Further characterization of the executive memory impairment following frontal lobe lesions.

In this article we describe a 41-year-old man who, following an operation to repair a ruptured anterior communicating artery aneurysm, manifested the "hallmark" features of a dysexecutive memory impairment. Of particular note was the patient's apparently normal level of recognition memory but impaired recall on tasks matched for difficulty in control subjects. However, further testing revealed that the patient's recognition memory was not normal under all circumstances. Implications of these data for the interpretation and further investigation of the dysexecutive deficit are discussed.

Basal interfalcine approach through a frontal sinusotomy with vein and nerve preservation. Technical note.

The authors describe a new method for a frontal interhemispheric approach when treating craniopharyngiomas of the third ventricle or anterior communicating artery aneurysms. This technique ensures preservation of the bridging veins and the olfactory nerves. This "basal interfalcine approach" involves a craniotomy in the centrobasal portion of the frontal bone (the frontal sinus), removal of the inner tables and the crista galli, and splitting the basal portion of the falx into two leaves, through which the basal interhemispheric fissure is opened. The olfactory nerves are protected by the leav

[A case of venous angioma complicated by unruptured aneurysm with TIA as the initial manifestation].

Venous angioma is a relatively rare vascular malformation of the brain. It is usually asymptomatic and may be an incidental finding at autopsy or on cerebral angiography. We report a very rare case in which TIA-like attack occurred as the initial manifestation. A 59-year-old woman was hospitalized because of a TIA-like attack about 5 months after left putaminal bleeding. Detailed examination allowed us to make a diagnosis of subcortical venous angioma of the left parietal lobe and multiple cerebral aneurysms. The cerebral aneurysms were treated surgically. Since the angioma was localized in an

Effects of prostaglandin E1 or trimethaphan on local cerebral blood flow and carbon dioxide reactivity during cerebral aneurysm surgery.

Effects of prostaglandin E1 (PGE1) or trimethaphan (TMP) on local cerebral blood flow (LCBF) and carbon dioxide (CO2) reactivity were studied in 26 patient undergoing cerebral aneurysm surgery for subarachnoid hemorrhage (SAH) under isoflurane anesthesia. Measurement of LCBF was made using a thermal gradient blood-flow meter. Hypotension was induced with a continuous i.v. infusion of PGE1 or TMP. The dose of PGE1 or TMP was adjusted to maintain the MAP at about 70 mm Hg, and CO2 reactivity was studied during and after PGE1 or TMP administration. PaCO2 was changed by the controlled hyperventila

[P3 portion aneurysm of the posterior cerebral artery; a case report].

A case was reported of surgically treated saccular aneurysm located at the right posterior temporoparietooccipital artery junction (P3 portion of PCA). An aneurysm of this portion is said to be rare, and only 7 cases have been described so far. A 74-year-old female was transferred to our clinic, after 3, 5 months of sustaining aneurysmal rupture, for surgical treatment. The patient had been treated conservatively because of her severe condition in the early stage. She was in nearly apallic state with left hemiparesis at the time of admission to our clinic. During the acute stage of her illness

[Vertebral dissecting aneurysm treated with trapping and bilateral posterior inferior cerebellar artery side-to side anastomosis; case report].

A case is reported of ruptured dissecting aneurysm of the intracranial vertebral artery (VA) operated on with VA trapping and bilateral posterior inferior cerebellar artery (PICA) side-to-side anastomosis. A 42-year-old male suddenly developed severe headache and vomiting. On admission, 3 hours later, he was in a state of moderate confusion (Japan Coma Scale 3) and had neck stiffness. Computed tomography (CT) revealed diffuse subarachnoid hemorrhage, especially thick in the posterior fossa with right side dominance. Right vertebral angiography disclosed a fusiform dilatation with proximal narr

Isolated trigeminal sensory loss secondary to a distal anterior inferior cerebellar artery aneurysm: case report.

A previously healthy 25-year-old woman suddenly developed right-sided facial numbness and a headache. The neurological examination was within normal limits with the exception of meningismus and right-sided facial sensory loss. A computed tomographic scan and a magnetic resonance imaging study demonstrated an acute hematoma in the right cerebellopontine angle. A 4-vessel cerebral angiogram revealed no abnormalities. Posterior fossa exploration disclosed a large, partially thrombosed, fusiform anterior inferior cerebellar artery aneurysm, which indented the pons at the trigeminal root entry zone

Spontaneous saccular cerebral aneurysm in a rat.

A saccular cerebral aneurysm was incidentally found at the bifurcation of a fenestrated anterior cerebral artery of 35-week-old Sprague-Dawley rat. The animal had been fed a normal laboratory diet. The internal lamina extended for a short distance into the neck of the aneurysm and then suddenly disappeared. The medial layer ended abruptly at the neck of the aneurysm. The aneurysmal wall consisted mainly of connective tissue, and of some poorly stained fragments of elastic lamina. No evidence of polyarteritis nodosa or any other inflammatory reaction was obtained. These findings were the same a

Ptosis as the sole manifestation of compression of the oculomotor nerve by an aneurysm of the posterior communicating artery.

Oculomotor palsy secondary to a berry aneurysm is usually present with pupillary dilatation, followed by other signs of third cranial nerve dysfunction, including oculomotor paresis and ptosis. Partial paralysis of the nerve with pupil sparing has been observed, but ptosis as the sole sign of oculomotor paralysis has not previously been reported until now.

Early operation of ruptured basilar artery aneurysm associated with bilateral carotid occlusion (moyamoya disease).

A 34-year-old Caucasian man presented with subarachnoid hemorrhage. Angiography revealed bilateral carotid occlusion at the cavernous sinus and an aneurysm at the basilar artery bifurcation. The whole brain was supplied with blood from the basilar artery and posterior cerebral arteries through a large number of collateral vessels to the internal carotid artery bifurcation, middle cerebral and anterior cerebral arteries: the moyamoya phenomenon. The aneurysm was clipped within hours of the subarachnoid hemorrhage. The relation between moyamoya disease and basilar artery aneurysms is discussed a

[Unusual postoperative hemorrhage in a patient with ruptured aneurysm and hemophilia B].

A case of ruptured cerebral aneurysm with hemophilia B is reported, and discussion is made concerning the management of mild type hemophilia in surgical operations. A 41-year-old male came to our hospital with complaints of severe headache, vomiting, and transient consciousness disturbance. His dentist said the patient had a mild bleeding tendency when he was 30 years old, however no postoperative hemorrhage was repeated in appendectomy in his childhood. He also had had no episodes of spontaneous bleeding. CT scan on admission showed subarachnoid hemorrhage, and angiography revealed a ruptured

Pseudoaneurysm complicating superficial temporal artery-superior cerebellar artery bypass.

A 60-year-old man with vertebrobasilar ischemia unrelieved by anticoagulation was found to have a midbasilar stenosis with an inadequate basilar collateral circulation. He underwent a right superficial temporal-superior cerebellar artery bypass. Following this operation he had a subarachnoid hemorrhage and right third nerve palsy that was due to formation of a pseudoaneurysm at the site of the anastomosis. This aneurysm was managed by ligation of the right superficial temporal artery. Subsequently it became necessary to perform a left superficial temporal artery-superior cerebellar artery bypa

Saccular aneurysm at the basilar artery fenestration.

The authors describe a 19-year-old patient who was presented with a subarachnoid hemorrhage and was found to have a saccular aneurysm arising at the proximal end of the fenestration of the basilar artery. Previously reported cases are reviewed. Stereoscopic magnification angiography facilitated the analysis of the three-dimensional-anatomical relationship between the aneurysm and its fenestrated parent artery.

Return of function after intracranial suture of the trochlear nerve. Case report.

An excellent functional result was obtained after microsuture of a trochlear nerve which was inadvertently divided during the course of a craniotomy. Successful intracranial suture of this nerve has not previously been reported. Ophthalmoscopic detection of excyclotorsion of the eye on the side of the trochlear nerve palsy is discussed, along with its value in the differential diagnosis of acquired vertical diplopia.

Invasive aspergillosis complicating Cushing's syndrome.

Patients with Cushing's syndrome are susceptible to opportunistic infections. Invasive fungal infections in patients with Cushing's syndrome caused by endogenous glucocorticoid excess rarely are reported, and aspergillosis occurring in this setting, to our knowledge, has not been described. Two patients with Cushing's syndrome and notably elevated levels of circulating cortisol had invasive aspergillosis develop. A patient with endogenous hypercortisolism caused by adrenal cortical carcinoma suffered a subarachnoid hemorrhage from rupture of an Aspergillus mycotic aneurysm. The other patient,

Barbiturate therapy in the management of cerebral ischaemia.

Two patients who underwent surgery for cerebral aneurysms are presented. In the first case 31 g thiopentone were given postoperatively over 20 h after the patient had already been comatose for many hours. Such high doses raise considerable problems in patient management. In the second case a loading dose of 50 mg/kg thiopentone was given prophylactically to a patient undergoing cerebral aneurysm surgery beginning at the induction of anaesthesia and before surface cooling was begun. Cerebral activity was monitored continuously with a cerebral function monitor (CFM). There was no significant car
