PURPOSE: There are significant inconsistencies in accounts in textbooks of the fitting in 1887 of a device resembling a scleral contact lens. The aim of this study was to establish the facts of this case and to provide an overview of the subsequent clinical performance of blown scleral shells and lenses.
METHODS: A literature search was conducted that included the original report published in 1910 of the fitting of the patient. The dimensions and back vertex power of a blown scleral shell were measured.
RESULTS: In 1887, the right eye of a patient with lagophthalmic keratitis as a sequel to

AIM: The antibacterial effect of topical anesthetics may lead to false-negative cultures from corneal specimens of bacterial keratitis. This in vitro study compared the antibacterial effect of 3 unpreserved topical anesthetics to indicate the most appropriate agent for corneal scrapes.
METHODS: Four bacterial strains (Staphylococcus epidermidis, Staphylococcus aureus, Pseudomonas aeruginosa, and Streptococcus pneumoniae) derived from the most frequently isolated microorganisms from corneal ulcers were cultured from stored control stocks and clinical specimens. These strains were used to deter

PURPOSE: One strategy to minimize bacteria-associated adverse responses such as microbial keratitis, contact lens-induced acute red eye (CLARE), and contact lens induced peripheral ulcers (CLPUs) that occur with contact lens wear is the development of an antimicrobial or antiadhesive contact lens. Cationic peptides represent a novel approach for the development of antimicrobial lenses.
METHODS: A novel cationic peptide, melimine, was covalently incorporated into silicone hydrogel lenses. Confirmation tests to determine the presence of peptide and anti-microbial activity were performed. Cation

Trachoma remains the leading cause of preventable corneal blindness in developing countries. The disease is contracted in early childhood by repeated infection of the ocular surface by C. trachomatis. Initial clinical manifestation is a follicular conjunctivitis which if not treated on timely basis, may lead to conjunctival and eyelid scarring that may eventually result in corneal scarring and loss of vision. Over the past two decades, a remarkable reduction in the prevalence of active trachoma has occurred due to the World Health Organization's (WHOs) program GET 2020 for the elimination of t

BACKGROUND: Little information is currently available on the long-term safety of eyes following limbal tissue donation. We have therefore evaluated long-term refractive changes, corneal surface stability, and visual outcome following limbal tissue donation of 180 degrees with permanent amniotic membrane transplantation (AMT).
METHODS: Four healthy eyes of four patients underwent limbal keratectomy involving half of the limbal circumference (half limbal) for the purpose of limbal donation with permanent AMT. The best corrected visual acuity (BCVA), astigmatic changes by manifest refraction, an

Microemulsion composed of Span20 + Tween20 isopropyl myristate (IPM) + H2O were investigated as potential drug delivery systems for eye drops. The system is important in that all its components are food grade so that the microemulsion is almost free of toxicity and irritation. The phase transition was investigated using the electrical conductivity measurements. The chloramphenicol is used to treat the eye diseases such as trachoma and keratitis. However, this drug in the common eye drops hydrolyzes easily. The main product of the hydrolysis is glycol. Here, the chloramphenicol was trapped into

Toxoplasmosis is the most common cause of posterior uveitis in immunocompetent subjects. The infection can be congenital or acquired. Ocular symptoms are variable according to the age of the subject. For instance, young children present with reduced visual acuity, strabismus, nystagmus, and leucocoria, while teenagers and adults complain of decreased vision, floaters, photophobia, pain, and hyperemia. Toxoplasmic retinochoroiditis typically affects the posterior pole, and the lesions can be solitary, multiple or satellite to a pigmented retinal scar. Active lesions present as grey-white focus

Leprosy control programmes are highly successful. As a result, leprosy control will be more and more integrated into the general health services. The existing vertical, specialized control programmes will be dismantled. Eye complications in leprosy have decreased. This is a result of earlier diagnosis and highly effective multidrug treatment (MDT) of leprosy, combined with timely treatment of secondary nerve damage by steroids. Most ocular morbidity is now found among elderly and disabled leprosy patients who were diagnosed before effective MDT treatment became available. Many of these patient

Infection caused by Penicillium spp. due to species other than P. marneffei is rare. We present three such cases of invasive disease. The first had chronic granulomatous disorder (CGD) with pulmonary infection caused by Penicillium spp. and he responded to amphotericin B therapy. Cases two and three were not known to be immunocompromised and both failed to respond to therapy. Case two had cerebral disease from an unknown source caused by P. chrysogenum. Case three probably acquired infection caused by P. decumbens peri-operatively and presented with paravertebral infection. The pertinent liter

Syphilis is an uncommon cause of uveitis in HIV-infected patients. We report a case of bilateral panuveitis and describe its characteristics as the initial manifestation of HIV infection. A 74-year-old heterosexual male complained of blurred vision and floaters in both eyes for 40 days. Slit lamp examination showed diffuse keratic precipitates and cells in the anterior chamber of both eyes. Fundus examination revealed multiple small white dots and scattered retinal hemorrhage over the mid-equatorial retina with marked vitritis. Physical examination disclosed multiple erythematous papules over

BACKGROUND: Floppy Eyelid is a rare condition causing chronic papillary conjunctivitis and chronic corneal disorders (superficial punctate keratitis, epithelial and stromal ulcers). It is characterized by an extremely enlarged and "floppy" upper eyelid which can be easily everted by slight elevation. Usually obese men are affected who use to sleep face down either on the right or on the left side. Pushing the eyelid against the pillow, the lid is intermittently everted at sleep. This lagophthalmus with rubbing of the exposed eye and lid structures causes all pathologic disorders. Shielding the

PURPOSE: Postoperative recurrence of pterygium occurs in many patients. Intraoperative and postoperative mitomycin therapy are two adjuvant treatment methods shown to lessen the high pterygium recurrence rate seen with simple excision alone. The authors designed a prospective, randomized study to explore the recurrence rate of pterygium after a single dosage of mitomycin C at the completion of pterygium excision, comparing it to postoperative mitomycin C therapy.
METHODS: Thirty-six patients with 40 primary and recurrent pterygia were randomized to 1 of 2 treatment groups: intraoperative mito

PURPOSE: Only epithelial cells are thought to express blood group antigens (bga) A and/or B in normal corneas, and ABO blood group (bg) matching seems to reduce immune rejection in high risk transplantations. This study attempts for the first time to investigate to what extent the bga A and/or B are expressed in diseased corneas.
METHODS: Thirty-nine diseased corneal buttons of 39 patients were examined. Immunohistochemical staining of paraffin-embedded sections was performed with monoclonal mouse anti human blood groups A and B antibodies using the Streptavidin-Biotin-peroxidase complex tech
