4 cases of acroangiodermatitis developing on paralytic feet are reported. Acroangiodermatitis might be an entity different from Mali's syndrome and Stewart-Bluefarb's syndrome. Paralysis could generate the dermatologic lesions by increasing venous stasis and enhancing arteriovenous channels.

INTRODUCTION: Nonattendance to dermatology appointments can disrupt continuity of care and may lead to worse outcomes among patients. Factors related to nonattendance among Hidradenitis suppurativa (HS) patients have not been adequately explored.
METHODS: This case-control study at an HS subspecialty clinic in the southeastern USA explores patient characteristics and encounter variables predictive of nonattendance using demographic, employment, health insurance, and distance to clinic data using both univariable and mixed-model multivariable logistical regression.
RESULTS: Return visits, Bla

Exosomes, owing to their potent capabilities for intercellular communication, have great potential in modern medicine. The pathogenesis of skin diseases is predominantly characterized by immune imbalance and impaired regenerative processes. Exosomes are rich in microRNAs, lipids, and proteins, which play a crucial role in immune regulation and regenerative medicine. Recent studies have highlighted the involvement of exosomes in the therapeutic management of dermatological disorders. This review provides a comprehensive overview of the potential therapeutic applications of exosomes in autoimmun

Colchicine, a natural alkaloid, has emerged as a promising anti-inflammatory therapy with applications in cardiovascular diseases, dermatological conditions, and COVID-19 management. Its mechanisms, including the modulation of neutrophil activity, the inhibition of the NLRP3 inflammasome, and the mitigation of cytokine storms, have expanded its therapeutic potential beyond traditional uses. This review synthesizes current evidence on colchicine's clinical applications and mechanisms of action. In cardiovascular medicine, colchicine has been shown to reduce risks of pericarditis, coronary arter

Ichthyosis, a group of genetic disorders of keratinization, often extends beyond the skin to affect mucosal surfaces, including the ocular surface. This case report of a 15-year-old male illustrates the critical interplay between dermatological conditions and ocular health. Initially treated for chronic eczema, the patient presented with recurrent ocular pain, photophobia, and corneal abnormalities including punctate epithelial erosions, punctate deep stromal opacities, and prominent corneal nerves. A skin biopsy revealed orthokeratosis and a normal granular layer, a key finding supporting a d

Although dermatological manifestations are common in hepatitis C virus (HCV) infection, nummular eczema is rarely reported in this context. We describe the case of a 73-year-old man with a seven-month history of pruritic lesions on the posterior thorax, unresponsive to symptomatic treatment. Physical and dermoscopic examination revealed well-demarcated, coin-shaped plaques with peripheral erythema and fine desquamation. Histopathology confirmed chronic inflammatory dermatitis. Laboratory tests showed elevated transaminases and high HCV viral load. The patient had a history of blood transfusion

The cutaneous microbiome represents a highly dynamic community of bacteria, fungi and viruses. Scientific evidence, particularly from the last two decades, has revealed that these organisms are far from being inconsequential microscopic hitchhikers on the human body, nor are they all opportunistic pathogens waiting for the chance to penetrate the skin barrier and cause infection. In this review, we will describe how dermatological diseases have been found to be associated with disruptions and imbalances in the skin microbiome and how this new evidence had shaped the diagnosis and clinical prac

The call for robust randomized clinical trials (RCTs) comparing Mohs micrographic surgery (MMS) with wide local excision for treatment of melanoma has stymied the development of guidelines for MMS despite growing evidence of benefit. This commentary explores the controversy by detailing opposing arguments, reviewing the relevant evidence supporting the use of MMS for early-stage melanoma, and discussing the role that RCTs may play in development of national guidelines for surgical treatment options for melanoma. Randomized clinical trials are considered the gold standard of clinical research,

Vulval lichen sclerosus (LS) is a chronic dermatological condition affecting the anogenital area, causing intense itching, pain and bleeding. It can change the terrain of the vulva, causing loss of vulval anatomy and altered texture and appearance of the skin. There has been little research into how women experience the materialities of a dermatological vulval disease. We aimed to understand experiences of living with LS, using a feminist lens to examine the influence of societal attitudes towards women's bodies and the vulva. We conducted qualitative interviews with 20 women with vulval LS, t

A number of chronic skin diseases, such as vitiligo and alopecia areata, are historically resistant to or respond poorly to treatment. Additionally, disorders such as atopic dermatitis and psoriasis have subtypes that are inadequately treated by current medications. Lastly, in the field of dermatology there are a number of conditions, some genetic (such as Darier's disease and Hailey-Hailey disease) and others caused by aberrant inflammatory responses (macrophage-driven conditions such as sarcoidosis and autoimmune conditions such as localized scleroderma) where effective treatments have been

Itch occurs in various dermatologic and systemic conditions. Many patients report that certain foods instigate itch, although there is limited published information in dermatology on food-induced pruritus. In addition, itch severity is rarely mentioned. Food can induce pruritus through either ingestion or direct contact with skin or mucosal membranes. The most common type of itch provoked by food is acute urticaria, often through the classical immunoglobulin E (IgE)-mediated pathway. Other mechanisms include non-IgE-mediated, mixed (IgE-mediated and non-IgE-mediated), T-cell-mediated, and noni

Dysautonomia refers to a group of autonomic nervous system disorders that affect nearly 70 million people worldwide. One subset of dysautonomia includes syndromes of orthostatic intolerance (OI), which primarily affect adolescents and women of childbearing age. Due to the variability in disease presentation, the average time from symptom onset to diagnosis of dysautonomia is 6 years. In general, there is a paucity of dermatological research articles describing patients with dysautonomia. The objective of this review is to summarize the existing literature on cutaneous manifestations in dysauto

INTRODUCTION: Pyogenic flexor tenosynovitis is an unusual complication of dyshidrotic eczema. The diagnosis has traditionally been made by Kanavel's signs. Point-of-care ultrasound can be a useful adjunct in the diagnosis of this surgical emergency.
CASE REPORT: We report the case of a 23-year-old male who presented with right middle finger pain and swelling and an overlying eczematous rash. The use of point-of-care ultrasound was performed to aid in the diagnosis of pyogenic flexor tenosynovitis. An incision and drainage was performed with deep wound cultures positive for Staphylococcus aure

The diagnosis of pyoderma gangrenosum (PG) is often difficult to establish based on a clinical presentation, which can mimic other dermatologic conditions. The formation of a mnemonic that incorporates the most prevalent clinical features of PG could aid in accuracy and speed of diagnosis. The 5 P's of PG: Painful, Progressive, Purple, Pretibial, Pathergy, and systemic associations, incorporate parameters recognizable on the first encounter with a patient with PG without reliance on histopathology and laboratory findings or treatment response. We postulate that this simple mnemonic will have t

Prurigo nodularis (PN) is a highly pruritic skin condition that is caused by chronic scratching. It occurs in patients with chronic itch and is characterized by multiple hyperkeratotic papules and nodules. The pathogenesis of PN is unclear, but involves a complex interplay of numerous pathways including neurogenic and inflammatory factors. As such, PN is very difficult to treat and patients are often refractory to multiple medications before finding a treatment that is effective. We present a woman with a 20-year history of exuberant prurigo nodularis who failed multiple therapies, including d

Neurofibromatosis 1 has various complications. To elucidate the frequency of neurofibromatosis 1-related major complications requiring medical intervention, a nationwide retrospective study was conducted of 3,530 patients with neurofibromatosis 1 registered from 2001 to 2014 in Japan. The ratio of certified patients requiring medical intervention (>stage 3) was 82%. Patients classified in the most severe grade experienced dermatological complications (71.8% of patients), neurological complications (38.1%) and bone complications (33.3%). In patients with dermatological manifestations, medical t

Studies have proven the relationship between cutaneous vasculature abnormalities and dermatological disorders, but to image vasculature noninvasively <italic<in vivo</italic<, advanced optical imaging techniques are required. In this study, we imaged a palm of a healthy volunteer and three subjects with cutaneous abnormalities with photoacoustic tomography (PAT) and optical coherence tomography with angiography extension (OCTA). Capillaries in the papillary dermis that are too small to be discerned with PAT are visualized with OCTA. From our results, we speculate that the PA signal from the pa

Vitamin D influences allergen-induced pathways in the innate and adaptive immune system, and its potential immunomodulatory role in allergic skin disorders has been explored. This comprehensive review article provides an overview of the role of vitamin D in three common dermatologic conditions: atopic dermatitis (AD), chronic urticaria, and allergic contact dermatitis (ACD). Whereas the literature regarding vitamin D and AD has resulted in mixed findings, several studies have described an inverse relationship between vitamin D levels and AD severity, and improvement in AD with vitamin D supple

BACKGROUND/AIMS: Prurigo nodularis (PN) has multiple reported disease associations, and this study analyzes the demographics, clinical distribution and underlying etiologies in an Asian cohort.
METHODS: A 1-year retrospective study was carried out on PN patients with extensive lesions (≥10) who attended a specialized itch clinic.
RESULTS: 37 patients (46% male, median age 53 years) were included. In all patients, an underlying etiologic factor associated with pruritus could be identified. 49% (n = 18) of patients had a single attributable etiology, whilst 51% (n = 19) had multiple etiologies
