Mini-dose Bier block intravenous regional anesthesia in the emergency department treatment of pediatric upper-extremity injuries.

The safety and effectiveness of the "mini-dose" Bier block, a technique of i.v. regional anesthesia using low-dose lidocaine (1.5 mg/kg) without routine premedication, was evaluated in the emergency department treatment of pediatric upper-extremity fractures and dislocations. We prospectively studied 69 patients, aged from 2 to 16 years, treated at a pediatric primary care/referral-based emergency department. Good to excellent anesthesia was achieved during closed reduction in 90% of the cases. All patients achieved an acceptable reduction, as demonstrated by follow-up radiographs. None requir

Traumatic lesions of the lower cervical spine in Poland.

From 1965 through 1990 the Spinal Department of the Metropolitan Rehabilitation Center in Konstancin provided treatment to 1330 patients with spinal injury in the C5-T1 area admitted to hospital in the early post-traumatic period. The most common causes of injury were falls from a horse cart, diving, and road accidents, in that order. This paper presents an analysis of the dependencies between the degree of spinal cord injury and the mechanism of spinal injury, as well as the results of conservative and surgical treatment. The most serious lesions of the spinal cord were those following spinal

Results following treatment of traumatic dislocations of the knee joint.

In a retrospective study, 31 patients were identified over a period of 25 years as having sustained traumatic knee dislocations without associated fractures. Three patients had early amputation secondary to vascular complications. Ten popliteal artery disruptions and five peroneal nerve palsies were diagnosed. At the time of follow-up evaluation (average, 40 months), 16 patients were examined. Operatively treated patients (n = 6) tended to have better motion (129 degrees) than nonoperatively treated patients (108 degrees). Varus instability was seen in only two patients treated nonoperatively.

Safe and effective IV regional anesthesia for use in the emergency department.

The mini-dose Bier block, a technique of intravenous (IV) regional anesthesia that uses low-dose lidocaine and provides safe and effective anesthesia for outpatient closed reductions of upper extremity fractures and dislocations, is presented. This procedure was evaluated in two hospital emergency departments in 105 patients (ages 2 to 86). Ninety-five percent achieved adequate anesthesia (minimal or no pain on closed reduction). No significant complications were noted. Full neurologic function returned in ten minutes in all cases. Both patient and physician satisfaction with the procedure wer

[Injuries of the cervical spine].

In connection with 44 patients, who suffered injuries of the cervical spine, the types of the fracture and dislocation of the cervical spine are discussed. The injuries of the cervical spine--with or without injury of the spinal marrow--signify hard tast in respect of therapy. The conservative (extension) treatment of surgical intervention are even nowadays much debated themes in the neurosurgery. The introduction of Crutchfield tongs was a definite advance in the latter years. With this treatment method the restitution of the form, continuity and stability of the cervical spine is possible. Confidence of graduating family practice residents in their management of musculoskeletal conditions.

A study was undertaken to determine the confidence of graduating family practice residents in the management of musculoskeletal conditions and to determine the level of exposure of graduating family practice residents to fracture care. A 2-page questionnaire consisting of 50 items was sent to 680 graduating family practice residents at 100 randomly selected residency training programs throughout the United States. Family practice residents were questioned about their fracture care experience, including the number of fractures diagnosed, the number of fractures reduced, the number of fractures

Indirect reduction of the radial head using an external fixator to treat chronic radial head dislocations.

OBJECTIVES: We evaluated the results of indirect reduction of the radial head via a circular external fixator in pediatric patients with unilateral chronic radial head dislocation. METHODS: Three male patients (mean age 6.3 years; range 5 to 8 years) with chronic radial head dislocation underwent ulnar lengthening using a circular external fixator for indirect reduction of the radial head. None of the patients had radiographic evidence for deformity of the radial head, dysplasia of the capitellum, or osteoarthritic changes. The etiologic factors were ulnar hemimelia, brachial plexus injury, a

Ceramic-on-ceramic versus polyethylene-based bearings in total hip arthroplasty: a meta-analysis.

BACKGROUND: Bearing surface selection is a key determinant of long-term performance in total hip arthroplasty (THA), yet comparative evidence across modern ceramic and polyethylene-based couples remains inconsistent. This meta-analysis provides a comprehensive comparative evaluation of ceramic-on-ceramic (CoC) bearings versus metal-on-polyethylene (MoP), metal-on-highly cross-linked polyethylene (MoHXLPE), ceramic-on-polyethylene (CoP), and ceramic-on-highly cross-linked polyethylene (CoHXLPE). METHODS: A systematic search of four databases identified randomized controlled trials and comparat

Shoulder Injuries in Patients with Epilepsy - a Multidisciplinary Approach.

The management of shoulder injuries resulting from epileptic seizures presents a particular challenge. These injuries often present complex patterns, ranging from dislocations and fractures to damage to the rotator cuff. Recurrent seizures can significantly impact the outcome and must be considered a central risk factor when choosing the therapeutic approach. Accordingly, these injuries require close multidisciplinary cooperation between orthopaedics/trauma surgery, neurology/epileptology and physiotherapy, both in diagnosis and treatment. Despite their clinical relevance, standardised treatme

Fatigue failure of constrained total knee replacement: A case report.

Hinged implants for total knee arthroplasty (TKA) improve stability and joint function in a ligamentously compromised knee, but potentially at the cost of reduced implant longevity due to increased mechanical stress on the construct. This report details the case of an 83-year-old woman who underwent a revision total knee arthroplasty for a chronically dislocated hinged knee prosthesis in August 2024 at our institution. The patient presented with a several month history of pain and instability with reported self-reduction of the prosthesis to aid in ambulation. The revision surgery revealed tha

[Forensic medical characteristics of fatal injuries in a traffic accident in the car cabin].

The study objective is to identify the most common fatal injuries in car accidents and observe the trends of the proportion of deaths at the scene, in an ambulance, and a hospital. We analyzed all forensic examination reports (protocols) of those who died in the car due to a traffic accident in Moscow from 2018 to 2020. Male drivers died 15 times more often than female drivers, and male passengers died twice as often as females. Most of the victims were young and middle-aged. Both drivers and passengers most commonly had traumatic brain injury with fractures of the bones of the vault and base

A Stepwise Guide to Performing Shoulder Ultrasound: The Acromio-Clavicular Joint, Biceps, Subscapularis, Impingement, Supraspinatus Protocol.

Shoulder pain is a common and painful patient condition. Unfortunately, diagnostic imaging of shoulder pain in the emergency department (ED) is often limited to radiography. While diagnostic for fractures and dislocations, drawbacks of radiography include time delays and non-diagnostic imaging in the case of rotator cuff pathology. While bedside ultrasound has been incorporated into many procedural and diagnostic applications in the ED, its use for musculoskeletal complaints and specifically shoulder pain is infrequent among ED clinicians. The incorporation of shoulder ultrasound in the ED may

CMS' Hospital-Acquired Conditions for the Neurohospitalist.

In 2008, the Centers for Medicare and Medicaid Services (CMS) implemented a provision that denies Medicare payment for hospital-acquired conditions (HACs). This provision brings attention to the quality of patient care and the financial impact associated with "never-events" occurring during a patient's hospitalization. Our review of HACs focuses on the 5 which are most pertinent to the neurohospitalist: stages III and IV pressure ulcers, catheter-associated urinary tract infection, vascular catheter-associated infection, manifestations of poor glycemic control, and falls resulting in fractures

Traumatic lumbosacral pseudomeningocele associated with spinal fracture.

BACKGROUND CONTEXT: Traumatic pseudomeningocele and avulsion of lumbosacral nerve roots are rare in the absence of fractures of the pelvic ring or hip dislocation. Clear prognoses for the root avulsion or pseudomeningocele do not exist. PURPOSE: To present a patient who developed a posttraumatic pseudomeningocele after lumbosacral injury. STUDY DESIGN: Case report and review of the literature. METHODS: A patient who sustained traumatic pseudomeningocele of the lumbar spine was treated nonsurgically and followed over a 2-year period. RESULTS: Nerve root deficit largely recovered, although a

Corrective osteotomy for volar instability of the distal radioulnar Joint associated with radial shaft malunion.

Volar instability of the distal radioulnar joint is an uncommon wrist disorder. We report three cases of recurrent volar instability of the distal radioulnar joint secondary to fracture of the radial shaft. In all cases, X-rays showed a volar apex deformity of the radial shaft. Opening wedge osteotomy and iliac bone grafting was performed on the distal diaphysis of the radius instead of on the radial shaft, in order to adjust the distal radioulnar joint more easily. Pre-operative dislocations and painful clunks disappeared in all three patients. However, slight instability of the distal radiou

Traumatic bilateral locked facet at L4-5: report of a case associated with incorrect use of a three-point seatbelt.

We present a case of traumatic L4-5 bilateral facet dislocation, without neurological deficit, in a 32-year-old female patient, as an unusual seatbelt injury caused by positioning the shoulder harness improperly under her armpit. Open reduction, posterior interbody fusion, and posterior segmental instrumentation were carried out. The aim of this report is to describe this rarely encountered condition and speculate regarding automotive shoulder harness misuse as a potential cause of bilateral locked facet at L4-5, and to emphasize the importance of multidirectional X-ray on first examination. Reduction of skiing-related anterior shoulder dislocation using Kocher's method without traction.

We evaluated the use of Kocher's original method (without humeral traction) for reduction of acute anterior glenohumeral dislocation in 28 alpine skiers and snowboarders at a single ski area during the 1995-1996 ski season. In all cases, reduction was begun within 1 hour of the acute injury. The Kocher method alone was successful in 23 (82%) patients. Of the patients having a successful reduction by means of the original Kocher technique, the mean reduction time was less than 5 minutes, and 9 (39%) of the reductions were achieved in less than 1 minute. Only 1 patient experienced discomfort sig
