Cerebral contusion as a rare cause of foot drop: case report.

A 74-year-old woman, taking anticoagulant therapy for chronic heart failure, presented to our emergency room with left dorsiflexion weakness 8 hours from after multitrauma. A detailed neurological examination revealed only 0/5 strength in the left foot dorsiflexion without any upper motor neuron signs. While there was no spinal cord pathology detected, cranial computed tomography demonstrated a lesion in the right parasagittal localization consistent with hemorrhagic contusion. Clinical follow-ups showed an improvement in neurological findings with muscle power of 3/5 in day 5 and 5/5 in day 4

Extradural en-plaque spinal meningioma with intraneural invasion.

BACKGROUND: Extradural spinal meningiomas are rare. Our understanding of purely extradural spinal meningiomas is incomplete because most reports rarely differentiate purely extradural meningiomas from extradural meningiomas with an intradural component. Occasionally, reports have described involvement of the adjacent nerve root, but there has never been a description of an extradural meningioma that actually infiltrates the nerve root. CASE DESCRIPTION: A 42-year-old woman presented with progressive lower extremity weakness and numbness below T3 during the span of 4 months with imaging eviden

[Paroxysmal hemicrania and SUNCT].

Paroxysmal hemicrania is experienced as headache attacks with pain and accompanying symptoms similar to those of cluster headaches. Attacks are, however of shorter duration, occur more frequently, affect predominantly women and respond reliably to indomethacin. Paroxysmal hemicrania can also occur secondary to an identifiable cause. To exclude symptomatic, paroxysmal hemicrania, especially with an atypical clinical picture and poor response to indomethacin, a careful diagnostic approach is necessary. The SUNCT syndrome (short-lasting unilateral neuralgiform headache attacks with conjunctival i

Effect of recombinant osteopontin on cerebral vasospasm after subarachnoid hemorrhage in rats.

BACKGROUND: osteopontin (OPN), a pleiotropic extracellular matrix glycoprotein, has been reported to have neuroprotective effects against early brain injury after subarachnoid hemorrhage (SAH). The aim of this study is to examine if osteopontin prevents cerebral vasospasm after SAH in rats. METHOD: the endovascular perforation model of SAH was produced, and 62 rats were randomly assigned to sham + vehicle, SAH + vehicle, and SAH+ r-OPN (0.1 microg) groups. Cerebral vasospasm was evaluated by India ink angiography at 24 and 72 h after SAH, as well as neurobehavioral tests. FINDINGS: significa

Seizure and altered mental status in a 12-year-old child with Shigella sonnei gastroenteritis.

We report a case of Shigella-associated altered mental status and seizure in a 12-year-old boy. Although seizure is a known complication of shigellosis, it is quite unusual in patients older than 9 years. This case illustrates the importance of considering the diagnosis of shigellosis in any pediatric patient with altered mental status or seizure in the context of gastrointestinal symptoms such as diarrhea.

Serological and clinical features of patients with myasthenia gravis in north Indian population.

Myasthenia gravis (MG) is a disorder of neuromuscular junction associated with presence of antibodies against nicotinic acetylcholine receptors (nAChRs). Here, we compared the clinical and serological profile of seropositive myasthenia gravis (SPMG) and seronegative myasthenia gravis (SNMG) patients. Anti-AChR antibody was measured using radio receptor immunoassay and correlated with clinical phenotype in 250 MG patients over 2004 and 2006. Out of 250 MG patients, 161 (64.4%) were males (male:female = 1.8:1). SNMG patients formed 40% (n = 101) of our MG patients which is much higher as compare

Catheter-tip mass mimicking a spinal epidural hematoma.

As > 95,000 spinal drug-delivery devices have been implanted since their inception in the 1980s, the recognition of associated adverse effects is essential. Since 1985, numerous reports have described the presence of catheter-tip granulomas. In the current case, the authors describe a less frequent complication of epidural bupivacaine precipitation. Regardless of origin, these unusual lesions have been increasingly recognized as a rare but potentially devastating complication of intrathecal infusions. A 34-year-old woman with an intrathecal pain-pump delivering fentanyl, bupivacaine, and cloni

Moisture status of the skin of the feet assessed by the visual test neuropad correlates with foot ulceration in diabetes.

OBJECTIVE: To examine the association between the moisture status of the skin of the feet with foot ulceration in subjects with diabetes. RESEARCH DESIGN AND METHODS: A total of 379 subjects with diabetes were examined. Assessment of peripheral neuropathy was based on neuropathy symptom score, neuropathy disability score, vibration perception threshold, and the 10-g monofilament perception. The moisture status of the skin of the feet was assessed using the visual test Neuropad. RESULTS: Patients with foot ulceration had more severe peripheral neuropathy and more often an abnormal Neuropad re

Nondysraphic pediatric intramedullary spinal cord lipomas: report of 5 cases.

OBJECT: Intramedullary spinal cord lipomas not associated with dysraphism are infrequently reported. When present, they typically occur in children and have a predilection for the cervical and thoracic spinal cord. The authors review the presentation, treatment, and disease course in 5 pediatric patients, and compare the outcomes with previously reported cases. METHODS: With institutional review board approval, a retrospective chart review was undertaken at Children's Hospital Los Angeles. RESULTS: Four patients with intramedullary spinal cord lipomas and 1 patient with a lipoblastoma, none

Two cases of frontotemporal dementia with predominant temporal lobe atrophy.

Frontotemporal dementia (FTD) is a subtype of frontotemporal lobar degeneration, which also includes semantic dementia (SD) and progressive non-fluent aphasia. Frontotemporal dementia is characterized by changes in personality and behavioral abnormalities, generally associated with predominant frontal lobe atrophy. Conversely, SD is typically characterized by Gogi (word meaning) aphasia based on semantic memory impairment and is associated with predominant temporal lobe atrophy. However, in the present cases, we diagnosed FTD on the basis of clinical symptoms, such as disinhibition, indifferen

Disappearance of episodic weakness during pregnancy in hyperkalemic periodic paralysis from the SCNA4 mutation T704M.

OBJECTIVE: Disappearance of episodic weakness during pregnancy and an increase in myotonia after delivery have not been reported in hyperkalemic periodic paralysis (hyperPP). CASE REPORT: In a 36-year-old white woman, with typical manifestations of episodic weakness of the limb muscles since age 3 years, hyperPP was diagnosed by demonstration of the T407M SCNA4 gene point mutation. At age 34 years, the index patient became pregnant for the first time. During the first trimester of her pregnancy, she noted a marked decrease in the frequency of the hyperPP attacks. During the second and third t

Sensory testing of inferior alveolar nerve injuries: a review of methods used in prospective studies.

PURPOSE: The inferior alveolar nerve (IAN) can be injured during trauma or surgery. So far there is no consensus for evaluating IAN injury. This study aimed to identify a testing method suitable for daily clinical practice which allows us to identify nerve injury, grade its severity, and monitor its recovery. MATERIALS AND METHODS: Covering a 20-year period, prospective studies on sensory changes after mandibular procedures were reviewed regarding sensory testing methods; 75 studies on third molar removal, osteotomy, fracture, and implants were included. RESULTS: These studies reported varyi

Long-lasting neurosensory disturbance following advancement of the retrognathic mandible: distraction osteogenesis versus bilateral sagittal split osteotomy.

Neurosensory disturbance (NSD) of the inferior alveolar nerve (IAN) is the most common complication after bilateral sagittal split osteotomy (BSSO) and distraction osteogenesis (DO) of the retrognathic mandible. It is suggested that the risk is lower after DO than after BSSO. This retrospective study compared both techniques with regard to long-lasting NSD and overall patient satisfaction. 91 patients (representing 182 IANs) were included, they completed a questionnaire and underwent an objective neurosensory test. In the BSSO-group (90 nerves), long-lasting NSD was reported in 27 cases (30%)

[Thoracic outlet syndrome: differential diagnosis and surgical therapeutic options].

The thoracic outlet syndrome (TOS) is a disputed syndrome in the field of the so-called compression syndromes of the upper extremity. This is no surprise, as the TOS is a complex and multifactorial compression syndrome diagnosed by different medical specialities. On average 6.5 physicians of different specialities need 4.3 years to come up with the diagnosis of TOS. The correct diagnosis is of great importance and crucial for successful operative treatment. The necessary diagnostic tests can only be accomplished by team work, especially interdisciplinary cooperation. This expressly includes ph

Stimulation-induced parkinsonism after posteroventral deep brain stimulation of the globus pallidus internus for craniocervical dystonia.

The authors report on a patient with craniocervical dystonia who was treated with bilateral GPi stimulation, with excellent improvement in dystonia but at the cost of stimulation-induced, reversible parkinsonism. Stimulation through ventral contacts resulted in maximal relief of craniocervical dystonia but induced considerable hypophonia, bradykinesia, rigidity, freezing, and impaired postural reflexes. Stimulation through dorsal contacts alleviated parkinsonism, but resulted in the return of dystonia. No stimulation parameters could alleviate the dystonia without inducing parkinsonism over th
