Hip adductor muscle function in forward skating.

Adductor strain injuries are prevalent in ice hockey. It has long been speculated that adductor muscular strains may be caused by repeated eccentric contractions which decelerate the leg during a stride. The purpose of this study was to investigate the relationship of skating speed with muscle activity and lower limb kinematics, with a particular focus on the role of the hip adductors. Seven collegiate ice hockey players consented to participate. Surface electromyography (EMG) and kinematics of the lower extremities were measured at three skating velocities 3.33 m/s (slow), 5.00 m/s (medium) a

An extreme lateral lumbar disc herniation mimicking L4 schwannoma.

OBJECTIVE: We herein describe a case with an extraspinal mass distorting the right L4 dorsal root ganglion. Initially presumed to be a nerve root schwannoma, the lesion turned out to be a free disc fragment. METHODS AND RESULTS: A 46-year-old woman presented with a history of mild low back and intense right leg pain. The leg pain was like electrical discharges. Right knee extension was weak together with hyperalgesia and loss of heat sensation in the right LA dermatome. The right patellar reflex was absent. Electromyography showed acute and chronic denervation of muscles innervated by the rig

Reduced neorectal capacitance is a more important factor for impaired defecatory function after rectal resection than the anal sphincter pressure.

OBJECTIVE: The role of the diverse anorectal diagnostic tools like manometry and determination of the preception threshold and the maximal tolerable volume is still a matter of debate. Currently, there is a scarcity of physiological data in the long-term follow-up of patients who underwent sphincter-preserving rectal resection. The aim of this study was therefore to perform these anorectal physiological measurements and to correlate the determined parameters with a faecal incontinence score. METHOD: In 45 patients, anorectal manometry, electromyography (EMG) and neorectal volume measurements

A new anchor electrode design for continuous neuromonitoring of the recurrent laryngeal nerve by vagal nerve stimulations.

PURPOSE: Intraoperative neuromonitoring has the limitation that the recurrent laryngeal nerve (RLN) is still at risk for damage between two stimulations with a handheld bipolar stimulation electrode. The purpose of this study was to establish the vagal anchor electrode for real-time monitoring of the RLN in surgical routine and to be alerted to imminent nerve failure by electromyography (EMG) signal analysis whereby the nerve damage becomes reversible. PATIENTS AND METHODS: This fully implantable electrode has been used in addition to a conventional handheld bipolar stimulation electrode duri

Effect of platelet-rich plasma on peripheral nerve regeneration.

Activated platelets release various growth factors, some of which are recognized to improve nerve regeneration. This study evaluated the effect of platelet-rich plasma (PRP) in end-to-end neurorrhaphy. A total of 45 Wistar rats were used, with the initial five used for PRP preparation. The right hind limbs were used as experimental, with the left as control. The animals were treated in five groups. Group A (n = 4): The right sciatic nerve was dissected only from the sciatic notch to the bifurcation. In all other groups, the nerve was sharply transected and repaired with: group B (n = 8): two s

Surgical anatomy of the external branch of the superior laryngeal nerve and its clinical significance in head and neck surgery.

Injury of the external branch of the superior laryngeal nerve (EBSLN) increases the morbidity following a variety of neck procedures and can have catastrophic consequences in people who use their voice professionally. Identification and preservation of the EBSLN are thus important in thyroidectomy, parathyroidectomy, carotid endarterectomy, and anterior cervical spine procedures, where the nerve is at risk. There are large variations in the anatomical course of the EBSLN, which makes the intraoperative identification of the nerve challenging. The topographic relationship of the EBSLN to the su

Immediate effects of a rucksack type orthosis on the elderly with decreased lumbar lordosis during standing and walking.

The spinal orthosis, the so-called rucksack type orthosis (RO), has been used to relieve low back pain and fatigue during prolonged standing and walking for the elderly with spinal deformities. However, little is known about the RO's kinematical effects. Twenty-three elderly (78.9 +/- 6.9 years old) participated in experiment 1, and 13 elderly (78.4 +/- 7.9 years old) in experiment 2. They had decreased lumbar lordosis or lumbar kyphosis. In experiment 1, using the "Spinal Mouse", which can measure spinal curvature, the effects of the RO on posture during standing were investigated. In experim

[A forty-five-years-old woman suffered from Sjögren syndrome with progressive tetraparesis].

We herein report the finding of a 45-year-old woman suffered from Sjögren syndrome with progressive tetraparesis, who later developed systemic muscle atrophy and respiratory failure with a one-year clinical history. Neurological examinations revealed progressive tetraparesis with absent deep tendon reflexes, whereas no upper motor neuron signs were observed. The motor and sensory nerve conduction velocity and sensory nerve action potential (SNAP) were both completely normal, but the prolongation of distal motor latency in the median nerve and a decrease in the compound muscle action potential

Line scan diffusion spectrum of the denervated rat skeletal muscle.

PURPOSE: To detect the earlier changes of the skeletal muscle of rats after peripheral nerve injury by measuring the apparent diffusion coefficient (ADC) on diffusion MR spectroscopy. MATERIALS AND METHODS: The posterior tibial nerve was transected in six rats (nerve transection group) and was only dissected in six rats (control group). At one, three, five, seven, 14, and 28 days after the surgery, both the T2 value and the ADC of gastrocnemius muscle were measured using a line-scan diffusion spectrum on a 1.5T clinical MR imager on both groups. RESULTS: In the nerve transection group, the T

The spinal accessory nerve in head and neck surgery.

PURPOSE OF REVIEW: To describe landmarks and tips used for minimizing surgical traumas to the spinal accessory nerve, and different options in case of its injury. RECENT FINDINGS: Modified radical and selective neck dissections reduce the prevalence of shoulder syndrome, a sequela of radical neck dissection. Impaired shoulder mobility and pain may be present even after nerve-sparing procedures, as shown using electromyography, particularly when dissection is extended to level V. In these cases physical therapy is mandatory to prevent shoulder pain and functional limitations. The issue of spin

Bowel dysfunction in patients with cauda equina lesions.

Despite their serious sequels on bowel function, lesions of the cauda equina have not been previously systematically studied in larger patient populations. This was the aim of the present report. From the registrars of a diagnostic and rehabilitation centres 67 patients with clinical, electrodiagnostic and radiological findings supportive of the cauda equina lesions were recruited. The Slovene versions of the standard questionnaires for anal incontinence and constipation were used. The responses were scored, impairments categorized and previous treatments noted. [Clinical consequences of muscle dysfunction in chronic obstructive pulmonary disease].

The function of respiratory muscles, and mainly inspiratory muscles, is impaired in COPD patients. Most of these impairments are essentially due to pulmonary hyperinflation that puts these muscles in a disadvantageous situation. The main consequence of this dysfunction is respiratory muscle fatigue that may cause shortness of breath, exertion intolerance, and hypoventilation with onset of hypercapnic respiratory failure. This function may be measured at the pulmonary function laboratory by means of unspecific (spirometry, pulmonary volumes) or specific tests (maxim respiratory pressures [MIP -

Neurophysiologic studies in early-onset cerebellar ataxia.

The discovery of the gene for Friedreich's ataxia (FRDA) has not only broadened the FRDA phenotype, but has also identified patients with early-onset cerebellar ataxia who resemble FRDA clinically but who do not carry a mutation in the frataxin gene. In order to identify subgroups that may represent a uniform underlying disorder, we performed neurophysiologic studies, including nerve conduction studies, electromyography, and transcranial magnetic stimulation, in 15 patients with a slowly progressive, unexplained, early-onset cerebellar ataxia (EOCA). In addition, sural nerve biopsy data were a
