Geographical location and genotyping analysis of pulmonary tuberculosis in the state of Nuevo Leon, Mexico.

Tuberculosis, caused by Mycobacterium tuberculosis, is a major global health problem. In Mexico, the State of Nuevo Leon is among the top ten in tuberculosis morbidity. Information about transmission patterns and case clustering for tuberculosis in Nuevo Leon is limited. The spoligotypes of 151 isolates from newly diagnosed pulmonary tuberculosis patients were obtained and its phenotypic drug susceptibility pattern for streptomycin, isoniazid, rifampin, ethambutol, and pyrazinamide was determined by using the Mycobacteria Growth Indicator Tube fluorometric method. Geographical data of isolates

Minimum Inhibitory Concentrations of Fluoroquinolones and Pyrazinamide Susceptibility Correlate to Clinical Improvement in Multidrug-resistant Tuberculosis Patients: A Nationwide Swedish Cohort Study Over 2 Decades.

BACKGROUND: Minimum inhibitory concentration (MIC) testing, unlike routine drug susceptibility testing (DST) at a single critical concentration, quantifies drug resistance. The association of MICs and treatment outcome in multidrug-resistant (MDR)-tuberculosis patients is unclear. Therefore, we correlated MICs of first- and second-line tuberculosis drugs with time to sputum culture conversion (tSCC) and treatment outcome in MDR-tuberculosis patients. METHODS: Clinical and demographic data of MDR-tuberculosis patients in Sweden, including DST results, were retrieved from medical records from 1

Role of real-time PCR for detection of tuberculosis and drug resistance directly from clinical samples.

BACKGROUND: Only a few studies done earlier in India reveal the utility of real-time PCR in detecting drug resistance in cases of pulmonary tuberculosis. OBJECTIVES: The study was carried out to standardise real-time PCR (Quantitative real-time PCR, qPCR) targeting 16s RNA for the rapid detection of tuberculosis and its drug resistance from suspected TB patients. MATERIALS AND METHODS: Sputum samples from 100 clinically suspected tuberculosis patients, after processing were subjected to microscopy, MGIT culture and qPCR. qPCR targeted 16sRNA for detecting Mycobacterium tuberculosis complex,

[The possibilities of shortening the treatment periods in patients with tuberculosis of the peripheral lymph nodes].

Outcomes of shorter periods of treatment of patients with tuberculosis of the peripheral lymph nodes are presented. It is possible to reduce the period of treatment provided early surgical intervention is performed to remove the affected nodes. One recurring case detected after the long-term therapy. The counterindications for reduction of treatment periods of the above disease are generalization of the process and Mycobacterium resistance to tuberculostatic drugs.

Mycobacterium gordonae genitourinary disease.

Mycobacterium gordonae is frequently isolated from urine, but M gordonae genitourinary disease is rare; the majority of the isolates are commensals. We describe a 40 year old housewife who presented with loin pain, dysuria and frequency. Urine contained excessive pus cells, was sterile on culture and she did not respond to broad spectrum antibiotics. There was repeated isolation of M gordonae from the urine and she responded to a standard antituberculosis regimen. She was irregular and non-compliant with supervised therapy and relapsed three months after stopping medications. She again had sym

[Specific features of tuberculosis in childhood].

Primary infection with Mycobacterium tuberculosis usually occurs during childhood. The source of infection is most often an adult. The risk of infection in exposed children is modulated by various factors related to the infectiousness of the index case, exposure conditions, and the child himself. This review aims to describe the specific diagnostic and therapeutic features of latent TB infection and TB disease in childhood.

Endovascular stent graft placement in the treatment of a ruptured tuberculous pseudoaneurysm of the descending thoracic aorta secondary to Pott's disease of the spine.

Ruptured thoracic aortic tuberculous pseudoaneurysms as a complication of mycobacterium tuberculosis infection of the spine are rare. Conventional treatment of a ruptured tuberculous pseudoaneurysm involves surgery with graft interposition or patch repair. We report successful repair of a ruptured tuberculous pseudoaneurysm of the descending thoracic aorta by endovascular stent graft placement and provide a literature review of such entities.

[Buruli ulcer--necrotizing infection of the hand of a plastic surgeon].

The Buruli ulcer is caused by mycobacterium ulcerans. The clinical features of this atypical mycobacteriosis are similar to tuberculosis. Differentiation is only possible by microbiological investigation. In cases of chronic soft tissue infections mycobacteriosis must be taken into consideration. Since microbiological differentiation needs 8-12 weeks and chemotherapy is not effective, early and wide surgical excision is the treatment of choice.

Utilization of the CRISPR-Cas system for investigating tuberculosis drug resistance.

Drug-resistant tuberculosis(TB) presents a significant challenge to global efforts to control the disease. Addressing this issue requires a comprehensive understanding of drug resistance in Mycobacterium tuberculosis (M.tb). The clustered regularly interspaced short palindromic repeat (CRISPR) and CRISPR-associated protein (Cas) system are efficient genome-editing tools for various organisms and have facilitated new research directions regarding drug resistance in M.tb. This review summarizes recent progress in applying CRISPR/Cas technology to investigate drug resistance mechanisms in M.tuber

Musculoskeletal Tuberculous Arthritis Mimicking Septic Arthritis.

A young woman presented with left hip pain for one week. She had been admitted to another hospital one month prior for right ankle pain and was undergoing treatment for septic arthritis with home IV antibiotics. Imaging of the left hip and right ankle was consistent with septic arthritis, but washout of the left hip and right ankle yielded negative gram stain and bacterial cultures. Due to persistent fevers, further imaging revealed miliary tuberculosis, and AFB testing of the washout fluid from the right ankle eventually resulted positive for Mycobacterium tuberculosis. The patient was placed

A rare encounter: Tuberculous pyomyositis in an immunosuppressed post-transplant patient.

Tuberculous pyomyositis is an uncommon form of extrapulmonary tuberculosis (EPTB). We report a rare case of EPTB in a 34-year-old male post-renal transplant recipient. He presented with pain in the left hip region and fever 9 years after the renal transplantation. A tender swelling was noted over the left hip region, visualized as a loculated hyperintense lesion on MRI. Ultrasound-guided aspiration revealed a purulent discharge which showed acid-fast bacilli on Ziehl-Neelsen staining. GeneXpert MTB confirmed the presence of Mycobacterium tuberculosis, sensitive to Rifampicin. The patient was d

Tuberculids: A Narrative Review.

Tuberculids are a group of dermatoses with tuberculoid histology and the absence of tubercle bacilli. They are considered to be hypersensitivity reactions to circulating Mycobacterium tuberculosis (M. tb) or its antigens in individuals with good immunity. The objective of the review is to provide a detailed literature review of all available articles on tuberculids in the past 10 years and provide an update on epidemiology, etiopathogenetic mechanisms, clinical manifestations, and treatment. A search was performed on PubMed using the keywords lichen scrofulosorum, papulonecrotic tuberculid, er

Tuberculous Osteomyelitis Mimicking a Lytic Bone Tumor: Report of Two Cases and Literature Review.

Tuberculous osteomyelitis is an uncommon form of tuberculosis (TB); the isolated involvement of the wrist joint is particularly rare. The symptoms and clinical manifestation mimic other conditions; hence, careful diagnosis is required. The authors present two cases of patients presenting with soft tissue mass and a lytic bone lesion. The biopsy revealed granulomatous osteomyelitis. Lesion culture identified Mycobacterium tuberculosis . The authors urge clinicians to include TB as a differential diagnosis when investigating the primary cause of lytic bone lesions, even in the absence of pulmona

Triple Immunochromatographic System for Simultaneous Serodiagnosis of Bovine Brucellosis, Tuberculosis, and Leukemia.

An immunochromatographic test system has been developed for the simultaneous rapid multiplex serodiagnostics of bovine brucellosis, tuberculosis, and leukemia. The test system is based on the use of a conjugate of gold nanoparticles with the chimeric protein Cysteine-A/G and three analytical zones with immobilized pathogen antigens: Brucella abortus lipolysaccharide, recombinant proteins MPB64 and MPB83-MPB63 of Mycobacterium bovis, and recombinant protein p24 of the bovine leukemia virus. Prototypes of the test system were tested on 98 samples of sera from healthy and infected animals. The di

Bilateral primary optic neuropathy as the presenting manifestation of tuberculosis in an immunocompetent patient.

Vision impairment is one of the devastating complications of central nervous system tuberculosis (CNS TB). Optic atrophy is a sequelae of various forms of CNS TB which, ultimately, is responsible for vision impairment. It is usually the secondary optic atrophy which occurs in CNS TB. In this case report, we present the story of an immunocompetent woman presenting as bilateral visual loss. She was found to have primary optic atrophy. Neuroimaging revealed features of chronic meningitis. Polymerase chain reaction test for Mycobacterium tuberculosis was positive in cerebrospinal fluid. Although s

Lepromatous leprosy and perianal tuberculosis: a case report and literature review.

Leprosy is a chronic infectious disease caused by Mycobacterium leprae, a microorganism that usually affects skin and nerves. Although it is usually well-controlled by multidrug therapy (MDT), the disease may be aggravated by acute inflammatory reaction episodes that cause permanent tissue damage particularly to peripheral nerves. Tuberculosis is predominantly a disease of the lungs; however, it may spread to other organs and cause an extrapulmonary infection. Both mycobacterial infections are endemic in developing countries including Brazil, and cases of coinfection have been reported in the

Rare Association of Severe Cryptococcal and Tuberculosis in Central Nervous System in a case of Sarcoidosis.

Sarcoidosis is a multisystem noncaseating granulomatous disease with a propensity for lung, eye, and skin which recently have been proposed that mycobacterium tuberculosis may contribute in its pathogenesis, and rarely involves central nervous system (CNS). Despite CD4+ lymphocytopenia, sarcoidosis by itself does not increase risk of opportunistic infections other than cryptococcosis. Nonetheless, simultaneous association of CNS cryptococcosis and tuberculosis infection remains extremely rare event in immunocompetent states, and has not been reported in sarcoidosis yet. We here presented such

[Necrotizing fasciitis as the first manifestation of tuberculosis in a immunocompromised patient].

Disseminated tuberculosis is a process that mainly affects immunocompromised hosts. Its initial appearance as necrotising fasciitis is exceptional especially if it is not associated to a bone disorder. The diagnosis of this rare complication requires a high degree of suspicion and the evidence of Mycobacterium tuberculosis in the muscular tissue, since the symptoms may mimic the underlying illness itself, for this reason the most sensitive method for demonstrating tuberculous infection is a muscular biopsy. Here we present the case of a patient who was receiving immunosuppressant therapy for n
