Characterizing the Diversity of Layer 2/3 Human Neocortical Neurons in Pediatric Epilepsy.

Childhood epilepsy is a common and devastating condition, for which many children still do not have adequate treatment. Some children with drug-resistant epilepsy require surgical excision of epileptogenic brain tissue for seizure control, affording the opportunity to study this tissue ex vivo to interrogate human epileptic neurons for potentially hyperexcitable perturbations in intrinsic electrophysiological properties. In this study, we characterized the diversity of layer L2/3 (L2/3) pyramidal neurons (PNs) in ex vivo brain slices from pediatric patients with epilepsy. We found a remarkable

Inflammation in Cerebral Cavernous Malformations: Differences Between Malformation Related Epilepsy vs. Symptomatic Hemorrhage.

BACKGROUND AND OBJECTIVE: Cerebral cavernous malformation (CCM) is a vascular disorder causing seizures, neurological deficits, and hemorrhagic stroke. It can be sporadic or inherited via CCM1, CCM2, or CCM3 gene mutations. Inflammation is broadly recognized as a promoter of cerebral vascular malformations. This study explores inflammatory mechanisms and differences behind CCM-related hemorrhage and epilepsy. MATERIAL AND METHODS: The study group comprised 28 patients, ten patients with CCM-related epilepsy, and 18 patients who clinically presented with a cerebral hemorrhage at diagnosis. All

Local brain volume reductions in patients with non-lesional epilepsy on 7T MRI.

OBJECTIVE: In patients with non-lesional epilepsy, MR imaging detects no structural or functional abnormalities. The aim of this study was to determine whether subtle local reductions or increases in brain volume, undetectable to the human eye, can indicate an epileptic focus directly or indirectly. To address this, we performed brain volumetry using 7T MRI. METHODS: We evaluated 7T MRI in patients with non-lesional epilepsy as part of a retrospective study and a healthy control cohort from another prospective study. FastSurfer segmentations were performed using T1 MPRAGE. Additionally, we al

Two-Year Retention Rate and Clinical Usefulness of Eslicarbazepine Acetate in Epilepsy Patients.

OBJECTIVE: Eslicarbazepine acetate (ESL) is a third-generation antiseizure medication (ASM) approved for the treatment of focal seizures. Because of the delayed approval of ESL in Asian countries, there is limited information on the clinical usefulness of ESL in the Asian population. We performed a retrospective analysis of retention rate and seizure outcomes for ESL treatment in Korean epilepsy patients. METHODS: A 2-year retrospective observational study was conducted, in which 160 epilepsy patients (82 males; age range, 17-81 years) were treated with ESL. The starting dose of ESL was 400 m

Aberrant HCN4 channel expression: Impacts on neuronal excitability and implications for epilepsy pathogenesis.

Hyperpolarisation-activated cyclic nucleotide-gated (HCN) channels are nonselective voltage-gated cation channels encoded by four genes (HCN1-4) that show distinct localisation patterns throughout the central nervous system and body. Among the four isoforms, HCN4 channels are most widely known for their pacemaker activity in cardiac tissue, with HCN4 pathogenic variants being strongly associated with cardiac abnormalities. In the brain, HCN4 channels are predominantly expressed in subcortical regions, where they play roles in regulating brain function including controlling transmitter release,

From Felbamate to Carbamazepine: Clinical Consequences of Unintentional Antiepileptic Substitution in Refractory Epilepsy.

Epilepsy is the most common chronic neurological disorder and one of the leading causes of disability. We report a rare and clinically significant complication involving the inadvertent substitution of felbamate with carbamazepine in a 37-year-old man with refractory epilepsy. He developed drowsiness, ataxia, dysarthria, and vomiting, followed by hematological abnormalities and respiratory symptoms. Although carbamazepine levels were within the reference range, a probable diagnosis of acute carbamazepine toxicity was made in the setting of refractory epilepsy. Therapeutic interventions include

Gene therapies alleviate absence epilepsy associated with Scn2a deficiency in DBA/2J mice.

Mutations in the voltage-gated sodium channel gene SCN2A, which encodes the NaV1.2 channel, cause severe epileptic seizures. Patients with SCN2A loss-of-function (LoF) mutations, such as protein-truncating mutations, often experience later-onset and drug-resistant epilepsy, highlighting an urgent unmet clinical need for new therapies. We previously developed a gene-trap Scn2a (Scn2a gt/gt ) mouse model with a global NaV1.2 reduction in the widely used C57BL/6N (B6) strain. Although these mice display multiple behavioral abnormalities, EEG recordings indicated only mild epileptiform discharges,

Rewiring the seizing brain: stem cell grafts as neuromodulatory architects in epilepsy therapy.

As an emerging therapeutic strategy, stem cell transplantation has demonstrated promising potential in the management of refractory epilepsy. Epilepsy, a prevalent neurological disorder characterized by recurrent seizures, affects approximately one-third of patients worldwide who exhibit resistance to existing antiepileptic drugs (AEDs). Consequently, exploring novel treatment modalities is imperative. Recent studies have indicated that stem cell transplantation improves neurological function in epilepsy through multiple mechanisms. Research has revealed that transplanted stem cells mitigate s

Molecular Basis of GABA Aminotransferase Inhibition in Epilepsy: Structure, Mechanisms, and Drug Development.

Epilepsy affects approximately 50 million people worldwide, with nearly one-third of patients experiencing inadequate seizure control with conventional anti-epileptic drugs. The GABAergic system, responsible for inhibitory neurotransmission in the central nervous system, represents a critical target for seizure management. GABA aminotransferase (GABA-T), the enzyme responsible for GABA catabolism, has emerged as a particularly attractive therapeutic target. Inhibition of GABA-T increases synaptic GABA availability, enhancing inhibitory neurotransmission and raising the seizure threshold. Vigab

Mesial-to-lateral gradients of epileptiform activity to localize mesial temporal lobe epilepsy.

OBJECTIVE: Mesial temporal lobe epilepsy is a common localization of drug-resistant epilepsy in adults. Patients often undergo intracranial electroencephalographic monitoring to confirm localization and determine candidacy for focal ablation or resection. Clinicians primarily base surgical decision-making on seizure onset patterns, with imaging abnormalities and information from interictal epileptiform discharge (spikes) used as ancillary data. How the morphology and timing of spikes within multielectrode sequences may inform surgical planning is unknown, in part due to the lack of measurement

Parvalbumin neurons in the nucleus accumbens shell modulate seizure in temporal lobe epilepsy.

A growing number of clinical and animal studies suggest that the nucleus accumbens (NAc), especially the shell, is involved in the pathogenesis of temporal lobe epilepsy (TLE). However, the role of parvalbumin (PV) GABAergic neurons in the NAc shell involved in TLE is still unclear. In this study, we induced a spontaneous TLE model by intrahippocampal administration of kainic acid (KA), which generally induce acute seizures in first 2 h (acute phase) and then lead to spontaneous recurrent seizures after two months (chronic phase). We found that chemogenetic activation of NAc shell PV neurons c

Language lateralization in temporal lobe epilepsy: A behavioral screening tool for surgical planning.

OBJECTIVE: Temporal lobe epilepsy can disturb eloquent areas, affecting language. We applied a visually-mediated task to measure lateralization of language recognition in drug-resistant temporal lobe epilepsy. METHOD: Patients with left (n = 26), right (n = 28) temporal lobe epilepsy and controls (n = 30) were administered the translingual lexical decision task. We performed repeated measures analyses of variance, with the visual half-field as an intrasubject factor and the group as an intersubject factor. RESULTS: A main effect of visual half-field was found, showing the right visual field

Differential reorganization of episodic and semantic memory systems in epilepsy-related mesiotemporal pathology.

Declarative memory encompasses episodic and semantic divisions. Episodic memory captures singular events with specific spatiotemporal relationships, whereas semantic memory houses context-independent knowledge. Behavioural and functional neuroimaging studies have revealed common and distinct neural substrates of both memory systems, implicating mesiotemporal lobe (MTL) regions such as the hippocampus and distributed neocortices. Here, we explored declarative memory system reorganization in patients with unilateral temporal lobe epilepsy (TLE) as a human disease model to test the impact of vari

MicroRNA-33 regulates the synaptic plasticity-related gene ARC in temporal lobe epilepsy.

This study aimed to elucidate the expression patterns of miR-33 and ARC in both a rat model of temporal lobe epilepsy (TLE) and human TLE patients, to explore the role of miR-33 in epilepsy onset through its regulation of ARC expression in the hippocampus. Our findings, supported by a Dual-Luciferase reporter assay, suggest that miR-33 can bind to the 3' UTR region of ARC. We observed that miR-33 levels were reduced at 1 hour and 60 days post-seizure, while ARC expression notably increased at these time points. In the hippocampal CA1 and CA3 regions of post-seizure rats, ARC expression signifi

Machine learning localization to identify the epileptogenic side in mesial temporal lobe epilepsy.

BACKGROUND: Mesial temporal sclerosis (MTS) is the most common pathology associated with drug-resistant mesial temporal lobe epilepsy (mTLE) in adults. Most atrophic hippocampi can be identified using MRI based on standard epilepsy protocols; however, difficulties can arise in cases where sclerotic changes in the hippocampus are subtle or non-epilepsy-specific protocols have been implemented. In such cases, quantitative methods, such as T1-weighted axial series MRIs, are valuable additional tools to complement epilepsy-specific protocols. In the current study, we applied machine learning (ML)

Adding the third dimension: 3D convolutional neural network diagnosis of temporal lobe epilepsy.

Convolutional neural networks (CNN) show great promise for translating decades of research on structural abnormalities in temporal lobe epilepsy into clinical practice. Three-dimensional CNNs typically outperform two-dimensional CNNs in medical imaging. Here we explore for the first time whether a three-dimensional CNN outperforms a two-dimensional CNN for identifying temporal lobe epilepsy-specific features on MRI. Using 1178 T1-weighted images (589 temporal lobe epilepsy, 589 healthy controls) from 12 surgical centres, we trained 3D and 2D CNNs for temporal lobe epilepsy versus healthy contr

Automated and Interpretable Detection of Hippocampal Sclerosis in Temporal Lobe Epilepsy: AID-HS.

OBJECTIVE: Hippocampal sclerosis (HS), the most common pathology associated with temporal lobe epilepsy (TLE), is not always visible on magnetic resonance imaging (MRI), causing surgical delays and reduced postsurgical seizure-freedom. We developed an open-source software to characterize and localize HS to aid the presurgical evaluation of children and adults with suspected TLE. METHODS: We included a multicenter cohort of 365 participants (154 HS; 90 disease controls; 121 healthy controls). HippUnfold was used to extract morphological surface-based features and volumes of the hippocampus fro

Exercise, memory, and the hippocampus: Uncovering modifiable lifestyle reserve factors in refractory epilepsy.

Physical exercise is an emerging target for improving cognition in aging and neurological disease. Due to the beneficial impact of exercise on hippocampal health and the vulnerability of the hippocampus in medication-resistant temporal lobe epilepsy (TLE), exercise could present a promising intervention in TLE. We investigated whether exercise engagement is associated with verbal memory function and hippocampal integrity in 29 young to middle-aged adults with refractory TLE and 21 demographically matched controls. Participants completed a self-reported questionnaire of weekly exercise, three t
