Successful Urethrorectal Fistula Repair Using GelPOINT Path Anal Access Sheath in Pelvic Fracture Urethral Injury.

INTRODUCTION: We present a case of rectourethral fistula complicating urethral trauma, successfully treated using a transanal exposure device. CASE PRESENTATION: A male in his twenties was referred to us with a pelvic fracture urethral injury. Preoperative imaging revealed an urethrorectal fistula. Excision and primary anastomosis urethroplasty was performed transperineally. The fistula was primarily closed transanally via GelPOINT Path access sheath, which provided wide access to the anal canal. The closure was reinforced perineally, with a gracilis muscle flap interposition. Postoperative u

Femoral Neck Stress Fractures in Athletes and the Military.

➤: Femoral neck stress fractures (FNSFs) are an infrequent condition in athletic and military populations. ➤: A high index of suspicion with liberal use of magnetic resonance imaging (MRI) is vital for early recognition and treatment initiation. ➤: An associated hip effusion on MRI is a risk factor for an evolving stress injury and requires close assessment and consideration for repeat MRI. ➤: Stress reactions and stable, incomplete FNSFs (<50% of femoral neck width) can be treated nonsurgically. ➤: Surgical intervention is accepted for high-risk, incomplete (≥50% of femoral neck width), a

Salvage of Bone Transport with the Induced Membrane Technique in an Open Tibia Fracture with a Segmental Defect: A Case Report.

CASE: A 29-year-old man with an open tibia fracture and a 15-cm bone defect was treated with irrigation, debridement, intramedullary nailing, soft-tissue flap, and bone transport with a frame over the nail. He developed osteomyelitis of his bone transport segment close to docking, which required bone resection and an induced membrane technique to salvage his transport segment and achieve union. CONCLUSIONS: This report illustrates the combined use of bone transport and induced membrane technique to achieve union in a 15-cm tibial defect. It demonstrates the use of the Masquelet technique to s

A 10-year follow-up of asymptomatic Charcot hip joints caused by CIPA syndrome (congenital insensitivity to pain with anhidrosis) with failure of any surgical reconstructive treatment.

Congenital insensitivity to pain with anhidrosis (CIPA) is a rare genetic disorder. Patients with CIPA lack among other things the protective sensation of pain. Orthopaedic manifestations are fractures, infections and hip joint dislocation. We report a 10-year follow-up of late developing hip dysplasia with CIPA and the result of several reconstructive surgical procedures. The final outcome was asymptomatic Charcot hip joints with superior migration and dislocation of both hips without pain. In conformance with the literature standard operative techniques in these rare cases usually seem to fa

Multiple carpometacarpal dislocations and an associated fracture of the hamate: an uncommon injury.

Dislocation of the four ulnar carpometacarpal (CMC) joints with a concurrent fracture of the hamate is a rare injury, with a paucity of published cases. A great force is required to dislocate a CMC joint. Diagnosis can easily be missed, due to other serious associated injuries. Appropriate treatment of CMC joint dislocations usually leads to excellent outcomes. We present a case of dorsal dislocation of the four ulnar CMC joints after punching a wall. The injury was treated with a closed reduction and percutaneous transfixation with Kirschner-wires. Despite the extensive injury, the functional

Traumatic Pseudoaneurysm of the Internal Maxillary Artery: A Rare Life-Threatening Hemorrhage as a Complication of Maxillofacial Fractures.

Pseudoaneurysm of the internal maxillary artery due to a traumatic event is a rare condition. Pseudoaneurysms are usually directly produced by arteries break with extravasation of blood. The compressed perivascular tissue forms the wall of aneurysmal sac. Then, this sac gradually expands and can be damaged. It is rare to see pseudoaneurysms of IMA. They are usually associated with fracture of the neck of the mandible. To the best of our knowledge the pseudoaneurysm of the internal maxillary artery related to maxillofacial trauma is an event extremely rare in the literature and if not quickly m

Management of common sports-related injuries about the foot and ankle.

Foot and ankle injuries are commonplace in competitive sports. Improvements in injury surveillance programs and injury reporting have enabled physicians to better recognize and manage specific foot and ankle injuries, with a primary goal of efficient and safe return to play. Athletes are becoming stronger, faster, and better conditioned, and higher-energy injuries are becoming increasingly common. Close attention is required during examination to accurately identify such injuries as turf toe, ankle injuries, tarsometatarsal (ie, Lisfranc) injuries, and stress fractures. Early diagnosis and man

Correction of a severe torsional malunion of the metacarpus in a calf by transverse osteotomy, transfixation pinning and casting.

A four-month-old Holstein heifer was presented for evaluation of an angular limb deviation. Upon initial presentation, the heifer was not clinically lame. An external rotation of 90 degrees of the fetlock was observed in the left forelimb. Radiographs identified a healing closed spiral fracture of the third and fourth metacarpal bones, with a 90 degrees external torsion. A mid-metacarpal bone transverse osteotomy was performed after insertion of transcortical pins in the distal and proximal part of the third and fourth metacarpal bones and in the distal radius. The reduction was performed in o

The partial excision greenstick (PEG) osteotomy: a novel approach to the correction of clinodactyly in children's fingers.

Closing wedge osteotomy is an accepted technique for correcting bony malalignment. This study reports the results of a novel osteotomy technique used in children's hands. Excision of a partial wedge generates a 'greenstick' type of fracture which can then be reduced and stabilised using only intraosseous wires. Eight consecutive patients who underwent surgery of this type between March 2003 and January 2008 were reviewed retrospectively. Union was obtained in all cases and there was good bone alignment and range of movement. No significant technique-related complications were encountered. This

Caiman bite.

Caiman crocodilus, commonly called the spectacled caiman, is a very widely distributed resident of the western-hemisphere wetlands. Caiman bites to humans can cause trauma and infection. There are few reports of caiman bites; however, there is information about bites by other members of the same family, including Alligator mississippiensis. A case of acute caiman bite to the hand is described, including initial treatment and outcome. The bite resulted in multiple lacerations, interarticular fracture, and infection. One of the lacerations was closed, and the patient was started on moxifloxacin

[Fatal course after osteosynthesis in a 18-year-old male].

A previously healthy patient became comatose 8 h after osteosynthesis of a fractured femur and humerus. After an initially normal cranial computed tomography, extended lesions were found 10 h later in the control scan as an indication of an embolization in the basilar artery. After neurosurgeons assessed the situation to be unfavorable, no other interventions were conducted and 6 days later brain death was confirmed. During the subsequent heart transplantation the suspicion of a patent foramen ovale as the reason for the paradoxical cerebral embolization was confirmed. This tragic course raise

Sixth cranial nerve palsy following closed head injury in a child.

A five year old female had an isolated abducens nerve palsy following closed head injury. There was no associated skull fracture, haematoma, or other cranial nerve injury. The significance, frequency, and differential diagnosis of traumatic sixth cranial nerve injury is discussed, particularly in paediatric patients. Management is symptomatic; occlusion with an eye pad may be used if diplopia is significant. In young children alternate day occlusion of each eye will help prevent amblyopia. Most cases improve within three months and many resolve by six months. Residual palsy at six months is li

Thigh compartment syndrome in a football athlete: a case report and review of the literature.

Although contusions of the thigh are common in all sports, a compartment syndrome from closed blunt trauma without a femur fracture is rare. Thigh compartment syndrome is unusual due to increased compliance of the thigh to accommodate increased expansion from hematoma or third space fluid. Compartment syndrome of the thigh is characterized by unrelenting pain, swelling, and limited knee range of motion. A single case of a thigh compartment syndrome caused by a direct blow to the anterior aspect of the thigh from a football helmet during kickoff occurred. Immediate thigh fasciotomy was performe

The primary treatment of frontobasal and midfacial fractures in patients with head injuries.

The timing of operation can be especially difficult when frontobasal fractures are combined with other head injuries and midfacial injuries. In these cases, the head injury represents a considerable hazard to the patient and therefore it is mandatory to avoid any additional strain that might be caused by treating the midfacial injury first. On the other hand, because of poorer results with the secondary treatment, the surgical therapy for midfacial injuries should be achieved as early as possible. This report describes the results obtained in 68 patients managed over a period of 5 years and di

Re-operation for aortic and mitral prosthetic dysfunctions.

The overall incidence of re-operation and prosthetic valve endocarditis was low in the present series as mechanical prostheses were used predominantly. The prosthetic dysfunctions were less frequent following the primary implantation with Bjork Shiley prostheses, but high operative risk was associated with the clotted Bjork Shiley prostheses. We also had unusual experience of strut fracture and sticking of Bjork Shiley discs in the closed position in both aortic and mitral positions. The early deaths were nil since the use of cardioplegic protection. Intra-operative bleeding due to adhesions c

Post-traumatic anterior hypopituitarism.

Reports of anterior pituitary insufficiency following closed head trauma are unusual. In those cases reported, extensive endocrinologic evidence to support the clinical syndrome is often lacking. The present report documents a case of anterior pituitary insufficiency in a 14-year-old girl following a skull fracture. Endocrine evaluation one year after her automobile accident revealed hypopituitarism minifested by cachexia, hypothyroidism, hypoadrenocorticism, and hypogonadism with regression of secondary sex characteristics. Laboratory studies demonstrated deficiencies of both trophic and peri

Comparative evaluation of pinning and cast fixation vs. external fixation after lateral closing-wedge osteotomy for cubitus varus in children.

BACKGROUND: Cubitus varus has been regarded as a poor functional and cosmetic consequence of supracondylar humerus fracture in children. The aim of this study was to assess the clinical and radiologic outcomes of cubitus varus treatments based on fixation methods: Kirschner (K)-wire and cast fixation or external fixation. METHODS: Forty consecutive patients with cubitus varus secondary to supracondylar fractures were retrospectively enrolled between October 2015 and December 2018. Following lateral closing-wedge osteotomy, those undergoing K-wire and cast fixation were included in group A (n

Type H vessels: functions in bone development and diseases.

Type H vessels are specialized blood vessels found in the bone marrow that are closely associated with osteogenic activity. They are characterized by high expression of endomucin and CD31. Type H vessels form in the cancellous bone area during long bone development to provide adequate nutritional support for cells near the growth plate. They also influence the proliferation and differentiation of osteoprogenitors and osteoclasts in a paracrine manner, thereby creating a suitable microenvironment to facilitate new bone formation. Because of the close relationship between type H vessels and oste

Posterior osteotomy techniques for rigid cervical deformity correction.

Posterior cervical osteotomies are powerful techniques for the correction of rigid cervical deformity. These include a variety of subtypes including partial facet joint resection, complete facet joint resection, opening wedge osteotomy (OWO), and closing wedge osteotomy (CWO). The partial facet joint resection provides limited lordosis but can be applied across multiple levels and provides bony surface for fusion. Complete facet joint resection can also be performed across multiple segments for a cumulative effect and like the partial facet joint resection requires mobility of the anterior col
