Transient electroencephalographic suppression with initiation of cardiopulmonary bypass in pigs: blood versus nonblood priming.

Electroencephalographic (EEG) changes have been reported with cardiopulmonary bypass (CPB). We tested whether the type of priming solution (blood versus nonblood) affected the EEG. Twenty-six anesthetized pigs (29.5+/-1.6 kg) were cannulated for CPB primed with 1 liter plasmalyte and 500 ml 6% hetastarch (nonblood prime). EEG signals were recorded during the initiation of normothermic CPB. Three minutes later, animals were weaned from CPB and allowed to stabilize. CPB was reinstituted using the animals' hemodiluted blood as prime. We found that with nonblood prime, abrupt and marked EEG suppre

Rett syndrome, EEG and the motor cortex as a model for better understanding of attention deficit hyperactivity disorder (ADHD).

EEG data in Rett syndrome (RS) are quite often characterized by spikes over the central region (especially in sleep). Furthermore, there has been a recent observation of a prominent and peculiar theta (4-6/s) rhythm over the central region and vicinity. In view of the clinical phenomena of RS, it is pointed out that clinical and electrical findings are based upon hyperexcitability of the motor cortex (or a state of motor cortex dyscontrol). States of dyscontrol (disinhibition) are caused by deficits in higher cortical functions. It is reasonable to assume a primary dysfunction for the frontal

Concomitance of childhood absence and Rolandic epilepsy.

Five children (3 girls and 2 boys) who showed generalized synchronous 3/sec spike and wave complexes as well as centrotemporal spikes in the same EEG or in different EEGs are described. Among these five patients only 1 boy and 1 girl showed clinically both absence seizures and partial motor seizures with or without secondary generalization. One girl and 1 boy have only absences and the other girl only partial motor seizures. A concomitance of generalized synchronous 3/sec spike and wave discharges and centrotemporal spikes in the same patient is rarely found. The clinical manifestation of abse

[Effects of the exercise intensities standardized by anaerobic threshold on EEG and heart rate variability].

The purpose of this study was to examine the effects of different exercise intensities with the anaerobic threshold (AT) as the standard on electroencephalograph (EEG) and heart rate variability (HRV). Eleven healthy males, with a mean age of 22 (SD 1.48) years, performed submaximal exercise to determine their ATs, and underwent four experimental conditions including rest (rest), 20% less than the AT level (-20), the AT level (AT), and 20% more than the AT level (+20) for about 20 minutes. EEG and electrocardiogram (ECG) were taken for 15 minutes before and after each experimental condition, r

Neuromonitoring in the operating room: why, when, and how to monitor?

This review considers the main principles and indications of EEG and evoked potential (EP) neuromonitoring in the operating room. Neuromonitoring has a threefold purpose: to warn the surgeon that he has to adjust his strategy, to confirm his decision, and to help him improve subsequent procedures. The pathophysiology of intraoperative events liable to alter the EEG or the EPs is first considered. The usefulness of neuromonitoring in preventing neurological complication relies on its ability to detect neurological dysfunction at a reversible stage. This applies especially to ischemia and compre

Interhemispheric EEG coherence in never-medicated patients with paranoid schizophrenia: analysis at rest and during photic stimulation.

We assessed functional relationships between hemispheres by calculating interhemispheric EEG coherence at rest and during photic stimulation in 18 never-medicated patients with paranoid schizophrenia and 30 control subjects. Although no significant group differences were found in the resting EEG, the schizophrenic patients had significantly higher coherence on EEGs recorded during photic stimulation, compared to the control subjects. In this study, we also examined the changes in interhemispheric coherence from rest to the stimulus condition (i.e., stimulation-related coherence reactivity); th

On the intercorrelation of some frequency and amplitude parameters of the human EEG and its functional significance. Communication II: neurodynamic imbalance in endogenous asthenic-like disorders.

In 67 borderline psychiatric patients suffering from schizotypal/slowly developing schizophrenic disorders and 18 patients suffering from cyclothymia, the factor structure of the period (interval-amplitude) parameters of the EEG proved to be similar to that obtained in normal subjects during mental activity and reported in part I (Lazarev, Int. J. Psychophysiol., 28 (1998) 77-98). However, 51 patients with schizotypal disorders with a predominance of asthenic-like symptomatology, characterized by mild thought disorders with difficulty in focusing attention, were distinguished from normal subje

[Landau-Kleffner syndrome (acquired aphasia with epilepsy). Etiopathology and response to treatment with anticonvulsants].

The Landau-Kleffner syndrome consists in the association of an aphasia acquired during childhood or adolescence after a period of normal development, accompanied by epileptic fits and, sometimes, psychological disturbances. The appearance of the symptoms may not be simultaneous. The nature of this syndrome has been widely discussed and even the possibility of being a non-unitary syndrome has been considered. It may be possibly due to diverse etiologies, genetic or acquired (infectious). The relation between the aphasic, the psychologic and the convulsive symptoms has also raised controversies.

Non-convulsive status epilepticus induced by antidepressants.

We report on a 27-year-old woman of non-convulsive status epilepticus (NSE) very probably induced by antidepressants. She has no history of febrile convulsions, or epileptic seizures. There are no genetic factors for epilepsy in her family. As she developed depressive illness, she consulted a doctor. Clomipramine 25 mg i.v. drip/day and maprotiline 75 mg/os/day were started. Recently she began to doze, so an EEG was performed. Continuous generalized 2-3 Hz spike and wave complexes were observed. Clomipramine and maprotiline were stopped the same day, and the patient's drowsiness gradually impr

Effect of subchronic mercury exposure on electrocorticogram of rats.

Mercury is a neurotoxic compound causing irreversible disorders of the central and peripheral nervous system. In some of the previous human and experimental studies mercury also affected some functional neurological parameters such as EEG, and cortical evoked potentials. In the present study, the effect of subchronic (4, 8, and 12 weeks) relatively low-level (0.4, 0.8, and 1.6 mg/kg mercury in form of HgCl2, per os by gavage) treatment on the basic cortical activity was investigated. Certain parameters of electrocorticogram (ECoG) recorded simultaneously from the primary somatosensory, visual

[Reflex epilepsy with seizures induced by mental calculation, playing chess and scrabble].

Reflex seizures induced by higher mental activity is rare. We report the case of a young man with myoclonic jerks and generalized tonico-clonic convulsion precipitated by calculation, playing chess and scrabble. Routine EEG, including hyperventilation and photic stimulation, showed no abnormality. Tests stimulation procedures, including spatial tasks, induced focal and generalized EEG spike-wave complex and myoclonic jerks. Valproate was effective in reducing epileptic seizures during a follow-up period of three years. Comparison of our case with previously reported reflex epilepsy with seizur

Negative pressure-induced deformation of the upper airway causes central apnea in awake and sleeping dogs.

We investigated the effects of negative pressure (NP) in the isolated upper airway (UA) in three unanesthetized dogs. The UA was isolated, and the dogs breathed through an endotracheal tube while wearing a fitted fiberglass snout mask. NP (-2 to -32 cmH2O) was applied in a square wave below the larynx or at the snout at end expiration and was held until inspiratory effort during wakefulness, non-rapid-eye-movement (NREM) sleep, and rapid-eye-movement (REM) sleep. During all states of consciousness, NP applied to the UA prolonged expiratory time (TE) 1) below a threshold of -8 to -10 cmH2O, whi

Pathophysiology of sensorimotor cortex in cortical myoclonus.

We have studied greatly enhanced cortical responses to somatosensory stimuli [giant somatosensory evoked potential (SEP) or magnetic field (SEF)] and cortical activities preceding the myoclonus (premyoclonus spike) in patients with cortical myoclonus, using magnetoencephalographic (MEG) techniques. A P1m component of giant SEF was estimated as a dipole positioned on the postcentral gyrus in all giant SEFs. This indicates that abnormally enhanced responses to sensory stimuli originate from the sensory cortex. In most of the patients with cortical reflex myoclonus, a dipole for the premyoclonus

Verbal auditory agnosia with focal EEG abnormality: an unusual case of a child presenting to an ENT surgeon with "deafness".

Verbal auditory agnosia implies the failure to recognize sounds in a patient who is nevertheless not deaf. We present a child referred to the ENT outpatients for suspected hearing loss investigation. At one point she had grommets inserted on the basis of a flat tympanogram but with no effect. All hearing tests, including electrocochleography and distraction tests, revealed normal hearing thresholds. At the age of three years and three months, and on direct questioning of the parents, we discovered that the child responded remarkably well to music. Paediatric neurologists made the diagnosis of

Clinical screening for narcolepsy/cataplexy in patients with fibromyalgia.

We report two cases of fibromyalgia occurring during the course of a long-standing narcolepsy/cataplexy. This association is probably not fortuitous and we propose an easy clinical procedure to search for narcolepsy/cataplexy in these patients. Narcolepsy/cataplexy is a treatable condition requiring drugs that in turn may improve in the long-term symptoms of fibromyalgia.
