Facial Nerve Palsy in a Patient with Acute Pancreatitis: A Complication of Systemic Inflammation or a Mere Co-existence?

Acute pancreatitis can trigger a systemic inflammatory response leading to wide range of complications that could be local or systemic. Viruses have been implicated in most of the infective etiology of acute pancreatitis. Cranial nerve palsy in acute pancreatitis patient is a rare event. A 35-year-old male had mild pancreatitis which resolved with treatment. After a week of admission, he developed right sided facial palsy which gradually improved with oral steroids, acyclovir, and physiotherapy. Whether it was a complication of acute pancreatitis or a mere co-existence secondary to some infect

Spontaneous regression of a cystic hypoglossal schwannoma causing unilateral tongue atrophy.

A 60-year-old lady presented with intermittent headaches. Examination revealed striking marked unilateral tongue atrophy. Magnetic resonance imaging (MRI) revealed a cystic lesion in the hypoglossal canal and a provisional diagnosis of cystic hypoglossal schwannoma made. Annual surveillance scans showed stable appearances but surprisingly at 3 years they showed a significant reduction in the size of the lesion. Most patients with hypoglossal schwannomas present with ipsilateral hypoglossal nerve palsy; careful cranial nerve examination is vital in diagnosing such rare lesions. Little is known

Horner syndrome and VI nerve paresis as a diagnostic clue to a hidden lesion.

A 33-year-old man treated elsewhere for an isolated VI cranial nerve paresis underwent an attempted transnasal biopsy of a large space-occupying lesion in the cavernous sinus and petrous apex seen on a CT scan. During the procedure, he developed severe bleeding and hypovolaemic shock. When he came to us 2 years later, he had Homer syndrome along with a mild VI nerve paresis that aided in localizing the lesion to the carotid canal and the posterior cavernous sinus. Digital subtraction angiography revealed a large internal carotid artery aneurysm of the laceral and petrous segments within the ca

[Third nerve palsy as the only manifestation of occult temporal arteritis].

CASE REPORT: Two patients (80 and 67 year-old) presented with diplopia for a sudden right 3rd cranial nerve palsy without other ocular or systemic symptoms related to temporal arteritis. Erythrocyte sedimentation rate and C-reactive protein levels were normal. Subsequent biopsy of the superficial temporal artery confirmed the diagnosis of temporal arteritis. DISCUSSION: Patients with ocular nerve palsy could have occult temporal arteritis. Aged patients with an acute ocular ischemic lesion, without elevated erythrocyte sedimentation rate, C-reactive protein levels and systemic symptoms, shoul

The role of radiation in delayed hearing loss in nasopharyngeal carcinoma.

Although radiation damage to the inner ear can be induced experimentally in animals, its incidence in humans as a complication of therapy for nasopharyngeal carcinoma (NPC) remains a matter of conjecture due to inadequate histological validation. A case of advanced NPC with a mixed conductive and neurosensory hearing loss is presented that at necropsy showed tumour invasion of the VIIIth cranial nerve in the internal auditory meatus with associated infection. The architecture of the organ of Corti was well-preserved despite heavy doses of radiation but degeneration, the cause of which could no

[Major symptoms of multiple myeloma and their management--neurologic disorders].

The incidence of neurologic complications in multiple myeloma has been reported as high as 10 to 40% of the patients and neurologic symptoms are sometimes recognized as first manifestation of the disease. Neurologic lesions in multiple myeloma are classified as follows. 1. cerebral disorders (due to intracranial lesion, or renal insufficiency, hypercalcemia, anemia without intracranial lesion), 2. cranial nerve and intracranial involvement, 3. spinal cord or cauda equina compression, 4. radiculopathy (involvement of a nerve root). 5. peripheral neuropathy, Incidence, symptoms, and management o

[Anatomic problems involved in the approach to the internal carotid artery].

The portion of the internal carotid artery (ICA) located above the line joining the tip of the mastoid and the angle of the mandible is in close relation with cranial nerves VII, IX, X, XI and XII as well as the middle ear at the beginning of its intrapetrous course. Several procedures have been described for exposure of the ICA at the base of the skull. Extension of the conventional sternocleidomastoid approach by mandibular subluxation associated with division of the posterior belly of the digastric muscle and the styloid apophysis or its attached muscles allows exposure of most of the ICA.

Ocular manifestations of Mycoplasma pneumoniae infection.

Ocular manifestations of Mycoplasma pneumoniae infection, other than conjunctivitis, are uncommon. Optic disk swelling, optic nerve atrophy, retinal exudates and hemorrhages, and cranial nerve palsies have been infrequently reported. We describe a 15-year-old patient who developed bilateral optic disk edema and iritis during an acute infection with M. pneumoniae and review the world literature on findings associated with ocular manifestations of infection with this pathogen. Although our patient experienced complete resolution of iritis and optic disk edema after 6 weeks, several patients desc

Guillain-Barré syndrome with unilateral oculomotor nerve palsy and anti-GM3 IgG antibodies.

Guillain-Barré syndrome (GBS) is a life-threatening acute paralytic neuropathy characterized by rapidly progressive limb weakness and bilateral cranial nerve involvement. We report an early 70s male with no relevant medical history diagnosed with anti-GM3 IgG-positive GBS, who developed unilateral oculomotor nerve palsy and autonomic dysfunction during intravenous immunoglobulin (IVIG) therapy, with spontaneous symptom resolution following IVIG completion. Six-month follow-up assessments confirmed complete remission. This case reinforces the therapeutic value of IVIG in GBS patients with speci

Vestibular Nucleus Involvement in Patients With Acute Vertigo Due to Herpes Zoster Oticus or Vestibular Neuritis.

OBJECTIVES: Herpes zoster oticus (HZO) typically provokes vestibular symptoms and is traditionally viewed as a cranial nerve equivalent of shingles, but in contrast to vestibular neuritis (VN), it is unclear whether the pathology of HZO is limited to the vestibular nerve (neuritis) or can also involve the brainstem (nucleitis). METHODS: We retrospectively compared brain MRIs of patients with HZO with those of patients with VN to study radiologic changes in the brainstem. RESULTS: Five of 10 patients with HZO showed signal abnormalities in the vestibular nuclei, which lie in multiple vascular

CIDP presenting as recurrent severe back pain without weakness or sensory loss.

A 71-year-old woman presented with severe back pain, limb weakness and cranial nerve dysfunction associated with high cerebrospinal fluid (CSF) protein; we diagnosed Guillain-Barré syndrome and her symptoms completely resolved after intravenous immunoglobulin. Over the next 4 years, she had three further episodes of excruciating back pain accompanied by raised CSF protein, but without weakness, sensory loss, or abnormalities in routine nerve conduction studies. Sensory evoked potentials suggested proximal demyelination and lumbosacral plexus imaging suggested inflammation. We argue that this i

Acute subdural hematoma caused by a ruptured cavernous internal carotid artery giant aneurysm following abducens nerve palsy: case report and review of the literature.

The authors report a 61-year-old female patient with a giant cavernous aneurysm in the right internal carotid artery (ICA) leading to acute subdural hematoma (ASDH) 7 days after the occurrence of abducens nerve palsy. She underwent ICA occlusion associated with high-flow bypass. In all five reported patients with a cavernous ICA aneurysm causing ASDH, the size of the aneurysm was giant and cranial nerve signs preceded the rupture. When a patient with a symptomatic cavernous ICA giant aneurysm experiences sudden-onset headache and/or consciousness disturbance, rupture of the aneurysm should be

Persistent dysphagia and dysphonia following cervical spine surgery.

Persistent dysphagia and dysphonia following anterior cervical spine surgery have been underdiagnosed. Understanding the physiologic mechanism resulting in post-surgical dysphagia and dysphonia is essential to providing appropriate treatment. Two cases of protracted dysphagia and dysphonia following left anterior cervical corpectomy were reviewed. Videofluoroscopic evaluation, videostroboscopic assessment and laryngeal nerve-conduction testing were used to determine swallowing, vocal fold and cranial nerve integrity; to assess the ability to achieve oral nutrition; and to direct therapy and su

Results and complications from acoustic neuroma excision via middle cranial fossa approach.

The middle cranial fossa (MCF) approach for acoustic neuromas has been criticized for limited exposure, anatomic surgical difficulty, and increased risk to facial nerve and temporal lobe. From 1986 to the present, 49 patients' acoustic neuromas were removed via the MCF approach. Hearing was preserved or improved in 69% of patients regardless of preoperative hearing levels, and facial nerve function was Grade II or better in 94%. Both results demonstrate improvement from our report 8 years ago. In addition, all patients are without recurrence. Our results continue to demonstrate the surgical ad

Middle cranial fossa transtemporal approach to the intrapetrous internal carotid artery.

Diseases involving the proximity of the internal carotid artery at the skull base require identification of this vessel in the temporal bone to gain vascular control for any maneuver in its vicinity. This article details the technique of surgical dissection and exposure of the internal carotid artery within the skull base through a transtemporal middle cranial fossa approach. The anatomic landmarks important in utilizing this procedure include the greater superficial petrosal nerve, the mandibular branch of the trigeminal nerve, and the middle meningeal artery. With this approach, the internal

[Interdisciplinary investigations on the criticism of the term 'anesthesia dolorosa'].

3291 patients with facial pain and/or complaints in the orofacial region were examined from 1972-1988 by both the maxillofacial surgeon and the neurologist at the departments of maxillofacial surgery and psychosomatic medicine of the Zentrum für Zahn-, Mund- und Kieferkrankheiten of the University of Münster. Only 27 cases of genuine trigeminal neuralgia could be found in those 17 years. At the same time, however, 31 patients presented with anaesthesia dolorosa after various operations on the fifth cranial nerve. Referring to the criteria set up by Marxkors and Müller-Fahlbusch (1981) these fi

Neuroradiographic diagnosis and surgical repair of tethered cord syndrome in an English bulldog with spina bifida and myeloschisis.

A 3-month-old English Bulldog had excretory incontinence and sensory deficits in the distribution of pudendal nerves. Noncontrast radiography, myelography, and computed tomography revealed spina bifida beginning at L7, an expanded subarachnoid space caudal to L7, and a taut, thick filum terminale. Microsurgical exploration of the lumbosacral spine confirmed the presence of a tethered cord, and the filum terminale was transected. The spinal cord immediately migrated cranially about 1 cm. Although some sensory improvement was evident during a 2-week postoperative period, the dog was euthanatized

[Cranial muscle activity in dissociated brain death].

The origin of spontaneous myogenic activity during the registration of isoelectric EEGs for documentation of total brain death has not yet been explained. In nine patients with total brain death, completely abolished acoustic evoked brainstem potentials (AEP), somatosensory evoked potentials (SEP) as well as electrically evoked blink-reflexes (BR) were demonstrated. The peripheral conduction capacity of the facial nerve, however, was never impaired. These findings suggest that peripheral nerve function may be well preserved although brain stem activity is completely abolished and therefore sup

The warning leak in subarachnoid hemorrhage and the importance of its early diagnosis.

A 40-year-old man had a severe headache of sudden onset and progressive palsy of the third cranial nerve from a minor subarachnoid hemorrhage. Despite assessment by two primary care physicians the possibility of serious neurologic disease was not raised and the urgency of the situation not appreciated. Two weeks after the onset of the headache the patient was urgently admitted to hospital at the request of a neuro-ophthalmologist. Cerebral angiography revealed an aneurysm of the posterior communicating artery, which was surgically occluded with a clip. The patient had a satisfactory recovery,

Mucormycosis of the temporal bone.

Mucormycosis of the temporal bone is described in a 60-year-old female with uncontrolled diabetes mellitus whose symptoms related to cranial nerve palsies and hearing loss, following spread of infection from the nasopharynx. The infection spread along the eustachian tube and tensor tympani muscle to the base of the skull, involving the internal carotid artery with mycotic thrombosis and rupture. Subsequent spread occurred from this area predominantly along nerve pathways and as mycotic emboli in blood vessels of the labyrinth and middle ear. Infection also spread from the anterior middle ear w
