Syphilitic Lymphadenitis and Clavicle Pathological Fracture Mimicking Malignant Tumor on 18 F-FDG PET/CT.

A 66-year-old woman presented with 9 days of left clavicle pain, with no history of recent trauma. CT images showed a left clavicle fracture with a surrounding soft tissue mass. An 18 F-FDG PET/CT scan revealed increased FDG uptake in the tonsils, multiple lymph nodes, and the pathologically fractured clavicle, initially suggesting malignancy. However, serologic tests and pathology confirmed syphilitic lymphadenitis and osteomyelitis. Given the inguinal and cervical lymphadenopathy with FDG uptake, potential bone destruction, or pathological fractures mimicking malignancy, syphilis should be c

Is Bartonella sp. infection relevant in hematological malignancies in HIV-negative patients? A literature review.

Bartonelloses are diseases caused by Bartonella sp., transmitted to humans by blood sucking arthropod vectors. Clinical presentations include bacillary angiomatosis, cat scratch disease and atypical forms. We performed a review of cases of bartonelloses and hematological malignancies published in HIV-negative patients. Terms used were Bartonella or Bacillary Angiomatosis and Leukemia, Lymphoma, Multiple Myeloma, or Cancer. Fifteen cases met our criteria. Clinical presentations included bacillary angiomatosis, chronic fever, chronic lymphadenopathy, osteomyelitis, neuroretinitis, chronic anemia

A Two-Stage Diabetic Foot Salvage Using Synthetic Bone Void Filler and Lesser Toe Fillet Flap: A Case Report.

CASE: A 54-year-old type 2 diabetic man with a diabetic forefoot ulcer and osteomyelitis successfully underwent a staged reconstruction to salvage his foot. An antibiotic-carrying bone void filler was used to decrease the infection burden after initial debridement. The fifth toe fillet flap was performed to cover the large soft-tissue defect. The patient returned to baseline ambulation at the 1-year follow-up with a well-incorporated flap. CONCLUSION: This case highlights the utility of combining an antibiotic-carrying bone void filler and a toe fillet flap in a two-stage approach to salvage

18F-FDG PET/CT in Clear Cell Sarcoma With Multiple Bone Metastases.

A 26-year-old man presented with right facial pain for more than 20 days. Admission diagnosis was considered as jaw osteomyelitis. CT of the chest before surgery showed multiple bone destructions, which were considered as bone metastases. Then the 18F-FDG PET/CT was recommended for finding the primary tumor and evaluation of disease extent. 18F-FDG PET/CT revealed multiple abnormal foci at the skeleton and the left arm. Pathological examination and molecular analysis revealed the diagnosis of clear cell sarcoma. We present a case of clear cell sarcoma with multiple bone metastases, which is an

Limb Salvage in Severe Diabetic Foot Infection.

Severe diabetic foot infections (DFI) are both limb threatening and life threatening and associated with negative impact on health-related quality of life. Most severe DFIs require surgical intervention, and the goal of treatment should be preservation of limb function in addition to eradication of infection. Minor amputations are required in approximately 40% and major amputations in approximately 20% of patients. Significant risk factors for lower extremity amputation included male gender, smoking, previous amputation, osteomyelitis, peripheral artery disease, retinopathy, severe infections,

Cefazolin-induced hypoprothrombinemia.

Long-term antibiotic use can be associated with a myriad of side effects, ranging from relatively benign to life-threatening. The most common side effects of cephalosporins include dermatologic reactions and diarrhea. Here, we present a rarer side effect: a spontaneous retroperitoneal hematoma in the setting of cefazolin administration for vertebral osteomyelitis. Several cephalosporins have been implicated in coagulopathy secondary to hypoprothrombinemia. However, only a few case reports have implicated cefazolin, and all have occurred in the context of acute or chronic renal failure. Our cas

Daptomycin-Related Rhabdomyolysis Complicated by Severe Hyperkalemia and Acute Kidney Injury.

We describe a case of a 65-year-old man who presented with progressive generalized weakness. He was started on daptomycin for methicillin-resistant Staphylococcus aureus (MRSA) right fifth metatarsal osteomyelitis. Laboratory testing revealed severe hyperkalemia of 7.5 mEq/L (normal range: 3.3-5.1) and acute kidney injury (AKI) stage 1 associated with a substantial elevation in serum creatine kinase (CK). He had been on a high-intensity statin for years, and daptomycin was switched to linezolid, and statin was held as daptomycin-related rhabdomyolysis was suspected. Our case highlights the imp

Brucellosis causing bone marrow aplasia in an 11-year-old patient with complete recovery after treatment.

Brucellosis is one of the most prevalent zoonotic infections in the Middle East. The disease may present with a range of symptoms from a simple febrile illness to severe invasive infections affecting different organ systems (meningitis, osteomyelitis). In this paper we present an eleven-year-old girl who was diagnosed with "idiopathic bone marrow aplasia" and planned for hematopoietic stem cell transplant (HSCT), when pre-transplant work-up showed high brucella titers. The patient was started on doxycycline, rifampin and gentamicin initially, with discontinuation of the latter 3 weeks into the

Salvage of Bone Transport with the Induced Membrane Technique in an Open Tibia Fracture with a Segmental Defect: A Case Report.

CASE: A 29-year-old man with an open tibia fracture and a 15-cm bone defect was treated with irrigation, debridement, intramedullary nailing, soft-tissue flap, and bone transport with a frame over the nail. He developed osteomyelitis of his bone transport segment close to docking, which required bone resection and an induced membrane technique to salvage his transport segment and achieve union. CONCLUSIONS: This report illustrates the combined use of bone transport and induced membrane technique to achieve union in a 15-cm tibial defect. It demonstrates the use of the Masquelet technique to s

Multimodal imaging of Spina Ventosa (TB Dactylitis) of the foot.

We present the case of a 29-year-old male healthcare worker with a 6 month history of progressive left foot pain resulting in presentation to the emergency department on 3 occasions. He denied systemic symptoms. Multimodal imaging demonstrated an expansile erosive inflammatory lesion centered on the neck of the second metatarsal with aggressive features. CT of the thorax, abdomen, and pelvis demonstrated calcified mediastinal lymph nodes and left inguinal adenopathy. The lesion was biopsied under ultrasound guidance demonstrating a necrotizing granulomatous osteomyelitis with acid fact bacilli

Central venous catheter placement leading to an emergent paramedian sternotomy: a case report.

Perioperative cardiac tamponade during central venous catheter placement is rare. We present a case of tamponade from pulmonary artery injury during dialysis catheter placement resulting in complicated sternotomy and hospital course. A 52-year-old female experienced intraoperative hypotension, rapidly identified as tamponade, that was treated with an emergent paramedian sternotomy. Patient experienced postdischarge dehiscence and osteomyelitis requiring multiple reoperations. This case is the first report of a deviated paramedian sternotomy performed mainly through ribs. The complications expe

A pediatric case of SAPHO-CNO syndrome with clinical correlation between cutaneous and osteoarticular features.

Synovitis acne pustulosis hyperostosis osteitis (SAPHO) and chronic nonbacterial osteomyelitis (CNO) represent overlapping osteoarticular autoinflammatory syndromes, with a minority displaying neutrophilic skin features at the time of diagnosis. The pathophysiological link and chronological timeframe between skin and osteoarticular findings remain ambiguous, which in turn can manifest in diagnostic delay. We present a rare pediatric case of SAPHO-CNO with a clear association between cutaneous and osteoarticular symptoms, treated with nonsteroidal anti-inflammatory medications, corticosteroids,

Polymicrobial community-acquired Acinetobacter baumannii and Burkholderia pseudomallei bacteremia: opportunistic infections with similar risk factors in northern Australia.

We report the case of a 61-year-old man from northern Australia with concurrent community-onset Acinetobacter baumannii complex and Burkholderia pseudomallei bacteremia presenting as severe tropical pneumonia requiring intensive care unit support. The pneumonia was complicated by L3/4 discitis and vertebral osteomyelitis presumed to be due to melioidosis. His risk factors included chronic lung disease and immunosuppression with etanercept. This case of concurrent infection highlights the similar risk factors, presentation and epidemiology of both infections, emphasises the importance of accura

Asymptomatic Bone Marrow Edema Detected by 67Ga Scintigraphy.

A 46-year-old woman presented with a complaint of fever. CRP was 2.1 mL/L in blood, white blood cell count was 20-29/hpf, and bacterial count was 3418/μL in a urinalysis. Ga scan revealed accumulation of Ga in the left distal femur, although she had no symptoms around the site. MRI demonstrated diffuse high signal intensity on T2-weighted STIR images. Osteomyelitis was suspected, and biopsy was performed. Bacterial culture of the bone marrow was negative, and histological examination showed no infiltration of inflammatory cells. Two months after the biopsy, disappearance of altered signal inte

Evidence-Based Gardening: Using Palliative Approaches to Cure Complex Wounds.

Complex wounds are commonly thought to require aggressive surgical intervention to achieve healing. However, optimal healing results can often be achieved when greater emphasis is placed on optimizing patient factors prior to any surgical intervention. A retrospective review was performed of pressure ulcers and complex wound cases treated over 5 years at the Yale New Haven Wound Center. Optimal healing including clinical resolution of osteomyelitis and improved scarring was achieved when patient factors, such as glucose control and nutrition, were optimized. Surgical intervention can be more e

Chronic Venous Insufficiency Causing a Diagnostic Conundrum on Tc-99m Bone Scan Done for Osteosarcoma Surveillance: Awareness is the Key!

Bone scans are the most commonly used imaging technique to rule out local recurrence or metastasis during surveillance of malignant bone tumors after treatment. Although bone scans are very sensitive in detecting recurrence or metastasis, they are less specific. There are many nonmalignant conditions which can mimic either recurrence or metastasis on a Tc-99m bone scan. Therefore, physicians must be aware of such conditions to avoid unnecessary workup and invasive procedures. We present such an interesting case where chronic venous insufficiency mimicked either osteomyelitis or regional metast

Management of oroantral communication using buccal advanced flap.

Oroantral communication (OAC) or fistula (OAF) is an open pathological communication between the oral cavity and maxillary sinus which mostly occurs as a result of extraction of upper molars and premolars, iatrogenic complications or from dental infections, osteomyelitis, radiation therapy or trauma. Several alternative techniques modalities have been described throughout the years for the management of OAC and OAF which show both advantages and limitations. The most employed surgical flaps are of three types: advanced buccal flap, palatal flap and buccal fat pad flap. The authors present two

Mycobacterial Musculoskeletal Infections.

Although less common as causes of musculoskeletal infection than pyogenic bacteria, both Mycobacterium tuberculosis and nontuberculous mycobacteria can infect bones and joints. Although tuberculous arthritis and osteomyelitis have been recognized for millennia, infections caused by nontuberculous mycobacteria are being identified more often, likely because of a more susceptible host population and improvements in diagnostic capabilities. Despite advances in modern medicine, mycobacterial infections of the musculoskeletal system remain particularly challenging to diagnose and manage. This artic

Prevertebral neck abscess with Salmonella sandiego: a case presentation and literature review.

We describe an unusual case of a 58-year-old man with type 2 diabetes mellitus (T2DM) developing sepsis secondary to a prevertebral neck abscess. Following cross-sectional imaging, the patient underwent surgical drainage. Salmonella sandiego was isolated from urine, blood and pus cultures. Salmonella species neck infections represent a rare cause of focal salmonellosis, though have been reported in the literature. This is the first reported case of S. sandiego causing neck abscess. The case highlights an atypical presentation of salmonellosis, with the surprising finding of a neck abscess on c

Daptomycin-induced Eosinophilic Pneumonia and a Review of the Published Literature.

A 53-year-old man was admitted to the hospital with a diagnosis of cellulitis and osteomyelitis. Twenty-four days after the initiation of daptomycin and sulbactam/ampicillin, he developed a fever and pulmonary infiltration. Bronchoalveolar lavage revealed a high number of eosinophils, while an intracutaneous test revealed positivity for daptomycin. The patient improved after discontinuing antimicrobial therapy. The plasma daptomycin minimum concentration (Cmin) was elevated (27.4 μg/mL), but plasma protein binding of daptomycin was low (87.8%). Although the pathophysiology of eosinophilic pneu
