We report on the treatment of invasive aspergillosis with the new triazole antimycotic agent itraconazole. All 11 patients suffered from pulmonary invasive aspergillosis. Two patients also had cerebral aspergillosis; in one of these patients the paranasal sinuses were also invaded. A 40-year-old woman developed a corneal ulcer with hypopyon following a traumatic episode. In a KOH mount dematiaceous, septate, branched hyphae were demonstrated, and heavy growth of Exserohilum rostratum was obtained in culture. No bacterial pathogens were isolated. The patient was treated with natamycin and subsequently with ketoconazole. In spite of antimycotic therapy a descemetocele of the eye remained. This observation represents the first documented case of an Exserohilum infection in India.

Three cases of paracoccidioidomycosis observed in 1990-1991 in Quillabamba, Peru, and its surroundings are described. The climate and the living conditions of the inhabitants favour paracoccidioidomycosis. It can be assumed that a majority of cases could not be identified, because the symptoms are disguised as other tropical diseases. The application of ketoconazole to the patients showed different results, from optimal to scarce, while temporary remission of the disease was obtained with itraconazole. The best therapy for paracoccidioidomycosis appears to be the application of amphotericin B

In the period between April 1984 and April 1988 a total of 4294 clinically suspected cases of dermatomycoses were examined for causative fungi. Of these cases 680 were suspected as tinea pedis and tinea manuum. These cases belonged to 21 different nationalities. Both sexes were represented and 649 cases (95.4%) were adults while 31 (4.6%) were prepubertals. A total of 90 cases of suspected dermatophytoses was observed over a period of 9 months in the city of Cusco (Peru) and in its environs. The age groups most commonly infected were 1-10 and 11-20 year old. The most common tineae diagnosed were tinea capitis (13.3%) and tinea unguium (11.1%). Only four species of dermatophytes were isolated: Microsporum canis (52.4%), Trichophyton mentagrophytes (35.7%), Tr. rubrum (9.5%) and Epidermophyton floccosum (2.4%). Dry weather, low temperature and high altitude do not seem to affect proliferation and infections by dermatophytes.

The protein profiles of different eumycetoma agents were compared by SDS gel electrophoresis. Dendrograms confirmed the homogeneity of isolates of Pseudallescheria boydii but amongst Madurella species, particularly isolates identified as M. grisea, there were substantial differences in protein composition. Pulmonary infection due to Nocardia sp. was investigated in 600 patients of various bronchopulmonary disorders by direct microscopy of sputum samples and cultures on appropriate media. Pathogenic Nocardia species were isolated from 25 patients, giving 4.1% incidence of infection. The species of Nocardia represented were N. asteroides (21 isolates), N. otitidis-caviarum and N. brasiliensis (2 isolates each). Paraffin baiting technique proved to be more efficacious than culturing on glucose nutrient agar.

Using a reproducible model of Candida albicans keratitis in rabbits we studied the effect of topical clotrimazole and bifonazole. Candida albicans DSM 70010 (2.5 X 10(5) cells) was injected into the corneal stroma of both eyes of 28 rabbits. All eyes developed a corneal ulcer. In 65 patients with scalp psoriasis or seborrhoic dermatitis of the scalp, stool specimens, tongue swabs and scalp scales were examined for yeasts. The stool specimens showed in 70.8% of the patient group massive and in 7.7% moderate yeast colonization. Yeasts were found in 47.7% of the tongue cultures and in 12.5% of the scalp scales. This retrospective study reviews the occurrence and treatment of various forms of tinea capitis at the out-patient clinic for Dermatology and Pediatric Dermatology of the University Hospital Sophia/Dijkzigt over the period 1977-1988. Tinea capitis was diagnosed in 64 children. Trichophyton violaceum was the pathogen most frequently isolated, with Microsporum canis ranking second. The potassium hydroxide preparation was positive in 58% of the cases. Four cases showed a positive potassium hydroxide preparation with a negative culture. Immigrant children from Morocco were the largest group (61%).

The authors report a case of subcutaneous mucormycosis in a diabetic patient with verrucous lesions in the fourth finger of the right hand. Initially diagnosed as a fixed cutaneous sporotrichosis case, success was obtained with the use of potassium iodide with healing of the lesion in a period of nearly 5 months. At present the patient remains under clinical observation with no treatment or relapse.

A Blastomyces dermatitidis yeast phase lysate antigen (T-58) prepared in our laboratory was used in an enzyme-linked immunosorbent assay (ELISA) for the detection of antibodies in 201 serum specimens from dogs with blastomycosis. The in vitro activities of two new miconazole and econazole salts with the sulphosalicylic acid against 71 clinical isolates of dermatophytes were evaluated in comparison with those of miconazole, miconazole nitrate, econazole and econazole nitrate. Miconazole sulphosalicylate and econazole sulphosalicylate were equally effective in inhibiting the fungal growth compared with miconazole, econazole, and their nitrates.

A case of localized cutaneous cryptococcosis successfully treated with oral itraconazole is reported. The patient, a 63-year-old Japanese housewife, had had an erythematous and partly eroded lesion in the area of her right earlobe for about 10 months.
