Serum levels of hydroperoxides and multimorbidity among older patients with mild cognitive impairment or late-onset Alzheimer's disease.

BACKGROUND: Oxidative stress (OxS) might be involved in the pathogenesis of late-onset Alzheimer disease (LOAD); noteworthy, the presence of multimorbidity is regarded as a common denominator of OxS and dementia. AIM: To evaluate the contribution of multimorbidity to OxS in LOAD and mild cognitive impairment (MCI). METHODS: Serum hydroperoxides and multimorbidity (CIRS-CI scale) were evaluated in 46 Controls, 104 MCI and 75 LOAD. RESULTS: A trend toward an increase of hydroperoxides from Controls to MCI to LOAD was observed (LOAD vs Controls p = 0.01). This OxS marker was positively and sig

Screening Mild and Major Neurocognitive Disorders in Parkinson's Disease.

INTRODUCTION: Among the nonmotor features of Parkinson's disease (PD), cognitive impairment is one of the most troublesome problems. New diagnostic criteria for mild and major neurocognitive disorder (NCD) in PD were established by Diagnostic and Statistical Manual of Mental Disorders 5th edition (DSM-5). The aim of our study was to establish the diagnostic accuracy of widely used screening tests for NCD in PD. METHODS: Within the scope of our study we evaluated the sensitivity and specificity of different neuropsychological tests (Addenbrooke's Cognitive Examination (ACE), Mattis Dementia Ra

Using cognitive decline in novel trial designs for primary prevention and early disease-modifying therapy trials of Alzheimer's disease.

BACKGROUND: Ideally putative disease-modifying therapies for Alzheimer's disease (AD) should be tested in patients who have minimal morbidity. Current barriers to such trials in early disease include the lack of disease-specific early biomarkers, insensitivity of quantitative cognitive outcome measures, and expensive trial designs requiring large sample sizes and long duration. This paper describes principles and progress towards a novel trial design that overcomes these problems, utilizing wide-scale cognitive performance screening to define pre-trial cognitive decline trajectories which can

Hierarchical anatomical brain networks for MCI prediction: revisiting volumetric measures.

Owning to its clinical accessibility, T1-weighted MRI (Magnetic Resonance Imaging) has been extensively studied in the past decades for prediction of Alzheimer's disease (AD) and mild cognitive impairment (MCI). The volumes of gray matter (GM), white matter (WM) and cerebrospinal fluid (CSF) are the most commonly used measurements, resulting in many successful applications. It has been widely observed that disease-induced structural changes may not occur at isolated spots, but in several inter-related regions. Therefore, for better characterization of brain pathology, we propose in this paper

Enlarged perivascular spaces and their association with motor, cognition, MRI markers and cerebrovascular risk factors in male fragile X premutation carriers.

FMR1 premutation carriers (55-200 CGG repeats) are at risk of developing fragile X-associated tremor/ataxia syndrome (FXTAS), a neurodegenerative disorder associated with motor and cognitive impairment. Bilateral hyperintensities of the middle cerebellar peduncles (MCP sign) are the major radiological hallmarks of FXTAS. In the general population, enlarged perivascular spaces (PVS) are biomarkers of small vessel disease and glymphatic dysfunction and are associated with cognitive decline. Our aim was to determine if premutation carriers show higher ratings of PVS than controls and whether enla

Obsessive-Compulsive Disorder from an Embodied Cognition Perspective.

Obsessive Compulsive Disorder (OCD) is characterized by problems of control over behavior and cognition. Although almost all of the studies on pathogenesis of OCD point out fronto-striatal dysfunction, it is still not possible to reveal mechanisms to explain the entire clinical course of OCD through these circuits. A more holistic explanation can be given through the Embodied Cognition (EC) perspective, which suggests that the alteration/dysfunction of low-level sensory-motor process may appear as a multifarious extent of dysfunction of high-level cognitive processes. Fronto-striatal circuits

Impaired intestinal function is associated with lower muscle and cognitive health and well-being in patients with congestive heart failure.

BACKGROUND: Small- and large-intestinal perturbations have been described as prevalent extracardiac systemic manifestations in congestive heart failure (CHF), but alterations in protein digestion and absorption and plasma short-chain fatty acid (SCFA) concentrations and the potential link with other systemic effects (muscle and cognitive health) have not been investigated in CHF. METHODS: We analyzed protein digestion and absorption with dual stable tracer method in 14 clinically stable, noncachectic CHF outpatients (mean left ventricular ejection fraction: 35.5% [95% CI, 30.9%-40.1%]) and 15

Gulf War Illness: Neurological Impacts, Pathophysiological Insights, and Therapeutic Prospects.

Gulf War Illness (GWI) is a chronic, multi-system condition affecting a substantial proportion of veterans deployed during the 1990-1991 Gulf War. Neurological complications, including cognitive impairment, musculoskeletal pain, fatigue, depression, and migraine, represent a major clinical burden. Evidence implicates neuroinflammation, oxidative stress, mitochondrial dysfunction, and epigenetic dysregulation as central mechanisms, with emerging data suggesting early tauopathy and sex-specific immune responses. Neuroimaging studies reveal hippocampal atrophy, white matter disruptions, and incre

Neuroendocrine-immune perturbations in metabolic disease: pathophysiological mechanisms underlying cognitive impairment.

The escalating global prevalence of obesity and associated metabolic disorders has catalyzed intensive investigation into their multisystemic ramifications. Recent scientific investigations elucidate the sophisticated bidirectional relationship between systemic metabolic aberrations and central nervous system (CNS) dysfunction, orchestrated via complex neuro-immune-endocrine signaling cascades. This integrative analysis consolidates extant literature regarding the mechanisms by which adiposity, metabolic-associated steatotic liver disease (MASLD), and systemic metabolic perturbations disrupt n

Recent advancements in the understanding of the alterations in mitochondrial biogenesis in Alzheimer's disease.

Alzheimer's disease (AD) is a common neurodegenerative disease characterized by progressive memory loss and cognitive decline. The processes underlying the pathophysiology of AD are still not fully understood despite a great deal of research. Since mitochondrial dysfunction affects cellular energy metabolism, oxidative stress, and neuronal survival, it is becoming increasingly clear that it plays a major role in the development of AD. This review summarizes the recent developments of mitochondrial dysfunction in AD, emphasizing mitochondrial biogenesis, dynamics, axonal transport, interactions

Systems-Level Interactome Mapping Reveals Actionable Protein Network Dysregulation Across the Alzheimer's Disease Spectrum.

Alzheimer's disease (AD) progresses as a continuum, from preclinical stages to late-stage cognitive decline, yet the molecular mechanisms driving this progression remain poorly understood. Here, we provide a systems-level map of protein-protein interaction (PPI) network dysfunction across the AD spectrum and uncover epichaperomes-stable scaffolding platforms formed by chaperones and co-factors-as central drivers of this process. Using over 100 human brain specimens, mouse models, and human neurons, we show that epichaperomes emerge early, even in preclinical AD, and progressively disrupt multi

The Role of Glial Cells in Synaptic Dysfunction: Insights into Alzheimer's Disease Mechanisms.

Alzheimer's disease (AD) is a devastating neurodegenerative disorder that impacts a substantial number of individuals globally. Despite its widespread prevalence, there is currently no cure for AD. It is widely acknowledged that normal synaptic function holds a key role in memory, cognitive abilities, and the interneuronal transfer of information. As AD advances, symptoms including synaptic impairment, decreased synaptic density, and cognitive decline become increasingly noticeable. The importance of glial cells in the formation of synapses, the growth of neurons, brain maturation, and safegua

Imaging Cognitive Impairment and Impulse Control Disorders in Parkinson's Disease.

Dementia and mild forms of cognitive impairment as well as neuropsychiatric symptoms (i. e., impulse control disorders) are frequent and disabling non-motor symptoms of Parkinson's disease (PD). The identification of changes in neuroimaging studies for the early diagnosis and monitoring of the cognitive and neuropsychiatric symptoms associated with Parkinson's disease, as well as their pathophysiological understanding, are critical for the development of an optimal therapeutic approach. In the current literature review, we present an update on the latest structural and functional neuroimaging

Hearing Loss and Dementia: Risk Factor, Early Marker, or Both?

Background/Objectives: Hearing loss and dementia are highly prevalent conditions in older adults and represent a growing public health challenge. Over the past decade, a substantial body of epidemiological evidence has demonstrated a consistent association between age-related hearing loss and cognitive dysfunction, including incident dementia. However, the nature of this relationship remains incompletely understood. Methods: This narrative review provides a structured overview of current evidence, focusing on epidemiological findings, mechanistic pathways, and clinical implications. Hearing lo

Preliminary evidence of a relationship between sleep spindles and treatment response in epileptic encephalopathy.

OBJECTIVE: Epileptic encephalopathy with spike-wave activation in sleep (EE-SWAS) is a challenging neurodevelopmental disease characterized by abundant epileptiform spikes during non-rapid eye movement (NREM) sleep accompanied by cognitive dysfunction. The mechanism of cognitive dysfunction is unknown, but treatment with high-dose diazepam may improve symptoms. Spike rate does not predict treatment response, but spikes may disrupt sleep spindles. We hypothesized that in patients with EE-SWAS: (1) spikes and spindles would be anti-correlated, (2) high-dose diazepam would increase spindles and d
