Bilateral central scotomata due to intracranial tumour.

Bilateral centrocaecal scotomata have been recognised as a sign of intrinsic optic nerve disease, usually associated with hereditary optic neuropathy, and nutritional or toxic amblyopias. This report describes four patients with central scotomata due to intracranial masses, three of whom recovered after surgical intervention. The clinician should be alerted to the association in patients with headaches, other neurological signs, and central visual loss.

Behçet's disease: an unusual case with bilateral obliterating retinal panarteritis and ischemic optic atrophy.

Report of an unusual case of Behçet's disease with bilateral obliterating retinal panarteritis and ischemic optic atrophy. The rarity of the primary optic nerve involvement and chiefly the exceptional full acute bilateral occlusion of the retinal arteries are emphasized. This ophthalmoscopic picture is further clinical evidence for the major importance of the vascular involvement in the pathogenesis of Behçet's disease.

Optic disc and peripapillary tumors.

This review covers the tumors either originating or seen close to the optic disc in the peripapillary area. Most of the optic disc tumors are diagnosed on clinical examination. In a few atypical cases, ancillary imaging, such as optical coherence tomography, fluorescein angiography, and ultrasonography, may be required to demonstrate characteristic features. The majority of these tumors have systemic associations, and hence, a multidisciplinary approach involving ocular oncologists, neurophysicians, radiation oncologists, and medical oncologists is the need of the hour. The treatment of optic

The Pulfrich Phenomenon: Practical Implications of the Assessment of Cases and Effectiveness of Treatment.

The Pulfrich phenomenon, originally described in normal observers, is a treatable disorder of the perception of movement in depth in cases of unilateral or asymmetric optic neuropathy. Treatment is highly bespoke and factors influencing treatment response and failure remain unclear. We assessed 25 adults with suspected Pulfrich phenomenon due to a range of conditions in two tertiary referral centres. Monocularly tinted spectacles were successful in reducing symptoms of the Pulfrich phenomenon under daylight conditions in nine subjects, eight of whom had optic neuritis. These spectacles were no

Bilateral vision loss associated with radiofrequency exposure.

A 57-year-old otherwise healthy woman presented with painless binocular vision loss 1 week after direct application of radiofrequency energy to her orbits. She had no light perception bilaterally. Pupils were dilated and not reactive to light. Fundoscopic exam initially showed optic disc swelling in the right eye and a normal-appearing disc in the left eye. Magnetic resonance imaging of the brain and orbits showed gadolinium enhancement of both intraorbital optic nerves. She underwent a course of high-dose steroid treatment without recovery of vision. Optic discs were pale 11 weeks after injur

[Visual evoked potentials in chessboard pattern reversion in children with optic neuritis].

Visual evoked potentials (VEP) to chess pattern reversion were studied in 17 children with optic neurites during the acute stage and in the course of convalescence 1-2 months and 1-3 years after the disease onset. Coarse VEP changes were detected in the patients during the acute stage of optic neuritis. A positive time course of VEP was followed up in late periods after the acute attack of the disease. Despite normalization of vision acuity, no complete restoration of VEP to small-cell chess patterns was seen. The findings evidence that studies of VEP to chess pattern reversion should be carri

Computed tomographic findings in septo-optic dysplasia: discordance between clinical and radiological findings.

The clinical and radiological findings in four children with septo-optic dysplasia are reported. All four had growth retardation associated with growth hormone deficiency, as well as varying degrees of ophthalmologic dysfunction. The CT scan findings spanned a spectrum from normal to the expected abnormalities involving the ventricular system and orbits. Only two of the four had an absent septum pellucidum. A third patient had normal CT scans except for optic nerve hypoplasia, while the fourth had entirely normal CT scans of both the brain and orbits. Because the anatomic defects of septo-opti

Understanding the molecular basis and pathogenesis of hereditary optic neuropathies: towards improved diagnosis and management.

Hereditary optic neuropathies result from defects in the human genome, both nuclear and mitochondrial. The two main and most recognised phenotypes are dominant optic atrophy and Leber hereditary optic neuropathy. Advances in modern molecular diagnosis have expanded our knowledge of genotypes and phenotypes of inherited disorders that affect the optic nerve, either alone or in combination, with various forms of neurological and systemic degeneration. A unifying feature in the pathophysiology of these disorders appears to involve mitochondrial dysfunction, suggesting that the retinal ganglion ce

[Qualitative and quantitative diagnostic possibilities of papillary colorimetry].

Papillary colorimetry is a qualitative and quantitative diagnostic method that defines the displacement of the temporal and nasal color ranges on the x;y plane of the German Industrial Standard (DIN) color chart. Starting with normal temporal and nasal papillary stain values, there are, for qualitative diagnosis, four different directions in which the color range can shift from normal values on the DIN color chart, i.e., in connection with simple optic atrophies, optic atrophies due to vascular conditions, hyperemic papillae, and deviations of the color range under vasodilative therapy. Advances in glaucoma biomechanics from 2000 to 2024: a bibliometric study and visualization analysis.

OBJECTIVES: Glaucoma is an irreversible progressive optic neuropathy characterized by neurodegeneration of retinal ganglion cells. Elevated intraocular pressure induces mechanical stress and strain at the optic nerve head, impairing axonal function and contributing to the onset and progression of glaucoma. This study aims to generalize and visualize the dynamic evolution and research hotspots of glaucoma biomechanics through bibliometric methods. METHODS: All documents about glaucoma biomechanics research in 2000-2024 were extracted from two databases (Web of Science Core Collection and PubMe

Optic Disc Microvasculature Reduction and Visual Field Progression in Primary Open-Angle Glaucoma.

PURPOSE: To investigate the relationship between optic disc vessel density (ODVD) reduction and visual field (VF) progression in primary open-angle glaucoma (POAG) patients. DESIGN: Retrospective case series. METHODS: A total of 187 POAG eyes underwent ≥5 consecutive VF, spectral-domain optical coherence tomography, and swept-source OCT angiography imaging sessions during ≥3 years of follow-up. ODVD reduction was defined as a statistically significant negative slope (P < .05) of ODVD, calculated as the ratio of pixels occupied by vessels within the temporal optic disc area for any global, su

When Is a Control Not a Control? Reactive Microglia Occur Throughout the Control Contralateral Pathway of Retinal Ganglion Cell Projections in Experimental Glaucoma.

PURPOSE: Animal models show retinal ganglion cell (RGC) injuries that replicate features of glaucoma and the contralateral eye is commonly used as an internal control. There is significant crossover of RGC axons from the ipsilateral to the contralateral side at the level of the optic chiasm, which may confound findings when damage is restricted to one eye. The effect of unilateral glaucoma on neuroinflammatory damage to the contralateral pathway of RGC projections has largely been unexplored. METHODS: Ocular hypertensive glaucoma was induced unilaterally or bilaterally in the rat and RGC neur

Melanopsin retinal ganglion cell loss in Alzheimer disease.

OBJECTIVE: Melanopsin retinal ganglion cells (mRGCs) are photoreceptors driving circadian photoentrainment, and circadian dysfunction characterizes Alzheimer disease (AD). We investigated mRGCs in AD, hypothesizing that they contribute to circadian dysfunction. METHODS: We assessed retinal nerve fiber layer (RNFL) thickness by optical coherence tomography (OCT) in 21 mild-moderate AD patients, and in a subgroup of 16 we evaluated rest-activity circadian rhythm by actigraphy. We studied postmortem mRGCs by immunohistochemistry in retinas, and axons in optic nerve cross-sections of 14 neuropath

Heterogeneous nuclear ribonucleoprotein K, an RNA-binding protein, is required for optic axon regeneration in Xenopus laevis.

Axotomized optic axons of Xenopus laevis, in contrast to those of mammals, retain their ability to regenerate throughout life. To better understand the molecular basis for this successful regeneration, we focused on the role of an RNA-binding protein, heterogeneous nuclear ribonucleoprotein (hnRNP) K, because it is required for axonogenesis during development and because several of its RNA targets are under strong post-transcriptional control during regeneration. At 11 d after optic nerve crush, hnRNP K underwent significant translocation into the nucleus of retinal ganglion cells (RGCs), indi

Expression of the diabetes risk gene wolframin (WFS1) in the human retina.

Wolfram syndrome 1 (WFS1, OMIM 222300), a rare genetic disorder characterized by optic nerve atrophy, deafness, diabetes insipidus and diabetes mellitus, is caused by mutations of WFS1, encoding WFS1/wolframin. Non-syndromic WFS1 variants are associated with the risk of diabetes mellitus due to altered function of wolframin in pancreatic islet cells, expanding the importance of wolframin. This study extends a previous report for the monkey retina, using immunohistochemistry to localize wolframin on cryostat and paraffin sections of human retina. In addition, the human retinal pigment epithelia
