Cardiac and pulmonary diseases. A pathophysiologic interelationship.

Left heart diseases, in particular mitral stenosis, are often associated with anatomic and functional alterations of the lung. According to the pulmonary structures involved they could be named chronic secondary intersticial and vascular lung diseases. Congenital heart diseases with pre- or post-tricuspid shunts are also often associated with anatomic and functional alterations of the lung. This condition also constitutes a chronic secondary vascular lung disease (atrial septal defect) or a chronic primary vascular lung disease ( ventricular septal defect, patent ductus arteriosus). Left Atrial Volumes and Strains in Healthy Mid-Term Pregnancy-A Detailed Investigation from a Three-Dimensional Speckle-Tracking Echocardiographic MAGYAR-Preg Study.

Introduction: Gestational physiology is characterized by an expansion of plasma volume and an elevation in cardiac output. Given the scarcity of existing data on pregnancy-related left atrial (LA) volumetric and functional features, this study aims to define LA volumes, volume-based functional properties and strains in healthy subjects during mid-term pregnancy. Methods: The present study comprised 19 healthy women in mid-term pregnancy (mean age: 30.5 ± 2.7 years, weight: 81.7 ± 14.0 kg, height: 166.9 ± 5.7 cm) without any symptoms, known diseases or other conditions, which could affect the r

Leadless Pacemaker Implantation in the Presence of the Bioprosthetic Tricuspid Valve: Case Presentation and Literature Review.

A 21-year-old man, known case of the repaired congenital heart disease, developed complete atrioventricular block (AVB) one week after simultaneous bioprosthetic pulmonary and tricuspid valve replacement and atrial septal defect repair. Considering the persistence of the AVB, it was decided to implant a permanent pacemaker. After considering all available options and the issues related to the patient, it was decided to implant a leadless pacemaker (LLP). A Micra pacemaker was implanted successfully, and the patient was discharged in good condition and without any complications. Follow-up evalu

Genetics of sinoatrial node function and heart rate disorders.

The sinoatrial node (SAN) is the primary pacemaker of the mammalian heart, initiating its electrical activation and ensuring that the heart's functional cardiac output meets physiological demand. SAN dysfunction (SND) can cause complex cardiac arrhythmias that can manifest as severe sinus bradycardia, sinus arrest, chronotropic incompetence and increased susceptibility to atrial fibrillation, among other cardiac conditions. SND has a complex aetiology, with both pre-existing disease and heritable genetic variation predisposing individuals to this pathology. In this Review, we summarize the cur

Remodelling in atrial fibrillation: the impact of amiodarone.

Atrial fibrillation (AF) is a common heart rhythm disorder with a prevalence of up to 2.9% in the general population. Its mechanism involves a particular electrophysiological profile as well as structural and biohumoral changes that are often irreversible. With the recent advances in pharmacology, amiodarone remains the cornerstone for the treatment of AF. Although it is one of the most controversial anti-arrhythmic agents due to the multitude of side effects, it is further recognised as the most effective drug available for the conversion and maintenance of sinus rhythm in the case of signifi

Endovascular therapy for left main compression syndrome. Case report and literature review.

Extrinsic compression of the left main coronary artery (LMCA) can occur in patients with severe pulmonary hypertension and enlarged pulmonary artery trunk. It has been usually described in the setting of congenital defects such as atrial septal defect, ventricular septal defect, and, more rarely, isolated persistent ductus arteriosus. Functional and structural evaluation of such patients can currently be performed noninvasively with the use of cardiac CT scanning and/or MRI. The optimal management of symptomatic patients remains unknown. We report a case of extrinsic compression of the LMCA in

Progressive pulmonary venous obstruction and pulmonary interstitial fibrosis associated with tetralogy of Fallot.

Stenosis of individual pulmonary veins is a rare condition associated with a very high mortality. The present report is the first known case of progressive pulmonary venous obstruction associated with the tetralogy of Fallot. This clinical and anatomical study suggests that initial dynamic and possibly reversible occlusion occurs at the junction of the pulmonary vein and left atrium. Anatomical lesions in pulmonary veins then develop, possibly secondary to functional stenosis. The usually fatal outcome of these cases is due to the development of fixed pulmonary venous occlusion and possibly pu

Ventriculo-pleural shunting for adult hydrocephalus.

In order to evaluate the efficacy and safety of ventriculo-pleural shunting as an alternative to conventional atrial and peritoneal procedures, we have carried out a retrospective study of 88 pleural shunting procedures. There was a 7% complication rate related to the use of the pleural space as the shunt terminus. Complications at the pleural end included shunt obstruction (functional or structural), pleural effusion, pneumothorax, and other technical problems. There were no deaths associated with shunt dysfunction or other complications. Ventriculo-pleural shunting for hydrocephalus, when us

Altered functional connectivity in lesional peduncular hallucinosis with REM sleep behavior disorder.

Brainstem lesions causing peduncular hallucinosis (PH) produce vivid visual hallucinations occasionally accompanied by sleep disorders. Overlapping brainstem regions modulate visual pathways and REM sleep functions via gating of thalamocortical networks. A 66-year-old man with paroxysmal atrial fibrillation developed abrupt-onset complex visual hallucinations with preserved insight and violent dream enactment behavior. Brain MRI showed restricted diffusion in the left rostrodorsal pons suggestive of an acute ischemic stroke. REM sleep behavior disorder (RBD) was diagnosed on polysomnography. W

Prevalence, Incidence, and Outcomes of Diastolic Dysfunction in Isolated Tricuspid Regurgitation: Perhaps Not Really "Isolated"?

BACKGROUND: In the absence of left-sided cardiac/pulmonary disease, functional tricuspid regurgitation (FTR) is referred to as isolated or idiopathic. Relationships between left ventricular diastolic dysfunction (DD) and FTR remain unknown. OBJECTIVES: The purpose of this study was to investigate the prevalence, incidence, and outcome of DD in patients with idiopathic FTR. METHODS: Adults without structural heart disease were identified. Severe DD was defined by ≥3 of 4 abnormal DD parameters (medial e', medial E/e', TR velocity, left atrial volume index) and ≥ moderate DD by ≥2. Propensity-

Heart-Brain Relationship in Stroke.

The patient presenting with stroke often has cardiac-related risk factors which may be involved in the mechanism of the stroke. The diagnostic assessment is predicated on recognition of this potential relationship. Naturally, an accurate history is of utmost importance in discerning a possible cause and effect relationship. The EKG is obviously an important clue as well as it allows immediate assessment for possible cardiac arrhythmia, such as atrial fibrillation, for possible acute ischemic changes reflective of myocardial ischemia, or there may be indirect factors such as the presence of lef

The significance of dissociation of conduction in the canine His bundle. Electrophysiological studies in vivo and in vitro.

Fractionated His bundle potentials were induced by ischemia or trauma in 30 anesthetized dogs, in vivo. Functional dissociation, i.e., alteration of the activation sequence of portions of these His bundle potentials was demonstrated in vivo as well as in 10 in vitro preparations of the His-Purkinje system. In vivo, plunge wire and electrode catheters were utilized to record from portions of the His bundle. During vagal-induced slowing of the heart rate, atrial pacing or His bundle pacing, His-Purkinje conduction as measured by the H-V interval was constant over a wide range of heart rates, 50-

Computational Model for Predicting Optimal Clinical Intervention in Pre-Operative Neonates with Transposition of the Great Arteries.

PURPOSE: Transposition of the Great Arteries (TGA) is a congenital heart defect characterized by the abnormal positioning of the pulmonary artery over the left ventricle and aorta over the right ventricle. Systemic oxygen delivery, therefore, is dependent on the ability for blood to flow (shunt) between the "parallel" (systemic and pulmonary) circulations. Prior to the standard procedure for treatment of TGA (the arterial switch operation (ASO)), shunting is commonly inadequate, resulting in systemic hypoxemia and hypoxia. In this case the standard intervention is the balloon atrial septostomy

The dominant morphology of fractionated atrial electrograms has greater temporal stability in persistent as compared with paroxysmal atrial fibrillation.

BACKGROUND: Measurements of both the dominant frequency (DF) and the time series morphology of complex fractionated atrial electrograms (CFAE) are useful to distinguish persistent from paroxysmal atrial fibrillation (AF). In this study, an algorithm was devised to extract morphologic components according to frequency, and its usefulness for distinguishing CFAE was shown. METHOD: CFAE of length 16s were obtained at two sites each from the four pulmonary vein ostia (PV), and from anterior and posterior left atrial free wall (FW), in nine paroxysmal and 10 longstanding persistent AF patients. Single Ventricle: Radiology Primer for Preprocedural Evaluation.

Single ventricle is an uncommon congenital abnormality characterized by a single anatomic or functional ventricle that cannot sustain two separate serial circulations. Imaging plays an important role in classifying a single ventricle, quantitating ventricular and valvular function, detecting associated cardiovascular anomalies, and identifying critical factors for management. Echocardiography is the first-line imaging modality. MRI and CT are useful when echocardiography is indeterminate or suboptimal, especially for evaluation of vascular anatomy. Morphologically, two atria can connect to a s
