Syringomyelia associated with type I Chiari malformation. A 21-year retrospective study on 75 cases treated by foramen magnum decompression with a special emphasis on the value of tonsils resection.

The purpose of the present study is to evaluate retrospectively the effects of several intra-operative manipulations on the results of foramen magnum decompression (FMD) in patients having syringomyelia associated with type I Chiari malformation. Seventy-five patients having syringomyelia associated with Chiari I malformation were operated on between 1975 and 1996. This population was grouped into 4 subgroups according to the surgical protocol: group I = 42 patients with FMD alone; group II = 16 patients with FMD and third ventricle shunting; group III = 9 patients with FMD and syringosubarach

[Surgical treatment of unstable, therapy-resistant and painful lumbosacral joints by distracting decompressive posterolateral spondylodesis over the divided sacral bar according to Zielke's method (author's transl)].

Here the procedure of the distracting decompressive fusion over the divided sacral bar which Zielke developed 8 years ago is being described. This procedure was applied in over 150 cases. In a follow-up only one pseudarthrosis was found in the area of L5/S1. 70% of the previously pensioned patients could at least be partially rehabilitated. 32 patients were asked of their opinion with a questionnaire. Only 2 patients (6%) assessed the operation results with "bad". Indication is in all instabilities of the lumbosacral joint. The problem of the multi-operated back can be largely solved through t

Intracranial Pressure Monitoring, Heart Rate Variability, Baroreflex Sensitivity, and Signal Complexity During Neurointensive Care after Decompressive Craniectomy in Malignant Middle Cerebral Artery Infarction.

BACKGROUND: Malignant middle cerebral artery infarction (MMI) is generally treated with decompressive craniectomy (DC) followed by neurointensive care (NIC). However, prognostic values of autonomic and complexity indices remain unclear. We aimed to explore heart rate variability (HRV), baroreflex sensitivity (BRS), and signal complexity (SC) in patients with MMI during post-DC NIC in relation to demographics, stroke severity, NIC variables [intracranial pressure (ICP), cerebral perfusion pressure (CPP), and Pressure Reactivity index (PRx)], and outcome. METHODS: This single-center, retrospect

Primary Ischemic Core Resection (Corectomy) with Immediate Bone Flap Replacement in Malignant Middle Cerebral Artery Infarction: A Case Series and Systematic Comparison with Decompressive Craniectomy Outcomes.

BACKGROUND: Malignant middle cerebral artery (MCA) infarction is associated with high mortality due to massive cerebral edema and herniation. While decompressive craniectomy (DC) reduces mortality, it often results in severe disability and necessitates secondary cranioplasty. Alternatives such as ischemic core resection with immediate bone flap replacement ("corectomy") may provide internal decompression while avoiding secondary surgery. OBJECTIVE: The aim of the study is to describe a single-center experience with ischemic core resection and immediate bone flap replacement in patients with m

A New Horizon in Hip Health: Using an Innovative Variable Loop Curette for Core Decompression and Bone Marrow Aspirate Concentrate in the Management of Pre-collapse Avascular Necrosis of the Hip.

Avascular necrosis (AVN) of the femoral head, or osteonecrosis (ON), is a debilitating condition characterized by disrupted blood supply to the hip joint, leading to subchondral bone necrosis, joint collapse, and arthritis. Emerging evidence suggests that the long-term use of corticosteroids, particularly in the context of COVID-19 treatment, may contribute to AVN development. This case report presents a male in his 50s with bilateral hip pain and a history of corticosteroid use. The patient underwent core decompression (CD) with a bone marrow aspirate concentrate (BMAC) infusion using the inn

Muscle activity and rehabilitation in spinal stenosis (MARSS) after conservative therapy and surgical decompression with or without fusion: Protocol for a partially randomized patient preference trial on rehabilitation timing.

BACKGROUND: Patients affected by lumbar spinal stenosis (LSS) suffer from a multifactorial degeneration of the lumbar spine resulting in narrowing of the neuroforamina and spinal canal, leading to various functional limitations. It remains unclear whether LSS patients after surgery would benefit from early post-operative rehabilitation, or if a delayed rehabilitation would be more advantageous. The purpose of this partially randomized patient preference trial is to evaluate the impact of post-operative rehabilitation timing as well as surgical intervention type on psychometric properties and f

Evaluation of the efficacy and safety of conventional and interlaminar full-endoscopic decompressive laminectomy to treat lumbar spinal stenosis (ENDO-F trial): Protocol for a prospective, randomized, multicenter trial.

Lumbar spinal stenosis is a common spinal degenerative condition. Minimally invasive interlaminar full-endoscopic decompressive laminectomy provides greater patient satisfaction and faster recovery than open decompressive laminectomy. The aim of our randomized controlled trial will be to compare the safety and efficacy of interlaminar full-endoscopic laminectomy and open decompressive laminectomy. Our trial will include 120 participants (60 per group) who will undergo surgical treatment for lumbar spinal stenosis. The primary outcome will be the Oswestry Disability Index measured at 12 months

Early Functional Improvement in American Spinal Injury Association Impairment Scale Grade C (AIS C) Non-Traumatic Spinal Cord Injury Following Staged Thoracic Decompression and Inpatient Rehabilitation: A Case Report.

Non-traumatic spinal cord injury (NTSCI) may result from degenerative spinal conditions, including spinal stenosis and multilevel disc herniation, and can cause substantial motor, sensory, and functional impairment. We present a 33-year-old man with NTSCI secondary to severe thoracic spinal stenosis and multilevel disc herniation. Before the initial surgical intervention, the available neurological examination documented 5/5 upper-extremity strength and 4/5 strength in the bilateral lower extremities. Sensation was impaired below a thoracic level, and hyperreflexia and other findings consisten

Radiological and Clinical Outcomes After Navigated Tubular Unilateral Laminotomy for Bilateral Decompression (ULBD) for Lumbar Spinal Stenosis Among Patients with Concurrent Degenerative Scoliosis: A Short-Term Retrospective Case Series.

Background/Objectives: Adult degenerative scoliosis (ADS) is a spinal disease causing pain and reduced mobility, often occurring with degenerative lumbar spinal stenosis (DLSS). While fusion stabilizes the spine, it has drawbacks like loss of motion and adjacent segment degeneration. Minimally invasive techniques, such as tubular unilateral laminotomy for bilateral decompression (tULBD), provide a less invasive alternative, but their impact on ADS with DLSS is underexplored. This study examines the short-term effects of navigated tULBD on radiological and clinical outcomes in this patient popu

Comparison of 30-day surgical outcomes of multi-level sub-axial anterior (ACDF) vs posterior (PCDF) approach to cervical decompression and fusion in the setting of pre-operative malnutrition.

Evidence continues to mount supporting the detrimental effects malnutrition has on spine surgery outcomes. However, improving nutrition with cervical pathology faces challenges, such as progressive neurological decline with surgical delays, which may force intervention despite patient malnutrition. The American College of Surgeons National Surgical Quality Improvement Project (NSQIP) database was queried to identify adult patients who underwent multi-level sub-axial anterior cervical discectomy and fusion (ACDF) or posterior cervical decompression and fusion (PCDF) from 2015 to 2022 in the set
