Retrospective assessment of the dismasking flap procedure as a craniofacial approach.

OBJECT: In 1997 the authors presented the case of a 3-year-old girl who underwent complete resection of a huge tumor via a new technique using a dismasking flap. Since that report, 14 patients have undergone surgery using this technique. There have been few reports on long-term follow-up after a craniofacial surgical approach and reconstruction of a huge tissue defect. The authors report details of this procedure based on these cases, including long-term follow-up in the original case. METHODS: The dismasking flap is a cranioorbitofacial degloving method that uses a circumpalpebral approach w

Treatment of human mucogingival defects utilizing a bioabsorbable membrane with and without a demineralized freeze-dried bone allograft.

BACKGROUND: The use of guided tissue regeneration (GTR) has become an effective procedure for the treatment of gingival recession. No reports exist on the use of a bioabsorbable membrane in combination with a demineralized freeze-dried bone allograft (DFDBA) for the treatment of these defects. METHODS: Fourteen (14) patients with 17 recession defects were included in this clinical study. Each patient had at least 1 tooth with 3 mm or greater marginal tissue recession on the facial surface as measured from the cemento-enamel junction (CEJ). Each patient was treated by GTR using a bioabsorbable

Treatment of murine mucopolysaccharidosis type VII by syngeneic bone marrow transplantation in neonates.

BACKGROUND: Bone marrow transplantation (BMT) proved an effective therapy for murine mucopolysaccharidosis type VII (MPS VII) in adult gusmps/gusmps mice with well developed clinical and pathologic characteristics of the disease. MPS VII mice transplanted as adults had a marked decrease in lysosomal storage material in many organs, although not in the skeleton and brain (1). Since untreated newborn MPS VII mice appear normal and have minimal lysosomal storage material detectable microscopically, we postulated that BMT in newborn mice might prevent the subsequent accumulation of storage materia

Long-term evaluation of treatment protocols for isolated midfacial fractures in a German nation-wide craniomaxillofacial trauma center 2007-2017.

An update on the trends in maxillofacial traumatology provides additional information on the actual and changing needs. This retrospective study aimed to review all patient records of patients treated for isolated midface fractures in the Department of Cranial- and Maxillofacial Surgery at the Dortmund General Hospital between 2007 and 2017. The patient radiographs and patient files were reviewed. The safety and efficacy of the applied methods were controlled by assessing complications based on the Clavien-Dindo classification system. The statistical analysis included descriptive methods inclu

A hybrid simulation model for pre-operative planning of transsphenoidal encephalocele.

Congenital transsphenoidal encephalocele (CTE) surgical correction is a challenging procedure. Although rare, this anomaly, characterized with neural herniation elements, including the pituitary gland or optic pathway through the sphenoid bone with anatomical alteration, can be presented in many different ways and should be individually analyzed. Significant advances in medical technology and the 3D models may simulate the complex anatomical relations of the human body. Nowadays, medical education relies on the availability of standardized materials that can reliably emulate human anatomy. The

Fibrous Dysplasia Characterization Using Lacunarity Analysis.

Fibrous dysplasia (FD) is a developmental anomaly in which the normal medullary space of the affected bone is replaced by fibro-osseous tissue. This condition is typically encountered in adolescents and young adults. It affects the maxillofacial region and it can often cause severe deformity and asymmetry. Therefore, accurate diagnosis is critical to determine the appropriate treatment of each case. In this sense, computed tomography (CT) is a relevant resource among the imaging techniques for correct diagnosis of this condition. Thus, in this paper, we propose to analyze fibrous dysplasia thr

Usefulness of vascularized galeal frontalis myofascial flap as treatment for postoperative infection in frontal sinus fracture.

Inappropriate treatments of frontal sinus fractures may lead to serious complications, such as mucopyocele, meningitis, and brain abscess. Assessment of nasofrontal duct injury is crucial, and nasofrontal duct injury necessitates sinus obliteration. For sinus obliteration, autogenous fat, muscle, bone, pericranium grafts, and alloplastic materials, such as Surgicel, hydroxyapatite (Bone Source), and methyl methacrylate, are commonly used. However, autogenous tissue grafts and alloplastic materials cannot prevent infection, which is the leading cause of complications of frontal sinus surgery. A

Computer-aided design and manufacturing and rapid prototyped nanoscale hydroxyapatite/polyamide (n-HA/PA) construction for condylar defect caused by mandibular angle ostectomy.

The fracture or defect of the mandibular condyle is one of the serious complications during angle-reduction ostectomy. Reconstruction of such defects also is a daunting task. The case report describes a method based on computer-aided design/computer-aided manufacturing (CAD/CAM) and rapid prototyping nanoscale hydroxyapatite/polyamide (n-HA/PA) for individual design, fabrication, and implantation of a mandibular condyle. A 27-year-old woman with a square-shaped face who had previously undergone mandibular angle reduction reported with malocclusion, deviated mouth, collapse of the right side of

Influence of the Furlow palatoplasty for patients with submucous cleft palate on facial growth.

The purpose of this study was to investigate the facial growth in patients with submucous cleft palate operated on using the Furlow palatoplasty. A total of 30 patients with submucous cleft palate underwent Furlow palatoplasty from 1993 to 1998. The mean follow-up period was 4 years and 3 months. Twenty-five of 30 patients were followed up. Mid-facial growth was measured using lateral cephalograms in 18 patients whose age was greater than 8 years. The parameters obtained in the lateral cephalogram were compared with those of a healthy population in Korea. Eleven (61.1%) of 18 patients observed

[Late sequelae of lateral and central mid-facial fractures after osteosynthesis with miniplates].

Despite significant progress in the therapy of craniofacial injuries permanent consequences of the trauma are still likely to occur. It has been possible to follow-up 105 (65.2%) of the 161 patients who were treated for lateral and central midfacial fractures in our department between 1989 and 1991. Pathological findings on follow-up were found in 48.6% of our patients. Most frequent were persistent disturbances of the sensitivity in the area of the second branch of the trigeminal nerve (32.4%). Patients who were operated within the first week after trauma did have significantly less sensory d

Aplasia cutis congenita of the scalp with large underlying skull defect: a case report.

Localised agenesis of the scalp is the most frequent pattern in aplasia cutis congenita (ACC), a congenital absence of the skin and occasionally of deeper layers. Several clinical groups are characterised by the location and pattern of skin defects, associated malformations and the mode of inheritance. Death occurs in 20% of cases, secondary to the associated anomalies, to infections or to haemorrhage from ulceration of the sagittal sinus when there is also a defect of the underlying skull. In this latter case, we close the defect by two rotational scalp flaps (Orticochea technique) at birth.

[Long-term results of fractured nasal bones in adults. Clinical and roentgenographic examinations (author's transl)].

In 100 adults the effect of treatment of nasal fractures was followed up after 14 to 65 months. In 30% of 53 patients with closed reduction of the nose long-term results as to shape and function of the nose were not satisfying. Nasal obstruction deformity slowly developed several months after treatment mainly because of untreated septal fractures and green-stick fractures of the nasal bones. After open reduction and septal reconstruction only 1 of 17 patients had a poor result. By roentgenographic examinations three types of fracturelines could be distinguished: osseous (40), fibrous (37), and

Sagliker Syndrome: A Diagnostic Challenge.

Sagliker syndrome (SS), commonly referred to as "Uglifying human face Syndrome," is a rare but serious outcome of chronic kidney disease characterized by skeletal deformities, especially in the skull and jaws, due to uncontrolled secondary hyperparathyroidism (SHPT). The diagnosis is made through a combination of clinical symptoms, radiographic evidence (demonstrating bone deformities), and laboratory evaluations. Timely and effective treatment of SHPT is essential to prevent or lessen the onset of SS. Treatment includes managing phosphorus levels (through dietary changes, phosphate binders, a

Piezoelectric shaping of radical mastoid cavity and dissection of ossified cochlea prior to cochlear implantation.

Cochlear implantation (CI) in radical cavities remains challenging due to the risk of postoperative electrode extrusion. We report a single-stage approach employing piezoelectric instrument (PEI) to optimize cavity osteoplasty, atraumatic cochlear access, and electrode stabilization in the setting of partial scala tympani ossification. A 72-year-old female with left profound sensorineural hearing loss and prior canal wall (CW)-down mastoidectomy underwent CI with posterior CW reconstruction. Angled and straight PEI inserts facilitated controlled bone removal over the facial nerve and creation

Renal Angiomyolipoma in a Young Woman With Tuberous Sclerosis: A Case Report.

Tuberous sclerosis is a rare autosomal dominant genetic disorder caused by mutations in tumor suppressor genes (TSC1/2), leading to hamartomas in multiple organs. Renal angiomyolipomas are often asymptomatic but can cause mass effects or bleeding if they enlarge, requiring treatment. A 29-year-old woman presented with severe left flank pain, an abdominal mass, and facial papules. Imaging revealed a large, 18.3 × 13 × 11.5 cm mass in the lower pole of the left kidney, which was fat-poor, a smaller lesion on the opposite kidney, pulmonary cysts, and osteoblastic bone lesions, all indicating tube

Computer-aided "Tetris" genioplasty using an intermediate trimming guide for complex asymmetry correction.

Computer-aided design and manufacturing (CAD/CAM) have improved the accuracy of genioplasty; however, complex three-dimensional movements such as the "Tetris genioplasty" remain surgically demanding. This technique requires precise trimming of bony interferences to achieve a perfect "lock-and-key" fit, which is difficult to perform freehand. We present a novel three-step digital workflow to address this challenge. Our protocol utilises a novel Intermediate Trimming Guide (ITG) designed to precisely resect specific bony interferences on the mobilised segment. In this case with facial asymmetry,
