A 19-year-old woman had a thunderclap headache, followed by left hemiparesis and left homonymous hemianopsia. Laboratory tests showed no signs of infection and immunological test results were unremarkable. MRI revealed a cerebral infarction in the right posterior cerebral artery territory, and digital subtraction angiography(DSA)showed right posterior cerebral artery stenosis on day 2. The first follow-up DSA demonstrated an irregular, bead-like appearance on day 9, but the stenotic lesion returned to normal on day 21. Reversible cerebral vasoconstriction syndrome should be suspected in cases

Diaper dermatitis is a common condition that often prompts parents to seek medical attention. Irritant diaper dermatitis is by far the most common cause, but numerous potentially serious diseases can present with changes of the skin in the diaper area. The differential diagnosis can include psoriasis, metabolic disorders, rare immune diseases and infection. Clinical examination can be helpful in distinguishing the underlying cause. General screening laboratory tests, as well as select testing when a specific condition is suspected, can be used to challenge or confirm the putative diagnosis.

Charcot neuroarthropathy (CN) is a difficult problem for the foot and ankle surgeon. If surgery is required, little is known or available regarding the best methods and timing. When the initial attempt of reconstruction fails, revision of CN is even more demanding. One must take in to account all aspects, including nutrition, vascular status, infection control, short- and long-term blood glucose management, as well as other factors requiring laboratory monitoring and consult services. Once optimized, the biomechanics of the deformity can be addressed and decisions can be made on fixation devic

A 16-year-old boy was diagnosed with Crohn's disease. Treatment with oral mesalazine was started at 3 g per day; however, he complained of high fever, a nonproductive cough, and left shoulder pain after 2 weeks. His chest radiography and chest computed tomography showed cardiomegaly and left pleural effusion, while an echocardiogram revealed pericardial effusion. Because no infection was detected by thoracentesis and the drug lymphocyte stimulation tests for mesalazine were positive, the patient was diagnosed with mesalazine-induced pleuropericarditis. After the cessation of mesalazine, the cl

Acute urinary tract disorders often manifest as flank pain and are a common complaint of patients who present to the emergency department. The pain is often a vague, poorly localized sensation that may have a variety of causes. Laboratory and clinical findings, such as hematuria, are neither sensitive nor specific for determining the cause of the flank pain. Accordingly, imaging is an important tool in determining a diagnosis and management plan. Patients with acute urinary tract disorders who present with pain include those with calculi as well as renal infection, vascular disorders, and hemo

Paragonimiasis is a food-borne parasitic zoonosis caused by the genus Paragonimus. Fresh water snails, crabs, and crayfish are the first and second intermediate hosts, respectively. Humans acquire this infection by ingesting uncooked/undercooked crustaceans. Laboratory diagnosis of Paragonimiasis is done by demonstration of ova in the sputum/feces/pleural fluid or by serology. A case of pulmonary Paragonimiasis is presented herewith; the patient having been diagnosed with pulmonary tuberculosis earlier. The aim of this presentation is to highlight this entity so that it is considered in the di

Acute leg ischemia after intra-arterial drug injection represents a critical vascular emergency scenario. Due to lack of evidence-based standards therapeutic strategies are oriented to the underlying pathomechanisms. For a sufficient therapy a close clinical monitoring and laboratory analyses as well as treatment with analgesics, anticoagulants, anti-inflammatory and spasmolytic agents are of utmost importance. This article reports on the diagnostic and therapeutic approaches in a 32-year-old patient with acute leg ischemia after intra-arterial administration of heroin and secondary infection

Crimean-Congo haemorrhagic fever (CCHF) is a fatal viral infection and an important public health issue in Turkey because of its high case fatality rate. Severity criteria of CCHF were defined previously in adults on the basis of epidemiological, clinical, and laboratory findings,. This study evaluated the course of CCHF in children. Between January, 2009, and November, 2012, 41 patients aged between 1 and 17 years (mean 9.78 ± 4.85) with a diagnosis of CCHF were included in the study. According to results of our study, Turkish pediatric patients had a milder course of CCHF.

Pulmonary cryptococcal infection is commonly seen in HIV patients and in patients who have compromised cell mediated immunity. Its occurrence in immunocompetent individuals is uncommon. Here we present a young adult who presented with cough and hemoptysis and is negative for HIV testing and has no clinical and laboratory evidences of conditions compromising his cell mediated immunity. Radiologically manifested as a solitary huge parenchymal mass lesion mimicking lung malignancy. He was finally diagnosed as having pulmonary cryptococcoma after open biopsy. This is an uncommon manifestation of c

Cytomegalovirus infection is often life threatening in immunocompromised patients. Three anti-CMV drugs, ganciclovir, valganciclovir and foscarnet are available in commercial basis in Japan. In addition to them, cidofovir is provided for patients with HIV by a research group supported by Ministry of Health, Labour and Welfare. Every anti-CMV drug has serious side effects, as bone marrow suppression with ganciclovir and valganciclovir, renal toxicity with foscarnet and both with cidofovir. Strict dose adjustment to the renal function and close monitoring with laboratory examinations are essenti

A seven year-old male presented to his pediatrician with choreiform movements and a recent history of sore throat. He was diagnosed with Sydenham's chorea based on clinical criteria and laboratory evidence. Worsening symptoms prompted a magnetic resonance imaging (MRI) of the brain which demonstrated evidence of Moyamoya disease. Sydenham's chorea is a common and well-documented complication of post-streptococcal infection, but has not been previously reported in association with Moyamoya disease. This case raises the quandary of causality of chorea in this patient and the need for neuroimagin

A precise assessment of the epidemiological extent of equine Lyme disease is not well established in metropolitan France, French Guiana, and Africa (Chad, Djibouti, Ivory Coast, Gabon, and Democratic Republic of Congo). Blood samples were obtained from 570 horses. The samples were tested for Borrelia burgdorferi infection by a commercial ELISA Dot-Blot method (SNAP 4 Dx; IDEXX S. Laboratory). Lyme disease antibodies were only detected in metropolitan France, specifically in the eastern and center-western regions (48% and 31%). The geographical distribution of the disease follows the distributi

In 1970s enterovirus type 71 (EV71) caused several epidemics of poliomyelitis-like disease with severe neurologic sequelae. In the last 20 years EV71 was the cause of series of outbreaks and epidemics of foot-and-mouth disease-like conditions with neurologic sequelae in countries of South-East Asia. During the last epidemic of EV71 infection, which occurred in China in 2008, more than 60,000 cases was registered, 38 of which were lethal. Some aspects of epidemiology and laboratory diagnostics of disease caused by EV71 are considered in this review.

An 8-month-old infant with cyanotic heart disease and transfusion-associated Babesia microti infection is reported here. At initial presentation, she was ill appearing, febrile and cyanotic. Laboratory tests revealed severe anemia, thrombocytopenia and an increase in hepatic enzymes. The diagnosis was made by the presence of intraerythrocytic parasites on thin blood smear and confirmed by serology and polymerase chain reaction. The infant was treated successfully with a combination of oral azithromycin and atovaquone. This combination is an alternative to clindamycin and quinine for the treatm

Forty years ago, in early August 1967, the first filovirus ever detected, Marburg virus, made its appearance in Europe, causing severe and often fatal hemorrhagic fever in laboratory workers in Marburg and Frankfurt and, about 4 weeks later, in Belgrade. The etiological agent was isolated and identified by the combined efforts of virologists in Marburg and Hamburg within the very short time of 3 months. Marburg was not the only town where the virus was isolated and identified for the first time, but most cases of infection occurred in Marburg.

Thomas Campbell Butler, at 63 years of age, is completing the first year of a 2-year sentence in federal prison, following an investigation and trial that was initiated after he voluntarily reported that he believed vials containing Yersinia pestis were missing from his laboratory at Texas Tech University. We take this opportunity to remind the infectious diseases community of the plight of our esteemed colleague, whose career and family have, as a result of his efforts to protect us from infection by this organism, paid a price from which they will never recover.

Corneal subepithelial infiltrates are a known complication of adenoviral conjunctivitis. In this article, we present a case of a 32-year-old man who developed these classic infiltrates after a well-documented adenoviral conjunctivitis. What is unique about this case is that the infiltrates recurred 8 months later after an upper respiratory infection without any clinical or laboratory evidence of a viral conjunctivitis. The possible confounding variable of corticosteroid withdrawal being responsible for the recurrence is highly unlikely because he had not used topical corticosteroids for almost
