Lymphoplasmacytic chronic cholecystitis and biliary tract disease in patients with lymphoplasmacytic sclerosing pancreatitis.

Lymphoplasmacytic sclerosing pancreatitis (LPSP) represents a distinctive form of chronic pancreatitis characterized by diffuse fibroinflammatory infiltrates that can involve both the pancreatic ducts and acinar parenchyma. Several cases of inflammatory infiltrates within the gallbladder have been reported in association with LPSP, but the spectrum of gallbladder pathology in patients with LPSP has not been systematically reviewed. Many patients with LPSP have distal CBD fibrosis, strictures, and inflammation, features that overlap somewhat with primary sclerosing cholangitis (PSC). In PSC, a

Acute cholecystitis: sonographic-pathologic analysis.

The study was performed to assess the pathogenic basis of sonographically demonstrable changes in the gallbladder wall in acute cholecystitis in an attempt to predict the degree of inflammation and to define a set of sonographic criteria for the diagnosis of acute cholecystitis. Sonograms in a control group of 30 patients (group A) without biliary tract symptoms, ascites, or cholelithiasis and 24 patients (group B) with proven diagnosis of acute cholecystitis were reviewed. The histologic sections of the gallbladder wall in the cholecystitis patients were correlated with sonographic findings.

A History of Cholecystectomy.

Benign disease of the gallbladder is common and frequently treated by surgical removal of the gallbladder. Today, cholecystectomy is the most common operation performed by general surgeons, and the majority are done via minimally invasive techniques. However, during the early history of the surgical management of gallbladder disease, cholecystectomy was seldom performed due to perceived risks. We present a historical review of the management of benign gallbladder disease, delineating 4 distinct phases of the surgical approach to the disease. The management of benign gallbladder disease has dra

Chronically symptomatic patients with undetectable gall bladder on ultrasonography could benefit from early cholecystectomy.

90 percent of symptomatic patients undergoing cholecystectomy have cholelithiasis with 10% categorized as asymptomatic cholecystitis. In both instances, the gallbladder is evident on ultrasonography. In children with symptomatic biliary dyspepsia, the decision to proceed to cholecystectomy is made difficult if choleliths are not seen on ultrasonography. This decision is made even more difficult if the gallbladder itself is not seen on repeated imaging. In a cohort of 54 cholecystectomies, 3 cases, with recurrent right upper quadrant pain and undetectable gallbladders on repeat ultrasonography,

Torsion of gall bladder, a rare entity: a case report and review article.

INTRODUCTION: Gallbladder torsion is a rare entity, which is often difficult to diagnose preoperatively. Since its first description in 1898 by Wendel, there have been over 500 documented cases in the literature. It is defined as rotation of the gallbladder on its mesentery along the axis of the cystic duct and cystic artery. Gallbladder torsion is more frequently encountered in the elderly with peak incidence in the 65-75 year old group, and a 3:1 female predominance. Gallbladder torsion typically presents as an acute abdomen requiring emergency surgery, and most cases are found as a surprise

Role of cholecystokinetic agents in 99mTc-IDA cholescintigraphy.

Cholecystokinin (CCK) and its C-terminal octapeptide analog, Sincalide, have been utilized in two separate roles for the evaluation of gallbladder disease. These are: (1) prior to cholescintigraphy to evacuate the gallbladder and optimized subsequent filling with radiotracers, and (2) to study contractile function of visualizing gallbladders on cholecystography and cholescintigraphy. As a preparation for 99mTc-IDA studies, it clearly facilitates earlier gallbladder filling in patients with chronic cholecystitis, thereby ruling out complete cystic duct obstruction. The problem lies in the fact

An Update on the Lithogenic Mechanisms of Cholecystokinin a Receptor (CCKAR), an Important Gallstone Gene for Lith13.

The cholecystokinin A receptor (CCKAR) is expressed predominantly in the gallbladder and small intestine in the digestive system, where it is responsible for CCK's regulation of gallbladder and small intestinal motility. The effect of CCKAR on small intestinal transit is a physiological response for regulating intestinal cholesterol absorption. The Cckar gene has been identified to be an important gallstone gene, Lith13, in inbred mice by a powerful quantitative trait locus analysis. Knockout of the Cckar gene in mice enhances cholesterol cholelithogenesis by impairing gallbladder contraction

Acalculous biliary pain: cholecystectomy alleviates symptoms in patients with abnormal cholescintigraphy.

A 45-minute infusion of an octapeptide of cholecystokinin (Kinevac; Squibb Diagnostics, New Brunswick, NJ) was used to measure the gallbladder ejection fraction during cholescintigraphy in 40 normal volunteers. Cholecystokinin cholescintigraphy was shown to be a reproducible test. The maximum mean gallbladder ejection fraction occurred 15 minutes after cholecystokinin infusion and was 74.5% +/- 1.9% (mean +/- SEM). A gallbladder ejection fraction greater than 40% (mean -3SD) was arbitrarily defined to be normal. The gallbladder ejection fraction test was then used to identify patients with aca

Acute cholecystitis treated by direct visualization endoscopy.

The common treatment of acute cholecystitis due to cholestasis is percutaneous transhepatic gallbladder drainage (PTGBD) or EUS-guided gallbladder drainage (EUS-GBD) or cholecystectomy. The new generation of direct vision endoscopy represented by Spy glass has successfully entered the gallbladder duct and gallbladder. On this basis, we apply similar direct visualization system for treatment.

Is Prophylactic Drainage Tube Placement Clinically Useful After Laparoscopic Cholecystectomy in Patients With Gallbladder Drainage for Acute Cholecystitis? A Propensity Score-Matched Study: A Secondary Analysis of the CSGO-HBP-017.

BACKGROUND: The aim of the study was to investigate whether prophylactic drainage tube (PD) is clinically useful after laparoscopic cholecystectomy (LC) in patients who undergo gallbladder drainage for acute cholecystitis (AC). METHOD: Using data from our previous multicenter retrospective study exploring the optimal timing of LC after gallbladder drainage for AC, we performed a secondary analysis in which we compared surgical outcomes between patients who underwent gallbladder drainage and subsequent LC for AC with (n = 134) or without (n = 149) PD placement. The results of the comparison we

Investigation of the association between gall bladder wall thickness and hypoalbuminaemia in dogs.

OBJECTIVES: To assess the correlation between gallbladder wall thickness and serum/plasma albumin concentrations in dogs. MATERIALS AND METHODS: Retrospective searches of digital medical record databases were conducted to identify dogs that had either severely low serum/plasma albumin concentration (<1.5 g/dL) or ultrasonographic evidence of gallbladder wall thickening (>2 mm). Analysis of covariance models were used to analyze gallbladder wall thickness with sample type (serum vs plasma), age, etiology, albumin, and albumin ' etiology as the covariates. RESULTS: A total of 216 dogs met incl

Evaluation of Preoperative Ultrasonography in Gall Stone Disease to Predict Technical Difficulties during Laparoscopic Cholecystectomy.

Laparoscopic cholecystectomy is the gold standard in the treatment of gallstones. This study was carried out to determine the technical difficulties that can be encountered during laparoscopic cholecystectomy and to evaluate the role of ultrasonography in identifying patients at risk of conversion returning to open surgery. The objective of this study was to evaluate the efficacy of preoperative ultrasonography in assessing technical difficulty faced during laparoscopic cholecystectomy in gallstone disease. This was an observational comparative study which was conducted in the Department of Su

Robotic cholecystectomy is safe and effective for all levels of gallbladder pathology in both the elective and emergent setting in a patient population with a high comorbidity load: outcomes from East Flatbush, New York.

The value of using the Da Vinci robotic platform to perform a cholecystectomy is still under investigation, particularly within emergent settings. The aim of our study is to analyze the safety, efficacy, and clinical outcomes associated with robotic cholecystectomy among patients with a high comorbidity load, comparative health disparities, and varying degrees of gallbladder pathology. To measure and compare 30-day postoperative complications seen in the robot-assisted cholecystectomy in a patient population with a high comorbidity load. We conducted a single-institution, retrospective analysi

Influence of Percutaneous Transhepatic Gallbladder Aspiration and Drainage for Severe Acute Cholecystitis on the Surgical Outcomes of Subsequent Laparoscopic Cholecystectomy: Post Hoc Analysis of the CSGO-HBP-017 (CSGO-HBP-017C).

OBJECTIVE: Percutaneous transhepatic gallbladder aspiration (PTGBA) and/or drainage (PTGBD) are useful approaches in the management of acute cholecystitis in patients who cannot tolerate surgery because of poor general condition or severe inflammation. However, reports regarding its effect on the surgical outcomes of subsequent laparoscopic cholecystectomy (LC) are sparse. The aim of this retrospective study was to investigate the influence of PTGBA on surgical outcomes of subsequent LC by comparing the only-PTGBA group, including patients who did not need the additional-PTGBD, versus the addi

Chronic cholecystitis and the oral cholecystogram.

Gallbladder image density at oral cholecystography has been compared with histological grading of the severity of chronic cholecystitis in resulting cholecystectomy specimens. (i) There was a linear relationship between gallbladder opacification and the grade of chronic cholecystitis. (ii) Removal of patients with either non-opacified gallbladders or abnormal liver function did not alter the linear trend. (iii) The occurrence of gallstones was independent of the degree of gallbladder opacification. (iv) The presence of gallstones did not correlate with the severity of chronic cholecystitis. A

Phrygian cap simulating mass lesion in hepatobiliary scintigraphy.

Hepatobiliary imaging has been used to demonstrate that defects in the region of the gallbladder fossa in sulfur colloid studies correspond to normal gallbladder and not tumor. We report a case of delayed filling of a portion of the gallbladder due to a Phrygian cap anomaly. The case demonstrates that without delayed images incomplete filling of the gallbladder can be confused with a mass lesion in the liver.
