BACKGROUND: To date, it remains unclear how amyloid plaques and neurofibrillary tangles are related to neural activation and, consequently, cognition in Alzheimer's disease (AD). Recent findings indicate that tau accumulation may drive hippocampal hyperactivity in cognitively normal aging, but it remains to be elucidated how tau accumulation is related to neural activation in AD.
OBJECTIVE: To determine whether the association between tau accumulation and hippocampal hyperactivation persists in mild cognitive impairment (MCI) and mild dementia or if the two measures dissociate with disease pr

Drugs targeting N-methyl-d-aspartate (NMDA) receptors and the ability to learn new associations have been proposed as adjunct treatments to boost the success of exposure therapy for anxiety disorders. However, the effects of the NMDA partial agonist d-cycloserine on psychological treatment have been mixed. We investigated potential neurocognitive mechanisms underlying the clinical effects of d-cycloserine-augmented exposure, to inform the optimal combination of this and similar agents with psychological treatment. Panic disorder patients were randomised to single-dose d-cycloserine (250 mg; N

Little is known about the neural functioning that underpins drug valuation and choice in addiction, including nicotine dependence. Following ad libitum smoking, 19 dependent smokers (smoked≥10/day) and 19 occasional smokers (smoked 0.5-5/week) completed a decision-making task. First, participants stated how much they were willing-to-pay for various amounts of cigarettes and shop vouchers. Second, during functional magnetic resonance imaging, participants decided if they wanted to buy these cigarettes and vouchers for a set amount of money. We examined decision-making behaviour and brain activi

INTRODUCTION: Osteoarthritis (OA) pain is a major cause of long-term disability and chronic pain in the adult population. One in five patients does not receive satisfactory pain relief, which reflects the complexity of chronic pain and the current lack of understanding of mechanisms of chronic pain. Recently, duloxetine has demonstrated clinically relevant pain relief, but only in half of treated patients with OA. Here, the aim is to investigate the neural mechanisms of pain relief and neural signatures that may predict treatment response to duloxetine in chronic knee OA pain.

Generalized anxiety disorder (GAD) is associated with brain functional and morphological changes in connected with emotional dysregulation and cognitive deficit. This study dealt with the neural functional deficits and metabolic abnormalities in working memory (WM) task with emotion-inducing distractors in patients with GAD. Fourteen patients with GAD and 14 healthy controls underwent functional magnetic resonance imaging (fMRI) and proton magnetic resonance spectroscopy ((1)H-MRS) at 3T. In response to the emotional distractors in WM tasks, the patients concurrently showed higher activity in

Early reports of cognitive and behavioural deficits in motor neuron disease might have been overlooked initially, but the concept of a frontotemporal dementia-motor neuron disease continuum has emerged during the past decade. Frontotemporal dementia-motor neuron disease is now recognised as an important dementia syndrome, which presents substantial challenges for diagnosis and management. Frontotemporal dementia, motor neuron disease, and frontotemporal dementia-motor neuron disease are characterised by overlapping patterns of TAR DNA binding protein (TDP-43) pathology, while the chromosome 9

Spatial variability in resting functional MRI (fMRI) brain networks has not been well studied in schizophrenia, a disease known for both neurodevelopmental and widespread anatomic changes. Motivated by abundant evidence of neuroanatomical variability from previous studies of schizophrenia, we draw upon a relatively new approach called independent vector analysis (IVA) to assess this variability in resting fMRI networks. IVA is a blind-source separation algorithm, which segregates fMRI data into temporally coherent but spatially independent networks and has been shown to be especially good at c

Social anxiety disorder (SAD) has been associated with hyper-reactivity in limbic brain regions like the amygdala, both during symptom provocation and emotional face processing tasks. In this functional magnetic resonance imaging study we sought to examine brain regions implicated in emotional face processing, and the connectivity between them, in patients with SAD (n=14) compared with healthy controls (n=12). We furthermore aimed to relate brain reactivity and connectivity to self-reported social anxiety symptom severity. SAD patients exhibited hyper-reactivity in the bilateral fusiform gyrus

INTRODUCTION: Longitudinal trajectories from healthy aging to Mild Cognitive Impairment and Alzheimer's Disease involve complex mechanisms.
METHODS: We evaluated five machine learning approaches (Random Forest, Support Vector Machines, Radial Basis Function Networks, Backpropagation Networks, Convolutional Neural Network) to assess the importance of potential predictive markers across the health-to-dementia continuum. Using the ADNI cohort across four phases (ADNI1, ADNIGO, ADNI2, ADNI3), we analyzed participants with distinct trajectories: stable, convertible, and reverse progression.

Uremic optic neuropathy (UON) is a rare but potentially reversible complication of advanced kidney failure characterized by optic disc edema and visual decline that improves with dialysis. We report the case of a 45-year-old man with no prior diagnosis of chronic kidney disease who presented with 3 months of progressive bilateral blurry vision, eye pain, and exertional fatigue. Ophthalmologic examination revealed bilateral optic disc edema with reduced visual acuity. Laboratory evaluation showed profound azotemia (blood urea nitrogen 182.9 mg/dL; creatinine 29.5 mg/dL), hyperkalemia, anemia, m

Thrombolytic therapy (TLT) is an effective treatment for ischemic stroke (IS). Three fibrinolytic agents are currently registered in the Russian Federation for the treatment of IS, including tenecteplase, which has demonstrated high efficacy and safety in several randomized controlled trials. We present three clinical case reports of patients with IS who, for the first time in the Russian Federation, underwent TLT with the domestically produced biosimilar of tenecteplase, Nektelisa. In all three patients with IS, we observed significant regression of neurological deficits after administration

Familial cerebral cavernous malformations (FCCM) are a heritable neurovascular disorder defined by clusters of dilated, thin-walled capillaries in the brain and spinal cord. Although rare, FCCM offers a tractable model for understanding how genetic disruptions in endothelial junction biology, mechanotransduction, and kinase signaling drive vascular instability in the central nervous system. Pathogenic loss-of-function variants converge on signaling abnormalities that promote barrier dysfunction, iron deposition, inflammation, and progressive lesional growth. Clinically, FCCM may manifest with

This report describes a case of bilateral linezolid-associated optic neuropathy in a 65-year-old woman treated for Mycobacterium abscessus infection and highlights the importance of early recognition and intervention. The patient developed progressive, painless bilateral visual decline after more than six months of linezolid therapy. Ophthalmic examination revealed optic disc edema (left greater than right), impaired color vision, and prolonged visual evoked potential latencies. Neuroimaging excluded compressive or inflammatory etiologies, while optical coherence tomography demonstrated bilate

Late-life depression involves neurodegeneration, inflammation, metabolic disruption, and psychosocial stress. Lipid metabolism has drawn increasing attention in this field. This review examines three interconnected aspects: peripheral metabolic disturbance, potential central lipid alterations, and dietary lipid regulation. Peripherally, adverse lipid profiles and lipidomic shifts correlate with depressive symptoms, cognitive decline, and cardiometabolic load. Mechanisms include inflammation, membrane remodeling, mitochondrial dysfunction, and oxidative stress. Centrally, the disorder may invol

Intracranial cardiac impulse propagation along penetrating arterioles is vital for both nutrient supply via blood circulation and waste clearance via CSF circulation. However, current neuroimaging methods are limited to simultaneously detecting impulse propagation at pial arteries, arterioles, and between them. We hypothesized that this propagation could be detected via paravascular CSF dynamics and that it may change with aging. Using dynamic diffusion-weighted imaging (dynDWI), we detected oscillatory CSF motion synchronized with the finger photoplethysmography in the subarachnoid space (SAS

Alzheimer's disease (AD) is an irreversible neurodegenerative disorder that poses a significant risk to human health and well-being. The high cost and invasiveness of neuroimaging and cerebrospinal fluid (CSF) analysis underscores the necessity for accessible early screening via blood samples. In this study, we developed an ultrasound-based strategy for emergent macroscopic that enhances the acoustic response enrichment of specific proteins by introducing functionalized microspheres. This customized macroscopic-directed aggregation method combines protein enrichment with specific fluorescent a

Post-traumatic stress disorder (PTSD) is a debilitating mental health condition resulting from exposure to traumatic events, marked by persistent psychological distress and impairment in daily functioning. Risk factors for PTSD include genetic predispositions, neurobiological factors, as well as psychosocial and environmental influences. Specific demographic groups, such as veterans, first responders, and individuals in high-risk environments, are more susceptible to developing the disorder. Despite growing research, there remain gaps in understanding the full pathophysiology of PTSD, and exis

There is growing evidence that the brain is not an immune-privileged area, with central nervous system (CNS) cells being susceptible to inflammation as the brain responds to infection. Among the myriad of pathogens capable of infiltrating the neural environment, cytomegalovirus (CMV) has been one of the most extensively studied. Notably, CMV has been implicated in a range of neuropsychiatric disorders, including schizophrenia (SCZ). The objective of this review is to provide a comprehensive analysis of the impact of CMV on cognitive function in SCZ. Our review covers studies that explore the i
