Imaging of chest wall disorders.

Pathologic processes that may involve the chest wall include congenital and developmental anomalies, inflammatory and infectious diseases, and soft-tissue and bone tumors. Many of these processes have characteristic radiologic appearances that allow definitive diagnosis. Sternal deformities can be visualized at radiography and their severity quantified with computed tomography (CT). In cervical rib, CT with multiplanar reconstruction may demonstrate relevant anatomic detail and the relationship between bone deformity and arterial compression. In Poland syndrome, radiography reveals an area of

Psoriatic arthritis and the spectrum of syndromes related to the SAPHO (synovitis, acne, pustulosis, hyperostosis, and osteitis) syndrome.

During the past year, the increasing use of nuclear magnetic resonance imaging techniques, with their ability to delineate cartilage and ligamentous structures and to identify edema, are providing a radical improvement in ascertainment of musculoskeletal abnormalities, although their significance remains incompletely delineated. A second theme has come from the study of spondyloarthropathies in different ethnic groups and societal environments, revealing that the Northern European and North American form of the disease, with its powerful association with HLA-B27, is little evident in the rest

The use of protein A columns in the treatment of cancer and allied diseases.

The staphylococcal cell-wall protein known as protein A has been explored as a therapeutic modality in the treatment of cancer and allied diseases. Protein A binds the Fc fragment of IgG 1, 2 and 4, and preferentially binds to IgG incorporated into immune complexes. Early investigators focused on the immune-suppressive effects of immune complexes in cancer and, based on in vitro experiments, postulated that clearance of immune complexes in vivo would permit effective immune clearance of cancer cells. A large clinical trial of the perfusion of cancer patient plasma over protein A was subsequent

[New advances in rheumatology].

(1) Molecular pharmacology which is a starting to reach the market place, what is expected to be the next big step in the biotechnology revolution: the creation of complex compounds that are custom-designed to meet human needs. The new technology known as protein engineering promises to take us one step further by modifying natural proteins or by creating new ones especially by using somatic gene transfer and germ gene transfer and its influence on progeny. By doing this we hope to control some of genetic diseases as well to find cure for arthritis. (2) Demonstration of infective agent as a ca

Pre-existing morbidity pattern in a Danish testicular cancer cohort: insights beyond the testicular dysgenesis syndrome hypothesis.

STUDY QUESTION: Do men diagnosed with testicular cancer (TC) exhibit increased pre-existing morbidity compared to matched controls? SUMMARY ANSWER: Men with TC had a significantly higher risk of hospital contacts and medicinal use prior to diagnosis compared to controls, reflecting excess morbidity across multiple health domains. WHAT IS KNOWN ALREADY: The testicular dysgenesis syndrome hypothesis suggests that, e.g. cryptorchidism, poor semen quality, and TC are all symptoms of a fetal gonadal dysgenesis. The association of TC with broader pre-existing morbidity remains unclear. STUDY DESI

Ayurvedic Management of Post-hysterectomy Osteoporosis: A Case Report.

Osteoporosis is a systemic metabolic bone disorder characterized by decreased bone mass and microarchitectural deterioration. These changes increase fragility and fracture risk. The condition is seen particularly in postmenopausal and post-hysterectomy women. Hormone decline-related bone loss is often linked to chronic musculoskeletal pain, functional limitation, and reduced quality of life. Long-term use of modern antiresorptive therapies, though effective, may be limited by tolerability concerns and adherence challenges. This creates a rationale for safer, multimodal conservative approaches.

Health-related quality of life in French adults with X-linked hypophosphatemia: real-world data from the International XLH Registry.

X-linked hypophosphatemia (XLH) is a rare genetic condition in which excess fibroblast growth factor 23 causes renal phosphate wasting, leading to skeletal morbidities. Patients experience musculoskeletal pain, stiffness, and fatigue, with impaired physical function and health-related quality of life (HRQL). Burosumab has been available in France for the treatment of XLH since 2021; European treatment guidelines suggest use in adults with pseudofractures or with insufficient response and/or intolerance to oral phosphate supplements and active vitamin D. The International XLH Registry is collec

Occupational and leisure-time physical activity and prospective musculoskeletal pain-relief prescribed medication among blue-collar workers: 24-h device-measured physical activity and Danish registers.

Musculoskeletal pain is the biggest contributor to burden of disease, causing excessive use of pain-relief medication. Non-pharmaceutical measures are called on for handling this burden. Guidelines recommend physical activity as an effective non-pharmaceutical measure. However, it is unknown if occupational physical activity has the same preventive effects on redeeming pain-relief medication as when performed during leisure-time ("The Physical Activity Paradox"). We aimed to investigate if the paradox contributes to the redemption of prescribed pain relief-medication. 24-h device-measured phys

Patient-Reported Reasons for Antihypertensive Medication Change: A Quantitative Study Using Social Media.

INTRODUCTION: Hypertension is the leading cause of heart disease in the world, and discontinuation or nonadherence of antihypertensive medication constitutes a significant global health concern. Patients with hypertension have high rates of medication nonadherence. Studies of reasons for nonadherence using traditional surveys are limited, can be expensive, and suffer from response, white-coat, and recall biases. Mining relevant posts by patients on social media is inexpensive and less impacted by the pressures and biases of formal surveys, which may provide direct insights into factors that le

Use of nonsteroidal anti-inflammatory drugs in patients with cardiovascular disease: a cautionary tale.

Nonsteroidal anti-inflammatory drugs (NSAIDs) inhibit production of prostaglandins by acting on cyclooxygenase (COX) isoenzymes 1 and 2. Nonselective NSAIDs inhibit both COX 1 and 2 isoenzymes (eg, ibuprofen and naproxen). Selective NSAIDs act on COX-1 (eg, aspirin) or COX-2 (eg, celecoxib) isoenzymes, respectively. Prostaglandins are produced in platelets and gastric mucosal cells through constitutively expressed COX-1 isoenzyme. They are involved in the regulation of hemostasis, functional integrity of the gastrointestinal and renal tracts, platelet function, and macrophage differentiation.

When exactly can carpal tunnel syndrome be considered work-related?

BACKGROUND: Carpal tunnel syndrome (CTS), compression of the median nerve at the wrist, is the most frequently encountered peripheral entrapment neuropathy. Whilst rates of all other work-related conditions have declined, the number of work-related musculoskeletal disorders (which include CTS) has not changed for the past 9 years in the USA. Median days off work are also highest for CTS: 27 compared to 20 for fractures and 18 for amputations. This results in enormous Workers Compensation and other costs to the community. Awareness of CTS as a disorder associated with repeated trauma at work is

Musculoskeletal health of the woman dentist: distinctive interventions for a growing population.

Female dentists face unique musculoskeletal demands and inherent gender differences that may place them at higher risk for occupational pain and injury than their male counterparts. By familiarizing themselves with unique muscle imbalances, specific pain syndromes, and positioning challenges to which they are predisposed, female dentists may take pre-emptive action and initiate ergonomic intervention in the operatory and specific exercise at home to reduce the risk of developing musculoskeletal disorders.

[Aging at work and musculoskeletal disorders].

By means of a critical review of the international literature and of their own published experiences, the Authors discuss the influence of the "age" factor on work related musculoskeletal disorders of the spine and upper limbs. Regarding the spine, the lumbosacral spine in particular, there is evidence (both in relation to pathways and from epidemiological data) of the influence of age in determining a progressive increase in the occurrence of spondyloarthropathy with clear radiological signs. For upper limb disorders the influence of the "age" factor is still under debate and in any case does

Disease-specific knowledge and differential diagnostic skill in clinical care.

BACKGROUND AND PURPOSE: Back and neck pain are among the most prevalent musculoskeletal complaints, imposing a substantial individual and societal burden on healthcare systems. Effective management requires adequate education, strong clinical decision-making skills, and accurate interpretation of red flags. This narrative literature review explores the educational, diagnostic, and decision-making aspects of back and neck pain among laypeople, healthcare students, and professionals. It aims to synthesize findings from studies assessing disease-specific knowledge to emphasize how targeted educat

Exome sequencing of 1190 non-syndromic clubfoot cases reveals HOXD12 as a novel disease gene.

BACKGROUND: Clubfoot, presenting as a rigid inward and downward turning of the foot, is one of the most common congenital musculoskeletal anomalies. The aetiology of clubfoot is poorly understood and variants in known clubfoot disease genes account for only a small portion of the heritability. METHODS: Exome sequence data were generated from 1190 non-syndromic clubfoot cases and their family members from multiple ethnicities. Ultra-rare variant burden analysis was performed comparing 857 unrelated clubfoot cases with European ancestry with two independent ethnicity-matched control groups (104

Patients with ankylosing spondylitis present a distinct CD8 T cell subset with osteogenic and cytotoxic potential.

OBJECTIVES: Ankylosing spondylitis (AS) is a chronic inflammatory rheumatic disease affecting mainly the axial skeleton. Peripheral involvement (arthritis, enthesitis and dactylitis) and extra-musculoskeletal manifestations, including uveitis, psoriasis and bowel inflammation, occur in a relevant proportion of patients. AS is responsible for chronic and severe back pain caused by local inflammation that can lead to osteoproliferation and ultimately spinal fusion. The association of AS with the human leucocyte antigen-B27 gene, together with elevated levels of chemokines, CCL17 and CCL22, in th

A Case of Native Joint Septic Arthritis Caused by Bacteroides fragilis.

BACKGROUND Bacteroides fragilis (B. fragilis) is an uncommon cause of native joint septic arthritis (NJSA). It is an anaerobic gram-negative rod that is normally found in the oral cavity, gastrointestinal tract, genitourinary system, and skin, and thus is rarely isolated from the musculoskeletal system. Risk factors for B. fragilis NJSA include inflammatory arthritis, malignancy, sickle cell disease, and ischemic heart disease. CASE REPORT We discuss a case of a 65-year-old man with coronary artery disease, heart failure, chronic kidney disease, and gout, who presented with 5 days of right kne

Hand dominance in early and established rheumatoid arthritis: evaluation by dynamometer, Ritchie index and musculoskeletal ultrasound: a cross sectional study.

Rheumatoid arthritis (RA) usually occurs as a symmetrical disease, which mainly affects the small joints of the hands and feet. The correlation of handedness with radiological changes shows significantly greater radiological changes in the dominant hand than in the non-dominant one. Additionally, the dominant hand is more severely affected in terms of strength, function and deformity. Our objective is to evaluate the influence of handedness on musculoskeletal ultrasound (US), Ritchie articular index (RAI) and digital dynamometer findings in patients with active RA (early, group B, vs. establis

Advanced Interventional Pain Management Approach to Neoplastic Disease Outside the Spine.

Neoplastic disease of the musculoskeletal system may result in serious morbidity and mortality secondary to cancer related bone pain, pathologic fracture, altered structural mechanics, and involvement of adjacent structures.1 Recent advances in cancer detection and treatment have allowed more patients to live longer. The prevalence of osseous metastatic disease has increased to 100,000 new patients developing bone metastases each year.2 These patients are seeing long-term exposure to chemotherapy and radiation leading to increased skeletal events, morbidity, and a negative impact on quality of
