Enhancing furcation involvement classification on panoramic radiographs with vision transformers.

BACKGROUND: The severity of furcation involvement (FI) directly affected tooth prognosis and influenced treatment approaches. However, assessing, diagnosing, and treating molars with FI was complicated by anatomical and morphological variations. Cone-beam computed tomography (CBCT) enhanced diagnostic accuracy for detecting FI and measuring furcation defects. Despite its advantages, the high cost and radiation dose associated with CBCT equipment limited its widespread use. The aim of this study was to evaluate the performance of the Vision Transformer (ViT) in comparison with several commonly

Predicting decompression surgery by applying multimodal deep learning to patients' structured and unstructured health data.

BACKGROUND: Low back pain (LBP) is a common condition made up of a variety of anatomic and clinical subtypes. Lumbar disc herniation (LDH) and lumbar spinal stenosis (LSS) are two subtypes highly associated with LBP. Patients with LDH/LSS are often started with non-surgical treatments and if those are not effective then go on to have decompression surgery. However, recommendation of surgery is complicated as the outcome may depend on the patient's health characteristics. We developed a deep learning (DL) model to predict decompression surgery for patients with LDH/LSS. MATERIALS AND METHOD: W

Deep learning reconstruction in prostate MRI: technical principles, clinical applications, and future directions.

Multiparametric magnetic resonance imaging (mpMRI) has become integral to the detection, localization, risk stratification, and management of prostate cancer. Increasing utilization of prostate MRI in screening, active surveillance, targeted biopsy guidance, and treatment planning has amplified the demand for faster imaging protocols while maintaining high diagnostic quality. Conventional MRI acceleration strategies, including parallel imaging and compressed sensing, improve acquisition efficiency but remain limited by trade-offs involving signal-to-noise ratio (SNR), image sharpness, artifact

Outcomes of Lobectomy vs Sublobar Resection for Clinical Stage IA3 Non-Small Cell Lung Cancer.

BACKGROUND: Recent trials demonstrate comparable outcomes between sublobar resection (SLR) and lobar resection (LR) for stage IA1-2 (≤2 cm) non-small cell lung cancer (NSCLC). Conversely, historical studies reported more recurrences with SLR than with LR for stage IA3 NSCLC (>2 cm, ≤3 cm), although these investigations predated diagnostic and therapeutic advancements. We aimed to compare SLR and LR outcomes in these patients in the contemporary era. METHODS: Patients assigned to clinical stage IA3 NSCLC undergoing resection at a single institution (2015-2024) were stratified by wedge resectio

Outcomes following left-sided pancreatectomy management with and without splenectomy for major pancreatic injury.

BACKGROUND: Traumatic pancreatic injuries carry significant mortality and often involve splenectomy because of the anatomic proximity of the spleen. However, current trauma guidelines provide no evidence-based recommendation for or against routine splenectomy at the time of left-sided pancreatectomy, instead advocating an individualized approach. Contemporary data comparing left-sided pancreatectomy management with splenectomy and without splenectomy (nonsplenectomy) remain limited. We evaluated perioperative outcomes following splenectomy versus nonsplenectomy left-sided pancreatectomy in a n

In Search of the "Eloquent Brain": History, Science, and Future.

BACKGROUND AND OBJECTIVES: Many decisions in neurosurgical practice are guided by whether "eloquent" or "noneloquent" cortex is involved in a patient's disease. Despite the ubiquity of these terms, their historical origins and scientific validity are rarely questioned. This review examines the divergent histories of cerebral localization and neurosurgical nomenclature to critically evaluate the utility of the "eloquence" framework in the modern era. METHODS: A literature review was conducted in PubMed to identify early and influential uses of this terminology. Historical textbooks and scienti

Metabolic Syndrome, Component Burden, and Incident Gastric Cancer Risk: A Prospective Cohort Study in the UK Biobank.

UNLABELLED: As Helicobacter pylori prevalence declines, understanding the contribution of modifiable systemic factors to gastric cancer risk becomes increasingly important. We evaluated the association of metabolic syndrome (MetS), its individual components, and anatomic subsites with incident gastric cancer in 471,540 UK Biobank participants. Multivariable Cox proportional hazards models were used to estimate the risk of overall, cardia, and non-cardia gastric cancers, and restricted cubic splines were used for exploratory nonlinear analyses. Over a mean follow-up of 6.5 years, 332 incident g

Warmed intravenous fluid administration attenuated a reduction in body temperature in anaesthetised horses.

BACKGROUND: Peri-anaesthetic hypothermia is a well-documented complication of general anaesthesia and is associated with prolonged recovery, impaired wound healing, and delayed coagulation. Fluid warmers can be used to mitigate heat loss, but there is little evidence supporting their effectiveness in equine anaesthesia. OBJECTIVES: To determine whether administration of warmed intravenous fluids attenuates heat loss during general anaesthesia in horses. STUDY DESIGN: Prospective, randomised, crossover in vivo experiment. METHODS: Six healthy adult horses were anaesthetised twice and adminis

Indocyanine green fluorescence angiography for prevention of anastomotic leak in colorectal surgery: a graded systematic review and meta-analysis with trial sequential analysis of randomized controlled trials.

BACKGROUND: Anastomotic leak (AL) is a major driver of morbidity after colorectal resection. Indocyanine-green fluorescence angiography (ICG-FA) provides real-time intraoperative perfusion assessment, but its impact across randomized trials remains debated. This review aims to evaluate the efficacy and safety of ICG-FA compared with conventional intraoperative assessment in preventing AL in colorectal surgery. METHODS: A systematic review and a meta-analysis was conducted in accordance with the PRISMA guidelines, including RCTs comparing ICG-FA with conventional assessment. The primary outcom

Exploring the relationships between multisite musculoskeletal pain, pain characteristics, and physical function: a cross-sectional analysis.

OBJECTIVE: Multisite musculoskeletal pain is highly prevalent and negatively impacts physical function through several potential mechanisms. However, it remains unclear how the number of affected pain regions relates to function. This study explored associations between different aspects of pain (global pain intensity, catastrophizing, kinesiophobia, pain interference) and physical function in individuals with multisite pain. METHODS: This cross-sectional analysis included 101 participants aged 18-85 years. Multisite pain was assessed across eight anatomic regions, using binary questions, wit

Influence of Refraction on Widefield Retinal and Choroidal Vascular Metrics in Optical Coherence Tomography Angiography in a Caucasian Population.

PURPOSE: Refraction-related differences in retinal and choroidal vascular metrics have been reported using optical coherence tomography angiography (OCTA), primarily within the central fundus. This study aimed to establish normative vascular reference values for the retina and choroid using widefield swept-source OCTA (WF-SS-OCTA), stratified by spherical equivalent (SE), to characterize associations between refractive status and OCTA-derived vascular measurements extending beyond the central fundus. METHODS: In this prospective single-center study, healthy volunteers underwent WF-SS-OCTA at

Etiologic heterogeneity across oral cavity cancers.

BACKGROUND: Oral cavity cancers are generally considered etiologically similar, with a majority (>75%) attributable to smoking/alcohol. Yet, incidence trends for oral cavity cancers have recently diverged in the United States and several European countries (with declines for most oral cavity cancers but increases for oral tongue cancers), suggesting etiologic differences. This study quantified etiologic heterogeneity across oral cavity cancers using data from the NIH-AARP Diet and Health Study. METHODS: Cox regression models were used to quantify associations (hazard ratios [HRs] and populati

[18F]AlF-NOTA-PCP2 PET/CT to Assess PD-L1 Status in Head and Neck Cancer: Head-to-Head Comparison With [18F]FDG PET/CT.

PURPOSE: While PD-L1 expression heterogeneity limits the predictive value of localized biopsies, molecular imaging offers a dynamic alternative. We developed [18F]AlF-NOTA-PCP2, a hydrophilic PD-L1-targeted peptide tracer, and first evaluated its clinical utility in head and neck (HNC) cancer through a head-to-head comparison with [18F]FDG PET/CT. METHODS: Preclinical validation included competitive binding assays and micro-PET/CT in PD-L1-positive HNC xenografts. A prospective trial (NCT04304066) enrolled 24 HNC patients for [18F]AlF-NOTA-PCP2 PET/CT, with paired [18F]FDG scans in 17 patient

The Genetics of Cerebellar Structure and Associations With Cognitive Performance: A Twin Magnetic Resonance Imaging Study.

The cerebellum, traditionally associated with motor control, is increasingly recognized for its involvement in higher-order cognitive functions. However, the role of cerebellar subregions in cognition remains underexplored, as are the roles of genetic factors on cerebellar structure and brain-behavioral associations. The primary goal of this study was to investigate the relationship between cerebellar subregion volumes and cognitive performance. A secondary aim was to quantify the genetic contributions to cerebellar structure and determine the degree to which any brain-behavior associations we

Higher postoperative gabapentin doses are associated with pulmonary complications after total shoulder arthroplasty.

BACKGROUND: Gabapentinoids are increasingly being used as adjunctive analgesic medications after total shoulder arthroplasty (TSA). However, data suggest that when used in conjunction with opioids, gabapentinoids may induce sedation and possibly potentiate respiratory depression. The aim of this study was to evaluate the impact of gabapentin postoperative pulmonary complications and total opioid utilization after primary TSA. METHODS: Patients who underwent primary, elective anatomic and reverse TSA from January 1, 2016, to December 31, 2020, were identified using the Premier Healthcare Datab

Patient-related prognostic factors in older adults with head and neck cancer: The EGeSOR clinical trial.

BACKGROUND: Due to its anatomic site, the treatment of head and neck cancer (HNC) significantly affects activities of daily living. Clinical heterogeneity and accelerated aging in older adults make it crucial to identify patient-related, prognostic factors. The objective of the present study was to assess the prognostic value of patient-related factors for 2-year overall survival (OS) in older adults with HNC, after adjustment for tumor-related factors. METHODS: This was a secondary analysis of the EGeSOR randomized, controlled trial conducted in France between 2013 and 2018. Patients with HN

Health care system factors associated with receipt of treatment and treatment intent in stage III non-small cell lung cancer: A population-based study in Ontario.

PURPOSE: Stage III non-small cell lung cancer (NSCLC) is a heterogeneous disease, with a spectrum of anatomic extent, health status, and treatment approaches. Receipt of treatment and its intent should be independent of health system factors where care quality is optimal. We investigated the degree that modifiable health system factors are associated with receipt of treatment and treatment intent in stage III NSCLC in a large, universal health system. METHODS: This was a population-based, retrospective cohort study with health administrative data from Ontario, Canada, 2010-2018 for those aged

Successful Cardiac Resynchronization Therapy Reduces Negative Septal Work in Patient-Specific Models of Dyssynchronous Heart Failure.

In patients with dyssynchronous heart failure (DHF), cardiac conduction abnormalities cause the regional distribution of myocardial work to be non-homogeneous. Cardiac resynchronization therapy (CRT) using an implantable, programmed biventricular pacemaker/defibrillator, can improve the synchrony of contraction between the right and left ventricles in DHF, resulting in reduced morbidity and mortality and increased quality of life. Since regional work depends on wall stress, which cannot be measured in patients, we used computational methods to investigate regional work distributions and their
