Effect of acupuncture for poor ovarian response: a multicenter randomized controlled trial.

INTRODUCTION: Acupuncture has been explored as a potential intervention for POR; however, high-quality evidence is limited. This multicenter randomized trial evaluated the effect of acupuncture on the number of oocytes retrieved following controlled ovarian hyperstimulation (COH) in women with POR. METHODS: This multicenter, randomized, controlled study was conducted at nine tertiary hospitals in China between August 2018 and March 2023, with follow-up extended through March 2024. A total of 140 women aged ≤ 40 years, who met the Bologna criteria and were eligible for the antagonist ovulation

Does the recovery of serum FSH influence the ovarian sensitivity index in the follicular-phase depot GnRH agonist protocol?

BACKGROUND: The follicular-phase long GnRHa protocol is a common protocol in COH. This study mainly analyzed the recovery of FSH levels on the day of Gn initiation compared with basal FSH levels on the day of downregulation and studied the correlation with the OSI to discover a new monitoring target for COH. METHODS: A total of 2286 patients who underwent IVF/ICSI with the follicular-phase depot GnRH agonist protocol from January 2018 to January 2023 were stratified into cohorts. The primary outcome was the OSI in patients who received the follicular-phase depot GnRH agonist protocol. The GnF

The polygenic score for age at menopause is independently associated with the response of controlled ovarian hyperstimulation in women undergoing IVF.

BACKGROUND: While Anti-Müllerian hormone (AMH) helps personalise In Vitro Fertilization (IVF) stimulation protocols, a significant number of women still do not achieve an optimal ovarian response. To address this, we explore the utility of a polygenic score for age at natural menopause (ANM) as a predictor for ovarian response during IVF. METHODS: This cohort study included 435 women who underwent standardised IVF stimulation, and they were genotyped to calculate a polygenic score (PGS) for ANM. Linear regression and ANCOVA models were used to assess the associations between the PGS and ovari

A commentary on the article "Controlled Ovarian Stimulation (COS) with follitropin delta results in higher cumulative live birth rates compared with r-hFSH alfa/beta in a large retrospectively analysed real-world data set".

Follitropin delta, a recombinant human follicle-stimulating hormone (r-hFSH), is administered using an individualized dosing algorithm based on serum anti-Müllerian hormone (AMH) levels and body weight and offers a different pharmacokinetic (PK) and pharmacodynamic (PD) profile compared to conventional gonadotropins. The retrospective analysis by Eggersmann et al. aimed to compare the cumulative live birth rate (CLBR) of r-hFSH delta versus r-hFSH alfa/beta using data from the German In-vitro Fertilization (IVF) registry (D.I.R.®; Deutsches IVF-Register), encompassing 113,936 stimulation cycle

Explainable artificial intelligence to identify follicles that optimize clinical outcomes during assisted conception.

Infertility affects one-in-six couples, often necessitating in vitro fertilization treatment (IVF). IVF generates complex data, which can challenge the utilization of the full richness of data during decision-making, leading to reliance on simple 'rules-of-thumb'. Machine learning techniques are well-suited to analyzing complex data to provide data-driven recommendations to improve decision-making. In this multi-center study (n = 19,082 treatment-naive female patients), including 11 European IVF centers, we harnessed explainable artificial intelligence to identify follicle sizes that contribut

Fertility stimulation protocols in women with cancer.

All female oncology patients undergoing controlled ovarian stimulation for fertility cryopreservation should be offered an antagonist protocol. Therapy can begin at the time of the first visit, providing consent is obtained. There is no need to delay until the next period. Stimulated patients should be monitored with ultrasound and estradiol levels as per normal clinic practice. For women with a diagnosis of breast cancer, and with the agreement of the treating oncologist, the stimulation protocol should include an aromatase inhibitor such as letrozole to reduce the circulating estradiol level

Serum luteinizing hormone trajectories during ovarian stimulation and their outcomes of IVF/ICSI: a retrospective cohort study.

BACKGROUND: An appropriate luteinizing hormone (LH) level during ovulation stimulation (OS) is important for positive in vitro fertilization/intracytoplasmic sperm injection (IVF/ICSI) outcomes. Although the gonadotrophin-releasing hormone antagonist (GnRH-ant) protocol can inhibit a premature rise of LH, it remains ineffective in some patients. This study aimed to identify the characteristics of patients who shared similar underlying LH trajectories and to determine how different serum LH trajectories influence IVF/ICSI outcomes. METHODS: This study was a retrospective cohort study that incl
