Magnetic Resonance Spectroscopy in ALS.

Proton magnetic resonance spectroscopy (MRS) provides a means of measuring cerebral metabolites relevant to neurodegeneration in vivo. In amyotrophic lateral sclerosis (ALS), neurochemical changes reflecting neuronal loss or dysfunction (decreased N-actylaspartate [NAA]) is most significant in the motor cortex and corticospinal tracts. Other neurochemical changes observed include increased myo-inositol (mIns), a putative marker of gliosis. MRS confirmation of involvement of non-motor regions such as the frontal lobes, thalamus, basal ganglia, and cingulum are consistent with the multi-system f

Optic perineuritis secondary to hyaluronic acid injections: a case report.

BACKGROUND: Although a safe, excellent administration method for hyaluronic acid derivatives has been documented; improper injections can lead to devastating and irreversible consequences. Here, we present the first known case of optic perineuritis caused by hyaluronic acid. CASE PRESENTATION: A young female experienced sudden orbital pain in the right eye after receiving hyaluronic acid injections to the eyebrows. She presented to the eye clinic two weeks later, after developing blurred vision in the right eye. Visual acuity was reduced significantly in the right eye. Automated visual field

Anterior transposition of inferior oblique for inferior rectus muscle aplasia.

Congenital absence of extraocular muscle is rare. The most common extraocular muscle found to be congenitally absent is superior oblique followed by inferior rectus. Patients with absent inferior rectus muscle can present with abnormal head posture and incomitant hypertropia with limitation of ocular motility in the field of action of the inferior rectus with or without torticollis. Microphthalmos, microcornea, coloboma, and Axenfeld-Rieger syndrome are known to be commonly associated with inferior rectus muscle aplasia. Orbital computed tomography (CT) or magnetic resonance imaging before sur

A swarm of slippery micropropellers penetrates the vitreous body of the eye.

The intravitreal delivery of therapeutic agents promises major benefits in the field of ocular medicine. Traditional delivery methods rely on the random, passive diffusion of molecules, which do not allow for the rapid delivery of a concentrated cargo to a defined region at the posterior pole of the eye. The use of particles promises targeted delivery but faces the challenge that most tissues including the vitreous have a tight macromolecular matrix that acts as a barrier and prevents its penetration. Here, we demonstrate novel intravitreal delivery microvehicles-slippery micropropellers-that

Susac's syndrome: isolated retinal artery occlusion after 10 years of remission.

Susac's syndrome, or microangiopathy of the retina, inner ear, and brain, is a rare condition characterized by the clinical triad of encephalopathy, branch retinal artery occlusion, and sensorineural hearing loss. The complete triad has been documented in 85% of reported cases. At clinical onset, the most common manifestations are central nervous system symptoms, followed by visual symptoms and hearing disturbances. Although the clinical course of Susac's syndrome is usually self-limiting, fluctuating, and monophasic, clinical polycyclic and chronic courses have also been described. Likewise,

B-Cell Lymphoma in a Patient With a History of Foreign Body Injection.

Dermal filler injection has been one of the most evolving areas of interest in the field of esthetic plastic surgery. This procedure is sometimes preferred to surgery due to shorter procedure and patient recovery times, and because it is suitable for a wide range of applications. Dermal filler injection is considered to be relatively safe, but various late complications such as infection and foreign body granuloma sometimes occur. Postprocedure development of lymphoma also occurs, but is extremely rare. The authors diagnosed extranodal marginal zone B-cell lymphoma of the mucosa associated lym

Nonamplification Sandwich Assay Platform for Sensitive Nucleic Acid Detection Based on AuNPs Enumeration with the Dark-Field Microscope.

A simple and efficient assay platform with high sensitivity, convenient implementation, and moderate cost in reagents and instrumentation is most appropriate for routine applications. On the basis of the gold nanoparticle (AuNP) enumeration signal readout mode established in our laboratory, we have developed a nonamplification sandwich assay for nucleic acid detection with the 3 fM limit of detection for a sequence related to Alzheimer's disease. This AuNP counting based method takes advantages of the distinctive and strong localized surface plasmon resonance light scattering with the dark-fie

Nanomedicines for advanced cancer treatments: Transitioning towards responsive systems.

The development of nanomedicines for the treatment of cancer focuses on the local targeted delivery of chemotherapeutic drugs to enhance drug efficacy and reduce adverse effects. The nanomedicines which are currently approved for clinical use are mainly successful in terms of improved bioavailability and tolerability but do not necessarily increase drug performance. Therefore, there is a need for improved drug carrier systems which are able to deliver high doses of anti-cancer drugs to the tumor. Stimuli responsive carriers are promising candidates since drug release can be triggered locally i

Low-pressure/spinal fluid leak headache.

Low cerebrospinal fluid (CSF) pressure results in neurologic deficits, of which the most common manifestation is headache. Typically, the headache is postural - and specifically, orthostatic - in presentation. There are three hypotheses to explain the occurrence of headache associated with low CSF fluid. The first is traction on pain-sensitive intracranial and meningeal structures; the second is CSF hypovolemia; and the third is spinal loss of CSF resulting in increased compliance at the caudal end of the CSF space. Spontaneous intracranial hypotension (SIH), once believed to be rare, is now m

Hypertrabeculation vs left ventricular noncompaction on echocardiogram: a reason to restrict athletic participation?

IMPORTANCE: Left ventricular noncompaction (LVNC) is a rare cause of progressive cardiomyopathy thought to result from incomplete myocardial development. It has been associated with an increased risk of sudden death, especially in those with a depressed left ventricular ejection fraction. Thus, the current recommendation for patients with this diagnosis is restriction from participation in competitive sports. OBSERVATIONS: An asymptomatic 18-year-old African American collegiate football player had a murmur on his preparticipation physical examination. Subsequent cardiology workup revealed hyp

Failure in developing high-level visual functions after occipitoparietal lesions at an early age: a case study.

Previous functional magnetic resonance imaging (fMRI) studies have identified several regions in the ventral visual pathway that are specialized for processing faces, words and general objects. However, little is known about the origin of the functional selectivity of these regions. Here, we reported a pediatric patient who suffered a left occipitoparietal lesion in the first year after birth from a subdural hematoma. After the hematoma was removed at the age of six, the hemianopia in the right visual field was alleviated, and no obvious deficits in low-level vision were observed in the patien

Current and new directions in MRI in multiple sclerosis.

PURPOSE OF REVIEW: This article summarizes the use of MRI in the diagnosis and treatment of multiple sclerosis (MS). Current and emerging imaging techniques are reviewed pertaining to their utility in MS. RECENT FINDINGS: Conventional T1-weighted and T2-weighted sequences are used to identify and characterize disease pathology in MS. T2 lesion burden, postcontrast enhancement, T1 hypointensities, and regional and global atrophy are all informative and correlate to clinical measures, such as disease disability, to a variable extent. Newer techniques such as diffusion tensor imaging, magnetizat

Low-dose cisplatin-etoposide regimen for patients with optic pathway glioma: a report of four cases and literature review.

Optic pathway gliomas (OPGs) account for 5% of all childhood brain tumors. For years it has been discussed which was the best method of examining tumor progression when the magnetic resonance imaging (MRI) scan does not change. The role of chemotherapy in their treatment still remains controversial. We treated four consecutive patients affected by progressive OPG with lower cumulative doses of cisplatin/etoposide. The extension of disease was assessed by brain MRI scan. A complete ophthalmologic examination was performed. Ototoxicity was monitored. Our OPG patients had reduced visual acuity (V

Surgically induced SMART syndrome: case report and review of the literature.

BACKGROUND: Strokelike migraine attack after radiation therapy is a recently described clinical entity characterized by transient hemispheric dysfunction manifesting as, but not limited to, visuospatial deficits, confusion, hemisensory deficits, hemiparesis, aphasia, seizures, and, most prominently, headache in patients with a history of remote external beam radiation therapy to the brain. The radiographic hallmark on magnetic resonance imaging is the presence of transient, diffuse, unilateral gadolinium enhancement of the cortex with white matter sparing, usually corresponding to the previous

Sequential then interactive processing of letters and words in the left fusiform gyrus.

Despite decades of cognitive, neuropsychological and neuroimaging studies, it is unclear if letters are identified before word-form encoding during reading, or if letters and their combinations are encoded simultaneously and interactively. Here using functional magnetic resonance imaging, we show that a 'letter-form' area (responding more to consonant strings than false fonts) can be distinguished from an immediately anterior 'visual word-form area' in ventral occipito-temporal cortex (responding more to words than consonant strings). Letter-selective magnetoencephalographic responses begin in

Prospective self-gated nonenhanced magnetic resonance angiography of the peripheral arteries.

Most nonenhanced MRA techniques for evaluating peripheral artery disease (PAD) require cardiac synchronization through physiological gating. Electrocardiographic gating is the most popular method for cardiac synchronization; however, it is subject to interference from switching magnetic field gradients and radiofrequency pulses. A method is described for self-gated nonenhanced MRA that does not require the use of electrocardiographic gating. Imaging was prospectively triggered by detecting the acceleration of blood flow during systole with a reference-less phase contrast navigator. The techniq

Possibilities and limitations in imaging the intracranial arteries in the context of a contrast-enhanced whole-body magnetic resonance angiographic screening protocol at 1.5 versus 3 Tesla.

OBJECTIVE: Evaluation of the diagnostic detectability of the intracranial vasculature on contrast-enhanced whole-body magnetic resonance angiographic (WBMRA) scans at 1.5 versus 3 T. METHODS: Twenty-seven patients with hereditary hyperlipidemia participated. Two experienced neuroradiologists scored the image quality regarding the intracranial arteries applying a 5-point scale. Stenoses and other findings were documented. Weighted κ-statistics were calculated to assess interobserver agreement. RESULTS: Interobserver agreement was very good. Image quality scoring resulted in the following mean

[A case of noninvasive sinus aspergillosis showing orbital apex syndrome].

A 78-year-old man was admitted to our hospital with headache, nasal pain, left-sided ptosis, loss of visual field in his left eye, and left ophthalmoplegia. Serum levels of beta-D-glucan were elevated. T1-weighted magnetic resonance imaging with gadolinium enhancement showed hyperintense lesions in the left orbital apex and dura mater of the left middle cranial fossa. A few days later, culture of specimens collected by surgical debridement from the left sphenoidal sinus revealed numerous branching hyphae. The aspergillus antigen was found in the cerebrospinal fluid (CSF). Therefore, aspergillo

Deficient responses from the lateral geniculate nucleus in humans with amblyopia.

Amblyopia or lazy eye is the most common cause of uniocular blindness in adults. It is caused by a disruption to normal visual development as a consequence of unmatched inputs from the two eyes in early life, arising from a turned eye (strabismus), unequal refractive error (anisometropia) or form deprivation (e.g. cataract). Animal models based on extracellular recordings in anesthetized animals suggest that the earliest site of the anomaly in the primate visual pathway is the primary visual cortex (corresponding to the striate cortex, cytoarchitectonic area 17 and area V1), which is where inp
