Single Patient Multiple Explosions: A Case Report on Exploding Head Syndrome.

Exploding head syndrome (EHS) is an uncommon sleeping disorder that is described by patients as a loud noise occurring while transitioning into and out of sleep. It is not accompanied by a headache but causes a sense of fright. We describe the case of a 58-year-old female patient, presenting with a total of 11 events of EHS occurring at bedtime. Events shared some features but also had unique characteristics. The workup was negative, making a diagnosis of EHS more likely. CPAP was recommended for a newly diagnosed obstructive sleep apnea, but despite non-compliance with the treatment plan, the

Head Shape After Sagittal Craniosynostosis Surgery: Open Versus Endoscopic Strip Craniectomy.

Purpose: Nonsyndromic sagittal craniosynostosis is treated surgically to improve skull cosmesis and to decrease the risk of raised intracranial pressure. The purpose of this study is to compare the outcomes of two current surgical techniques for craniosynostosis treatment, open and endoscopic strip craniectomy. Methods: A single institution retrospective chart review was conducted of patients with nonsyndromic sagittal craniosynostosis treated surgically from 2011 to 2016. Patients were divided into two groups based on surgical technique: open or endoscopic strip craniectomy. The head shape wa

Femoral head cartilage reconstruction using autologous osteochondral mosaicplasty: A case report.

RATIONALE: Cartilage injuries of the femoral head may occur following hip dislocation. As a rare injury, controversy persists regarding ideal treatment of damaged femoral head cartilage. Here we report the case of a patient who developed a large cartilage injury to the femoral head following anterior hip dislocation for which autologous osteochondral mosaicplasty with a graft harvested from the ipsilateral femoral head achieved a satisfactory outcome. PATIENT CONCERNS: A 62-year-old man developed a right hip dislocation after a fall from a 5-m height and was referred to our institution. DIAG

Sclerotic zone in femoral head necrosis: from pathophysiology to therapeutic implications.

This review summarizes the sclerotic zone's pathophysiology, characterization, formation process, and impact on femoral head necrosis. The sclerotic zone is a reaction interface formed during the repair of femoral head necrosis. Compared with normal bone tissue, the mechanical properties of the sclerotic zone are significantly enhanced. Many factors influence the formation of the sclerotic zone, including mechanics, bone metabolism, angiogenesis, and other biological processes. The sclerotic zone plays an essential role in preventing the collapse of the femoral head and can predict the risk of

Somatostatin Receptor-PET/CT/MRI of Head and Neck Neuroendocrine Tumors.

BACKGROUND AND PURPOSE: Due to its high sensitivity, somatostatin receptor-PET may detect smaller lesions and more extensive disease than contrast-enhanced MR imaging, while the superior spatial resolution of MR imaging enables lesions to be accurately localized. We compared results of somatostatin receptor-PET/MRI with those of MR imaging alone and assessed the added value of vertex-to-thigh imaging for head and neck neuroendocrine tumors. MATERIALS AND METHODS: Somatostatin receptor-PET/CT was acquired as limited brain or head and neck imaging, with optional vertex-to-thigh imaging, followi

Reversible positioning head tilt observed in 14 cats with hypokalaemic myopathy.

CASE SERIES SUMMARY: Positioning head tilt (PHT) is a dynamic neurological sign in which the head tilts to the opposite side to which it is moving. This sign is triggered in response to head movement and is thought to be due to the lack of inhibition of vestibular nuclei by the cerebellar nodulus and uvula (NU). The occurrence of PHT in animals has been suggested to be an indicator of NU dysfunction. Here, we describe the acute onset of PHT in 14 cats. All the cats were diagnosed with hypokalaemic myopathy caused by a range of pathologies. The PHT resolved along with other signs related to myo

Ictal head roll: a seizure semiology from the anterior prefrontal lobe.

Longstanding epilepsy can lead to modulation of cortical networks over time and unexpected seizure onset zones. Frontal lobe seizures, in particular, can have diverse semiologies and evolution patterns. We present a male patient with drug-resistant epilepsy secondary to severe traumatic brain injury who underwent bilateral stereo electroencephalography (SEEG) for surgical planning. SEEG localized an ictal circular head roll to the right anterior prefrontal region. This was followed by spread to the left orbitofrontal region and later the left amygdala and hippocampus, at which point a differen

18F-fluorodeoxyglucose PET/Computed Tomography: Head and Neck Salivary Gland Tumors.

Salivary gland tumors (SGTs) are a heterogeneous group of neoplasms arising from the 3 pairs of major salivary glands (parotid, submandibular, and sublingual) or numerous minor salivary glands located throughout the oral cavity. This review discusses the role of PET/computed tomography (CT) in evaluation of SGTs, including staging, restaging, prognostication, and response assessment. 18F-fluorodeoxyglucose (FDG) PET/CT is useful for staging and restaging malignant SGTs and offers important prognostic information in these patients. It is less useful for differentiating benign and malignant SGTs

Distant metastasis from head and neck cancer: role of surgical treatment.

PURPOSE OF REVIEW: The rates of distant metastases in patients with head and neck squamous cell carcinoma varies from 3 to 50%. Metastases are usually multiple, diagnosed within 24 months after treatment and sited in the lungs, bone or liver. This review highlights the importance of a personalized treatment approach in such patients. RECENT FINDINGS: In patients with primary tumor controlled and with oligometastatic disease, surgical options can be considered, especially for lung metastases. Overall survival for patients who are candidates for lung metastasectomy can be as high as 59%. In bon

Computed Tomography Scans in Children With Hemophilia After Minor Head Trauma.

OBJECTIVES: Children with hemophilia have the usual childhood risk of falls and head trauma. Head computed tomographies (HCTs) are fast, detailed, and readily available, but increased radiation exposure in the pediatric population is now recognized as causing increased brain malignancy. By examining the incidence of intracranial cerebral hemorrhage in this population, we will be able to weigh risks and benefits of HCT use more accurately. METHODS: Using a retrospective chart review, we examined past medical records of pediatric patients, aged 0 to 15 years, with hemophilia presenting to 1 aca

Routine Repeat Imaging Is Unnecessary for Coagulopathic Patients Sustaining Head Trauma.

INTRODUCTION: In trauma patients using warfarin, current guidelines recommend computed tomography of the brain (CTH), 24-hour observation, and repeat CTH to monitor for stability. Despite growing evidence of uncommon delayed hemorrhage, this remains standard practice even in mild traumatic brain injury without intracranial hemorrhage (ICH). Our study sought to determine the incidence and outcomes of delayed ICH (DICH) in trauma patients on supra-therapeutic warfarin without initial ICH. METHODS: A retrospective, single institutional study was performed of all adult trauma patients (>18 years

Consensus Head Acceleration Measurement Practices (CHAMP): Origins, Methods, Transparency and Disclosure.

The use of head kinematic measurement devices has recently proliferated owing to technology advances that make such measurement more feasible. In parallel, demand to understand the biomechanics of head impacts and injury in sports and the military has increased as the burden of such loading on the brain has received focused attention. As a result, the field has matured to the point of needing methodological guidelines to improve the rigor and consistency of research and reduce the risk of scientific bias. To this end, a diverse group of scientists undertook a comprehensive effort to define cur

Defining Experimental Variability in Actuator-Driven Closed Head Impact in Rats.

Traumatic brain injury (TBI) is a world-wide health challenge that lacks tools for diagnosis and treatment. There is a need for translational preclinical models to effectively design clinical tools, however, the diversity of models is a barrier to reproducible studies. Actuator-driven closed head impact (AD-CHI) models have translational advantages in replicating the pathophysiological and behavioral outcomes resulting from impact TBI. The main advantages of AD-CHI protocols include versatility of impact parameters such as impact angle, velocity, depth, and dwell time with the ability to inter

Diffusion Imaging of Sport-related Repetitive Head Impacts-A Systematic Review.

Repetitive head impacts (RHI) are commonly observed in athletes participating in contact sports such as American football, ice hockey, and soccer. RHI usually do not result in acute symptoms and are therefore often referred to as subclinical or "subconcussive" head impacts. Epidemiological studies report an association between exposure to RHI and an increased risk for the development of neurodegenerative diseases. Diffusion magnetic resonance imaging (dMRI) has emerged as particularly promising for the detection of subtle alterations in brain microstructure following exposure to sport-related
