Electronic surveillance criteria for non-ventilator-associated hospital-acquired pneumonia: Assessment of reliability and validity.

OBJECTIVE: Surveillance of non-ventilator-associated hospital-acquired pneumonia (NV-HAP) is complicated by subjectivity and variability in diagnosing pneumonia. We compared a fully automatable surveillance definition using routine electronic health record data to manual determinations of NV-HAP according to surveillance criteria and clinical diagnoses. METHODS: We retrospectively applied an electronic surveillance definition for NV-HAP to all adults admitted to Veterans' Affairs (VA) hospitals from January 1, 2015, to November 30, 2020. We randomly selected 250 hospitalizations meeting NV-HA

Blockchain for Digital Health Governance: Evidence Gap Map and Scoping Review.

BACKGROUND: Digital health increasingly depends on data exchange across institutions, technologies, and jurisdictions, creating persistent challenges for the governance of access, consent, interoperability, provenance, and accountability. Blockchain and distributed ledger technology (DLT) systems have been proposed as mechanisms for coordinating and verifying governance processes across distributed actors. However, existing research has examined individual applications or technical domains, leaving unclear how blockchain/DLT systems function as governance infrastructure across health care, and

Comparison of the Prevalence of Ruptured and Unruptured Cerebral Aneurysms in a Poor Urban Minority Population.

BACKGROUND: Most ruptured cerebral aneurysms are small (<7 mm). Evidence suggests low rupture rates for such lesions (<1% per year). Population studies demonstrate a prevalence rate of 3.2%. This study simultaneously estimates the prevalence of aneurysms in a single geographic population while reporting the observed rate of aneurysmal subarachnoid hemorrhage (aSAH) in the same geographic region composed of a poor urban minority demographic. METHODS: This is an institutional review board-approved, Health Insurance Portability and Accountability Act of 1996-compliant retrospective study perform

Automated identification of an aspirin-exacerbated respiratory disease cohort.

BACKGROUND: Aspirin-exacerbated respiratory disease (AERD) is characterized by 3 clinical features: asthma, nasal polyposis, and respiratory reactions to cyclooxygenase-1 inhibitors (nonsteroidal anti-inflammatory drugs). Electronic health records (EHRs) contain information on each feature of this triad. OBJECTIVE: We sought to determine whether an informatics algorithm applied to the EHR could electronically identify patients with AERD. METHODS: We developed an informatics algorithm to search the EHRs of patients aged 18 years and older from the Partners Healthcare system over a 10-year per

Improving timing and sequencing of postoperative radiation therapy in head and neck cancer.

BACKGROUND: The Commission on Cancer recognizes the importance of timing and sequencing care to improve the quality of health care delivery and cancer outcomes. In 2021, the Commission on Cancer established a quality metric for head and neck cancer to achieve postoperative radiation therapy within 6 weeks of surgery. The 4R Oncology Model is a teams-based framework that accounts for interdependency of care and allows for sustainable multilevel quality improvement in multiple other cancer types. We demonstrate the development of a 4R quality improvement framework to achieve this new Commission

Surgeon Attitudes to Guideline-Concordant Extended Pharmacologic Venous Thromboembolism Prophylaxis after Cancer Surgery Within a Regional Health System: A Qualitative Study.

BACKGROUND: While infrequent, venous thromboembolism (VTE) is a significant cause of morbidity and mortality after cancer surgery. Extended pharmacologic VTE prophylaxis (ePPX) decreases VTE risk and is recommended by professional societies. Observational studies have shown limited ePPX utilization, although reasons for non-adherence have not been forthcoming from existing data. OBJECTIVE: The aim of this study was to obtain insight toward surgeon practices and attitudes regarding ePPX within a regional health system. METHODS: Semi-structured interviews were conducted with 13 surgeons and 2

Enhancing hemotherapy safety through pertinent diagnostic testing and electronic "provisional diagnosis" reporting: expanding roles for blood bank technologists in biovigilance and patient safety.

BACKGROUND: Suspected transfusion reaction (STR) investigations are foundational for biovigilance. Diagnostic evaluations performed by blood banks may prolong turnaround times (TATs) for final STR results reporting. We identified a quality improvement opportunity using diagnostic testing reflex algorithms and our hospital's patient electronic health record to enhance TATs regarding one aspect of STR results reporting. STUDY DESIGN AND METHODS: We conducted a descriptive quality improvement study of reported STR cases investigated by our hospital's blood bank from March 1, 2014, to December 31

IMPORTANCE: Timely outpatient treatment for COVID-19 reduces the risk of hospitalization and death in high-risk patients. However, large racial and ethnic inequities in treatment have been observed across the US. The relative contributions of care mechanisms that create barriers (eg, access to home tests or telemedicine) are unknown. OBJECTIVE: To examine the relative contribution of structural and encounter-level factors associated with racial and ethnic disparities in oral antiviral prescribing for COVID-19. DESIGN, SETTING, AND PARTICIPANTS: In this cross-sectional study using electronic

Optimal Data Collection in Lifestyle Medicine at the Point of Care: A Position Statement From the American College of Lifestyle Medicine.

The full potential of lifestyle medicine has yet to be realized within the healthcare system, and it is hindered by the lack of systematic data collection focused on modifiable lifestyle factors that influence chronic disease. It is the position of the American College of Lifestyle Medicine (ACLM) that (1) current data collection practices at point of care are typically inconsistent across practice settings, electronic health record (EHR) environments, and specialties, limiting usability of data collected; (2) optimal data collection across all settings will demonstrate the effectiveness of li

Generalized anxiety disorder screening scores are associated with greater treatment need among Veterans with depression.

Comorbid anxiety and depression predict a poorer prognosis than either disorder occurring alone. It is unclear whether self-reported anxiety symptom scores identify patients with depression in need of more intensive mental health services. This study evaluated how anxiety symptoms predicted treatment receipt and outcomes among patients with new depression diagnoses in the Veterans Health Administration (VHA). Electronic medical record data from 128,917 VHA patients (71.6% assessed for anxiety, n = 92,237) with new diagnoses of depression were analyzed to examine how Generalized Anxiety Disorde
