs and seem to be unaware of the serious complications arising from their overuse. This ov

[The role and status of standardized cancer pain management in comprehensive cancer care].

Standardized cancer pain management is an essential component of comprehensive cancer care. Pain is one of the most common and distressing symptoms in palliative care for patients with advanced cancer. Persistent pain severely impairs daily function and quality of life. The implementation of standardized cancer pain management, including standardized assessment, analgesic treatment, adverse effect management, follow-up, and end-of-life care, has significantly improved pain control and quality of life, with the goal of achieving optimal pain relief and preserving dignity throughout the disease

Ultrasound-Guided Serratus Anterior Plane Block for Acute Zoster Pain in an Immunocompromised Heart Transplant Patient in the Emergency Department.

BACKGROUND: Zoster rashes are painful and may be difficult to manage with conventional oral, topical, and intravenous analgesics. Ultrasound-guided regional nerve blocks may have a role as part of a multimodal analgesia strategy for managing acute pain in these patients. CASE REPORT: We report the case of a 54-year-old immunocompromised man with a remote history of heart transplant who presented with an acute multi-dermatomal thoracic zoster rash and was awaiting admission for antiviral therapy and close monitoring. After negligible pain relief from multiple doses of intravenous opioids, he u

Intraoperative Ultrasound-Guided Selective Glossopharyngeal Nerve Block for Post-tonsillectomy Analgesia: A Case Report.

Tonsillectomy can lead to significant postoperative pain, which can impact the recovery process. Traditional analgesic approaches may entail risks due to medication use. Considering that the tonsils are innervated by the glossopharyngeal and maxillary nerves, implementing glossopharyngeal and maxillary nerve blocks can provide analgesia. Ultrasound guidance may improve its effectiveness and safety. A woman in her 30s with recurrent tonsillitis underwent tonsillectomy under general anesthesia. After induction, we performed an ultrasound-guided selective glossopharyngeal nerve block and an ultra

Pain Management in Animals with Oncological Disease: Opioids as Influencers of Immune and Tumor Cellular Balance.

Advancements in understanding pain physiopathology have historically challenged animals' absence of pain senses. Studies have demonstrated that animals have comparable neural pain pathways, suggesting that cats and dogs likely experience pain similarly to humans. Understanding brain circuits for effective pain control has been crucial to adjusting pain management to the patient's individual responses and current condition. The refinement of analgesic strategies is necessary to better cater to the patient's demands. Cancer pain management searches to ascertain analgesic protocols that enhance p

A transcriptome data set for comparing skin, muscle and dorsal root ganglion between acute and chronic postsurgical pain rats.

Chronic postsurgical pain (CPSP), with a high prevalence and rising epidemic of opioids crisis, is typically derived from acute postoperative pain. Our knowledge on the forming of chronic pain mostly derives from mechanistic studies of pain processing in the brain and spinal cord circuits, yet most pharmacological interventions targeting CNS came to be unhelpful in preventing CPSP. Revealing the peripheral mechanisms behind the transition from acute to chronic pain after surgery could shine a light on the novel analgesic regimens. Based on two recognized animal models in simulation of acute an

Mechanisms of transcranial direct current stimulation (tDCS) for pain in patients with fibromyalgia syndrome.

Fibromyalgia syndrome (FMS) is a recurrent pain condition that can be challenging to treat. Transcranial direct current stimulation (tDCS) has become a promising non-invasive therapeutic option in alleviating FMS pain, but the mechanisms underlying its effectiveness are not yet fully understood. In this article, we discuss the most current research investigating the analgesic effects of tDCS on FMS and discuss the potential mechanisms. TDCS may exert its analgesic effects by influencing neuronal activity in the brain, altering cortical excitability, changing regional cerebral blood flow, modul

Tapentadol: A Review of Experimental Pharmacology Studies, Clinical Trials, and Recent Findings.

Tapentadol is an analgesic compound that acts centrally to attenuate pain. Previous studies have shown that tapentadol has dual mechanisms of action as a mu-opioid receptor agonist and noradrenaline re-uptake inhibition. Therefore, tapentadol provides a great advantage over classic opioids in pain management from nociceptive to neuropathic. Cumulative evidence from in vitro data suggests that tapentadol effect of norepinephrine re-uptake could be a new target that overcomes other classic opioids in chronic neuropathic pain. Compared to tramadol and other opioids, tapentadol is associated with

Case report: Electroacupuncture for acute pain flare-up of knee osteoarthritis.

Acute pain flare-up of knee osteoarthritis (KOA) is a common disease in orthopedics and is mainly treated with analgesic drugs. Patients usually refuse to take western medicines orally owing to gastrointestinal side effects or unsatisfactory treatment results. We report the case of a 69-year-old woman who had an acute pain flare-up of right KOA induced by long-distance walking. As the patient refused medication, we used electroacupuncture (EA) to relieve her symptoms. EA with a 2-Hz frequency and a 1-2-mA intensity had an analgesic effect on the acute pain flare-up of KOA. After 12 weeks of EA

Local and Regional Anaesthetic Techniques in Canine Ovariectomy: A Review of the Literature and Technique Description.

Canine ovariectomy is an elective surgery with a moderate level of pain. Despite its relative simplicity, it requires surgical pain management. This study aimed to collect all recent information about local and regional anaesthetic/analgesic techniques in a review of the literature describing the technique utilised. The various procedures described in this review use local anaesthetics to improve analgesia in the routine systemic anaesthetic protocol. The approach described in this paper is called multimodal analgesia and is used in addition to the normal standard anaesthetic protocol. These t

Respiratory depression and analgesia by opioid drugs in freely behaving larval zebrafish.

An opioid epidemic is spreading in North America with millions of opioid overdoses annually. Opioid drugs, like fentanyl, target the mu opioid receptor system and induce potentially lethal respiratory depression. The challenge in opioid research is to find a safe pain therapy with analgesic properties but no respiratory depression. Current discoveries are limited by lack of amenable animal models to screen candidate drugs. Zebrafish (Danio rerio) is an emerging animal model with high reproduction and fast development, which shares remarkable similarity in their physiology and genome to mammals

The Role of Organic Small Molecules in Pain Management.

In this review, a timeline starting at the willow bark and ending in the latest discoveries of analgesic and anti-inflammatory drugs will be discussed. Furthermore, the chemical features of the different small organic molecules that have been used in pain management will be studied. Then, the mechanism of different types of pain will be assessed, including neuropathic pain, inflammatory pain, and the relationship found between oxidative stress and pain. This will include obtaining insights into the cyclooxygenase action mechanism of nonsteroidal anti-inflammatory drugs (NSAID) such as ibuprofe

Piperidine propionamide as a scaffold for potent sigma-1 receptor antagonists and mu opioid receptor agonists for treating neuropathic pain.

We designed and synthesized a novel series of piperidine propionamide derivatives as potent sigma-1 (σ1) receptor antagonists and mu (μ) opioid receptor agonists, and measured their affinity for σ1 and μ receptors in vitro through binding assays. The basic scaffold of the new compounds contained a 4-substituted piperidine ring and N-aryl propionamide. Compound 44, N-(2-(4-(4-fluorobenzyl) piperidin-1-yl) ethyl)-N-(4-methoxy-phenyl) propionamide, showed the highest affinity for σ1 receptor (Ki σ1 = 1.86 nM) and μ receptor (Ki μ = 2.1 nM). It exhibited potent analgesic activity in the formalin t

Cryoneurolysis and Percutaneous Peripheral Nerve Stimulation to Treat Acute Pain.

Two regional analgesic modalities currently cleared by the U.S. Food and Drug Administration hold promise to provide postoperative analgesia free of many of the limitations of both opioids and local anesthetic-based techniques. Cryoneurolysis uses exceptionally low temperature to reversibly ablate a peripheral nerve, resulting in temporary analgesia. Where applicable, it offers a unique option given its extended duration of action measured in weeks to months after a single application. Percutaneous peripheral nerve stimulation involves inserting an insulated lead through a needle to lie adjace

Managing neonatal pain in the era of non-invasive respiratory support.

Non-invasive ventilation is currently the preferred respiratory support for premature infants with respiratory distress. The lung-protective effects of non-invasive ventilation should however not prompt disregard for the possible pain and discomfort it can generate. Non-pharmacological interventions should be used in all premature infants, regardless of their respiratory support, and are not detailed in this review. This review includes currently available evidence and gaps in knowledge regarding three aspects of pain management in premature infants receiving non-invasive ventilation: optimisa

Pain management for chemotherapy-induced oral mucositis.

The number of children and young people diagnosed with cancer is increasing every year. Pain is a significant side effect of disease, surgery and treatments including chemotherapy. After a course of intensive chemotherapy, some children develop oral mucositis, a debilitating condition causing bleeding, pain and inflammation. Moderate and severe mucositis pain is treated with continued good oral hygiene and parenteral analgesia. The aim of this article is to identify challenges in managing chemotherapy-induced oral mucositis pain in children, and to highlight the benefits of adding ketamine as

C5a and pain development: An old molecule, a new target.

Pain is a distressing sensation, resulting from real or potential tissue damage. It is crucial to protect our body, but it can be so intense that it requires treatment. Furthermore, in some circumstances, pain can become persistent/chronic, such as that triggered by inflammatory disease or neuropathy. Treatments for pain are still a clinical challenge. An advance in the knowledge of the neurobiological mechanisms involved in the genesis of acute and chronic pain might be the fundamental approach for developing novel classes of analgesic drugs. In this context, there is emerging evidence indica
