Primary Lateral Sclerosis.

Primary lateral sclerosis is characterized by insidious onset of progressive upper motor neuron dysfunction in the absence of clinical signs of lower motor neuron involvement. Patients experience stiffness; decreased balance and coordination; mild weakness; and, if the bulbar region is affected, difficulty speaking and swallowing, and emotional lability. The diagnosis is made based on clinical history, typical examination findings, and diagnostic testing negative for other causes of upper motor neuron dysfunction. Electromyogram is normal, or only shows mild neurogenic findings in a few muscle

Two distinct neural mechanisms underlying indirect reciprocity.

Cooperation is a hallmark of human society. Humans often cooperate with strangers even if they will not meet each other again. This so-called indirect reciprocity enables large-scale cooperation among nonkin and can occur based on a reputation mechanism or as a succession of pay-it-forward behavior. Here, we provide the functional and anatomical neural evidence for two distinct mechanisms governing the two types of indirect reciprocity. Cooperation occurring as reputation-based reciprocity specifically recruited the precuneus, a region associated with self-centered cognition. During such coope

Robust T1-weighted structural brain imaging and morphometry at 7T using MP2RAGE.

PURPOSE: To suppress the noise, by sacrificing some of the signal homogeneity for numerical stability, in uniform T1 weighted (T1w) images obtained with the magnetization prepared 2 rapid gradient echoes sequence (MP2RAGE) and to compare the clinical utility of these robust T1w images against the uniform T1w images. MATERIALS AND METHODS: 8 healthy subjects (29.0 ± 4.1 years; 6 Male), who provided written consent, underwent two scan sessions within a 24 hour period on a 7T head-only scanner. The uniform and robust T1w image volumes were calculated inline on the scanner. Two experienced radiol

Headache Following Occipital Brain Lesion: A Case of Migraine Triggered by Occipital Spikes?

This study describes the case of an 8-year-old boy who developed a genuine migraine after the surgical excision, from the right occipital lobe, of brain abscesses due to selective infestation of the cerebrum by Entamoeba histolytica. After the surgical treatment, the boy presented daily headaches with typical migraine features, including right-side parieto-temporal pain, nausea, vomiting, and photophobia. Electroencephalography (EEG) showed epileptiform discharges in the right occipital lobe, although he never presented seizures. Clinical and neurophysiological observations were performed, inc

Training transfers the limits on perception from parietal to ventral cortex.

Visually guided behavior depends on (1) extracting and (2) discriminating signals from complex retinal inputs, and these perceptual skills improve with practice. For instance, training on aerial reconnaissance facilitated World War II Allied military operations; analysts pored over stereoscopic photographs, becoming expert at (1) segmenting pictures into meaningful items to break camouflage from (noisy) backgrounds, and (2) discriminating fine details to distinguish V-weapons from innocuous pylons. Training is understood to optimize neural circuits that process scene features (e.g., orientatio

Parturitional injury of the head and neck.

Parturitional injuries refer to injuries sustained during and secondary to fetal delivery. The skull, brain, and head and neck regions are frequently involved. Accurate differentiation and classification of the various injuries is essential for treatment, prognosis, and parental counseling. In this review, we discuss the various "bumps and lumps" that maybe encountered along the neonatal skull as well as the most frequent calvarial and intracranial parturitional injuries. In addition, a short discussion of the most common head and neck, facial, and spinal lesions is included. Various mimickers

Polymicrogyria: a common and heterogeneous malformation of cortical development.

Polymicrogyria (PMG) is one of the most common malformations of cortical development. It is characterized by overfolding of the cerebral cortex and abnormal cortical layering. It is a highly heterogeneous malformation with variable clinical and imaging features, pathological findings, and etiologies. It may occur as an isolated cortical malformation, or in association with other malformations within the brain or body as part of a multiple congenital anomaly syndrome. Polymicrogyria shows variable topographic patterns with the bilateral perisylvian pattern being most common. Schizencephaly is a

Nervous system Lyme disease.

Lyme disease, the multisystem infectious disease caused by the tick-borne spirochete Borrelia burgdorferi involves the nervous system in 10-15% of affected individuals. Manifestations include lymphocytic meningitis, cranial neuritis, radiculoneuritis, and mononeuropathy multiplex. Encephalopathy, identical to that seen in many systemic inflammatory diseases, can occur during active systemic infection. It is not specific to Lyme disease and only rarely is evidence of nervous system infection. Diagnosis of systemic disease is based on demonstration of specific antibodies in peripheral blood by m

EEG/MEG- and imaging-based diagnosis of Alzheimer's disease.

In recent years, researchers have embarked on a search of computer-aided methods for diagnosis of the Alzheimer's disease (AD) to help clinicians make the diagnosis earlier and more accurately such that treatment of the disease can begin sooner when there is a higher chance of success in slowing down the progression of this disease. This article presents a review of journal articles on brain signal- and image-based diagnosis of AD published in the past few years. The areas of signal processing, electroencephalogram and magnetoencephalogram are considered. In the area of image analysis, the fol

Neurocircuit function in eating disorders.

OBJECTIVE: Eating disorders are serious psychosomatic disorders with high morbidity and lifetime mortality. Inadequate response to current therapeutic interventions constitutes a challenging clinical problem. A better understanding of the underlying neurobiological mechanisms could improve psychotherapeutic and drug treatment strategies. METHOD: A review highlighting the current state of brain imaging in eating disorders related to the anxiety and pathological fear learning model of anorexia nervosa (AN) and the impulsivity model of binge eating in bulimia nervosa (BN). Toxocariasis.

Toxocariasis is one of the most commonly reported zoonotic helminth infections in the world with a higher prevalence in tropical settings and in rural populations. It is caused by the larval stages of the ascarids Toxocara canis, the common roundworm of dogs, and probably also by the larval stages of Toxocara cati, the roundworm of cats. The clinical spectrum of toxocariasis in humans varies from asymptomatic infection to severe organ injury caused by larval migration to the major organs ("visceral larva migrans"). Clinical involvement of the nervous system in visceral larva migrans due to Tox

Haemangiopericytoma with no dural attachment.

The authors describe a case of a gentleman in his 40s who presented with a space-occupying lesion in the right precentral gyrus which showed no dural attachment and was well circumscribed, both on imaging and at surgery. This lesion was histologically challenging to diagnose but favoured to be a haemangiopericytoma. The lack of dural attachment is unusual for this tumour type but is typically associated with metastases to the brain, and less commonly a solid primary central nervous system tumour. The authors propose that haemangiopericytoma should be regularly considered as a differential diag

Biomarkers for trials of neuroprotection in Parkinson's disease.

With increased understanding of disease pathogenesis and the foreseeable reality of disease-modifying therapies, there is a growing need to find biomarkers that will allow early (preferably preclinical) detection of disease and that will provide an independent readout of disease progression. In this article, we review a variety of markers, with a focus on functional imaging techniques, which while imperfect, currently provide the best approach to this problem. We consider the limitations of functional imaging of the dopamine system in assessing the progression of Parkinson's Disease (PD) as we

Emotion processing and regulation in bipolar disorder: a review.

OBJECTIVES: Bipolar disorder (BP) is characterized by a dysfunction of mood, alternating between states of mania/hypomania and depression. Thus, the primary abnormality appears to be an inability to regulate emotion, the result of which is emotional extremes. The purpose of this paper is to review the current functional magnetic resonance imaging (fMRI) literature on adult patients with BP using emotion processing or regulation paradigms. METHODS: A search was conducted on PubMed using the keywords: bipolar disorder, fMRI, mania, bipolar depression, bipolar euthymia, emotion, and amygdala. On
