Unilateral papilloedema after hepatitis B vaccination in a migraine patient. A case report including forensic aspects.

PURPOSE: To report on a unilateral optic nerve reaction appearing 9-10 hours after vaccination against hepatitis B. METHODS: To describe the case and discuss the underlying mechanisms. There was a scintillating scotoma leaving a permanent inferior notch in the visual field, but central vision was left normal and with only a slight affection of colour perception. Shortly the optic nervehead appeared engorged, then slightly ischaemic, but was hardly left with evidence of atrophy. RESULTS: Immune-based reactions could hardly be held responsible, multiple sclerosis was unlikely, and ultrasonogra

The development of choroidal neovascularization in pregnancy.

PURPOSE: To evaluate the possible association between the development of choroidal neovascularization (CNV) and pregnancy. METHODS: A retrospective review was performed of the clinical records of three patients who were pregnant at the time a choroidal neovascular membrane (CNVM) was diagnosed. The clinical presentations and treatment of the CNVM occurring in association with the pregnancies are described. RESULTS: Each patient had a decrease in visual acuity during her pregnancy: one in the first trimester, one in the second trimester, and one in the third trimester. Two patients were diagn

Autologous platelet treatment for optic disc pit associated with persistent macular detachment.

PURPOSE: To determine the effectiveness of autologous platelets in repair of optic disc pit associated with persistent macular detachment. PATIENT: We report on a 44-year-old woman with optic disc pit associated with persistent macular detachment. The patient underwent an uneventful pars plana vitrectomy with autologous platelets injected over the optic disc pit. Good anatomical and functional results were obtained. CONCLUSION: This surgical approach may be a valuable adjunct to the surgical management of optic disc pit associated with persistent macular detachment.

Inherited retinal arteriolar tortuosity with retinal hemorrhages.

BACKGROUND: Familial arteriolar tortuosity is an autosomal dominant disorder affecting the retinal arterioles. OBJECTIVES: To report a pedigree with this disorder and describe a systemic workup to determine whether this vascular abnormality is limited to the eye. RESULTS: A 58-year-old woman referred for retinal hemorrhages was found to have retinal arteriolar tortuosity of both eyes, especially in the macular area. Her 63-year-old brother had a history of retinal hemmorhages beginning at age 18 years and had similar fundoscopic examination findings. The proband had an extensive systemic wor

Visual field loss caused by retinal vascular occlusion after vitrectomy surgery.

PURPOSE: To report complete superonasal retinal arterial occlusion with resulting temporal visual field loss after uneventful pars plana vitrectomy with gas-fluid exchange. METHOD: Case report. RESULTS: Superonasal retinal arterial occlusion was observed 12 days after vitrectomy surgery. Within 10 weeks, the artery reperfused, leaving minimal vascular attenuation and mild optic atrophy. CONCLUSIONS: Uneventful vitrectomy surgery can be associated with notable peripheral visual field loss caused by arterial occlusion in the immediate postoperative period. Rapid resorption of the intraocular

Epiretinal membrane formation in Terson syndrome.

Clinical features of epiretinal membranes were examined in 22 eyes of 13 patients with Terson syndrome who were treated with pars plana vitrectomy. The shape and localization of the epiretinal membranes were intraoperatively evaluated and correlated with the presence or absence of posterior vitreous detachment (PVD). Patients with complete PVD, but with no membrane found during surgery, were followed postoperatively. Membrane formation ultimately developed in 13 of the 22 eyes. In eight eyes, PVD was incomplete and the epiretinal membrane was found at the optic disc or along the temporal vascu

Myelinated nerve fibers and retinal vascular abnormalities.

BACKGROUND/PURPOSE: Abnormal retinal vessels may develop in a region of myelinated nerve fibers, and these vessels may cause vitreous hemorrhages. METHODS: The clinical histories of seven patients with retinovascular abnormalities in a patch of myelinated nerve fibers are presented. None of the reported patients had other evidence of systemic disease. The cases were traced by a multicentric retrospective study. RESULTS: Retinal vascular abnormalities ranged from mild telangiectasis to frank neovascularization, with or without obstruction of the capillary network and signs of branch artery an

Cranial MRI in idiopathic retinal vasculitis.

Ten patients with clinically isolated idiopathic retinal vasculitis who had a positive family history for multiple sclerosis (MS) or positive typing for HLA B7 underwent magnetic resonance imaging (MRI) of brain and optic nerves in order to establish the frequency of clinically silent lesions. Brain MRI was normal in seven and abnormal in three: one had a single small white matter lesion, two had extensive white matter abnormalities resembling those seen in MS. In two patients a lesion was shown in the optic nerve. These findings suggest that a minority of patients with idiopathic retinal vasc

[AIDS: infections of the retina and choroid].

Various viral, bacterial, parasitic and fungal agents have been found to cause infections of retina and choroidea in HIV-infected patients. Usually these infections are opportunistic infections caused by the profound immunodeficiency, which is a result of the decay of lymphocytes by HIV. Before the HIV epidemic only rare cases of cytomegalovirus (CMV) retinitis were known in the literature. Now CMV retinitis has become the most common infection of the eye in AIDS patients. Ocular toxoplasmosis in HIV-infected patients can have a severe clinical appearance without treatment. Spontaneous recover

Acute bilateral optic disc neovascularization.

PURPOSE: A case of bilateral acute neovascularization of the optic disc and peripapillary epiretina of unknown etiology in a previously healthy 26-year-old woman is discussed. METHODS: The clinical course of the disease, investigation, and treatment are presented in detail. RESULTS: Histopathologic examination of epiretinal tissue removed from the second eye at vitrectomy provided evidence of inflammatory disease and suggested a possible etiology. CONCLUSION: The cause of this patient's disease remains uncertain. Known causes of this disease were excluded as far as possible. Histopathologic

Combined occlusion of the central retinal artery and vein.

A 56-year-old man, complaining of sudden visual loss in his left eye, demonstrated cream-colored retinal edema along a macular branch of the central retinal artery, overall delay of fluorescence in angiography and general depression of the central visual field, which were interpreted as the incomplete form of central retinal artery occlusion (CRAO) mimicking cilioretinal or branch retinal artery occlusion. After receiving paracentesis and fibrinolytic agents, the patient recovered his vision gradually, while the ophthalmoscopic findings progressed to show central retinal vein occlusion (CRVO)

The blinding mechanisms of incontinentia pigmenti.

The ocular and cerebral abnormalities associated with incontinentia pigmenti, an X-linked dominant disease with characteristic cutaneous features, are far worse than the name would indicate. Although some patients have normal vision, total blindness or permanent visual deficiency may occur. Retinal vascular abnormalities, involving the periphery as well as the macula, appear to represent the primary disease process in the eye. Retinal detachment may then ensue, due to mechanisms that seem analogous to those of retinopathy of prematurity. Optic nerve atrophy and occipital lobe infarction are ad

[Pathogenesis and therapy of central serous retinopathy].

UNLABELLED: There is no consensus in the literature on the etiology and therapy of central serous retinopathy. We conducted a retrospective study to find out whether patients treated with argon laser coagulation (ALK) experienced long-term visual acuity improvement. We were also concerned to find whether choroidal perfusion was involved in the etiology. PATIENTS: In all, 28 patients (4 women/24 men) with RCS confirmed by angiography were analyzed. All patients were followed up from 1988 to 1992. Patients with chronic recurrent RCS were not considered. Angiography was performed to examine the

Presumed photic retinopathy after cataract surgery: an angiographic study.

To ascertain the prevalence rate of presumed photic retinopathy after cataract surgery, 276 fluorescein angiograms of 147 patients were reviewed. The diagnosis was based on strict angiographic criteria. Lesions consistent with healed photic retinopathy were noted in four patients (prevalence rate of 3%). This finding is discussed and comparisons with the literature are made.

Isolated ocular cryptococcosis in an immunocompetent patient.

A 62-year-old woman without evidence of immunocompromise was evaluated for uveitis and a subretinal lesion in the right eye. Laboratory evaluation, including cerebrospinal fluid analysis, revealed no apparent cause. The diagnosis of subretinal cryptococcosis was established by transscleral needle biopsy of the subretinal mass. Treatment with intravenous amphotericin B and oral 5-flucytosine brought recovery of visual acuity to 20/30-1 and resolution of the inflammation. This patient demonstrates that ocular cryptococcal infection must be suspected, even in the absence of predisposing factors o

[Emoxipin as an inhibitor of angiogenesis].

The effect of emoxypin on angiogenesis in rabbit cornea in aseptic inflammation induced by intracorneal implantation of a piece of quartz and on the development of the vessels of the chick embryo yolk sac was studied. 1% emoxypin pipetted thrice a day for 10-14 days inhibited corneal neovascularization and reduced the formation of new blood vessels. We observed an inhibitory effect on the development of vascular bed of the embryo yolk sac on incubation hour 64-72. The drug affected neither general growth of the embryos no the number of somites.

[Para-papillary chorioretinal atrophy in eyes with shallow glaucomatous optic disk cupping].

Parapapillary chorioretinal atrophy is present in some, but not all glaucomatous eyes. We examined whether eyes with shallow or with deep optic cupping showed more pronounced parapapillary atrophy. To this end, we analyzed 15 degree color stereo optic disc photographs of 141 eyes of 95 normal subjects and 211 eyes of 120 patients suffering from chronic open-angle glaucoma morphometrically. Optic cup depth was semiquantitatively estimated in a relative score ranging from 0 to 5. It increased significantly with increasing glaucoma damage indicated by smaller neuroretinal rim area, higher visual
