Microsurgical tissue autotransplantation was used in 133 patients with postburn neck and extremities deformation and contractures. The free nonstretched vascularized scapular, deltoid and thoracodorsal flaps were used in 93 patients with scarring deformations and contractures of the neck. The free stretched scapular flaps on the microvascular anastomoses were used in 30 patients. The stretched and nonstretched flaps can be used successfully for correction of extensive neck and upper extremities scarring deformations and contractures. Good functional and cosmetic results were achieved in 88.7%

We are reporting here a rare case of congenital complete sternal cleft in an adult of 25 years of age, with absent anterior pericardium and "diastasis recti." Successful surgical repair was achieved along with the preservation of the already existing anatomic and mechanical sanctity of thoracic cage, sternal reconstruction, and protection of the underlying heart and great vessels with a good cosmetic result using autogenous tissue i.e., iliac bone and "V-Y" myoplasty of pectoralis major muscles with their functional preservation and correction of associated anomalies.

The indications for splenectomy in hematological diseases are well known. In particular, they include idiopathic thrombocytopenic purpura (ITP), hereditary spherocytosis (HS) and Hodgkin's disease (HD) (as a part of subdiaphragmatic staging). We present here our initial experience of 10 cases (6 ITP, 2 HS and 2 HD) managed with a laparoscopic approach as opposed to the traditional laparotomy. Advantages over the open operation include decrease of post-operative pain, pulmonary sequelae and infections, cosmetic advantages, faster recovery and reduced hospitalization.

Radial clubhand is an uncommon congenital deformity in which a part of or a whole radius is absent. This is a report on two cases of radial clubhand-like deformity resulting from osteomyelitis of the distal radius in which the distal radial metaphysis was destroyed and resorbed, resulting in a defect in that part. This is a very rare condition. The patients were successfully treated by cancellous bone graft and plating, combined with ulna shortening. At follow-up after 1 year, both patients showed good functional and cosmetic results.

The incidence of allergic contact dermatitis from neomycin evaluated in relation to 1381 verified cases of allergic contact dermatitis showed a progressive increase (5.00, 7.69, 10.18%) over a three-year period (1990-1992). Sensitivity to neomycin was investigated with special reference to possible cross-reactions between neomycin and the allergens that are commonly used in the manufacture of cosmetic products. Contact sensitivity to neomycin was found to be present with the other diagnoses, such as atopic dermatitis, seborrhoeic dermatitis, hypostasic dermatitis and psoriasis vulgaris.

Medium peels are an excellent modality for treating actinically damaged skin. Because they create wounds that extend only down into the upper dermis, they have less risk of complications than deeper phenol peels. Conversely, because of their more superficial nature, they also are not as effective as phenol peels in improving deep scars and wrinkles. Cosmetic surgeons should consider medium-depth peels a useful addition to their current armamentarium against aging skin, rather than a replacement for phenol peels.

A solitary lid mass was identified as a primary solitary plasmacytoma. A 71 year old male presented with a left upper lid mass. The mass was found to be firmly intermingled with the underlying tarsal plate. A total excision was successfully performed and the lid margin was reconstructed with a good cosmetic result. There were no positive findings in the systemic work-up. The light microscopic findings suggest that the mass is of tarsal plate origin. It is the first case of plasmacytoma of tarsal plate origin presented as a lid mass.

A new 1-stage procedure for clitorovaginoplasty in severely masculinized female pseudohermaphrodites is presented. The technique consists of clitoral reduction with preservation of the neurovascular supply to the glans and vaginal formation by joining the inverted penile skin and the divided distal "urethra" (urogenital sinus) to form a cylinder that is then inserted into the perineum and anastomosed to the true vagina. This is a report detailing further experience with our one-stage repair of severe hypospadias and scrotal transposition performed on 18 patients. Initial success was obtained in 12 cases, while two more were improved by secondary repair. The operative technique, as modified, assures a parameatal preputial flap, which is well vascularized and easily constructed into a neourethra. The advantages of this improved "glanulomeatoplasty" and scrotoplasty are discussed. The method is recommended as an excellent functional and cosmetic procedure for one-stage correction of severe hypospadias.

There are several infants and young children with a large recto vaginal fistula or common recto vaginal opening. This could arise either as a result of previous surgery, or entirely congenitally. Abscesses of the nasal septum are most likely to occur after trauma. Proper management of a nasal septal abscess requires prompt diagnosis, adequate surgical drainage, and parenteral antibiotics to prevent the potentially dangerous spread of infection and the development of severe functional and cosmetic sequelae. Without a history of prior trauma, physicians should be alert to other, less obvious causes for nasal septal abscesses. A rare case of an isolated sphenoid sinus infection leading to a nasal septal abscess illustrates the surgical treatment of these entities.

The scalaire incisions were indicated in 34 hands with 58 longitudinal palmar contracture which restrict extension and usually result of deep burns. The technique was suggested by the incisions used in Dupuytren's followed by directed cicatrization and wearing dynamic extension splints for 2 months. The functional results are good and fair in 85 percent of cases. The failure usually come from important articular stiffness. The cosmetic result is most often very good especially when comparing with the disadvantage of skin graft pigmentation.

The physician who initially examines the patient with a nasal or facial injury should thoroughly inspect the external and internal aspects of the nose and also evaluate the facial bones. General anesthesia may be necessary in examining children. Although roentgenograms of the nose and facial bones are usually obtained in cases of nasal injury, they rarely influence treatment. Treatment goals should be to maintain or restore satisfactory function and to ensure a good cosmetic result. The patient (or parents) should be advised to be alert to possible delayed abnormalities from the injury.

A new vascular musculocutaneous flap from the lower buttock is described that is based on the inferior gluteal vessels and includes skin from the gluteal fold and a part of the gluteus maximus muscle. The inferior gluteal flap has been clinically used either as an island flap for sacral repair or as a free flap. The results of 6 clinical cases are presented and the features of the flap are exposed. The minimum residual deformity of the buttock contour and, even in some cases of trochanteric lipodystrophy, the cosmetic improvement of the donor site appearance are emphasized.
