Characteristics of diplopia as the presenting symptom to an ocular emergency room.

The medical records of 350 patients (median age, 59 years) who presented emergently at Bascom Palmer Eye Institute from 2014 to 2019 for evaluation of diplopia were reviewed retrospectively. Diplopia was binocular in 305 cases (median duration, 5 days). Strabismus was present in 301 patients (horizontal in 49.2% of cases). The most common cause of binocular diplopia was cranial nerve palsy: of the 208 cases, 134 (64%) were microvascular. Nearly half of the neuroimaging scans obtained (102/205) were abnormal. Ten patients with abnormal scans required urgent neurosurgical evaluation.

The emerging role of neuroimmune interactions in atopic dermatitis and itch.

Millions of people globally suffer from allergic diseases, and the cases have been rising in the past decades. One of the major manifestations of allergic diseases is itch, which is an unpleasant symptom that triggers the urge to scratch and greatly affects the quality of life. Thus, research on how sensation of itch is detected/transmitted from the contact of the allergen to the nervous system is crucial in mitigating itch. Recent studies have attempted to elucidate the mechanisms of itch in allergic diseases. Here, we aim to review the endogenous mediators released from immune/nonimmune skin

The emerging role of proteases in α1-antitrypsin deficiency and beyond.

α1-Antitrypsin deficiency (AATD) has been historically under-recognised and under-diagnosed; recently it has begun to receive greater interest in terms of attempts at deeper elucidation of pathology and treatment options. However, the concept of disease phenotypes within AATD (emphysema, chronic bronchitis, bronchiectasis or a combination of phenotypes) has not been proposed or studied. Of the three neutrophil serine proteases, neutrophil elastase was historically believed to be the sole contributor to disease pathology in AATD. Recently, Proteinase-3 has been increasingly studied as an equal,

Emerging Human Babesiosis with "Ground Zero" in North America.

The first case of human babesiosis was reported in the literature in 1957. The clinical disease has sporadically occurred as rare case reports in North America and Europe in the subsequent decades. Since the new millennium, especially in the last decade, many more cases have apparently appeared not only in these regions but also in Asia, South America, and Africa. More than 20,000 cases of human babesiosis have been reported in North America alone. In several cross-sectional surveys, exposure to Babesia spp. has been demonstrated within urban and rural human populations with clinical babesiosi

Emergence of Morganellaceae Harboring blaIMP-27 Metalloenzyme in Canada.

In 2018 to 2019, PCR for carbapenemases in routine Gram-negative isolates submitted to the National Microbiology Laboratory revealed an increase in IMP-type metalloenzyme-positive isolates, mostly among Morganellaceae Whole-genome sequencing revealed that 23 Morganellaceae harbored blaIMP-27 within a chromosomal Tn7 element. Phylogenomics indicated diversity of isolates but also the presence of a few clonal isolates dispersed geographically. These isolates may be difficult to detect due to carbapenem susceptibility and false-negative results in phenotypic testing.IMPORTANCE Over the last decad

Emerging Biomarkers and Therapeutic Strategies for Refractory Bullous Pemphigoid.

Bullous pemphigoid (BP) is the most common autoimmune subepidermal blistering disorder in the elderly. Systemic and topical use of glucocorticoids and immunosuppressants has been shown to be effective in most patients. However, refractory BP patients are challenged to clinicians with severe clinical symptoms, resistance to treatment, and high relapse rate. How to predict and assess the refractory and severity of bullous pemphigoid is the key issue in clinical practice, and the urgent need for precision medicine in refractory patients is driving the search for biomarkers and biologics. Recently

The utility of laboratory testing for suspected hyperthyroidism in the emergency department.

BACKGROUND: Given signs and symptoms of hyperthyroidism are non-specific, thyroid studies may be completed to evaluate patients with vague complaints such as tachycardia in the emergency department (ED). We sought to determine how often a new diagnosis of hyperthyroidism was made in patients who underwent thyroid laboratory testing in the ED. OBJECTIVES: The primary outcome of this study was a new diagnosis in the ED, or in the following 30 days, of hyperthyroidism or thyrotoxicosis following ED thyroid laboratory testing. METHODS: This was a retrospective chart review study in a single ED o

The emergence of the brain non-CpG methylation system in vertebrates.

Mammalian brains feature exceptionally high levels of non-CpG DNA methylation alongside the canonical form of CpG methylation. Non-CpG methylation plays a critical regulatory role in cognitive function, which is mediated by the binding of MeCP2, the transcriptional regulator that when mutated causes Rett syndrome. However, it is unclear whether the non-CpG neural methylation system is restricted to mammalian species with complex cognitive abilities or has deeper evolutionary origins. To test this, we investigated brain DNA methylation across 12 distantly related animal lineages, revealing that

COVID-19-Associated Pneumomediastinum: An Emerging Clinical Presentation.

Recent publications have suggested an association between coronavirus disease 2019 (COVID-19) pneumonitis and pneumomediastinum. The association has been linked to the frequent use of mechanical ventilation in these patients; however, there have also been increasing reports of spontaneous pneumomediastinum in the absence of mechanical ventilation. These reports suggest a direct association between COVID-19 pneumonitis and increased alveolar fragility. In this report, we present a case of a spontaneous mediastinum in a 64-year-old male patient with COVID-19 without any history of mechanical ven

Hepatitis E Virus: An emerging enigmatic and underestimated pathogen.

Hepatitis E virus (HEV) is an RNA virus causing hepatitis E disease. The virus is of one serotype but has diverse genotypes infecting both humans and animals. Based on evidence from seroprevalence studies, about 2 billion people are estimated to have been infected with HEV globally. HEV, therefore, poses a significant public health and economic challenge worldwide. HEV was discovered in the 1980s and was traced back to the 1955 - 1956 outbreak of hepatitis that occurred in India. Subsequently, several HEV epidemics involving thousands of individuals have occurred nearly annually in different c

High-Risk Medicolegal Conditions in Pediatric Emergency Medicine.

The top 5 reasons for pediatric malpractice are cardiac or cardiorespiratory arrest, appendicitis, disorder of male genital organs, encephalopathy, and meningitis. Malpractice is most likely to result from an "error in diagnosis." Claims involving a "major permanent injury" were more likely to pay out money, but of all claims, only 30% result in a monetary pay out. Consideration of "high-risk misses" may help to direct a history, examination, testing, and discharge instructions.

Gender differences between adolescents with autism in emergency psychiatry.

Among adolescents seen for psychiatric emergency consultation, the percentage of adolescents with autism is increasing over the years. This applies even more to girls than to boys. We collected data of 1378 adolescents aged 12-18 years who were seen for urgent consultation by mobile psychiatric emergency services in the Netherlands. Among these, there were 64 autistic girls and 125 autistic boys. We wanted to know more about differences in problems between autistic and typical developing adolescents in crisis, both to prevent crisis and to improve services. The percentage of adolescents with a

The Effect of Homelessness on Patient Wait Times in the Emergency Department.

BACKGROUND: Prolonged emergency department (ED) wait times could potentially lead to increased morbidity and mortality. While previous work has demonstrated disparities in wait times associated with race, information about the relationship between experiencing homelessness and ED wait times is lacking. OBJECTIVES: The purpose of this study was to explore the relationship between residence status (undomiciled vs. domiciled) and ED wait times. We hypothesized that being undomiciled would be associated with longer wait times. METHODS: We obtained data from the National Hospital Ambulatory Medic

Emerging role of air pollution in chronic kidney disease.

Chronic kidney disease (CKD), a global disease burden related to high rates of incidence and mortality, manifests as progressive and irretrievable nephron loss and decreased kidney regeneration capacity. Emerging studies have suggested that exposure to air pollution is closely relevant to increased risk of CKD, CKD progression and end-stage kidney disease (ESKD). Inhaled airborne particles may cause vascular injury, intraglomerular hypertension, or glomerulosclerosis through non-hemodynamic and hemodynamic factors with multiple complex interactions. The mechanisms linking air pollutants exposu

Neglected No More: Emerging Cellular Therapies in Traumatic Injury.

Traumatic injuries are a leading cause of death and disability in both military and civilian populations. Given the complexity and diversity of traumatic injuries, novel and individualized treatment strategies are required to optimize outcomes. Cellular therapies have potential benefit for the treatment of acute or chronic injuries, and various cell-based pharmaceuticals are currently being tested in preclinical studies or in clinical trials. Cellular therapeutics may have the ability to complement existing therapies, especially in restoring organ function lost due to tissue disruption, prolon

Out of tragedy can emerge the finest human qualities.

In March 2019 the Pacific Association of Pediatric Surgeons held its annual conference in Christchurch, New Zealand, a normally peaceful city that was in the process of rebuilding following the devastating earthquakes of 2010 and 2011. Then the day after the conference concluded a horrendous atrocity was committed in two nearby mosques which presented the hospital with 25 years of major gunshot injuries in just one hour. The remarkable response of the hospital and its staff, the victims and the broader community are outlined. Although the surgical response was impressive, lessons could still b

Emerging B-Cell Therapies in Systemic Lupus Erythematosus.

Systemic lupus erythematosus (SLE) is a chronic, multisystem, autoimmune disease of unknown etiology, whose hallmark is the production of autoantibodies. B cells are promising targets for novel SLE therapies. In 2011, belimumab (Benlysta®), a fully humanized monoclonal antibody inhibiting B-cell activation and proliferation, was the first medication in 50 years to be approved by the US Food and Drug Administration to treat adult SLE. This review discusses the current experience with B-cell-targeted therapies, including those targeting B-cell-surface antigens (rituximab, ocrelizumab, ofatumumab
