Presumed Idiopathic Replacement Resorption in a Permanent First Molar in a Child With Multiple Tooth Agenesis: A Case Report.

Replacement resorption is a rare pathological condition in permanent teeth, characterized by progressive substitution of root dentin with bone tissue, typically following traumatic injuries. This case report presents an unusual occurrence of replacement resorption in the mesial root of the mandibular right first permanent molar (46) in an eight-year-old boy with no documented history of trauma, associated with non-syndromic agenesis of multiple permanent teeth (15, 34, 35, 44, 45). Despite the reserved long-term prognosis, conservative endodontic management was pursued due to the significant t

Dyke-Davidoff-Masson syndrome in a young adult: Lifelong hemiparesis and characteristic MRI findings.

Dyke-Davidoff-Masson syndrome (DDMS) is a rare disorder characterized by cerebral hemiatrophy and compensatory calvarial changes. We report a 22-year-old male with lifelong left-sided spastic hemiparesis and developmental delay, with a history of prematurity, neonatal meningitis, and childhood head trauma. Brain MRI demonstrated right cerebral hemiatrophy, exvacuo dilatation of the lateral ventricle, ipsilateral sulcal prominence, calvarial thickening, frontal sinus enlargement, and Wallerian degeneration of the corticospinal tract. This case is notable for the coexistence of multiple early-li

Iatrogenic chest tube placement within the heart following a large pneumothorax.

This case report describes an uncommon occurrence of cardiac injury and perforation resulting from chest tube placement. The patient, a 44-year-old female, presented to the ED with severe blunt force trauma following a pedestrian motor vehicle collision. Imaging revealed a large pneumothorax and multiple fractures. Pulsatile bleeding was observed during chest tube placement, and the tube's tip was later found on CT of the thorax to be located within the heart. Although the patient recovered by postoperative day 13 and was subsequently discharged, this case highlights the importance of performi

Treatment of hypothalamic obesity in people with hypothalamic injury: new drugs are on the horizon.

Hypothalamic obesity (HO) is a complex and rare disorder affecting multiple regulatory pathways of energy intake and expenditure in the brain as well as the regulation of the autonomic nervous system and peripheral hormonal signaling. It can be related to monogenic obesity syndromes which often affect the central leptin-melanocortin pathways or due to injury of the hypothalamus from pituitary and hypothalamic tumors, such as craniopharyngioma, surgery, trauma, or radiation to the hypothalamus. Traditional treatments of obesity, such as lifestyle intervention and specific diets, are still a the

Unilateral Internuclear Ophthalmoplegia Following Minor Head Injury.

A lesion in the medial longitudinal fasciculus (MLF) causes internuclear ophthalmoplegia (INO). Many intracranial lesions, such as multiple sclerosis or vascular disorders may be associated with INO; however, INO is a rare complication of minor head injury. The mechanism underlying injury to the MLF may be shear force on the brain stem during head trauma. The shear force can tear or stretch the fibers of the MLF and can also lead to compromise or rupture of the perforating branches of the basilar artery. We present an unusual case of unilateral INO after minor head injury in a patient with a s

Costal cartilage fracture: A commonly missed thoracic injury in trauma patients.

In this report, we describe a case of a 43-year-old male who sustained a displaced mid chondral costal cartilage fracture along with multiple ipsilateral rib fractures following a jet ski collision. Costal cartilage fractures (CC) are commonly missed on imaging evaluation, and accordingly underreported in the literature. High-energy blunt chest trauma represents the most common mechanism for CC injuries. Computed Tomography (CT) is the modality of choice to diagnose CC fractures along with the associated cardiopulmonary and abdominal injuries in polytrauma patients. There is currently no conse

Superolateral Dislocation of Bilateral Mandibular Condyles With a Mandibular Body and Condyle Fracture.

An 82-year-old man fell from a motorcycle and was admitted with superolateral dislocation of the bilateral mandibular condyles. The displaced condyles on the right and left were hooked in the posterior root of the zygomatic arch and in the temporal fossa beyond the zygomatic arch, respectively. There was a sagittal split fracture on the medial side of the right mandibular condyle and a right mandibular body fracture. Under general anesthesia, manual reduction of the dislocation was successfully performed the day after the injury. Bilateral superolateral dislocations hooked to the temporal foss

Protecting the tracheal tube cuff: a novel solution.

We describe the successful insertion of a nasotracheal tube following repeated cuff rupture. The patient was a 55-year-old woman with a history of nasal trauma and multiple rhinoplasties, who underwent elective Lefort I osteotomy and bilateral sagittal split osteotomy for correction of skeletal facial deformity. During fiberoptic bronchoscope-guided nasal intubation after the induction of general anesthesia, the tracheal tube repeatedly ruptured in both nares, despite extensive preparation of the nasal airways. We covered the cuff with a one-inch tape, intubated to the level of the oropharynx,

Kindler Syndrome: A Multidisciplinary Management Approach.

Kindler syndrome is a very rare form of bullous epidermolysis. It is a hereditary condition caused by a mutation in the FERMT1 gene that encodes the protein kindlin-1. It is clinically characterized by trauma-induced blistering, diffuse skin atrophy, poikiloderma, pseudosyndactyly, and photosensitivity. The most common mucosal manifestations are conjunctivitis, ectropion, hemorrhagic gingivitis, periodontal disease, premature tooth loss, and severe colitis. We present the first 4 cases of Kindler syndrome diagnosed at the Instituto Nacional de Salud del Niño in Lima, Peru. These cases highligh

A Novel Treatment for Massive Hemorrhage after Maternal Trauma in Pregnancy.

Background  Trauma in pregnancy can lead to life-threatening hemorrhage. Conventional treatments of hemorrhage include medical and surgical management. However, if these measures fail uterine compression is an option to control bleeding. We present a case where this management was employed. Case  A patient presented at 36 weeks of gestation with multiple injuries after a motor vehicle collision and experienced disseminated intravascular coagulation (DIC). The use of a Bakri balloon in combination with external compression with Coban, a sterile self-adherent bandage, after delivery temporized h

Polyvictimization in childhood and its adverse impacts across the lifespan: Introduction to the special issue.

Although much empirical work has focused on the adverse impact of specific types of childhood victimization (e.g., sexual, physical, or emotional abuse), researchers and clinicians increasingly are recognizing the prevalence of polyvictimization, or exposure to multiple types of victimization. Polyvictimization during formative developmental periods may have detrimental and potentially lifelong biopsychosocial impacts over and above the effects of exposure to specific types of adversity. In this guest editorial, we summarize the key questions and findings for six empirical studies on polyvicti

Traumatic asphyxia.

Traumatic asphyxia is a rare condition in children that usually occurs after severe compression to the chest or abdomen. We report 3 cases in patients 18, 20, and 36 months of age who presented signs and symptoms of traumatic asphyxia after car accidents. Two clinical features were consistent in all 3 patients: multiple petechiae on the face and bulbar conjunctival hemorrhage; 2 patients had facial cyanosis, and 1 had facial edema. In children, the number of clinical manifestations that should be evident to diagnose traumatic asphyxia has not been ascertained. However, in any history of trauma

"Synchronous" multifocal necrotizing fasciitis.

BACKGROUND: Necrotizing fasciitis (NF) is an infection of the soft tissue, and is fatal if not promptly and aggressively treated. Although it is rare, it is not exceptional; nevertheless, its presentation may be misleading and may delay the diagnosis. We highlight the possible synchronous development of NF in multiple noncontiguous areas. CASE REPORT: A 44-year-old diabetic man with no history of trauma complained of nonspecific lower back pain, which he treated with analgesics and oral antibiotics. Erythema at the left arm appeared, and the general condition worsened. The patient was admitte

Aneurysmal bone cyst of thoracic spine: case report and brief review of literature.

A 16-year-old girl was admitted with insidious onset, gradually progressive spastic sensori-motor paraparesis, with a sensory level at D10 dermatome without bowel or bladder involvement for the last 2 months following trivial trauma on the lower back. MRI of the spine showed a low-to-intermediate signal intensity, heterogeneous mass with multiple fluid levels. A diagnosis of aneurysmal bone cyst was made. A D8-D9 laminectomy with near total excision of mass was performed. Histopathology of the mass showed cyst cavity filled with haemorrhage surrounded by bony trabeculae confirming the diagnosi

Effectiveness of group psychotherapy for adult outpatients traumatized by abuse, neglect, and/or pregnancy loss: a multiple-site, pre-post-follow-up, naturalistic study.

The New Experience for Survivors of Trauma (NEST) is a group psychotherapy intervention for clients traumatized by consequences of abuse, neglect, and pregnancy loss. This multiple site study is the first investigation of its effectiveness. Ninety outpatients from a naturalistic setting completed the Symptom Checklist and the Sense of Coherence questionnaire at baseline, end of treatment, and one-year follow-up. Effectiveness was tested with statistical significance, effect size, and clinical significance. Clients from the total sample as well as from the abortion subsample showed improvement

Cricoid chondronecrosis: a complication of endotracheal intubation.

Stenosis of the subglottic area is a common complication of endotracheal intubation and external blunt trauma. Chondronecrosis of the cricoid cartilage is a rare but known complication of neck radiotherapy for laryngeal carcinoma. It has also been reported as a complication of prolonged endotracheal intubation. The clinical and bronchoscopic findings are non-specific and the diagnosis can not be accurately established without surgical evidence or computed tomographic (CT) findings. In addition to technical issues related intubation, multiple anatomic and host factors can contribute to the path

Excessive glycation of pericapillary pericytic matrix components is an essential mechanism of arterial hypertension development in hyperglycemia.

In patients with essential hypertension, inhibition of glucose utilization in tissues was detected similar to that in diabetics. High levels of glycated compounds were found in cerebral, cardiac, and renal tissues of patients dead from coronary heart disease complicated and not complicated from diabetes mellitus and essential hypertension compared to victims dead from traumas. Experimental studies on rabbits and rats showed that the content of glycoconjugates in tissues did not increase in short-term (exogenous or stress-associated) hyperglycemia, while accumulation of glycoconjugates in peric

Transmylohyoid/Submental intubation: review, analysis, and refinements.

Successful outcome in any surgery is dependent on unobstructed access. Management of patients with polytrauma of the face or those undergoing multiple/complex facial osteotomies has always been a challenge, not only to maxillofacial surgeons but also to the anesthetists, as both specialists fight for the same anatomic territory. Hernandez in 1986 published the first article on the submental route for endotracheal intubation. He developed this technique to avoid tracheotomy particularly in maxillofacial trauma where short-term maxillomandibular fixation was required.Since our first report in 19
