The therapeutic role of bone marrow stem cell local injection in rat experimental periodontitis.

Mesenchymal stem cell therapy brings hope for regenerating damaged periodontal tissues. The present study aimed to investigate the therapeutic role of local bone marrow stem cell (BMSC) injection in ligation-induced periodontitis and the underlying mechanisms. Alveolar bone lesion was induced by placing ligatures subgingivally around the bilateral maxillary second molars for 28 days. The alveolar bone lesion was confirmed by micro-CT analysis and bone histomorphometry. Allogeneic BMSC transplantation was carried out at 28 day after ligation. The survival state of the transplanted BMSC was obse

Haematopoietic toxicity of radium-223 in patients with high skeletal tumour burden.

UNLABELLED: In patients with metastasized, castration resistant prostate cancer (mCRPC) treatment with radium-223 (Xofigo) is an attractive therapeutic option. In particular, patients with high tumour load seem to profit from this treatment in regard of survival and quality of live. Aim of this study was to stratify mCRPC patients according to a quantitative imaging marker derived from routine bone scans (EXINI bone) and analyze haematopoietic toxicity of Xofigo in these patients. PATIENTS, METHODS: Toxicity and oncologic outcome were investigated in a cohort of 14 patients with high tumour l

[Present status and perspective of articular cartilage regeneration].

Because the capacity of articular cartilage for repair is limited, defects are a major clinical problem, and there is at present no satisfactory clinical technique to regenerate cartilage defects. Current clinical practice involves the bone stimulation technique, which breaks subchondral bone to facilitate cartilage repair from bone marrow derived cells and cytokines. This consists of multiple perforations, abrasions, and micro-fractures. However, with this procedure, cartilage defects are repaired with fibrocartilage, which is known to be biochemically and biomechanically different from norma

IMPeTUs parameters correlate with clinical features in newly diagnosed multiple myeloma.

OBJECTIVES: To investigate the correlations between IMPeTUs-based 18 F-FDG PET/CT parameters and clinical features in patients with newly diagnosed multiple myeloma (MM). MATERIALS AND METHODS: PET/CT were analysed according to the IMPeTUs criteria. We correlated these PET/CT parameters with known clinically relevant features, bone marrow plasma cell (BMPC) infiltration rate and the presence of cytogenetic abnormalities. RESULTS: A total of 149 patients (86 males, 63 females; mean age, 59.9 ± 9.7 years) were included. Bone marrow metabolic state correlated with the most clinical features inc

Predictors of survival in patients with surgical spine multiple myeloma metastases.

BACKGROUND: Multiple myeloma (MM) is the most common primary malignant tumor of the spine and bone. Spinal metastases are part of the evolution and progression of the MM disease. Therefore, this study aims at investigating prognostic factors associated with overall survival (OS) in patients with symptomatic spine MM metastases. METHODS: Consecutive spine surgery procedures were performed in the largest series reported to date and comprising 51 patients with osteolytic vertebral compression fractures resulting from MM, diagnosed by either bone marrow or tumor biopsy. RESULTS: The mean age of

[Multiorgan failure after sickle cell vaso occlusive attack: integrated clinical and biological emergency].

We describe the case of a 30-year-old patient, suffering from composite S/β + sickle cell disease. He was hospitalized following a vaso-occlusive attack with acute bone pains. Despite an analgesic treatment and transfusion of three units of red blood cells, a non-regenerative anemia appeared within 24 hours. One day later an acute chest syndrome with atelectasis of the left lung and desaturation and multi-organ failure occurred and necessitated the patient's intubation and required him to be placed in an artificial coma. A bronchoalveolar lavage was performed, which eliminated pneumonia but pr

Pathophysiology and natural history of avascular necrosis of bone.

Avascular necrosis of bone (AVN) occurs as two main variants, local and systemic. Local AVN is usually caused by trauma or microtrauma; examples include primary osteonecrosis of the medial condyle, vertebral osteonecrosis, necrosis after meniscectomy, and osteonecrosis of the mandible or small bones. Systemic AVN manifests as epiphyseal necrosis or bone infarction, which is often multifocal. Little is known about the factors that trigger AVN. One possible mechanism is intraluminal obliteration of blood vessels by microscopic fat emboli, sickle cells, nitrogen bubbles (caisson disease), or foca

A genetic locus controlling aging-sensitive regression of B lymphopoiesis in an autoimmune-prone MRL/lpr strain of mice.

Aging readily affects immune system under the influence of environmental and/or intrinsic factors while accelerating the development of various immune disorders including autoimmune diseases. Little is known about molecular and cellular mechanisms connecting between immune senescence and development of autoimmune diseases. Here, we first show strain-specific and aging-sensitive onset of B-cell abnormality in a lupus-prone MRL/Mp.Fas(lpr) (MRL/lpr) strain of mice. This abnormality was characterized by the regression of B lymphopoiesis in the bone marrow of this strain. We next examined the asso

Long-term follow-up of patients who achieved complete remission after donor leukocyte infusions.

Donor leukocyte infusions (DLI) can induce a direct graft-vs-leukemia (GVL) reaction and restore complete remission for patients who relapse after allogeneic bone marrow transplantation (BMT). A critical and unanswered concern is the long-term safety and durability of DLI. To determine remission duration, long-term toxicity, and survival after DLI-induced remissions, we identified 73 patients who achieved complete remission after DLI. Follow-up information was obtained for 66 of the 73 patients, including 39 patients with chronic myelogenous leukemia (CML) and 27 patients with other diseases.

Oral Trypanosoma cruzi Acute Infection in Mice Targets Primary Lymphoid Organs and Triggers Extramedullary Hematopoiesis.

During the acute phase of Chagas disease, Trypanosoma cruzi circulation through the bloodstream leads to high tissue parasitism in the host. In primary lymphoid organs, progenitor cell reduction paralleled transient immunosuppression. Herein we showed that acute oral infection in mice promotes diffuse parasitism in bone marrow cells at 14 and 21 days post-infection (dpi), with perivascular regions, intravascular regions, and regions near the bone being target sites of parasite replication. Phenotypic analysis of hematopoietic differentiation in the bone marrow of infected mice showed that the

The effects of tranylcypromine on osteoclastogenesis in vitro and in vivo.

Identification of anti-osteoclastogenic agents is important for the treatment of bone loss diseases that feature excessive osteoclast (OC) activity and bone resorption. Tranylcypromine (TCP), an irreversible inhibitor of monoamine oxidase (MAO), has been used as an antidepressant and anxiolytic agent in the clinical treatment of mood and anxiety disorders. TCP has been discovered to exert anabolic effect on osteoblasts, and MAO-A has also been verified as an important mediator in prostate cancer cells to accelerate osteoclastogenesis. In current study, we were focused on TCP and MAO-A effects

Bone marrow transplantation for thalassemia. Experience in Pesaro, Italy.

PURPOSE: We reviewed the results of transplanting allogeneic marrow from HLA-identical donors in patients with beta-thalassemia. Among the 484 consecutive patients who have received transplants since 1981, survival and disease-free survival rates leveled off at approximately 1 year after transplantation, at 82 and 75%, respectively. PATIENTS AND METHODS: Clinical characteristics of patients before transplant have been studied to determine their impact on survival, disease-free survival, and graft rejection. By multivariate analysis, portal fibrosis, hepatomegaly, and a history of inadequate c

Experimental studies of osteopetrosis in laboratory animals.

Osteopetrosis is a metabolic bone disease characterized by a systemic increase in skeletal mass. It results from a defect in the production or function of osteoclasts and is inherited in nine genetically distinct osteopetrotic animal mutations and man. Studies of these mutations have revealed that osteopetrosis is a complex, heterogeneous disorder in its expression, etiology, and response to treatment by bone marrow transplantation or by hormone/growth factor therapy. These animal mutations have been valuable tools for probing the pathogenesis and treatment of osteopetrosis, and information ob

Lipid nanoparticles for bone marrow-targeted RNA therapeutics.

Lipid nanoparticle (LNP)-mediated delivery of therapeutic RNA molecules to the bone marrow (BM) holds transformative potential to treat hematological malignancies such as leukemia and for advancing immune and regenerative therapies. Despite this promise, LNPs exhibit poor BM accumulation following systemic administration. Formidable physiological barriers including rapid clearance by the mononuclear phagocyte system, predominant off-target sequestration in the liver, complement activation and inefficient endosomal escape currently limit their efficacy for extrahepatic applications. This review

Diagnosing Myelodysplastic Syndrome in a Rural Setting: A Case Report.

Myelodysplastic syndrome (MDS) is a rare group of diseases defined by cytopenias, abnormal cellular morphology and various symptoms including anemia, infections, and bleeding. This wide array of nonspecific symptoms creates a challenge in diagnosing MDS, especially in a rural setting. In this case study we present an 81-year-old female who presented to a rural family medicine clinic for evaluation of a severe macrocytic anemia that persisted despite repeated transfusions. The patient was evaluated with a variety of labs including complete blood count, vitamin levels, endocrine evaluation and i

Rheumatoid arthritis flare mimicry by parvovirus B19.

Parvovirus B19 infection can closely mimic flares of autoimmune diseases such as rheumatoid arthritis, posing diagnostic challenges. We report a case of a 50-year-old man with seropositive rheumatoid arthritis who presented with acute symmetrical polyarthropathy, a transient rash, and profound anaemia. Initially managed as a rheumatoid flare, further evaluation revealed severe normocytic anaemia with reticulocytopenia. Bone marrow aspiration demonstrated pure red cell aplasia with characteristic giant proerythroblasts containing intranuclear inclusions. Parvovirus B19 DNA was detected by polym

Vitamin B12: For more than just the treatment of megaloblastic anemia?

Vitamin B12, or cobalamin, belongs to the group of water-soluble vitamins and is ingested through food of animal origin such as eggs, milk, red meat and poultry, fish, and shellfish. Its clinical indication is the treatment of hypovitaminosis B12 administered orally or intramuscularly in the form of hydroxocobalamin. Hypovitaminosis B12 is mainly caused by deficient dietary intake (individuals with malnutrition, vegetarians or vegans, older adults, pregnant people, individuals with alcohol use disorder); when intestinal absorption is reduced (atrophic gastritis, malabsorption syndrome, gastroi
