The Impact of Digital Intervention Messages Targeting Users With High Blood Pressure Events: Retrospective Real-World Study.

BACKGROUND: Effective hypertension management, particularly through self-care strategies, remains a significant public health challenge. Despite widespread awareness, only approximately 1 in 5 adults achieves adequate blood pressure (BP) control. There is a growing need for scalable digital health interventions that enhance awareness, support behavioral change, and improve clinical outcomes. However, real-world evidence evaluating the impact of such interventions on BP levels and their underlying mechanisms is limited. OBJECTIVE: This study aimed to evaluate the effectiveness of a digital int

Optimizing Pediatric Blood Pressure Readings: The Role of Timing and Medical Personnel Presence: A cross-sectional study.

INTRODUCTION: Hypertension  is  a  major  global  health  concern, playing  a  significant  role  in  the  development  of  cardiovascular diseases  and  other  non-communicable  conditions.  Accurate blood pressure (BP) measurement is a key to its diagnosis. While guidelines recommend three BP readings with 1-2-minute intervals for adults, there is no clear consensus for children. Additionally, various factors, such as the presence of medical personnel, could impact  BP  readings.  This  study  aimed  to  evaluate  the  impact  of measurement  timing  and  the  presence  of  medical  personne

Blood pressure patterns during pregnancy and early postpartum in preeclampsia with severe features: predictors of chronic hypertension.

RATIONALE: Women with a history of preeclampsia are at increased risk of chronic hypertension and subsequently cardiovascular disease. Early identification of high-risk patients facilitates personalized interventions to mitigate these risks. Reliable predictors for chronic hypertension after preeclampsia are lacking. This study analyzed blood pressure patterns during pregnancies with preeclampsia in relation to development of chronic hypertension. METHODS: This prospective cohort study included only women with preeclampsia with severe features, treated at the Erasmus Medical Center between 20

Current status of the use of validated home blood pressure monitoring devices among Korean patients with hypertension.

The implementation of Home Blood Pressure Monitoring (HBPM) has improved the diagnosis and treatment of hypertension. Current clinical guidelines advocate the use of validated HBPM devices. However, few studies have examined the current use of validated HBPM devices. We aimed to investigate the current status of the use of validated HBPM devices in real-world hypertensive patients. This study was conducted on Korean patients with hypertension using CareforMe®, a smartphone healthcare application which allows the patients to record their HBPM data which can be shared with their physicians. Comparison of ambulatory blood pressure monitoring among patients with postural orthostatic tachycardia syndrome, autonomic dysfunction, and controls.

The utility of ambulatory blood pressure monitoring (ABPM) among patients with various forms of autonomic dysfunction (AD) is unknown. Twenty-four-hour ABPM among patients with postural orthostatic tachycardia syndrome (POTS), AD without POTS, and control patients without AD were compared. Patients with AD without POTS had high rates of uncontrolled blood pressure (76%), whereas 19% of patients with POTS had uncontrolled blood pressure, suggesting ABPM may provide less value among patients with POTS.

Using PREVENT Equations to Compare Intensive vs Standard Systolic Blood Pressure Control for Primary Prevention in SPRINT.

BACKGROUND: The Predicting Risk of Cardiovascular Disease Events (PREVENT) equations may provide more precise risk stratification than older calculators by incorporating estimated glomerular filtration rate, excluding race, and capturing total cardiovascular disease (CVD) events including heart failure. OBJECTIVES: The aim of this study was to quantify the relative and absolute benefits and harms of intensive vs standard systolic blood pressure (SBP) treatment by the new PREVENT risk levels. METHODS: A secondary analysis of SPRING (Systolic Blood Pressure Intervention Trial) was performed am

Effectiveness of Four-Limb Blood Pressure in Distinguishing Between Aortic Stenosis and Bilateral Lower-Extremity Arterial Stenosis.

In this study, we investigated the applicability of the ankle-brachial index (ABI) and brachial-ankle index (BAI) in distinguishing aortic stenosis (AS) from lower-extremity arterial stenosis. The difference between the ABI on both sides (∆ABI) was defined as the absolute value of the left ABI minus the right ABI. Lower BAI (L-BAI) was defined as the value of the side with the lowest BAI. We obtained four-limb blood pressure measurement data from 6435 patients. AS and bilateral lower-extremity arterial stenosis (BLEAS) were diagnosed. The performance of combined bilateral ABI decline, ΔABI, an

Blood pressure load associated with progressive kidney function decline in biopsy-proven atherosclerotic chronic kidney disease patients.

BACKGROUND: Hypertension is a major risk factor for the progression of chronic kidney disease (CKD). However, the impact of blood pressure (BP) load, especially when office BP is normal, remains unclear. Thus, we conducted a retrospective cohort study to elucidate the impact of elevated BP load in patients with biopsy-proven CKD, BP load elevation, and a normal office BP compared with normotensive patients with CKD but without BP load elevation and those with hypertension. METHODS: We retrospectively analyzed patients with histologically confirmed atherosclerotic CKD who underwent kidney biop

Low on-treatment blood pressure and cardiovascular events in patients without elevated risk: a nationwide cohort study.

Insufficient blood pressure control among patients with hypertension without elevated risk is a global concern, suggesting the need for treatment optimization. However, the potential harm of excessive blood pressure lowering among these patients is understudied. This study addressed this evidence gap by using nationally representative public health insurer database covering 30 million working-age population. Patients who were continuously using antihypertensive drugs with 10-year cardiovascular risk <10% were identified. They were categorized by on-treatment systolic and diastolic blood pressu

Association between intraprocedural drops in blood pressure and infarct growth rate patterns after acute large-vessel occlusions.

BACKGROUND: Infarct growth rate (IGR) differs among patients with acute ischemic stroke due to large vessel occlusion (LVO-AIS), and this variability has critical clinical repercussions. We explored IGR patterns and their association with blood pressure during endovascular therapy (EVT). METHODS: This is a two-center cohort observational study that included consecutive anterior circulation LVO-AIS patients who underwent EVT and achieved modified Thrombolysis in Cerebral Infarction (mTICI) 2 c-3. Initial and final infarct volumes (FIV) were defined using admission computed tomography perfusion

Pulse-wave analysis during routine blood pressure measurement shows potential to distinguish between sinus and nonsinus rhythm.

OBJECTIVE: To investigate whether pulse-wave analysis (PWA) performed by trained evaluators facilitates detection of nonsinus rhythm. ANIMALS: Same-day, high-definition oscillometry pulse-wave data and ECG results of 155 animals (144 dogs and 11 cats) were analyzed. METHODS: In this cross-sectional study, we enrolled 18 participants from various backgrounds, all of whom received PWA training. The ability to distinguish between sinus and nonsinus rhythms was assessed using receiver operating characteristic curve analysis. The pulse-wave datasets were divided into 5 ECG categories. Twenty-four-hour blood pressure trajectories and clinical outcomes in patients who had an acute ischaemic stroke.

OBJECTIVE: The management of blood pressure (BP) in acute ischaemic stroke remains a subject of controversy. This investigation aimed to explore the relationship between 24-hour BP patterns following ischaemic stroke and clinical outcomes. METHODS: A cohort of 4069 patients who had an acute ischaemic stroke from 26 hospitals was examined. Five systolic BP trajectories were identified by using latent mixture modelling: trajectory category 5 (190-170 mm Hg), trajectory category 4 (180-140 mm Hg), trajectory category 3 (170-160 mm Hg), trajectory category 2 (155-145 mm Hg) and trajectory categor

Non-dipping blood pressure pattern is associated with cardiovascular events in a 21-year follow-up study.

Non-dipping blood pressure (BP) pattern is a predictor for cardiovascular (CV) events and mortality. We evaluated dipping status change and its association with incidence of non-fatal CV events in middle-aged subjects. The OPERA study was carried out during the years 1991-1993, with a follow-up study 21.7 years later. In this study, we included 452 participants with 24-h ambulatory BP measurements (ABPM) available in both surveys. The study population was divided into four groups according to the dipping pattern change: dipping-dipping (n = 152/33.6%), dipping-non-dipping (n = 198/43.8%), non-

Use of Impedance Cardiography to Guide Blood Pressure Lowering Medication Selection: Systematic Review of Randomized Controlled Trials.

BACKGROUND: Blood pressure (BP) control can be difficult to attain due to multiple factors, including choosing and titrating antihypertensive medications. Measurement of hemodynamic parameters using impedance cardiography (ICG) at the point of care may allow better alignment of medication with the mechanism(s) underlying an individual's hypertension. We conducted a systematic review of randomized controlled trials of ICG compared to usual care for attainment of BP control. METHODS: We searched Medline inclusive of the year 1946 to January 31, 2024, using a combination of MeSH terms and keywor
