An anomalous foveal OCT-sign after posterior capsule rupture in cataract surgery: Complicated cataract surgery maculopathy.

PURPOSE: To report a series of novel optical coherence tomography (SD-OCT) foveal abnormalities, that we called "T-sign," that were noticed after a complicated cataract surgery with posterior capsule rupture and vitreous loss. METHODS: Retrospective case series of persistent foveal changes that incurred after anteroposterior vitreo-foveal traction secondary to phacoemulsification in presence of posterior capsule rupture. RESULTS: The study included three eyes of three patients that incurred in complicated cataract surgery and intraoperative vitreo-foveal traction. During 8-month follow-up pe

Transperitoneal laparoscopic heminephroureterectomy in pediatric population: A single-centre experience using a sealing device.

INTRODUCTION: We sought to report the outcomes of transperitoneal laparoscopic heminephroureterectomy (LHNU) in pediatric population and to describe the technical details of this minimally invasive surgery. METHODS: Seventeen pediatric patients (18 renal units), who had consecutive transperitoneal LHNU in our department between January 2012 and July 2017 were included in the study. In all patients, diagnostic cystoscopy and retrograde pyelography were carried out immediately before the operation. A catheter was inserted in the unaffected ureter and fixed. LHNU with a transperitoneal approach

[Transurethral resection in the treatment of benign prostatic hypertrophy: analysis of its results].

OBJECTIVE: To analyze the results achieved with transurethral resection in the treatment of benign prostatic hyperplasia. METHODS: We analyzed the results achieved with transurethral resection in 400 patients with benign prostatic hyperplasia who had been treated at the Joaquín Albarrán teaching hospital. The procedure was utilized for prostates weighing up to 50 gm and middle lobe involvement independent of its size. Sterile water was used as irrigant since it permits excellent vision, it is low-cost and carries a low risk of complications associated with intraoperative osmotic diuresis. RE

Intraoperative radiation therapy in biliary tract cancer.

From 1983 to 1986, nine patients with advanced cancers of the proximal biliary tract were treated with intraoperative electron beam radiation therapy (IORT) following surgical resection or intubation. Five patients also received external beam radiation and four received chemotherapy. Early complications were minimal. Late complications included cholangitis, gastroduodenal ulceration, gastric outlet obstruction and portal vein thrombosis. Symptomatic recurrent or residual disease developed in eight patients with a median disease-free survival of 6 months. Seven patients failed locally; four rec

Can Aortic Balloon Occlusion Reduce Blood Loss During Resection of Sacral Tumors That Extend Into the Lower Lumber Spine?

BACKGROUND: Although aortic balloon occlusion has been shown to reduce blood loss during sacral tumor resections, it has not been validated in larger sacral tumors involving the lower lumbar spine. If such an approach were shown to be associated with less blood loss, it might aid the tumor surgeon in resecting these difficult tumors. QUESTIONS/PURPOSES: (1) Is the use of aortic balloon occlusion associated with reduced blood loss in sacral tumor resections when the lower lumbar spine is also involved? (2) Does the use of the aortic balloon prolong total operating time? (3) What complications

Regional Anesthetic Techniques in Chronic Obstructive Pulmonary Disease Patients.

The growing incidence of chronic obstructive pulmonary disease (COPD) has implications for anesthesia. Historically general anesthesia (GA) was the preferred anesthetic technique for laparoscopic, upper limb orthopedic surgeries, and open abdominal surgeries. However, in severe COPD the incidence of postoperative pulmonary complications (PPC) is increased. Regional anesthetic (RA) techniques offer several advantages by significantly reducing PPCs. We review anesthetic considerations for COPD patients undergoing RA, discussing benefits and anticipated risks, postoperative pain management, and i

Autologous breast reconstruction in patients with primary hypercoagulability disorders.

The rising demand for autologous breast reconstruction presents challenges for patients with primary hypercoagulability disorders due to their elevated thrombotic risks. In this article, we reviewed 13 patients who underwent 23 free flap breast reconstructions from 1998 to 2023 with various anticoagulation regimens. Results showed a flap loss rate of 4%, with additional intraoperative and postoperative anticoagulation potentially reducing thrombotic complications. The data concludes that autologous breast reconstruction in hypercoagulable patients is reliable with careful anticoagulation manag

[Cochlear implantation in far advanced otosclerosis: series of four cases].

In this article, we present four patients who underwent cochlear implantation due to far advanced otosclerosis. Preoperative evaluations, intraoperative findings, complications, and postoperative benefits were analyzed. Cochlear implantation is a treatment option providing excellent audiological results for rehabilitation of patients with far advanced otosclerosis. However, facial nerve stimulation after cochlear implantation is observed more frequently in patients with otosclerosis. Also, caution should be paid in patients with otosclerosis in terms of cochlear ossification and inconsistent r

Combined spinal-epidural anesthesia for cesarean section in a patient with Moyamoya disease -A case report-.

Moyamoya disease is a rare progressive occlusive disease of the internal carotid arteries. We report a case of combined spinal-epidural anesthesia in a patient with Moyamoya disease presenting for Cesarean section. Hypotension associated with spinal anesthesia for Cesarean section is the most common and serious adverse effect despite the use of uterine displacement and volume preload. We continuously infused phenylephrine and ephedrine to prevent hypotension. The intraoperative hemodynamic state was stable. The patient had no significant postoperative complications.

Amplatzer septal occluder associated with late pulmonary venous obstruction requiring surgical removal with acquired aorta to left atrial fistula.

The Amplatzer septal occluder is currently the preferred device for the transcatheter closure of secundum atrial septal defects. Multiple studies have shown that device complications with the Amplatzer occluder are rare and often acute in presentation. The investigators describe the first reported case of late obstruction of the right pulmonary veins with an Amplatzer septal occluder and, in the same patient, an unusual intraoperative finding of a noncoronary aortic sinus to left atrium fistula after device removal.

Videothoracoscopic resection of intrathoracic neurogenic tumors: report of two cases.

Two cases of intrathoracic neurogenic tumors are discussed. The benign neoplasms were located in the posterior mediastinum and caused no clinical symptoms. In both cases, complete resection of the lesion was achieved by video-assisted thoracic surgery (VATS). There were no intraoperative complications. Postoperatively, one patient presented with Claude Bernard-Horner syndrome, which resolved spontaneously after 1 week. Although the great majority of mediastinal neurogenic neoplasms are benign, resection is necessary to prevent malignant transformation and intraspinal extension.

Dacryocystorhinostomy with silicone tubes: evaluation of 253 cases.

Two hundred fifty-three patients with chronic epiphora underwent dacryocystorhinostomy combined with silicone intubation of the lacrimal drainage system. An overall success rate of 91.3% was achieved. Since the tubes were well-tolerated postoperatively, complications were rare and benign. Although our success rate is similar to those reported for similar cases using other techniques, we find that silicone intubation facilitates surgery in cases with excessive intraoperative bleeding or nasal mucosal tears, and simplifies postoperative follow-up.

The reproductive performance of women before and after metroplasty.

Nineteen women with congenital uterine anomalies underwent metroplasty. Prior to surgical correction of the uterine malformation, 44 of their 45 (98%) pregnancies had ended with a spontaneous abortion, and no woman had a living child. Following metroplasty, only 4 of 18 (22%) pregnancies aborted, and 12 of the 14 (86%) women attempting pregnancy had a living child. There were no unusual intraoperative or postoperative complications. Metroplasty procedures can be performed safely, and are associated with excellent reproductive outcomes.
