The outcome of kidney grafts from multiorgan donors and kidney only donors.

From 1988 to 1994, 15356 renal cadaveric transplantations have been performed within the Eurotransplant area (Austria, Belgium, Germany, Luxembourg and The Netherlands); 8746 kidneys were obtained from multiorgan donors and 6610 from kidney only donors. To evaluate the impact of the procurement policy, multiorgan donor (MOD) versus kidney only donor (KOD), on renal graft survival, an observational study has been performed. Multivariate analysis using Cox's proportional hazards model served to quantify the role of the procurement policy on renal graft survival after adjustment for other prognos

Molecular heterogeneity of steroid sulfatase deficiency: a multicenter study on 57 unrelated patients, at DNA and protein levels.

Steroid sulfatase (STS) deficiency is the biochemical defect of X-linked ichthyosis (XLI), one of the most common X-linked disorders. We studied 57 European unrelated patients affected by STS deficiency. Twenty-eight patients were from Italy, 24 from the United Kingdom, 4 from The Netherlands, and 1 from Denmark. In two families XLI was associated with Kallmann syndrome (hypogonadotropic hypogonadism and anosmia). STS enzymatic activity was profoundly deficient in all cases. Direct DNA analysis, using cDNA and genomic probes from the STS gene and linked regions, demonstrated heterogeneity of t

AIDS health education for intravenous drug users.

Intravenous (IV) drug users are the second largest risk group for AIDS and the main source of infection for heterosexual partner and pediatric AIDS cases. IV drug users have an addiction and a subculture that make risk reduction difficult; for example, to refuse to share needles can endanger personal relationships, and carrying clean works (rather than renting them in a shooting gallery) risks arrest. In New York City, at least, knowledge about AIDS transmission is widespread among IV drug users, and most drug injectors report having changed their drug use practices to reduce their risks. The

Probe-based metagenomic sentinel surveillance of viral respiratory infections in primary care: a prospective, national, pilot study.

BACKGROUND: With the introduction of metagenomics in clinical diagnostics unfolding and the expanding role of pathogen genomics in national surveillance, conditions are favourable for the further maturation of these approaches in public health surveillance. In this study, we aimed to pilot the use of probe-based metagenomics for nationwide sentinel surveillance through general practitioner (GP) networks and for the genomic characterisation of both anticipated and emerging respiratory viruses in primary care. METHODS: This prospective, pilot study included patients with acute respiratory illne

Quality of life and patient satisfaction after one-stage implant-based breast reconstruction with an acellular dermal matrix versus two-stage breast reconstruction (BRIOS): primary outcome of a randomised, controlled trial.

BACKGROUND: There is increasing interest in the use of acellular dermal matrices (ADMs) in implant-based breast reconstruction (IBBR). Suggested advantages are that ADMs facilitate one-stage IBBR and improve aesthetic outcomes. We compared immediate one-stage ADM-assisted IBBR with two-stage IBBR (current standard of care). Our previously reported secondary endpoint showed that one-stage ADM-assisted IBBR was associated with significantly more adverse outcomes. Here, we present the primary endpoint results aiming to assess whether one-stage IBBR with ADM provides higher patient-reported qualit

Cerebrovascular function in presymptomatic and symptomatic individuals with hereditary cerebral amyloid angiopathy: a case-control study.

BACKGROUND: Previous work suggests that impairments of cerebrovascular flow or reactivity might be early markers of cerebral amyloid angiopathy (CAA). Hereditary cerebral haemorrhage with amyloidosis-Dutch type (HCHWA-D) is a genetic form of CAA that can be diagnosed before the onset of clinical symptoms by DNA testing. We aimed to investigate whether haemodynamic measures are decreased in presymptomatic and symptomatic HCHWA-D mutation carriers compared with healthy controls. METHODS: In this case-control study, we included presymptomatic and symptomatic HCHWA-D mutation carriers diagnosed t

Evaluating COVID-19 Vaccine Masking and Unmasking Methods in Two National Pharmacovigilance Databases.

BACKGROUND: The COVID-19 mass vaccination led to a substantial increase in spontaneous reports submitted to pharmacovigilance (PV) databases, potentially introducing masking effects that could conceal safety signals. OBJECTIVES: To examine the masking effect of COVID-19 vaccines on disproportionality analyses and to compare two unmasking interventions in the Dutch (Lareb database) and Spanish (Farmacovigilancia Española, Datos de Reacciones Adversas, FEDRA) national PV databases: removal of all drug-event combinations (DEC) involving a COVID-19 vaccine versus excluding influential outliers DE

Exploring spillover effects beyond workplace dietary interventions using agent-based modelling.

OBJECTIVE: We applied an innovative agent-based modelling approach to investigate how and to what extent spillover effects occur beyond dietary interventions. DESIGN: We developed an agent-based model in NetLogo to simulate the dietary practices of the Dutch adult population during lunch and dinner, representing participants in the Dutch National Food Consumption Survey. SETTING: The model assessed spillover effects beyond a hypothetical workplace canteen intervention targeting increased plant-based protein consumption during lunch. Direct effects were defined as the percentage of non-vegeta

A community-based programme to reduce non-communicable disease risk among persons living in disadvantaged neighbourhoods in three European countries: protocol for a controlled trial.

BACKGROUND: Non-communicable diseases (NCDs) are the leading cause of premature death in Europe and disproportionately affect persons experiencing disadvantages (i.e., low-income populations, migrants and ethnic minorities). The Health Outcomes from Raised Urban Settings (HORUS) evaluation study aims to implement and evaluate a community-based programme, based on the social-ecological model, to reduce key modifiable NCD risk factors. The study is performed in three European cities (Rijeka, Croatia; Rotterdam, The Netherlands, and Valencia, Spain). METHODS: The HORUS programme will be evaluated

Unintentional Water Intake During Swimming and Post-Race Gastrointestinal Illness in Triathletes: Results from 6 Triathlons and 1294 Athletes.

This study aimed to investigate the relationship between water intake and post-race gastrointestinal illness in triathletes. Following a post-event survey approach, we evaluated the association between water intake and gastrointestinal illness in triathletes. We collected data among participants of six different triathlons in the Netherlands using an online questionnaire about personal characteristics (age, sex, swimming experience, chronic illness, and athletic level), the completed triathlon (length and duration), water intake, and illnesses during the 7 days following the triathlon. Ethical Implementation of Organ Donation Following Medical Assistance in Dying: Recommendations of the Ethics Committee of the Transplantation Society.

BACKGROUND: Medical assistance in dying (MAiD) is legally permitted in a growing number of jurisdictions, and in 6, it can currently be followed by organ donation. Organ donation following MAiD offers patients an opportunity to address transplant needs as a final act of altruism, but it also raises complex ethical questions that require strong safeguards to protect patients, professionals, and public trust. METHODS: We identified key ethical issues and safeguards by analyzing guidelines and protocols from the 6 countries where organ donation following MAiD is performed: Australia, Belgium, Ca

Between-centre variation in antibiotic therapeutic drug monitoring and obtaining surveillance cultures in Dutch ICUs: A retrospective observational study.

PURPOSE: Antimicrobial resistance is a major concern in intensive care units (ICUs), where infections and antibiotic use are common. Antimicrobial stewardship programs (ASPs) promote optimal infectious disease management through interventions such as Therapeutic Drug Monitoring (TDM) and selective digestive/oropharyngeal decontamination (SDD/SOD), but data on ICU-specific quality indicators (QIs) and between-centre variation are limited. This study aimed to evaluate infectious disease management in Dutch ICUs by assessing adherence to, and variation in, TDM and SDD/SOD practices. METHODS: In

Neutrophil phenotyping reflects injury severity when base excess and lactate remain normal.

INTRODUCTION: Accurate assessment of injury severity is essential for optimal trauma care, as it dictates timing and intensity of interventional strategy. Conventional markers, such as base excess and lactate levels are usually borderline deranged in patients with midrange injury severity. This study explored whether determining circulating neutrophil phenotypes in these patients (ISS 9-35) might aid in the assessment of injury severity. METHODS: A descriptive single-center cohort study of trauma patients ≥ 16 years between 2013 and 2023 was conducted. Data was collected from the Dutch Nation

Understanding Why Certain Patients With Hand and Wrist Conditions Are Dissatisfied Despite Achieving Their Expressed Personal Improvement Goals Anchored at Satisfaction: A Qualitative Study.

PURPOSE: The Personal Meaningful Gain (PMG) is a construct identifying a patient's self-defined improvement goal, capturing the individual's threshold to be satisfied with treatment results. Previous research shows that patients who achieve their expressed PMG should be satisfied with their treatment results, but this is not always the case. This study explored the lived experiences of patients who were dissatisfied with their treatment results despite having achieved their expressed PMG. METHODS: We conducted a qualitative study comprising semistructured interviews using Interpretative Pheno

Unraveling dynamic factors in Dialectic Behavioural Treatment for Adolescents (DBT-A): A study protocol.

Dialectical Behavioural Therapy for Adolescents (DBT-A) is an effective treatment for adolescents exhibiting features of borderline personality disorder (BPD). However, some do not benefit, potentially leading to major negative outcomes later in life. Previous quantitative studies have highlighted the importance of dynamic (changeable) factors in determining treatment success, including the therapeutic alliance, emotional dynamics between therapist and adolescent, and the motivation of those involved. Nevertheless, the specific contribution of these factors to the therapeutic process and perce
