Pediatric Emergency Department Care for Septic Shock in Sub-Saharan Africa: Single-Center Outcomes Before-Vs.-After Implementation of a Telesimulation Program, 2023-2024.

OBJECTIVES: Sepsis is a leading cause of preventable death in low-resource settings, where delays in recognition and emergency department (ED) treatment are common. Limited access to training also contributes to poor outcomes. We hypothesized that a contextualized telesimulation and debriefing program would be associated with better sepsis-related outcomes and time-critical care processes in children presenting to our center in Kumasi, Ghana. We also determined the program's acceptability and feasibility in our clinical providers. DESIGN: We conducted a 12-month mixed-method quasi-experimenta

Resuscitation following emergency room clamshell thoracotomy and cardiac laceration repair.

The resuscitative thoracotomy (RT) is a time-sensitive procedure performed on trauma patients with impending fatality, particularly those with penetrating thoracic injuries. We present a case of a male in his twenties with pulseless electrical activity following a gunshot wound (GSW) to the right chest. This patient experienced rapid haemodynamic deterioration in concurrence with signs of cardiac tamponade, leading to the performance of a clamshell thoracotomy, followed by a pericardiotomy, and subsequent myocardial repair. Direct haemorrhage control, cardiac massage and rapid transfusion achi

Reducing antibiotic overuse in immunocompetent children with febrile neutropenia in a pediatric emergency department: a quality improvement initiative.

INTRODUCTION: Management of febrile neutropenia in previously healthy, presumed immunocompetent children varies. Unnecessary treatment impacts the patients, families, and the healthcare system. With guidance from a Canadian Paediatric Society Practice Point, most well-appearing children with a first episode of febrile neutropenia can be managed with reduced exposure to antibiotics and close outpatient follow-up. The aim of this initiative was to safely reduce antibiotic use in this low-risk population presenting to the emergency department (ED). METHODS: A multidisciplinary team designed a qu

VExUS-ED: Early venous congestion is associated with clinical fluid overload in patients with sepsis.

BACKGROUND: Initial fluid resuscitation is central to sepsis management, yet excessive fluids may exceed a patient's fluid tolerance, increasing the risk of fluid overload and organ dysfunction. The Venous Excess Ultrasound (VExUS) score allows noninvasive bedside assessment of venous congestion, but its prevalence and predictive value in the emergency department (ED) are unclear. METHODS: We analyzed prospectively collected data from the Acutelines data-biobank (University Medical Center Groningen, Netherlands). Adults with suspected sepsis requiring initial fluid resuscitation underwent VEx

Viral hepatitis detection through emergency hospital departments in a Spanish region with high immigration: implications for elimination strategies.

BACKGROUND: Chronic infection with hepatitis B virus (HBV) and hepatitis C virus (HCV) remains a major global public health problem, as many infected individuals remain undiagnosed. Emergency departments (ED) represent a strategic setting for opportunistic screening. This study aimed to evaluate implementation strategies of opportunistic screening in ED within the Girona Health Region in Spain. METHODS: A prospective observational study was conducted in the ED of the Hospital Universitari de Girona Dr. Josep Trueta (HUGJT) and the Hospital Santa Caterina (HSC) between October 2023 and January

Bacteremia evaluation with T2 magnetic resonance in the emergency room (BETTER).

Conventional blood culture (BC) is limited by long turnaround times and low sensitivity. The T2Bacteria is a novel culture-independent system identifying six bacteria from whole blood within a few hours. We conducted an observational study in an Australian Emergency Department to assess the performance of T2Bacteria, including its diagnostic accuracy, time to results, and potential role in early adjustment of antimicrobials. Patients with suspected bacteremia were enrolled and had BC drawn with T2Bacteria samples. We enrolled 101 patients. Thirty-five patients had positive BC, of which 31 (88.

The use of non-mydriatic ocular fundus imaging in the emergency department for detection of ocular syphilis presenting as presumed "papilledema": a case series.

BACKGROUND: Ocular syphilis and neurosyphilis can present as seemingly "isolated" bilateral optic disc edema mimicking papilledema from intracranial hypertension. As a result, patients are often sent to the emergency department (ED) for "papilledema work-up," which can lead to delayed or missed ocular syphilis diagnosis since serologic testing for syphilis is not routinely performed in patients with presumed "papilledema." Our study describes eight patients initially referred to our ED for "papilledema" workup who were ultimately diagnosed with ocular syphilis based on characteristic findings

Comparison of Elastic Tourniquet and Blood Pressure Cuff for Peripheral Intravenous Access in Hypotensive Patients: A Randomized Single-Blind Study.

BACKGROUND: Peripheral intravenous catheterization (PIVC) is one of the most common invasive procedures performed in emergency departments (EDs). Elastic tourniquets (ET) and blood pressure cuffs (BPC) are most commonly used for venodilatation. Difficult vascular access can be predicted in some patients, and the rate of successful first-time PIVC placement in this group is low. OBJECTIVE: In this study, we investigated which venodilatation method is most appropriate for hypotensive patients, a group in whom vascular access is predicted to be difficult. METHODS: This was a prospective, single

Evaluating patients with chest pain in the emergency department.

Identifying patients with chest pain that is of cardiac origin is crucial owing to the high mortality and morbidity of cardiovascular diseases. History, electrocardiogram, and cardiac biomarkers are the core elements of the assessment. High sensitivity cardiac troponins (hs-cTn) are the preferred biomarkers for diagnosing acute myocardial infarction, allowing for more accurate detection and exclusion of myocardial injury. However, elevations in hs-cTn can occur in patients who are not experiencing acute myocardial infarction. Therefore, it is important for clinicians to recognize how to integr

Ultrasound evaluation of the optic nerve in patients with severe symptomatic hyponatremia.

Severe hyponatremia can precipitate significant neurological symptoms due to osmotic imbalances that induce intracranial hypertension (ICH). This study investigates the correlation between serum sodium levels and optic nerve sheath diameter (ONSD), measured by ultrasound, in patients presenting with symptomatic severe hyponatremia in the Emergency Department. Sixty-four patients with severe hyponatremia (Na+ < 125 mmol/L) and neurological symptoms were enrolled and compared to a matched control group. Serial measurements of serum sodium and ONSD were obtained at 3, 12, and 24 h post-correction

Artificial Intelligence-Based Early Prediction of Acute Respiratory Failure in the Emergency Department Using Biosignal and Clinical Data.

PURPOSE: Early identification of patients at risk for acute respiratory failure (ARF) could help clinicians devise preventive strategies. Analyzing biosignals with artificial intelligence (AI) can uncover hidden information and variability within time series. We aimed to develop and validate AI models to predict ARF within 72 h after emergency department admission, primarily using high-resolution biosignals collected within 4 h of arrival. MATERIALS AND METHODS: Our AI model, built on convolutional recurrent neural networks, combines biosignal feature extraction and sequence modeling. The mod

Surgical Disease Burden, Outcomes, and Roles of Non-Physician Clinicians in Ugandan Emergency Departments.

BACKGROUND: Delivery of emergency surgical care remains a challenge in much of Sub-Saharan Africa, with physician shortages in Uganda resulting in only one surgeon per 100,000 people. Emergency units in Uganda receive emergency surgical patients, but it is unknown how great of a burden these emergency surgical patients represent in terms of total number, care required, or outcomes. METHODS: We performed a retrospective review of a quality assurance database for all patients treated at two emergency units in Uganda from 2009-2019. Patients were defined as "surgical" if they were admitted direc
