Two competing views have been expressed to explain ectoparasite speciation patterns, one emphasizing co-speciation and one emphasizing host-switching. Though recent authors preferred a combination of these views, the available phylogenetic trees cannot be interpreted without revisiting island biogeography theory. Parasite extinction due to a temporal decline in host population size seems to be a prerequisite of subsequent speciation by host-switch. The need for a re-evaluation of "stragglers" (ectoparasites found on non-specific hosts) is emphasized.

Rectoprostatic fistulas can pose challenging problems for surgical reconstruction. We describe and illustrate the successful repair of a difficult, post-traumatic prostatic urethrorectal fistula by combining use of a vascularized scrotal dartos flap interposition with a modified anterior rectal wall flap fistula repair (Park's technique). An anterior perineal transanorectal, transanorectal, transsphincteric approach provided excellent operative exposure as well as a direct path for the perineal pedicled dartos island flap en route to the fistula repair site.

Murine typhus is endemic in many South East Asian countries but it has not been reported from the island of Borneo. Recently we attended to an English lady who contracted the disease in Brunei. Her clinical course unexpectedly turned stormy in the third week of the illness accompanied by severe thrombocytopenia. She made a complete recovery. Serological studies here and subsequently in England confirmed that she suffered from recent murine typhus infection complicated by dengue haemorrhagic fever.

The demography of scabies was studied in the sole dermatology practice on the island of Kauai, Hawaii, over a 50-month period. Scabies was observed far more frequently in whites and Hawaiians than in Japanese and Filipinos. Within the white ethnic group, scabies was most frequently diagnosed in persons between 15 and 44 years old, while in Hawaiians, the disease was most common in the 0-14-year age group. It is possible that learned or acquired behavior patterns may play a significant role in contracting the disease.

A new vascular musculocutaneous flap from the lower buttock is described that is based on the inferior gluteal vessels and includes skin from the gluteal fold and a part of the gluteus maximus muscle. The inferior gluteal flap has been clinically used either as an island flap for sacral repair or as a free flap. The results of 6 clinical cases are presented and the features of the flap are exposed. The minimum residual deformity of the buttock contour and, even in some cases of trochanteric lipodystrophy, the cosmetic improvement of the donor site appearance are emphasized.

Two persons who had been splenectomized later contracted babesiosis, one on Cape Cod and one near Islip, Long Island, areas where human cases of babesiosis had not been previously identified. One of the patients received pentamidine, but parasitemia persisted after therapy. No deaths have been recorded for persons who had had splenectomies and were later infected with Babesia microti. Until more effective therapy is available, such patients should be treated conservatively.

The emergence and spread of antibiotic resistance genes (ARGs) mediated by mobile genetic elements (MGEs) pose a significant threat to public and environmental health. While aquaculture ecosystems are recognised as critical reservoirs for ARGs, the genomic architecture of MGEs that facilitate resistance dissemination in aquatic pathogens - particularly Aeromonas veronii (A. veronii) - remains underexplored. This study isolated a highly virulent, multidrug-resistant strain of A. veronii (Y6) from diseased largemouth bass, underscoring its potential dual threat to aquaculture and public health.
