Procedure-related vasospasm and reperfusion vulnerability during mechanical thrombectomy.

Mechanical thrombectomy (MT) has reshaped the treatment of acute ischemic stroke caused by large-vessel occlusion (LVO), yet successful angiographic reperfusion does not reliably translate into meaningful neurological recovery. Landmark randomized trials and the Highly Effective Reperfusion Evaluated in Multiple Endovascular Stroke Trials (HERMES) patient-level meta-analysis established MT as standard of care, but they also brought into focus the persistent problem of incomplete tissue-level rescue despite technically successful recanalization. Procedure-related vasospasm is usually handled as

Renal Denervation for Hypertension Management: Past, Current Evidence and Future Directions.

Hypertension remains the most prevalent modifiable cardiovascular risk factor worldwide, and continues to account for substantial morbidity, mortality, and health care burden despite established pharmacologic and lifestyle-based therapies. Poor long-term adherence, therapeutic inertia, polypharmacy, and persistent implementation gaps have generated interest in device-based therapies that provide durable BP reduction, independent of daily medication use. Renal denervation (RDN) has emerged as the most extensively studied catheter-based intervention targeting the sympathetic pathways that contri

Decoding Alzheimer's disease through down syndrome: insights from a genetically defined population.

PURPOSE OF REVIEW: This review explores Alzheimer's disease (AD) in individuals with Down syndrome (DS), a genetically defined population with near-universal development of AD neuropathology by age 40. We examine the genetic basis of DS-AD, epidemiology, biomarker trajectories, and clinical trial innovations, highlighting how insights from DS research inform broader AD pathogenesis, early detection, and therapeutic strategies. RECENT FINDINGS: Advances in biomarker research, including longitudinal studies such as ABC-DS, have mapped predictable trajectories of amyloid, tau, and neurodegenerat

Artificial intelligence (AI)-assisted diagnosis of skin diseases: From image classification to dermatology-specific multimodal clinical reasoning.

Artificial intelligence (AI) in dermatology has moved beyond the early paradigm of single-image classification. Dermatological diagnosis is achieved based on morphology, distribution, symptoms, tactile findings, temporal evolution, patient history, histopathology, and treatment response. Clinically important differentials, such as eczema versus psoriasis, cutaneous T-cell lymphoma versus chronic dermatitis, drug eruption versus viral exanthem, lupus erythematosus versus dermatomyositis, and melanoma versus atypical nevus, are rarely resolved with one photograph alone. This review therefore fra

Infection, inflammation and immune dysregulation: evolving perspectives on the host response.

Host inflammatory responses contribute substantially to infection-related morbidity, and in severe cases such as sepsis, acute respiratory distress syndrome (ARDS) and haemophagocytic lymphohistiocytosis, immune-mediated tissue injury may exceed the direct effects of pathogens themselves. Although traditional wisdom has considered immunosuppression risky when antimicrobial therapy is required, modern insights into immune biology reveal a more nuanced landscape in which targeted immunomodulators can attenuate harmful inflammation without broadly compromising host defence. This evolution is part

Refractory Celiac Disease: Nutritional Failure, Immune Dysregulation, and Lymphomagenesis.

Refractory celiac disease (RCeD) is a rare but severe complication of celiac disease characterized by persistent or recurrent malabsorptive symptoms and villous atrophy despite a strict gluten-free diet, after exclusion of ongoing gluten exposure, alternative enteropathies, and overt lymphoma. RCeD comprises two biologically distinct entities. RCeD-I is associated with phenotypically normal, polyclonal intraepithelial lymphocytes and generally reflects persistent gluten-independent mucosal inflammation with a relatively favorable prognosis. RCeD-II is defined by expansion of aberrant clonal in

From living well to dying well with dementia: The significance of an expanded understanding of loss for end-of-life care.

As rates of dementia increase, the need for care is clear. Understanding what this looks like and how people might orientate the narratives and practices of care against a universally relatable version of success is less clear cut yet seems crucial if we are to progress towards an ideal of care that allows for a 'good' or 'dignified' death with palliative care. With this as a central focus, the paper examines the orientating principles that govern the current approach to dementia care. We map the evolution in academic theory from deficit-based models of care that identify impairments as a mean

Ortho Plastics The Adoption and Evolution of Polyethylene in Orthopedic Surgery.

Total joint arthroplasty relies on the use of biomaterials that are biologically inert and capable of forming wear-resistant articulating surfaces. Polyethylene use in arthroplasty has become ubiquitous since its introduction in the 1960s. Early arthroplasty procedures of the hip utilized poly-tetra-flouroethylene, or "Teflon," due to its low coefficient of friction that was presumed to closely mimic the hyaline cartilage of native joints. Early catastrophic wear of Teflon caused a significant local tissue reaction contributing to osteolysis, aseptic loosening, and clinical failure ultimately

Fructose might be a clue to the origin of preeclampsia insights from nature and evolution.

Preeclampsia is a hypertensive disorder of pregnancy and is due to abnormal placentation. The pathogenesis remains unclear. Fructose is biologically distinct from glucose and has a critical role in fetal growth in early pregnancy. Many species, including humans, produce fructose in their placenta during the first trimester to assist fetal growth and survival during a time when hypoxia is significant. Fructose is preferred over glucose in hypoxic tissues, and in the developing fetus, fructose has a critical role in stimulating the production of nucleic acids, lipids and glycosaminoglycans. Fruc

BACKGROUND: Targeted therapies have broadened the available treatment options for patients with severe eosinophilic asthma (SEA). However, differences in the magnitude of treatment responses among patients indicate the presence of various underlying pathophysiological processes and patient subgroups. OBJECTIVES: We aimed to describe the characteristics of SEA and identify its patient subgroups. METHODS: Clinical data from the Cohort for Reality and Evolution of Adult Asthma in Korea were analyzed. Cluster analysis was performed among those with SEA using 5 variables, namely, prebronchodilato

Predicting functional disability in patients with spondyloarthritis using a CRP-based algorithm: A 3-year prospective study.

This prospective study explored the link between values of C-reactive protein (CRP) in patients with SpA (ankylosing spondylitis, psoriatic arthritis, reactive arthritis, or arthritis-related inflammatory bowel disease) and functional disability in order to derive an algorithm that may predict functional disability based on disease activity. Patients diagnosed with Spa were classified into five groups based on the type of therapy and they were followed up for 3 years. Group 1: Symptomatic medication alone; Group 2: Disease-modifying antirheumatic drugs (DMARDs); Group 3: DMARDs and 30 rehabili

Biochemical, biological and molecular characterization of an L-Amino acid oxidase (LAAO) purified from Bothrops pictus Peruvian snake venom.

An L-amino acid oxidase from Peruvian Bothrops pictus (Bpic-LAAO) snake venom was purified using a combination of size-exclusion and ion-exchange chromatography. Bpic-LAAO is a homodimeric glycosylated flavoprotein with molecular mass of ∼65 kDa under reducing conditions and ∼132 kDa in its native form as analyzed by SDS-PAGE and gel filtration chromatography, respectively. N-terminal amino acid sequencing showed highly conserved residues in a glutamine-rich motif related to binding substrate. The enzyme exhibited optimal activity towards L-Leu at pH 8.5, and like other reported SV-LAAOs, it i

Nonradiographic axial spondyloarthritis background and confounding factors of this new terminology: an appraisal.

The advent of biologic therapy in the treatment of rheumatic diseases has intensified the need to further define and characterize spondyloarthritis (SpA). There has been a long debate over nomenclature of the SpA subtypes. There are those who are considered "lumpers," favoring the notion that different entities of the SpA groups are manifestations of the same disease, and "splitters," those who believe the different SpA groups represent separate diseases with shared clinical features. The influential work by Moll et al. has led to separation of entities and recognition of etiological processes

The rhizome of the multidrug-resistant Enterobacter aerogenes genome reveals how new "killer bugs" are created because of a sympatric lifestyle.

Here, we sequenced the 5,419,609 bp circular genome of an Enterobacter aerogenes clinical isolate that killed a patient and was resistant to almost all current antibiotics (except gentamicin) commonly used to treat Enterobacterial infections, including colistin. Genomic and phylogenetic analyses explain the discrepancies of this bacterium and show that its core genome originates from another genus, Klebsiella. Atypical characteristics of this bacterium (i.e., motility, presence of ornithine decarboxylase, and lack of urease activity) are attributed to genomic mosaicism, by acquisition of addit

Genetic and phenotypic analysis of B-cell post-transplant lymphoproliferative disorders provides insights into disease biology.

B-cell post-transplant lymphoproliferative disorders (PTLD) are classified as early lesions, polymorphic lymphomas (P-PTLD) and monomorphic lymphomas (M-PTLD). These morphologic categories are thought to reflect a biologic continuum, although supporting genetic data are lacking. To gain better insights into PTLD pathogenesis, we characterized the phenotypes, immunoglobulin (Ig) gene alterations and non-Ig gene (BCL6, RhoH/TTF, c-MYC, PAX5, CIITA, BCL7A, PIM1) mutations of 21 PTLD, including an IM-like lesion, 8 P-PTLD and 12 M-PTLD. Gene expression profile analysis was also performed in 12 cas

A general mathematical framework to model generation structure in a population of asynchronously dividing cells.

In otherwise homogeneous cell populations, individual cells undergo asynchronous cell cycles. In recent years, interest in this fundamental observation has been boosted by the wide usage of CFSE, a fluorescent dye that allows the precise estimation by flow cytometry of the number of divisions performed by different cells in a population, and thus the generation structure. In this work, we propose two general mathematical frameworks to model the time evolution of generation structure in a cell population. The first modeling framework is more descriptive and assumes that cell division time is di

Pegylated interferon alpha therapy in acute hepatitis C: relation to hepatitis C virus-specific T cell response kinetics.

Pegylated interferon alpha (PEG IFN-alpha) improves sustained virological response rates in chronic hepatitis C, but neither its role in acute hepatitis C nor the biologic basis for its action has been defined. This prospective study assessed the efficacy of PEG IFN-alpha treatment in acute hepatitis C in relation to the kinetics of hepatitis C virus (HCV)-specific CD4(+) T cell responses during therapy and follow-up. Forty subjects with proven acute hepatitis C who received either PEG IFN-alpha plus ribavirin (n = 20) or PEG IFN-alpha monotherapy (n = 20) for 24 weeks in addition to 14 untrea

Genomic abnormalities in monoclonal gammopathy of undetermined significance.

Translocations involving immunoglobulin (Ig) loci and chromosome 13 monosomy (Delta 13) are frequent cytogenetic findings in multiple myeloma (MM). Similar chromosomal aberrations have been identified in the monoclonal gammopathy of undetermined significance (MGUS), but their prevalence and significance remain uncertain. Bone marrow from 72 patients with MGUS (n = 62) and smoldering MM (n = 10) was evaluated for translocations between the Ig heavy chain (IgH) and chromosomes 4, 11, and 16, translocations involving Ig light chain-lambda (IgL-lambda, and Delta 13.
