Association Between Use of WATCHMAN Device and 1-Year Mortality Using High-Dimensional Propensity Scores to Reduce Confounding.

BACKGROUND: Previous observational studies showed left atrial appendage occlusions with the WATCHMAN device reduced 1-year mortality, which conflicted with evidence generated from randomized controlled trials. We proposed to use the high-dimensional propensity score (hdPS) to assist in nonactive comparator selection (prevalent user of medication) and compared 1-year mortality between patients with atrial fibrillation who received the WATCHMAN device (percutaneous left atrial appendage occlusion device [pLAAO]) and direct oral anticoagulants in 2 matched cohorts based on (1) traditional propens

Stereotactic radiosurgery for brain metastases: evolving practice patterns from the national cancer database (2004-2020).

PURPOSE: Stereotactic radiosurgery (SRS) offers less neurotoxicity and comparable survival to whole-brain radiation therapy (WBRT) for brain metastases (BM). Current SRS practice patterns are understudied. We examined national trends in SRS and WBRT utilization. METHODS: We queried the National Cancer Database for patients with BM from twelve cancers (≥ 18 years; diagnosed 2004–2020) treated with radiotherapy. Patients were grouped by first-course radiotherapy modality (SRS:1–5 fractions; WBRT:5–15 fractions). Multivariable logistic regression assessed SRS predictors, adjusting for sociodemogr

Persistent inpatient delirium associated with increased length of stay and mortality.

INTRODUCTION: Delirium is associated with an increased risk of post-hospitalization mortality. However, the impact of persistent delirium on mortality is not well defined. METHODS: We conducted a retrospective cohort study on the association of non-persistent or persistent delirium and mortality. We included patients aged > 65 years admitted to the Owensboro Health Regional Hospital between August 2021 and August 2022. Delirium was determined based on a score of ≥ 2 on the Nurse Delirium Screening Scale (NuDESC) recorded on all admitted patients three times a day during nursing shift change.

Ambient Air Pollution and Chronic Obstructive Pulmonary Disease: The Multiethnic Cohort Study.

Rationale: Globally, chronic obstructive pulmonary disease (COPD) was the third leading cause of death in 2019. Although tobacco smoking is the predominant risk factor, the role of long-term air pollution exposure in increasing the risk of COPD remains unclear. Moreover, few studies that account for smoking history and other known risk factors have been conducted in racially and ethnically minoritized and socioeconomically diverse populations. Objectives: We sought to evaluate the association of ambient air pollution with COPD in a multiethnic population in California. Methods: In the Multieth

Clinical Outcomes of Switching From Warfarin to Apixaban or Rivaroxaban in Patients With Atrial Fibrillation: A Nationwide Multidatabase Study.

BACKGROUND: The benefits of switching from warfarin to direct oral anticoagulants in atrial fibrillation remain unclear. METHODS: This retrospective study used the Medicare fee-for-service (2013-2020) and Optum Deidentified Clinformatics Data Mart databases (2013-2023). Among patients with atrial fibrillation who received warfarin for at least 180 days, we created 2 cohorts: (1) patients switching to apixaban versus continuing warfarin (the apixaban cohort) and (2) patients switching to rivaroxaban versus continuing warfarin (the rivaroxaban cohort). The index date was the switch date for swi

Prevalence and Predictors of Self-Reported Swallowing Difficulties in Community-Dwelling Older Adults: A Population-Based Study from the National Health and Aging Trends Study (NHATS).

It is well established that dysphagia is common amongst older adults, yet there is a paucity of evidence from population-based studies confirming prevalence and risk factors. The current study aimed to determine prevalence and predictors of self-reported swallowing difficulty in community-dwelling older adults. The National Health and Aging Trends Study is a nationally representative population study that has followed American Medicare beneficiaries aged 65 and older since 2011. Annual, interviews collect detailed information on biological, medical, psychological, social, lifestyle and economi

Epidemiology of Kidney Disease in the United States: Highlights from the Centers for Disease Control and Prevention Kidney Disease Surveillance System.

Kidney diseases are an important public health problem, rising in global significance. In place for nearly 2 decades, the Centers for Disease Control and Prevention's Kidney Disease Surveillance System is the first comprehensive surveillance system developed in the United States, focused exclusively on tracking kidney disease before ESKD. The Kidney Disease Surveillance System incorporates key data and trends from multiple, large national-level survey data sources (National Health and Nutrition Examination Survey), electronic medical record data (Military and Veterans Affairs Health Systems),

Cemented Femoral Stem Design Is Not Associated With Risk of Revision After Total Hip Arthroplasty in Patients Aged ≥ 65 Years: An Analysis of the American Joint Replacement Registry.

BACKGROUND: Cemented femoral fixation for total hip arthroplasty (THA) in those aged ≥ 65 years has potential benefits. However, few resources exist to assist in selecting cemented femoral implant designs. We examined the associated risk for all-cause revision, risk of periprosthetic femoral fracture, and aseptic loosening based on a modern classification of cemented femoral stem designs. METHODS: An analysis of primary THA cases in patients aged ≥ 65 years was performed with American Joint Replacement Registry (AJRR) data linked to Centers for Medicare and Medicaid Services data from 2012 to

Racial and Ethnic Disparities in Degenerative Lumbar Disc Disease: A Population-Based Study Using the All of Us Research Program.

BackgroundDegenerative lumbar disc disease (DLDD) can contribute to substantial low back pain and radicular leg pain. Emerging evidence suggests that racial and ethnic disparities in DLDD care could impact clinical outcomes, yet these trends remain largely unexplored. This study investigates differences in health literacy, access to care, and healthcare utilization among patients with DLDD using the All of Us (AoU) Research Program.MethodsAdults diagnosed with DLDD were identified from the AoU database using ICD-9 and ICD-10 diagnostic codes. Participants were stratified by race and ethnicity

Rediscovering Posterior Cervical Foraminotomy/Diskectomy With the Endoscope: A Favorable Cost Comparison with the Traditional Treatment of a Cervical Radiculopathy.

BACKGROUND AND OBJECTIVES: If nonoperative therapy fails, a unilateral cervical radiculopathy has traditionally been treated with anterior cervical diskectomy and fusion or posterior cervical foraminotomy/diskectomy. More recently, full-endoscopic posterior cervical foraminotomy/diskectomy (FEPCF) has gained popularity. In a cohort of patients with isolated, unilateral cervical radiculopathy, we sought to conduct a preliminary, pilot study to compare the total cost of surgical care between open and endoscopic posterior foraminotomy/diskectomy. METHODS: A single-center, retrospective, cohort s

Neighborhood socioeconomic disadvantage is not associated with adverse outcomes following elective spine surgery in older Veterans.

BACKGROUND: In the US, over half of all cervical and lumbar arthrodesis spine surgeries are for people ≥60 years of age. The extent to which adverse outcomes vary by social (eg, disadvantaged neighborhoods) and demographic factors have been scarcely investigated in spine surgery. We investigated the association of social, demographic, and clinical factors with complications, 30-day readmission, and 30-day mortality in older Veterans undergoing elective spine surgery. METHODS: Veterans (N=5,277) aged ≥65 years who underwent inpatient elective spine surgery for degenerative disease in the Veter

Predicting Postoperative Discharge Status and Readmissions in Spinal Metastatic Disease Using Machine Learning Models.

Operative management of spinal metastatic disease is largely for symptom palliation rather than curative and revolves around the expectation that postoperative survival will exceed recovery time. While several scoring systems and models to predict survival exist, few studies have unified diverse predictors into integrated models to predict short-term postoperative outcomes as indicators of recovery. The Merative™ MarketScan® Commercial Database and the accompanying Medicare Supplement were queried for adult patients receiving surgery for extradural spinal metastatic disease between 2006 and 202

Cefazolin Is Associated With a Lower Periprosthetic Joint Infection Risk than Alternative Antibiotic Regimens Following Total Hip Arthroplasty.

BACKGROUND: There have been conflicting reports on the risk of periprosthetic joint infection (PJI) with the use of alternative antibiotics to cefazolin following arthroplasty. We leveraged recent data from a nationally representative multipayer dataset to investigate this question. METHODS: We queried a health care dataset from 2016 to 2022 to identify primary total hip arthroplasties (THAs) using Current Procedural Terminology and International Classification of Disease (ICD)-Procedure Coding System codes (27130, 0SR90xx, 0SRB0xx) with a corresponding osteoarthritis diagnosis (ICD-Clinical

Evaluation of Head and Neck Squamous Cell Carcinoma (HNSCC) Patient Outcomes Based on Rurality: A Retrospective Cohort Study.

INTRODUCTION: Patients with head and neck squamous cell carcinoma (HNSCC) receiving chemoradiotherapy require ongoing treatment and follow-up, which may be influenced by distance to care. Rural patients often face limited access to specialized treatment centers, yet data on rurality's impact on compliance and outcomes remain limited. This study evaluates differences in treatment adherence and outcomes between rural and urban HNSCC patients. METHODS: Retrospective chart review identified patients with HNSCC treated with chemoradiotherapy from 2010-2019 at Sanford Health in Sioux Falls, South D
