Full-Endoscopic Decompression for Thoracic Ossification of the Ligamentum Flavum Combined with Dural Ossification.

Ossification of the ligamentum flavum (OLF) can result in spinal stenosis. Thoracic spinal cord compression due to spinal stenosis is a common cause of progressive thoracic myelopathy in Asian countries. The incidence of complications is high in open decompression surgeries for thoracic OLF. With dural ossification (DO), the risk of complications is even higher in thoracic OLF. We introduce a full-endoscopic decompression surgery for thoracic OLF combined with DO under local anesthesia. Hemilaminectomy is performed using a high-speed burr under the endoscopy first, and then decompression of th

Risk Factors for Intraoperative Hypotension During Decompressive Craniectomy in traumatic Brain Injury Patients.

BACKGROUND: Decompressive craniectomy (DC) is an important therapy for treating intracranial pressure elevation following traumatic brain injury (TBI). During this procedure, about one-third of patients become complicated with intraoperative hypotension (IH), which is associated with abruptly decreasing sympathetic activity resulting from brain decompression. This study aimed to identify factors associated with IH during DC procedures and the mortality rate in these patients. METHODS: The records of adult TBI patients aged 18 years and older who underwent DC at Songklanagarind Hospital betwee

Incidence and risk factors of dural ossification in patients with thoracic ossification of the ligamentum flavum.

OBJECTIVE: Dural ossification (DO) is a common clinical feature in patients with thoracic ossification of the ligamentum flavum (OLF) and associated with the increased risk of perioperative complications. However, few studies have been conducted to determine the incidence and independent risk factors of DO in patients with thoracic OLF. The aim of this retrospective study was to determine the incidence and independent risk factors of DO in patients with thoracic OLF. METHODS: A total of 107 patients with thoracic OLF who were admitted to the authors' hospital from December 2020 to December 20

Transforaminal Full-Endoscopic Ventral Facetectomy: Midterm Results and Factors Associated with Poor Surgical Outcomes.

BACKGROUND: Full-endoscopic spine surgery (FESS) is a well-established procedure for herniated nucleus pulposus. It is a minimally invasive surgery that can be performed under local anesthesia through only an 8-mm skin incision. With improvements in surgical equipment such as high-speed drills, the indications for FESS have expanded to include lumbar spinal stenosis (LSS). We perform a transforaminal full-endoscopic ventral facetectomy (TF-FEVF) for unilateral nerve root-type lateral recess stenosis (LRS) using a transforaminal approach under local anesthesia. The aim of this study was to exami

Posterior vertebral column resection as a safe procedure leading to solid bone fusion in metastatic epidural spinal cord compression.

OBJECTIVE: Cancer is one of the leading causes of death and greatly decreases a patient's quality of life. Vertebral metastases often lead to epidural spinal cord compression (ESCC) requiring surgical therapy. It has previously been shown that in patients with metastatic ESCC (MESCC), a surgical intervention leads to an improved outcome. Although the treatment paradigms in spinal metastases have changed and separation surgery followed by stereotactic radiosurgery is considered the best strategy, there are still cases in which 360° decompression with stabilization is indicated. In these patient

Predicting cerebrospinal fluid leakage prior to posterior circumferential decompression for the ossification of the posterior longitudinal ligament in the thoracic spine.

BACKGROUND: Cerebrospinal fluid leakage (CSFL) is one of the most common complications after posterior transarticular osteotomy and circumferential decompression (CD) for the ossification of the posterior longitudinal ligament (OPLL) in the thoracic spine. Predicting CSFL preoperatively would be of great value to surgeons. The aim of this study was to discover the factors that can predict CSFL prior to CD and incorporate them into a predictive model. METHODS: Sixty-one patients with OPLL who underwent posterior transarticular osteotomy and CD at Peking University International Hospital were d

Three region spinal decompression in the same session: a case report.

BACKGROUND AND IMPORTANCE: Multiregional spinal stenosis [tandem spinal stenosis (TSS)] is not rare but operating on multiple regions at the same sitting is. Decompression of cervical and lumbar spine in the same session has a frequency of 5-25% all TSS cases, the most frequent one is TSS. Decompression in three different regions is so rare that there is only one case in the literature. We report the second. CLINICAL PRESENTATION: A 72-year-old man with pain in legs and arms, neurogenic claudication, progressive loss of balance, radiculopathy and myelopathy in lower and upper extremities whos

Obese people benefit from lumbar spinal stenosis surgery as much as people of normal weight.

BACKGROUND: Lumbar spinal stenosis (LSS) is a common degenerative condition of the spine that causes back pain radiating to the lower extremity. Surgical treatment is indicated to treat progressive radical symptoms. Obesity has been associated with inferior results in the domains of quality of life (QoL) following an LSS operation, but the research findings have been limited. This paper aims to identify whether obesity affects QoL due to back pain among patients who underwent an operation for LSS. METHODS: This study is based on a series of patients operated on for LSS between 2012 and 2018.

Treatment of posttraumatic syringomyelia: evidence from a systematic review.

BACKGROUND: Following spinal cord injury (SCI), the routine use of magnetic resonance imaging (MRI) resulted in an incremental diagnosis of posttraumatic syringomyelia (PTS). However, facing four decades of preferred surgical treatment of PTS, no clear consensus on the recommended treatment exists. We review the literature on PTS regarding therapeutic strategies, outcomes, and complications. METHODS: We performed a systematic bibliographic search on ("spinal cord injuries" [Mesh] AND "syringomyelia" [Mesh]).
