Risks and consequences of puerperal uterine inversion in the United States, 2004 through 2013.

BACKGROUND: Puerperal uterine inversion is a rare, potentially life-threatening obstetrical emergency. The current literature consists of small case series and a single nationwide study from Europe with only 15 cases. OBJECTIVE: We aimed to define the incidence, temporal trends, and outcomes in women with uterine inversion using a nationally representative US cohort. STUDY DESIGN: We used the Nationwide Inpatient Sample, a 20% sample of US hospital admissions, to identify all deliveries from 2004 through 2013. International Classification of Diseases, Ninth Revision diagnosis codes were used

Ultra-low dose of rituximab in rheumatoid arthritis: study protocol for a randomised controlled trial.

BACKGROUND: A standard low-dosing schedule of rituximab (RTX; 2 × 500 mg or 1 × 1000 mg) is as effective for active rheumatoid arthritis (RA) as the registered dose (2 × 1000 mg). Moreover, several small uncontrolled studies suggest that even lower-dosed treatment with RTX also leads to good treatment response in patients with RA. Retreatment with such an 'ultra-low' dose RTX in patients who responded well to RTX induction treatment is of special interest, as long-term use of lower RTX doses may lead to shorter infusion duration, lower risk of adverse events and lower costs. However, the effec

Effects of perinatal protein malnutrition and fenfluramine action on food intake and neuronal activation in the hypothalamus and raphe nuclei of neonate rats.

In neonatal rats, hunger and satiety responses occur particularly via dehydration and gastric distention, respectively. The control of food intake in newborns is yet to be fully consolidated, particularly with respect to the participation of the hypothalamic nuclei and their relationship with the serotonergic pathway. Moreover, it is unclear how the environmental stressors in early life, like undernutrition, interfere in these events. Therefore, this study examined the serotonin-system's impact on food intake in rat neonates at postnatal day (P) 10 and P18 and the manner in which protein under

Balance chiropractic therapy for cervical spondylotic radiculopathy: study protocol for a randomized controlled trial.

BACKGROUND: Cervical spondylosis is a very common disorder and cervical spondylotic radiculopathy (CSR) is the most common form of spinal degenerative disease. Its clinical manifestations focus on pain and numbness of the neck and arm as well as restricted movement of the neck, which greatly affect the patient's life and work. The orthopedic of traditional Chinese medicine (TCM) theory holds that the basic pathologic change in spinal degenerative diseases is the imbalance between the dynamic system and the static system of the cervical spine. Based on this theory, some Chinese physicians have

Osteocalcin. A biochemical marker of bone turnover during puberty.

During bone remodelling, osteocalcin is produced by osteoblasts and its level increases during the events characterized by rapid bone turnover. Osteocalcin is a bone matrix protein, which is specific for bone metabolism and it is not influenced by metabolic bone disorders. Osteocalcin is an important marker of bone turnover in physiological and pathological conditions. Physiologically, serum osteocalcin was increased in children, particularly during the first year of life and during puberty, when evolution of the concentration was related to rapidity of physical growth. Evidence of a correlati

Interventions to increase patient and family involvement in escalation of care for acute life-threatening illness in community health and hospital settings.

BACKGROUND: There is now a rising commitment to acknowledge the role patients and families play in contributing to their safety. This review focuses on one type of involvement in safety - patient and family involvement in escalation of care for serious life-threatening conditions i.e. helping secure a step-up to urgent or emergency care - which has been receiving increasing policy and practice attention. This review was concerned with the negotiation work that patient and family members undertake across the emergency care escalation pathway, once contact has been made with healthcare staff. It

Efficacy and evaluation of dose-response relationship of selective internal radiation therapy for the management of liver metastases in neuroendocrine neoplasia.

PURPOSE: The liver is the commonest site of metastases in neuroendocrine neoplasias (NENs) and is an independent predictor of poor outcome. Selective internal radiation therapy (SIRT) allows selective delivery of high dose radiation to liver tumours and has shown promise in the management of NENs. We determined the safety and efficacy of SIRT for inoperable liver metastases secondary to NENs. Dose-response and dose-toxicity relationship were assessed. METHODS: A prospective, multicentre, phase 2 study was conducted. Primary outcomes were objective response rate (ORR) in the treated liver volu

Relieving pain with OnabotulinumtoxinA in refractory burning mouth syndrome: protocol and outcomes of an exploratory case series.

BACKGROUND: Burning mouth syndrome (BMS) is a chronic orofacial neuropathic pain disorder, occurring in the absence of clinically evident mucosal lesions or identifiable oral pathology that can explain the symptoms. A significant proportion of patients experience uncontrolled symptoms despite multiple therapies. OnabotulinumtoxinA (onaBoNT/A) is used in neuropathic pain, as clinical trials have shown benefits in peripheral and central neuropathic pain management with its application. We present an exploratory case series of four patients with refractory BMS treated with intradermal onaBoNT/A i

TRADE: tolerability of adjuvant abemaciclib at 6 months after initial dose escalation in patients with early-stage HR-positive/HER2-negative breast cancer.

BACKGROUND: Adjuvant abemaciclib decreases recurrence rates and improves overall survival for patients with high-risk early-stage hormone receptor (HR)-positive/human epidermal growth factor receptor 2 (HER2)-negative breast cancer. However, toxicity, particularly diarrhea, may lead to dose reductions and early discontinuation. Primary results of the TRADE trial demonstrated that early dose escalation of abemaciclib can help patients reach and maintain the target dose of 150 mg twice daily (b.i.d.) by 12 weeks on therapy. In this article, we report a preplanned analysis of clinical outcomes at

Comparative effectiveness of pharmacotherapy for heart failure with preserved ejection fraction: A systematic review and network meta-analysis.

AIM: Heart failure with preserved ejection fraction (HFpEF) presents a therapeutic challenge, characterised by a paucity of validated treatments. Emerging data suggest that targeting adiposity is central to HFpEF pathogenesis. We conducted an updated network meta-analysis to compare the efficacy of emerging and established HFpEF therapies. MATERIALS AND METHODS: We systematically searched PubMed, Embase and Cochrane Library from inception to April 2025 for randomised controlled trials enrolling patients with HFpEF and evaluating pharmacotherapies, including angiotensin-converting enzyme inhib
