Radiation dermatitis, burns, and recall phenomena: Meaningful instances of immunocompromised district.

Ionizing and ultraviolet radiations, as well as burns, can selectively damage and immunologically mark the cutaneous area they act on through direct and indirect mechanisms. After the causal event has disappeared, the affected skin district may appear clinically normal, but its immune behavior is often compromised forever. In fact, irradiated or burned skin areas undergo a destabilization of the immune control, which can lead to either a reduction of immunity (as suggested by the facilitated local occurrence of tumors and infections) or an excess of it (as suggested by the possible local onset

A novel procedure for transtracheal resection for recurrent thyroid cancer involving a trachea and esophagus.

BACKGROUND: Surgery remains the main treatment for locally advanced thyroid cancers invading the trachea, esophagus, and recurrent laryngeal nerve. However, extensive resection of such tumors can sometimes involve difficulties and may result in the deterioration of the patient's quality of life. The surgeon should consider not only the patient's prognosis but also the preservation of postoperative function. METHODS: This report describes a minimally invasive surgical procedure for recurrent poorly differentiated papillary thyroid carcinoma involving the trachea and esophagus. To decrease the

Evaluation of stapled versus hand-sewn techniques for colo- rectal anastomosis after low anterior resection of mid-rectal carcinoma: a study on 50 patients.

AIM: To evaluate the outcome of stapled versus sutured colo-rectal anastomosis after low anterior resection of mid-rectal carcinoma. PATIENTS AND METHODS: A prospective study of fifty patients who underwent colo-rectal anastomosis following low anterior resection (LAR) of T2 mid-rectal cancers at the Egyptian National Cancer Institute during the time period from June 2010 to June 2013 was conducted. Classification was into two groups; a stapled anastomosis group I (25 patients) and a hand-sewn anastomosis group II (25 patients). All operations are evaluated regarding intra-operative complicat

All-arthroscopic repair of Palmer 1B triangular fibrocartilage complex tears using the FasT-Fix device.

PURPOSE: The FasT-Fix device (Smith and Nephew Endoscopy, Andover, MA), initially developed for knee meniscal tears, is described for all-arthroscopic triangular fibrocartilage complex (TFCC) repairs. Potential benefits of this technique are ease of use, the lack of prominent suture knots, and strength of repair. This case series evaluates the early clinical outcomes of this technique. METHODS: We conducted a retrospective review of patients with TFCC Palmer type 1B injuries treated by 1 hand surgeon from 2005 to 2009. The patients' charts were reviewed for postoperative complications, range

Psychogenic movement disorders.

PURPOSE OF REVIEW: This review focuses on recent studies assessing clinical features and laboratory findings that may help diagnose psychogenic movement disorders, and the ongoing controversy about the relationship of these disorders with preceding peripheral injury. RECENT FINDINGS: 'Organic' movement disorders may still be misdiagnosed as psychogenic. Probably more commonly, however, psychogenic movement disorders are underdiagnosed. Most features typically associated with recognized movement disorders, including geste antagoniste or treatment-induced dyskinesias, can be seen in psychogenic

Clinical impact of MRI in acute wrist fractures.

The purpose of this study was to evaluate the clinical impact of MRI in the early diagnosis of wrist trauma. High-resolution MR imaging was performed on a 1.5-T unit (Symphony Quantum, Siemens, Erlangen, Germany) using coronal and axial T1- and T2-weighted fat-saturated turbo-spin-echo sequence via a dedicated wrist coil within a mean of 6.6 days after initial radiographs in 54 patients (56 wrists) with clinical suspicion of wrist fractures and normal plain or indistinct radiographs. Initial radiographs were evaluated independently by two senior radiologists and the hand surgeon without knowle

Fifth Metacarpal Fracture in 65-Year-Old Female.

Metacarpal fractures account for 30% to 40% of all hand fractures. These fractures usually occur in adolescents and young adults. The mechanism of injury is often direct trauma, but can also occur with an axial load or torsional loading (twisting). Fractures of the metacarpals are subdivided into the metacarpal head, neck, shaft, and base. The most common metacarpal fractures involve the neck of the fifth metacarpal, known as the boxer fracture. Fifth metacarpal fractures can involve the head, neck, shaft, or base. This article highlights a patient with a fifth metacarpal fracture near the bas

Noninvasive Respiratory Support in Acute Respiratory Distress Syndrome.

Noninvasive respiratory supports have been successfully used as an alternative to endotracheal intubation especially in patients with a milder degree of hypoxemia. In patients with acute respiratory distress syndrome (ARDS), the main goals of noninvasive oxygenation strategies are to improve oxygenation, unload the respiratory muscles, and relieve dyspnea. On the other hand, recent studies have suggested that spontaneous breathing could represent an additional mechanism of lung injury, especially in the more severe forms. The aim of this review is to describe the role of different noninvasive

Thumb Metacarpal Periosteal Sleeve Avulsion: A Case Report.

Periosteal sleeve fractures, or avulsions of cartilage and/or periosteum with or without an osseous fragment in skeletally immature individuals, are notoriously easy to miss and a high index of suspicion is necessary for accurate diagnosis and treatment. While periosteal sleeve avulsion fractures are classically reported in the patella, they have also been reported in the shoulder, clavicle, and elsewhere in the knee. However, no published reports exist for a periosteal sleeve avulsion fracture in the hand. This case details the first reported instance of such an injury involving a thumb metac

Wide awake surgery for rupture of the lateral expansion of the long-finger extensor tendon: A case report.

Spontaneous extensor digitorum communis (EDC) tendon rupture is uncommon but easily confusing injury that lead to functional impairment. There is no any strict first-line treatment for nonrheumatoid EDC tendon rupture. We report a kidney transplant recipient with spontaneous rupture of the lateral expansion of the long-finger extensor tendon treated by wide awake surgery. Surgical repair with wide-awake local anesthesia was performed to realign the tendon for the patient. Three months following surgery, the patient obtained a full pain-free range of motion of the hand without recurrent disloca

Bone-Retinaculum-Bone Autografts for Scapholunate Interosseous Ligament Reconstruction.

Carpal instability arising from an injury to the scapholunate interosseous ligament (SLIL) is commonly seen and treated by hand surgeons. No technique to this date has proved to provide optimal results for primary repair of acute SLIL tear and the treatment of chronic tears of the SLIL. Recently, attention has shifted toward replacement of the dorsal aspect of the SLIL, which is the most structurally and functionally important aspect of the SLIL. This article describes the indications, surgical technique, postoperative treatment and expected results of the use of a bone-retinaculum-bone autogr

Iatrogenic Mycobacterium abscessus infection in a trigger finger.

An immunocompetent 63-year-old lady developed Mycobacterium abscessus soft tissue infection of the hand following local corticosteroid injection for trigger finger. The patient was successfully treated with repeated radical debridement and prolonged antimicrobial therapy. Atypical mycobacterial infections, including those caused by M. abscessus, albeit rare, should be considered in cases of late-onset indolent infection following local injury surgical procedures, and injections. Clinical vigilance, timely diagnosis, combined directed antimicrobial treatment, coupled with adequate surgical debr

Tracheo-innominate artery fistula: two cases.

Tracheo-innominate artery fistula (TIF) is a rare but catastrophic and almost always fatal complication of tracheostomy. Two surgically intervened TIF cases are presented. In both cases, innominate artery ligation was performed. The first patient died due to respiratory failure 10 hours after operation. The second patient was discharged with normal neurologic examination on the 12th day of operation. Since the mechanism of injury leading to TIF is pressure necrosis, it is of vital importance to be aware of the predisposing factors and to take preventive measures. On the other hand, only patien

Outcome of conservative management of spiral/long oblique fractures of the metacarpal shaft of the fingers using a palmar wrist splint and immediate mobilisation of the fingers.

Conservative management of spiral/long oblique fractures of the shaft of the metacarpal almost always results in shortening and hence these fractures have the reputation of ending up with an extension lag and reduced grip strength. In a prospective study, 42 patients with 54 fractures were treated with a palmar wrist splint and immediate mobilisation of all finger joints. All fractures united. Extension lag of fractured fingers was initially seen in all fingers, but eventually recovered. Grip strength progressively increased to reach a mean of 94% of the contralateral hand by 1 year after inju

Robot-aided sensorimotor arm training methods based on neurological rehabilitation principles in stroke and brain injury patients.

We developed the upper extremity compound movements (UECM) rehabilitation training robot and designed straight-line paths tracking and circle paths tracking exercise based on neurological rehabilitation principles. Rehabilitative exercises should be on hand paths rather than on individual joints. robot can guide patients during forward reaching movements. The exterior oriented circle movement performed the coactivation of shoulder abductors with elbow extensors and the coactivation of shoulder adductors with elbow flexors at most of tracks against the stereotypic movement pattern in stroke and

Pectoralis major ruptures postsuction lipectomy for surgical management of gynecomastia.

Over the last decade, suction-assisted lipoplasty has been shown to be a safe and reliable procedure in the hands of trained individuals. A large number of plastic surgeons employ liposuction as an adjunct to surgery for gynecomastia with excellent results and low morbidity. An unusual problem, traumatic rupture of the pectoralis major muscle, has been observed by us in a patient who had undergone the procedure. The physical deficit arising from the injury may go unnoticed in all but the most physically active people, and surgeons should be wary of the possibility of this previously unreported

The structural basis of Felice Fontana's spiral bands and their relationship to nerve injury.

In 1779, Felice Fontana, using a six-power hand-held magnifying lens, described what appeared to be spiral bands surrounding the peripheral nerve. He hypothesized that these bands were due to an optical illusion related to the underlying undulations of the individual nerve fibers, which he was the first to observe with an early microscope. The present study examines the historical basis of Fontana's work, confirms with intrafascicular dissection that the bands are an illusion created by unstretched nerve fibers, and relates their clinical disappearance to current concepts of the pathophysiolog

Humeral head osteonecrosis caused by electrical injury: a case report.

Various musculoskeletal abnormalities caused by electrical injury are described. Such abnormalities usually include fractures or dislocation of adjacent bones and joints. Osteonecrosis is a noteworthy, but less common, consequence of electric shock. The case is discussed of a 52-year-old woman who had received an electric shock (220-V alternating household current) to the right hand and developed osteonecrosis in the ipsilateral humeral head, most likely caused by bone "melting." An osteonecrotic lesion may therefore develop in a joint at a distance from the point of electrical contact, and th

Anomalous facial nerve: the otologic surgeons' abyss.

Iatrogenic injury to the facial nerve is certainly the most apparent if not the most feared complication faced by the otologic surgeon. The temporal bone with its tightly packed sensory structures is no place for the "occasional operator". A thorough knowledge of temporal bone anatomy is of course required. On occasion, as in other areas of life, one encounters the unexpected and must be prepared to "adjust" to the situation at hand. An instructional case involving an anomalous facial nerve is presented. A review of the developmental anatomy of the facial nerve, preoperative diagnostic imaging

[Late results after meniscectomy].

Lesion of the meniscus is the most frequently occurring lesion in the region of the knee joint. Opinions are still divided as to the effects of partial, subtotal or total meniscectomy in provoking and/or promoting arthrosis. In this study, 70 patients with isolated injury of the meniscus were followed up on an average 11 years after meniscectomy. The subjective and clinical findings are juxtaposed with the x-ray findings. It was found that no arthrosis or no increase of the already preoperatively existing degree of arthrosis was seen in only 21.4% of the patients. Correlations are set up betwe
