[Behavior modification therapy: application in renal nursing].

Kidney patients often suffer numerous comorbidities (e.g., hyperphosphatemia, renal osteodystrophy, and bodyweight gain induced by fluid overload or hypertension), which often impart discomfort and a significantly reduced quality of life. The literatures points to the efficacy of behavior modification therapy in changing inappropriate self-health care behavior in order to improve comorbidity symptoms. This article introduces behavior modification theory and its application as well as clinical strategies related to kidney patient care. The authors hope this article will promote knowledge of beh

[Rethinking oxygen therapy for premature infants in terms of oxidative stress].

Oxygen therapy is one of the most common interventions in the care of premature infants in the neonatal intensive care unit (NICU). Premature infants are at risk of oxidant injury due to inadequate anti-oxidant defenses. Common complications of premature infants such as retinopathy, chronic lung disease, intraventricular hemorrhaging, and periventricular leukomalacia may also be associated with secondary oxidant injuries. This article explores relationships between oxygen and complications linked to premature delivery. It also outlines correct concepts and practices of oxygen therapy for NICU

Diffuse white matter injury and neurologic outcomes of infants born very preterm in the 1990s.

Neurocognitive outcomes of infants born very preterm (less than 32 weeks gestation) remain a major concern in perinatal practice. Very preterm birth rates have increased, with enhanced survival since 1990. As focal brain lesions become less common, diffuse injury to both gray and white matter is now the primary focus for improving neurologic outcomes in survivors. Recent evidence supports preoligodendrocytes as the principal cellular target of diffuse white matter injury due to their susceptibility to hypoxic-ischemic and inflammatory insults. An understanding of their development and vulnerab

Potassium additive algorithm for use in continuous renal replacement therapy.

The performance of continuous renal replacement therapy (CRRT) brings about alterations in the serum potassium levels in patient. Potassium is an electrolyte essential for the regulation of nerve conduction and muscle contraction, particularly important in the case of cardiac muscle. This article describes the physiological mechanisms that affect potassium distribution throughout the body and also describes the effects of hypokalaemia or hyperkalaemia on the heart. This article justifies the need for serum potassium control during CRRT, which recognizes and reconciles the differing areas of re

Memories of former intensive care patients six months following discharge.

This exploratory/descriptive study describes the memories of former ICU (Intensive Care Unit) patients after discharge. Six patients described their memories of ICU and discharge to the ward and home. Memories for the former ICU patients varied, and there were multiple layers of awareness, with periods of unreal experiences and nightmares. In the memories collected, there was little reference to nurses, or connection to the care they provided. Once discharged from ICU, patients' memories changed to new challenges they faced as they coped with the realization of how sick they had been and tried

Anesthesia-related cardiac arrest in children. An update.

The improvement in mortality rates for anesthetized children over the past 50 years reflects the many improvements that have been made in pediatric perioperative care. The modern pediatric anesthesiologist is better trained than the predecessors of half a century ago, and has a vastly improved arsenal of monitoring devices and anesthetic agents from which to choose. The modern pediatric perioperative environment is better equipped to meet the unique needs of children. Techniques practiced by surgeons, nurses, radiologists, and pharmacologists help create a far more sophisticated infrastructure

Family-centered care and the anticipated death of a newborn.

Providing support for a family whose infant is expected to die requires planning and coordinated intervention by members of the health care team. From diagnosis until the death of the infant, the family's choices and needs must be integrated into an acceptable plan of care. The role of the health care team in this process includes four principal phases: planning for the baby's birth, preparing the siblings, caring for the baby, and supporting the family both at the time of the death and afterward. Specific strategies used by physicians, nurses, and child life specialists can encourage the fami

Teaching safer sex in a long-term psychiatric setting.

The trend toward normalizing life in mental health institutions poses a challenge to the facility's staff in the area of sexual relations among clients. In addition to their other duties, nurses now are being asked to discuss sexual feelings with the clients as well as teach sex education, AIDS information, and safer-sex practices. Such programs need to begin with retraining of staff and institution-wide classes to determine the clients' knowledge base. One model program experienced initial resistance among staff and patients, but worked well after an adjustment period. Different methods were

Experiences of Older Adults Living With HIV.

Background and Purpose: Older people living with HIV (OPLWH) face complex challenges as they age. Although advances in antiretroviral therapy have transformed human immunodeficiency virus (HIV) infection into a chronic condition and increased longevity, aging with HIV is often accompanied by multimorbidity, polypharmacy, stigma, social isolation, and mental health concerns. These intersecting factors complicate clinical management and are not always fully addressed in traditional HIV care models. Methods: This study aimed to explore the clinical care experiences of OPLWH over age 50. A qualita

Determinants and promotion strategies for type 1 diabetes screening in children: A qualitative study from a parental perspective.

BACKGROUND: First-degree relatives (FDRs) of type 1 diabetes (T1D) patients are at high risk for T1D. Early screening enables timely intervention, yet participation remains suboptimal, missing opportunities in this high-risk group. Understanding determinants of screening participation and translating them into actionable, theory-informed, family-centered strategies is essential to optimize preventive care. METHODS: In this qualitative study in China, semi-structured interviews were conducted with 21 adult T1D probands or parents of pediatric T1D patients who have unaffected children. The Theo

Student-related impacts on health professional educator wellbeing: A scoping review of influences and recommendations.

INTRODUCTION: There are growing institutional and regulatory imperatives to provide increasing support to higher education students, particularly those at risk of failure to succeed. These demands can be especially challenging for Health Professional Educators (HPEs), who are tasked with delivering meaningful and supportive learning experiences, while also ensuring graduates meet minimum competency thresholds for safe professional practice-a complex dynamic that can negatively impact educator wellbeing. Despite strong evidence linking student success to educator wellbeing, limited systematic a
