The left ventricle in atrial septal defect: Looking through 3D glasses.

OBJECTIVES: To identify left ventricular (LV) dyssynchrony and associated factors in atrial septal defect (ASD) patients, using real time three-dimensional echocardiogram (RT3DE). BACKGROUND: Left ventricular dysfunction has been observed in ASD. Few studies have utilized RT3DE to assess LV wall-motion abnormality in ASD. METHODS: Patients aged ≥1 year with ASD, with or without partial anomalous pulmonary venous drainage (PAPVD), were included over 1 year. Additional cardiac defects or abnormalities independently affecting LV function were excluded. 2DE and RT3DE-derived LV function data wer

MEIS2 gene is responsible for intellectual disability, cardiac defects and a distinct facial phenotype.

Myeloid ecotropic insertion site 2 (MEIS2) gene, encoding a homeodomain-containing transcription factor, has been recently related to syndromic intellectual disability with cleft palate and cardiac defects. Here, we present a male patient, aged 10, with cardiac defects, intellectual disability, facial dysmorphisms and gastroesophageal reflux. Whole exome sequencing revealed a novel de novo nonsense mutation in the MEIS2 gene. This patient represents another reported case with a de novo MEIS2 point mutation and helps to characterize a distinct facial phenotype consisting in low anterior hairlin

Systemic-to-pulmonary shunt vs right ventricle to pulmonary artery connection in the treatment of pulmonary atresia, ventricular septal defect, and major aortopulmonary collateral arteries.

OBJECTIVE: The systemic-to-pulmonary shunt (SPS) and right ventricle to pulmonary artery (RV-PA) connection were evaluated to pursue the goal of rehabilitating dysplastic native PAs via establishment of antegrade blood flow. However, the application of these two palliative operations was still confusing. We compared the two operations to determine their different effects on patients who have pulmonary atresia, ventricular septal defects, and major aortopulmonary collateral arteries (MAPCAs). METHODS: From January 2011 to January 2016, 44 patients received the SPS procedure, and 54 patients re

Recommendations for management of equine athletes with cardiovascular abnormalities.

Murmurs and arrhythmias are commonly detected in equine athletes. Assessing the relevance of these cardiovascular abnormalities in the performance horse can be challenging. Determining the impact of a cardiovascular disorder on performance, life expectancy, horse and rider or driver safety relative to the owner's future expectations is paramount. A comprehensive assessment of the cardiovascular abnormality detected is essential to determine its severity and achieve these aims. This consensus statement presents a general approach to the assessment of cardiovascular abnormalities, followed by a

Surgical closure of apical multiple muscular septal defects via right ventriculotomy using a single patch with intermediate fixings.

BACKGROUND: Multiple apical muscular ventricular septal defects (VSDs) remain a challenge for surgeons because of their anatomical features. We used single patch with intermediate fixations to repair multiple apical muscular VSDs through right ventriculotomy. METHODS: We analysed the data of 16 children (median age 8 months, range 2 months to 144 months) with multiple apical muscular VSDs who underwent a single patch technique via apical right ventriculotomy. Perioperative data were collected and analysed, and the patients were followed up for three months to 66 months (median, 46 months) to

Repairing the posterior postinfarction ventricular septal defect: a left ventricular approach with a sealant reinforced multipatch technique.

An uncommon complication of acute myocardial infarction (AMI), postinfarction ventricular septal defect (PI-VSD), often yields devastating outcomes. Because of the strikingly poor quality of the residual tissue, the repair of PI-VSD poses a surgical challenge and is associated with high operative mortality as well as residual or recurrent shunting. Among the various techniques that have been developed, we prefer a left ventricular approach to repairing PI-VSD by using a multipatch technique reinforced with a sealant as an adjunct to surgical repair. In this method, 3 patches are used: two over

Ventricular septal defect as a result of direct trauma from mechanical mitral valve prosthesis.

We present a case of a patient with preexisting mechanical mitral valve who developed a traumatic ventricular septal defect following a motor vehicle accident. Serial transthoracic echocardiography revealed the progressive nature of the defect. Surgical repair was delayed due to significant noncardiac comorbidities. The ventricular septal defect was found in an unusual location in the membranous portion of the interventricular septum. We discuss the possible etiologic mechanisms of injury as well as the importance of timely surgical repair.

Pattern of pulmonary venous flow in patients with ostium secundum atrial septal defect.

AIMS: The purpose of this study is to determine the possible influence of a left to right atrial shunt over the pulmonary venous flow pattern in children with ostium secundum atrial septal defect (ASD). METHODS AND RESULTS: Complete two-dimensional, spectral Doppler and colour flow Doppler examination were undertaken to examine the pulmonary vein flow pattern in 74 patients with ASD. This group was compared with a control group of 49 non-cardiological patients of similar ages and gender. Ultimately, pulmonary venous flow patterns changed. Pulmonary vein systolic velocities were significantly

[A long-term prognosis of ventricular septal defect associated with aortic regurgitation].

Fifty-one cases of ventricular septal defect associated with aortic regurgitation (VSD and AR) were examined for past twenty-two years. Only VSD closure was performed for AR cases under Sellers II regurgitation. After the average of 11.6 years' follow-up, 14 cases retained over 60 mmHg of diastolic pressure and belonged to NYHA functional class 1. VSD closure and aortic cusp suspension were performed for AR cases over Sellers II regurgitation. Three cases operated before 1969 died of cardiac failure and one case recently died of infectious endocarditis (IE). Because of residual AR, one case wa

Results of the Mustard operation for dextro-transposition of the great arteries.

One hundred twenty patients with dextro-transposition of the great arteries (TGA) underwent intra-arterial baffle repair using thin Dacron fabric from 1971 to 1979. The ages of the patients ranging from 29 days to 17 years (mean age 28 months). Thirty-five patients had undergone 49 preliminary palliative operations. Early postoperative mortality was 4.8% for patients with simple TGA but was higher among patients with associated ventricular septal defect (26%), VSD and left ventricular outflow tract obstruction (28.5%) and intact ventricular septum with left ventricular outflow tract obstructio

Double-outlet right ventricle: wide-angle two-dimensional echocardiographic observations.

M-mode echocardiographic delineation of double-outlet right ventricle (DORV) has relied primarily on the demonstration of mitral-semilunar valve discontinuity--a feature that requires an interpretation of spatial anatomic relationships. Thirty-six patients with DORV were examined by wide-angle real-time two-dimensional echocardiography. Anatomic diagnosis was established by surgery in 28 patients and by angiography alone in eight. Typical two-dimensional echocardiographic features were (1) parallel orientation and origin of both great arteries from the anterior right ventricle, (2) mitral-semi

Assessment of ventricular size and function in congenitally corrected transposition of the great arteries.

Twenty-four quantitative cineangiographic studies were performed in 19 patients with congenitally corrected transposition of the great arteries to assess right and left ventricular size and function. Ages ranged from 7 days to 44 years and associated lesions included ventricular septal defect (13 of 19), pulmonary stenosis (9 of 19), and systemic (tricuspid) valvular insufficiency (7 of 19). Systemic (anatomically right) ventricular end-diastolic volume was within normal limits in most patients and averaged 119% of predicted normal. Pulmonary (anatomically left) ventricular end-diastolic volum

Cardiac malformations and midline skeletal defects in mice lacking filamin A.

The X-linked gene filamin A (Flna) encodes a widely expressed actin-binding protein that crosslinks actin into orthogonal networks and interacts with a variety of other proteins including membrane proteins, integrins, transmembrane receptor complexes and second messengers, thus forming an important intracellular signalling scaffold. Heterozygous loss of function of human FLNA causes periventricular nodular heterotopia in females and is generally lethal (cause unknown) in hemizygous males. Missense FLNA mutations underlie a spectrum of disorders affecting both sexes that feature skeletal dyspla

Urgent right ventricular outflow tract stenting in a dual risks of thrombosis and bleeding in uncorrected adult with cyanotic congenital heart disease: a case report.

BACKGROUND: Uncorrected adults congenital heart disease (ACHD) may present with profound hypoxia and dual risks of thrombosis and bleeding, posing major challenges for definitive surgical repair. In such settings, right ventricular outflow tract (RVOT) stenting may provide a life-saving palliative option. CASE PRESENTATION: We report a 30-year-old woman with double-outlet right ventricle, subaortic ventricular septal defect, and severe infundibular pulmonary stenosis who suffered cardiac arrest due to profound hypoxia. Her clinical profile was complicated by polycythemia, upper extremity deep

A case report of late physiologic repair of congenitally corrected transposition of the great arteries and pulmonary stenosis in a severely cyanotic patient: better late than never.

BACKGROUND: Patients with congenitally corrected transposition of great arteries (ccTGA) not infrequently seek medical attention for the first time late in life. Optimal management of natural history ccTGA is debated and must be tailored. CASE SUMMARY: A 38-year-old male patient was referred to our centre because of severe cyanosis and worsening dyspnoea. Investigations disclosed situs solitus, mesocardia, double discordance, large ventricular septal defect (VSD), severe pulmonary stenosis, and no significant atrio-ventricular valves regurgitation. The patient underwent physiologic repair: VS
