This review focuses on nail changes that do not belong to the group of benign or malignant nail tumors. These common afflictions of the nail include structural changes in and under the nail plate as well as inflammation around and in the nail bed. They include onychomycoses, nail psoriasis, onychodystrophies, subungual hematoma, paronychia, ingrown nails and pincer nails. Due to the peculiar anatomy and physiological growth conditions of the nail, the pathology does not necessarily stem from the site of the clinical problem and calls for careful inspection and interpretation.

In this work, we compare the blood aggregation parameters measured in vitro by laser aggregometry and optical trapping techniques in blood samples with the parameters of blood rheology measured in vivo by digital capillaroscopy in the nail bed capillaries of patients suffering from the hypertension and coronary heart disease. We show that the alterations of the parameters measured in vivo and in vitro for patients with different stages of these diseases are interrelated. A practically useful coupling reaction between aromatic halides and redox-active esters was realized by nickel catalysis through the use of a packed zinc bed column in continuous flow. Multiple reuse of the column showed a negligible decrease in efficiency, affording high space/time yields. A wide range of substrates, including a number of heteroaryl halides and polyfunctional materials were coupled in generally good yields. Longer-time and larger-scale experiments further demonstrates the robustness of the system.

Lichen planus (LP) is a chronic inflammatory disease that affects the skin and oral mucosa. Although its etiology is unknown, its potential for cancerization has been confirmed. We herein report the cases of a brother and sister, both with a 15-year history of nail lichen planus of the fingers and toes, who developed squamous cell carcinomas of the nail bed and matrix. This article shows the potential for malignant transformation in lichen planus disease and highlights the importance of periodic clinical follow-up.

With a special focus on advanced wound healing (wound bed preparation, epithelialisation and hydration) and negative pressure wound therapy, the Hartmann LINK for Wound Healing Congress took place in Belfast, Northern Ireland, on 19 September. The event aimed to promote the latest advances in wound care and highlight the main challenges that clinicians face when treating different types of wounds. Annabel De Coster, editor of the British Journal of Community Nursing, gives an overview of selected talks from the congress.

In 2004, a 61-year-old male presented to a chiropractic clinic complaining of neck pain after hearing a 'crunch' when getting out of bed that morning. The initial history intake and physical examination identified no red flags or indications for the patient's pain, with the exception of traction being pain-provoking. Conventional radiographs were ordered, which identified a pathological burst fracture of the fourth cervical vertebra. This Imaging Case Review (ICR) is to provide clinicians with a follow-up to the patient's care and current state.

We studied the effects of autologous biomedical cell product (bone marrow multipotent mesenchymal stromal cells and their conditioned media) on the parameters of the microcirculatory bed in the broad ligament of the uterus of normal Wistar rats were studied. The parameters of microcirculation and lymph drainage in the broad ligament changed in opposite directions in response to injection of autologous biomedical cell product via different routes. This fact should be taken into consideration when prescribing cell therapy for inflammatory degenerative processes in the pelvic organs.

We studied disorders in ciliary body microcirculation in experimental chronic glomerulonephritis with tubulointerstitial nephritis and evaluated the hemodynamic effects of low-frequency pulsed electromagnetic field in this pathology. Pediatric nail findings are plentiful and can range from benign processes to the initial signs of a systemic condition. The examination of the nail is an essential part of the pediatric physical examination. The nail unit consists of the nail plate, the nail matrix, the hyponychium, the nail bed, and the surrounding nail folds. Conditions that affect the nail unit have a variety of etiologies; these include inflammatory, congenital, and hereditary conditions as well as tumors and trauma. This review describes many of the nail conditions that are seen in the pediatric population.

BACKGROUND: Onycholysis, which is separation of the nail plate from the nail bed at its distal and lateral attachments, can cause pain and impair function and is a well-known side effect of traditional chemotherapeutic agents.
OBJECTIVES: We present two cases of onycholysis associated with capecitabine in women with advanced breast cancer, who were initially referred for evaluation of onychomycosis and review the literature on capecitabine.
CONCLUSION: Owing to the increased frequency of use of capecitabine for different cancers, physicians should be aware of this side effect.

Wound debridement is a critical component of promoting optimal healing for a wound with necrotic tissue. Although much is known about the multiple barriers to healing that necrotic detritus presents, much is still unknown about the best ways, timing, and approaches to constructing a healthy wound bed. Falanga's "black box," a metaphor for the unknown components of wound healing and debridement, should remind all practitioners that future research needs to address the continuing questions and issues associated with promotion of quality chronic wound healing outcomes.

Pulmonary hypertension frequently complicates mitral stenosis. Increased pulmonary artery pressure results from raised left atrial pressure, pulmonary arteriolar constriction, and obliterative changes in the pulmonary vascular bed, and usually responds to surgical relief of mitral stenosis. However, severe pulmonary hypertension may persist after surgical treatment of mitral stenosis. We describe a patient whose severe pulmonary hypertension following mitral valve replacement was treated successfully with continuous intravenous epoprostenol.

Photo-onycholysis is a phototoxic reaction, which is usually drug-induced. It consists of the separation of the nail from the nail bed due to ultraviolet radiation. We report two cases of female patients who developed distal onycholysis while receiving doxycycline. Among the drugs that cause photo-onycholysis, the most frequently cited are tetracyclines, psoralens and fluoroquinolones. Photo-onycholysis is often distal, half-moon shaped and can be surrounded by pigmentation. Spontaneous recovery follows within a few months of discontinuing the drug.

A short cut review was carried out to establish whether nail removal and nail bed repair is better than simple trephining in patients with significant subungual haematoma. Altogether 312 papers were found using the reported search, of which four presented the best evidence to answer the clinical question. The author, date and country of publication, patient group studied, study type, relevant outcomes, results and study weaknesses of these best papers are tabulated. A clinical bottom line is stated.

Two cases in whom the subsegmental biliary branch of the anterio-inferior area (B5) coursed to the right laterally and inferiorly, forming a caudally convex arc, and running near the neck of the gallbladder are reported. In these two patients, the B5 branches were not aberrant, however, these branches were running near the gallbladder bed extrahepatically, which alerted us to the risk of their inadvertent division. The relationship between the cystic duct and the B5 branch is clinically important during cholecystectomy, especially laparoscopic cholecystectomy.

172 patients were operated for complicated extrasphincter high fistulas and some transsphincter fistulas of the rectum. All the patients were treated by the proposed method--resection of the fistula in intestinal lumen, splitting of the sphincter, repair of anal canal with pullthrough of the distal rectal segment. Spinal anesthesia leading to good relaxation of the anal canal was used. There were no recurrences in patients folloved up during 2 years. Postoperative bed-day reduces to 12-15. There were no disorders of rectal sphincter apparatus.

A 74-year-old man who had been institutionalized for severe multiinfarct dementia was found dead in bed. He had a history of compulsive ingestion of food and other materials, including feces. At autopsy, a 65-g bolus of fecal material was impacted within the laryngopharynx, occluding the inlet of the larynx. This case demonstrates an unusual coprophagic variant of cafe coronary syndrome and emphasizes that demented individuals with pica are at risk of sudden death from ingestion of a wide variety of materials, including, but not limited to, food.

Fungal dermatitis was diagnosed in two captive gray seals (Halichoerus grypus) and four harbor seals (Phoca vitulina) between 1992 and 1994. Trichophyton mentagrophytes, Malassezia spp., and Yarrowia (Candida) lipolytica were isolated. Erythematous, thickened, alopecic skin lesions were present on the face and on the flippers, particularly around the nail bed. The two most important environmental factors associated with development of fungal dermatitis appeared to be excessive chlorination of pool water and warm water temperature.
